1.Mycoplasma infection and drug sensitivity analysis in 3026 patients with urogenital tract infections

Objecfive To investigate the prevalence of Ureaplasma urealyticum(UU)and Mycoplasma hominis(MH)infections in urogenital tract and their sensitivities to drugs in Qinhuangdao area from 2006 to 2007.Methods UU and MH were detected by Mycoplasma IST kits in urine samples from 3062 patients suspected as urogenital tract infection and the sensitivity tests for 8 antimicrobial agents were performed.Results Mycolasmas were detected in 1262 patients from 3062 patients suspected as urogenital tract infection,in which 1037 were of UU infection,41 were of MH infection and 184 were of UU+MH infection.Both strains were sensitive to josamycin,doxycycline and tetracycline,but were less sensitive to ofloxacin and ciprofloxacin.Conclusions Drug resistance is increasing for Mycoplasma and sensitivity test is important in treatment of urogenital tract infection by Mycoplasma.
2.Cardiac Screening in the First Trimester Using Three-dimensional Ultrasound with Spatiotemporal Image Correlation

[Objective] To evaluate the value of cardiac screening in the first trimester by using three-dimensional ultrasound with spatiotemporal image correlation (STIC) and to analyze the value of different views for cardiac assessment.[Methods] This study retrospectively reviewed the eighty-seven fetuses with congenital heart diseases which were diagnosed in our institution between January 4th 2013 and June 30th 2016.We compared the detection rate of prenatal ultrasound screening with STIC in the first trimester and the results of postnatal echocardiography or autopsy.In addition,the detection rates of the different planes,including four-chamber view (4CV),outlet of the left ventricle (OLV),outlet of the right ventricle (ORV) and three vessels-trachea view (3VT) were analyzed.[Results] Severe cardiac anomalies were identified in 62 fetuses (71.3%) and non-severe cardiac anomalies were identified in 25 fetuses (28.7%).Seventy-eight cases (89.7%,78/87) with congenital heart diseases could be detected by using three-dimensional ultrasound with STIC.The detection ratesfor 4CV,4CV+OLV,4CV+ORV,4CV+3VT,4CV+OLV+ORV and 4CV+OLV+ORV+3VT were 48.3%,64.4%,66.7%,79.3%,69.0% and 89.7% respectively.[Conclusions] Three-dimensional ultrasound with STIC could detect congenital heart disease in the first trinester.The four-chamber view plus outlet of the left ventricle,outlet of the right ventricle and three vessels-trachea view could increase the detection rates of congenital heart diseases.
3.Rational thinking of precision medicine from the perspective of syndrome differentiation and treatment of Chinese medicine

This paper mainly analyzed the similarities and differences of theory, practice and development between precision medicine and TCM syndrome differentiation. After the analysis, we found that these two medicine body could be integrated to improve the progress of medicine albeit some differences.
4.Early therapeutic intervention for congenital hypothyroidism with L-thyroxin sodium

Objectives To observe the effects of varied doses of L-thyroxin sodium ( L-T4) on congenital hypothyroidism ( CH) and to determine appropriate dose for early therapeutic intervention. Methods Totally, 108 cases of confirmed CH were divided into two groups (54 in each group). All the cases were administered with L-T4 at varied doses( 10. 1? - 15. 0 ?g ? kg-1? d-1 for large-dose group and 6. 0 - 10. 0 ?g ?kg-1? d-1 for small-dose group) after their parents were trained for basic knowledge of CH by specialists. Their serum levels of T3, T4 and TSH, as well as their body height, weight and head circumference, were measured during follow-up. L-T4 was maintained at doses of 1. 0 - 4. 0 ?g ? kg -1 ? d-1 when their serum levels of T3, T4 and TSH returned to normal. Results All the 108 cases had their clinical symptoms quickly improved and development and growth accelerated after L-T4 substitution. Their serum level of T3 returned to normal in all cases, that of T4 did not return to normal in six cases of large-dose group and in 16 cases of small-dose group, and that of TSH did return to normal in 22 cases of large-dose group and in 39 cases of small-dose group, respectively, after one-month treatment, with a significant difference between the two groups. Cases in large-dose group gained more weight in average than those in small-dose group when they were three months old. Conclusions Early therapeutic intervention for babies with CH with L-thyroxin sodium of 10. 1 -15. 0 ?g ? kg-1? d-1 could quickly correct abnormal function of their thyroid and could avoid abnormal development of their intelligence due to CH.
5.High-performance liquid chromatographic determination of floxuridine in human serum

OBJECTIVE To develop an HPLC method for the determination of floxuridine in human serum.METHODS With metronidazole as the internal standard,200 μL of serum was extracted by n-propyl alcohol/methyl t-butyl ether in a two-step extraction.The organic layer was evaporated under nitrogen stream and the residual was reconstituted with the mobile plase.A Shim-Pack CLC-ODS column was selected and the mobile phase was consisted of acetonitrile-phosphate buffer-water (75∶100∶900) at a flow rate of 0.6 mL*min-1.The detection wavelength was 268 nm.RESULTS A linearitywas obtained from 0.005 to 0.5 mg*L-1 of floxuridine in serum with a good correlation coefficient (r=0.9999,n=8).The intra-run and inter-run coefficients of variation were less than 4.09%.The mean recoveries were 103.00%,107.00% and 100.88% for the low,middle and high concentrations of check samples,respectively.The limit of detection was 0.001 mg*L-1.CONCLUSION The method was sensitive,specific and simple.It is suitable for clinical pharmacokinetic study.
6.The role of soluble intercelluar adhesion molecule-1 in the pathogenesis of bronchial asthma

Objective To study the role of soluble intercellular adhesion molecule-1 ( sICAM-1 ) in the pathogenesis of adults bronchial asthma and analyze the relationship between the level of sICAM-1 and the severity of bronchial asthma.Methods Serum levels of sICAM-1 in 134 cases with different periods of bronchial asthma patients and healthy volunteers were measured by ELISA and pulmonary functions of acute asthma patients were determined by pulmonary function analyzer. Results Serum sICAM-1 levels in 63 cases of acute asthma patients were increased significantly when compared with those in remission asthma patients and healthy volunteers and its value was increased significantly with the exacerbation of asthma;A negatively correlation between FEV1% of pulmonary function and serum sICAM-1 level in patients with acute asthma was found in this study. Conclusion sICAM-1 was one of the important adhesion molecules in the pathogenesis of bronchial asthma. It could be used as one indicator of disease severity and guide the drug application in clinic.
7.Efficacy of laryngeal mask airway Supreme and Streamlined Liner of the Pharynx Airway in patients undergoing general anesthesia

Objective To compare the efficacy of laryngeal mask airway Supreme (LMAS) and Streamlined Liner of the pharynx Airway (SLIPA) in patients undergoing general anesthesia. Methods Eighty ASA Ⅰ or Ⅱ patients aged 18-70 yr weighing 45-80 kg undergoing general anesthesia were randomly divided into 2 groups ( n = 40 each): group LMAS and group SLIPA. Pharyngeal airway was inserted after induction of anesthesia with fentanyl 3μg/kg and propofol 2.0-2.5 mg/kg. MAP, HR, number of insertion, rate of successful placement at first attempt, placement time, airway sealing pressure, peak and mean airway pressure and side effects were recorded. Results There were no significant differences in MAP, HR and rate of successful placement at first attempt between the two groups. The placement time was significantly longer, the airway sealing pressure lower and the incidence of side effects higher in SLIPA group than in LMAS group. There was no significant difference in the peak and mean airway pressure between the 2 groups. Conclusion Both LMAS and SLIPA can assure good airway sealing and adequate ventilation. The complication is rare. The efficacy of LMAS is better.
8.Effect of ginsenoside Rg1 on the transcriptional activation of NF-κB induced by H_2O_2 in 293T cell

Objective:To observe the influence of ginsenoside Rg1 on transcriptional activation of NF-κB induced by hydrogen peroxide (H_2O_2) in 293T cell,and probe into the antioxidant mechanism of ginsenoside Rg1.Methods:In the experiment,cells was exposed to H_2O_2 after pretreatment with Rg1,cell proliferation and cytotoxicity studies were detected by MTT and Trypan blue.The quantities of generation of intracellular reactive oxygen species (iROS) was analyzed by flow cytometric analysis measured with fluorescent probe 2,7-dichlorofluorescin diacetate (DCFH-DA).NF-κB-responsive element-luciferase reporter gene was transfected and dual-luciferase cis-reporting systems were used to assay the transcriptional activity of NF-κB under the stimulated circumstance of oxidative stress induced by H_2O_2.Results:The results of MTT showed that ginsenoside Rg1 apparently protected the proliferation of 293T cell,which were repressed by H_2O_2 (P<0.05).The results by trypan blue showed that H_2O_2 stimulated substantial cytotoxicity.This effect was markedly attenuated by treatment with ginsenoside Rg1.Oxidant production,measured as the fluorescence of dichlorofluorescein,was significant increased by 40%-50% through H_2O_2 stimulation.The decrease in iROS generation was significant blocked by 35%-40% through Rg1 and antioxidant.The relative luciferase reporter assay of NF-κB was apparently improved by H_2O_2-induced(P<0.05),but Ginsenoside Rg1 significantly repressed the relative value of luciferase (P<0.05).Conclusion:Ginsenoside Rg1 has the obvious protective function from the damage of oxidative stress damage,whose possible mechanism is to eliminate excessive free radicals of the cells effectively,to reduce transcriptional activation of nuclear factor kappa B(NF-κB),and subsequently to suppress the NF-κB circuit activation.
9.Optimization of Conditions of Homologous Gene Quantitative Polymerase Chain Reaction

OBJECTIVE To evaluate the standard of non-invasive diagnosis of Down′s syndrome between the different PCR machines. METHODS Two kinds of PCR machines were used respectively to amplify the homologous genes-the human liver-like phosphofructokinase gene (PFKL) and the human muscle-like phosphofructokinase gene (PFKM). RESULTS The best annealing temperature of the two PCR machines were 64 ℃ and 60 ℃,and only in this reaction conditions PFKL′s and PFKM′s electrophoresis strips had the same optical density value. CONCLUSIONS This approach has proven that the conditions of amplified the PFKL and PFKM are different between the two kinds of PCR machines.
10.Clinical observation of albumin-bound paclitaxel plus S-1 in patients with advanced pancreatic cancer after the failure of gemcitabine

Background and purpose: Pancreatic cancer is often diagnosed at advanced stage, therefore, chemotherapy remains the cornstone of treatment for advanced pancreatic cancer. However, no standard regimen has been established as second-line therapy for advanced pancreatic cancer. The purpose of the study was to evaluate the efifcacy and safety of albumin-bound paclitaxel plus S-1 for the treatment of advanced pancreatic cancer patients in second-line setting after the failure of gemcitabine treatment. Methods:Clinical outcomes of 19 patients with advanced pancreatic cancer were analyzed. These patients received albumin-bound paclitaxel plus S-1 as second-line therapy after the failure of gemcitabine treatment. Albumin-bound paclitaxel was administered at a dose of 125 mg/m2 over 30 minutes on day 1 and 8 of a 21-day cycle. From d1-14, all patients received oral S-1 40 mg/m2, twice daily. Results:All patients were available for evaluation. Of the 19 patients, 1 case got complete response (CR), 4 cases had partial response (PR) and 9 cases had stable disease (SD). The objective response rate (ORR) was 26.3%, the disease control rate (DCR) was 73.7%and the median progression free survival (PFS) was 5.2 months. The main toxicities include hematological toxicity, myodynia, gastrointestinal reactions, sensory neuropathy, fatigue and alopecia. Conclusion:The combination of albumin-bound paclitaxel and S-1 is effective and tolerated in the treatment of advanced pancreatic cancer patients who resistant to gemcitabine.
