1.Anesthesia methods during artificial femoral head replacement in elderly patients

Artificial femoral head replacement is commonly performed in elderly patients with femoral neck fracture. However, the patients are always complicated by other diseases and their major organ functions are low, which increases risks of anesthesia and replacement. The anesthesia methods for artificial femoral head replacement include epidural anesthesia, combined spinal-epidural anesthesia, and general anesthesia. Respiratory system and nervous system complications, bleeding and transfusion complications, deep venous thrombosis and lung embolism, and bone cement-associate complications are common complications in anesthesia for femoral head replacement. Therefore, appropriate anesthesia pattern, rigid monitoring, and proper treatment can reduce anesthesia risks.
2.Supervision of Schizophrenia in Community

Objective To explore the significance of Community Mental Health Service funded by the government, organised by the Health Department and participated by the Community Psychiatric Service, which consists of community case managers, policemen, community health station, doctors, nurses and family of patients.Methods 150 patients, having the tendency to cause trouble or disturbance in their psychotic episodes, were placed in a case management system, monitored and treated throughout the year, gathered statistics for their number and extent of violence every month, and scored according to Social Disability Screening Schedule (SDSS).Results After one year's monitoring and treatment, the total number of occurrence of violence 5 grade among the 150 patients decreased from 20 person-times to 0 person-times; violence 4 grade decreased from 68 person-times to 2 person-times; violence 3 grade decreased from 66 person-times to 4 person-times; violence 2 grade decreased from 258 person-times to 12 person-times; violence 1 grade decreased from 432 person-times to 24 person-times. The number of patients who lost their violent tendency increased from 0 person-times to 72 person-times.Conclusion To establish community case management systems, monitor, treatment, conduct follow-up studies and providing appropriate recovery guidance for schizophreniac patients through the Community Psychiatric Service, are the effective ways to decrease the rate of causing trouble or disturbance, reduce the patients' social disability, and facilitate the patients' recovery of social functions.

4.Safety and efficacy of endoscopic retrograde cholangiopancreatography for children with pancreaticobiliary diseases

Objective To assess the safety and efficacy of endoscopic retrograde cholangiopancreatography (ERCP) for pancreaticobiliary diseases in children. Methods Data of 9 patients younger than 14 years who underwent ERCP at between November 2004 and May 2010 were indentified through a computer database search. Therapeutic methods, success rate and procedure-related complications were evaluated.Results A total of 9 patients underwent 17 ERCP procedures under anesthesia, including 16 therapeutic and 1 diagnostic procedure. The success rate was 94. 1% (16/17) and the complication rate was 11.8%(2/17), including 1 mild pancreatitis and 1 peri-pancreatic infection. Conclusion ERCP is an important tool with high safety and efficacy for diagnosis and treatment of pancreaticobiliary diseases in children.

7.Role of endothelial cell injury in the pathogenesis of diabetic nephropathy

[Summary] Diabetic nephropathy is one of the major chronic microvascular complications of diabetes ,which is the leading cause of end‐stage renal disease ,as well as the main cause of death in diabetic patients. Glomerular endothelial cell is an important component of the glomerular filtration barrier ,which is directly related to the materials of circulation ,and it can be easily damaged by glucose ,lipid and inflammatory factors. Under the hyperglycemia ,the PKC pathway ,the polyol pathway and oxidative stress were activated ,producing an excess of advanced glycation end products and reactive oxygen species ,which damage the endothelial nitric oxide synthase ,reduce the generation of nitric oxide ,while produce a large number of Ang Ⅱ. Ang Ⅱ damage the endothelial cell. In addition ,there are crosstalk between glomerular endothelial cells and endothelial cells ,which also cause endothelial cell injury. Here ,we reviewed the role of endothelial cell injury in the pathogenesis of diabetic nephropathy.
8.A Meta analysis on NOS3 G894T polymorphism and cancer susceptibility

Objective To investigate the relation between polymorphism of NOS3 G894T and susceptibility of cancer .Methods Literatures under the inclusion and exclusion criteria about relation between polymorphism of NOS 3 G894T and susceptibility of cancer were collected by computer -based retrieval and manual retrieval .The pools ORs with 95% CI were calculated to assess the association strength between polymorphism of NOS 3 G894T and cancer risk by Meta methods .Sensitivity and publication bias were evaluated .Results 19 literatures with 7679 cases of cancer and 8180 cases as control group were included .The pooled result indica-ted that no significant association between its polymorphism and cancer (ORT versus G=1 .030 ,95% CI=0 .964 -1 .101 ;ORTT versus GG=1 .056 ,95% CI=0 .895-1 .245 ;ORTG versus GG=1 .045 ,95% CI=0 .977-1 .119 ;ORTT/TG versus GG=1 .047 , 95% CI=0 .981-1 .117 ;ORTT versus TG/GG=1 .005 ,95% CI=0 .900-1 .123) .No significant association was observed in sub-group analysis based on ethnicity ,cancer type ,and source of control group .Conclusion There was no relationship between NOS3 G894T polymorphism and cancer susceptibility .
9.Effect of Different Purity Galangin on Melanocyte Proliferation and Melanin Synthesis of A375-HaCaT Co-culture System

Objective To investigate the effect of Galangin of different purity on melanocyte proliferation and melanin synthesis of A375-HaCaT co-culture system. Methods The A375-HaCaT co-culture system was established with Transwell technology, and 0.1, 0.5, 1.0, 5.0, 10 μg/mL Galangin of the purity of 70%and 90%was used to act on the co-culture system. Cell proliferation, melanin content and tyrosinase of A375 were measured by MTT assay, NaOH lysis assay and L-DOPA oxidation assay respectively. The cytokines such as ET-1 and SCF in HaCaT were detected by ELISA. Results A375 cells in co-culture system grew well, and 0.1, 0.5, 1.0, 5.0, 10 μg/mL Galangin of the purity of 70% and 90% had up-regulation effect on cell proliferation, melanin content and tyrosinase of A375. Moreover, Galangin could increase the expression level of ET-1 and SCF of HaCaT at more than 0.5 μg/mL, and the effect of Galangin of 70% purity was better than 90% purity. Conclusion Galangin could promote the cell proliferation, melanin content and tyrosinase of A375, and mechanism of the pathways is probably related to the up-regulation on the expression of ET-1 and SCF of HaCaT.
10.Liver transplantation in 64 liver donors with hepatic steatosis

Objectives To investigate the results of liver transplantation using steatosis liver donors in order to provide a scientific basis for the use of marginal donors.Methods From 2002 to 2011,80 of 407 were steatosis liver donors.There were 69 males and 11 females.Their age ranged from 20-54 years old.Sixteen donor livers with severe fatty liver and reperfusion injury were not used.The remaining 64 livers were divided into a S1 group (mild steatosis,n=22),a S2 group (moderate steatosis,n=25),and a S3 group (severe steatosis,n=17).A S0 group was used as a control (randomly selected fat-free liver,n=80).Results The occurrence rates of delayed graft function (DGF) in the S0,S1,S2,S3 groups were 5%,9.1%,20%,29.41%,respectively.Primary nonfunctioning occurred in 2 cases of the S3 group,which represented a 11.76% of the S3 group (2/17),and 3.12% of the total 64 cases (2/64).Conclusions Although the incidence rate of DGF was higher in the steatosis liver donor groups than the S0 group,there was no correlation in the mortality rate of the S1-S3 group within one year of transplantation.Attentions should be paid to the treatment of complications after steatosis liver transplantation.By minimizing ischemia-reperfusion injury,improving microcirculation,strengthening routine therapy and reducing the amount of immunosuppression,the same results could be achieved using steatosis donor liver and normal liver for transplantation.
