2.MRI Findings of Juvenile Acute Articular Cartilage Injury of the Knee Joint

Objective To suty the MRI manifestations of juvenile acute articular cartilage injury of the knee joint.Methods MRI findings of cartilage,subcartilage low signal line and subcarilage bone were analysed retrospectively in 53 juvenile patients (ranged in age from 4~27 years) with acute articular cartilage injury confirmed by arthroscopy.Results Sixty-nine cartilage injuries were showed by MRI in 53 patients,including patellas in 25,femoral lateral condyles in 22,femoral medial condyles in 11,trochlea of the femur in 2,and tibial plateau in 9.Acute articular cartilage injury appeared as pure cartilage fracture in 46, including complete split of the cartilage in 22 sites,partly split of the cartilage in 20 sites,and fissur-like fracture in 4 sites.Osteochondral fracture were observed in 23 sites,including avulsion fracture in 13 and osteochondral subsided in 10.Articular cartilage loose bodies and osteochondral loose bodies were found by MRI in 6 and 13,respectively.Conclusion MRI is the best non-invasive method for studying cartilage injury.
3.Clinical effects of cytokine induced killer cells treatment combined with radiofrequency ablation on liver metastases

Objective To observe the efficacy of cytokine induced killer (CIK) cells combined with radiofrequency ablmion (RFA) in treatment of liver metastases.Methods 23 patients were treated with CIK plus RFA, while 27 patients with RFA.Tumot markers and CD3+、CD4+、CD8+、CD4+/CD8+ were tested before and after treatment.Recurrence rate and Kamofsky score were also evaluated.Results The local recurrence rate in the study group was lower (13.04% vs.40.74%)(P<0.05),CD3+、CD4+、and CD4+/CD8+ were increased (P<0.01). The percentage of patients with decreased tumor markers and increased Kamofsky is higher (P<0.05);Conclusion The therapy of CIK combined with RFA has better efficacy on liver metastases,can enhance the cellular immune functions and improve the quality of life significantly.
4.Effects of amino acid on growth and secondary metabolites contents of adventitious roots of Tripterygium wilfordii.

The adventitious root of Tripterygium wilfordii was used as experiment material to study effects of various concentration of aspartic acid, isoleucine, cysteine and arginine in MS medium on the growth and triptolide, wilforgine, wilforine contents of the adventitious roots. The results showed that compared with the control, supplemented with 0.25 mmol x L(-1) aspartic acid at 3rd week, the growth of the adventitious roots only accounted for 80%, but the content of triptolide of the adventitious roots and the medium was 1.36, 1.30 times, the content of wilforgine was 1.16, 1.37 times, the content of wilforine was 1.22, 1.63 times, respectively. At 3rd week 0.05 mmol x L(-1) isoleucine, the growth of adventitious roots was 97.3%, wilforgine of adventitious roots and medium 1.02, 1.27 times, wilforine 1.36 times and 1.15 times. At 1st week 0.25 mmol x L(-1) cysteine, the growth of the adventitious roots comprised 77.5% of the control, while content of triptolide of adventitious roots reached 1.87 times. At 2nd week 1.00 mmol x L(-1) cysteine, the growth of adventitious roots was 44.6% of the control, the content of wilforine in medium was 2.97 times. At 3rd week 0.50 mmol x L(-1) arginine, the growth of adventitious roots was 124.2%, the content of wilforgine and wilforine was 1.3, 1.4 times, respectively.

5.Fingerprint of Polygala tenuifolia by high performance liquid chromatography.

AIMTo study and establish the fingerprint of Polygala tenuifolia by RP-HPLC.
METHODSThe fingerprint of P. tenuifolia was built by using Kromasil C18 as column and acetonitrile-0.05% phosphoric acid aqueous in gradient as mobile phase. The flow rate was 1.0 mL x min(-1), and the detecting wavelength was set at 318 nm. Total 14 batches of P. tenuifolia from different habitats were detected, and different commodities and preparative methods were compared.
RESULTSSamples from different habitats were of high similarities, and the qualities of "yuanzhitong" and "yuanzhirou" were better than that of "yuanzhigun", while the fingerprints of P. tenuifolia with different preparative methods have not obvious differences. Moreover, the standard fingerprint of P. tenuifolia was originated from the "Computer Aided Similarity Evaluation" software, and 29 common peaks existed in the fingerprint. Each peak in the fingerprint was separated very well under the above chromatographic condition, with good accuracy, stability and repeatability, satisfied to the requests of fingerprint.
CONCLUSIONThe RP-HPLC fingerprint method can be used in the quality control of P. tenuifolia.

6.Study on the Optimization of Enrichment Medium of Lactobacillus casei Zhang

The effects of different carbon source,nitrogen source,proportion of carbon and nitrogen source, microelement and buffer salts on the growth of Lactobacillus casei Zhang isolated from Koumiss were studied.The enrichment medium for Lactobacillus casei Zhang was optimized by response surface methodology and its composition was glucose 20.9 g/L,soy peptone 10.45 g/L,yeast extract 10.45 g/L,K2HPO4 3.5 g/L, sodium acetate 14.6 g/L,sodium citrate 2.35 g/L,MgSO4?7H2O 1.0 g/L,MnSO4?5H2O 54 mg/L, CuSO4?5H2O 10 mg/L,tween 80 1.0 g/L.After cultivated in enrichment medium for 18h at 37℃,the living cells of Lactobacillus casei Zhang were 4.78?109 CFU/mL,which was about 10 times higher than in MRS (4.8?10 8 CFU/mL).
7.Study on the correlation between mesencephalic structural characteristics and motor function in patients with cerebral small vessel disease

Objective:To explore whether there were changes in midbrain morphology related to motor function in patients with cerebral small vessel disease (CSVD).Methods:The study was conducted on a subset of patients with CSVD registered at the Department of Neurology of Peking Union Medical College Hospital from 2010 to 2018. All magnetic resonance imaging images were taken with a 3.0 T nuclear magnetic resonance imager. The measurement of anteroposterior diameter of the mesencephalon and the tegmentum of mesencephalon was performed on a personal computer with the image processing software RadiAnt DICOM Viewer. Several clinical manifestations of dyspraxia, dysphagia, dysarthria and dysuria were evaluated by interviewing the patient and family members living with the patient. Kinect depth camera combined with self-developed software platform was used to conduct quantitative evaluation of patients′ motor function. Three parameters, namely walking speed, walking time of three meters and time of standing up and sitting down, were selected from the obtained parameters. SPSS 17.0 software was used for statistical analysis.Results:A total of 176 patients were included, aged 30-88 (64.16±11.57) years. One hundred and fifteen patients were males, accounting for 65.34%. In patients with CSVD, anteroposterior diameter of the mesencephalon and the tegmentum of mesencephalon were negatively correlated with age ( B=-0.032, P<0.001; B=-0.020, P=0.006). The anteroposterior diameter of the mesencephalon was negatively correlated with symptoms of dyskinesia ( OR=0.006, 95% CI 0-0.135, P=0.001), even when the age and gender were adjusted ( OR=0.014,95% CI 0-0.416, P=0.013). The anteroposterior diameter of the mesencephalon was negatively correlated with symptoms of dysuria ( OR=0.046,95% CI 0.002-0.936, P=0.045), but no longer correlated when the age and gender were adjusted. The anteroposterior diameter of tegmentum of mesencephalo was also negatively correlated with symptoms of dyskinesia ( OR=0.035,95% CI 0.002-0.684, P=0.027), but no longer correlated after adjusting for age and gender. Video recording and evaluation of motor function were performed on 87 patients. Spearman correlation analysis showed that none of the three motor function scores was correlated with age or gender. The anteroposteric diameter of the midbrain was positively correlated with walking speed ( r=0.231 ,P=0.040) and negatively correlated with walking time of three meters ( r=-0.304, P=0.005), but not with standing up and sitting down time, while the anteroposteric diameter of tegmentum of mesencephalo was not correlated with all of them. Conclusion:There are changes in mesencephalon morphology in patients with CSVD, structural parameters of mesencephalon are related to motor function and urination function, and mesencephalon atrophy may be an independent related factor for symptoms of dyskinesia in patients with CSVD.
8.Clinical and laboratory findings of patients with anti-dipeptidyl-peptidase-like protein 6 encephalitis: two cases report

Objective:To identify anti-dipeptidyl-peptidase-like protein 6 (DPPX) antibody in patients with encephalitis of unknown etiology and describe the clinical features of anti-DPPX antibody-associated encephalitis in Chinese patients.Methods:For patients registered in the Peking Union Medical College Hospital Encephalitis and Paraneoplastic Syndrome Registration Project from 2016 to 2019 with negative findings in autoimmune encephalitis routine antibody profile and paraneoplastic antibody profile, but with positive tissue-based assay (TBA) results, further tests for rare antibodies, including cell-based assay (CBA) of anti-DPPX antibody, were performed. Patients positive for anti-DPPX antibody were enrolled and the clinical data were collected.Results:Two patients with anti-DPPX antibody-associated encephalitis were found from 2016 to 2019 among about 15 000 patients. Both were females, aged 46 and 75 years. One patient had diarrhea, cachexia, cognitive dysfunction, agitation, myoclonus, tremor, and seizures. The other had cognitive impairment, restlessness, memory loss, disorientation, and sleep disturbance. The second patient had medical history of systemic lupus erythematosus and secondary Sj?gren′s syndrome.Conclusions:TBA should be combined with CBA in identification of anti-DPPX antibody to confirm the diagnosis. Anti-DPPX antibody-associated encephalitis has clinical manifestations of encephalopathy with diarrhea and cachexia, and can coexist with systemic lupus erythematosus.
9.Application and effect of standardized management model of quality control in neurosurgery

Objective To discuss the methods and effect of structuring standardized management model and to search the strict quality control method of first class nursing quality in order to provide reference bases for improving the quality of nursing and ensuring nursing security.Methods We formatted a first class nursing control group and formulated standardized quality control methods.Then a reasonable supervisory mechanism was established and we used the hierarchical management mode to inspect the nursing quality per week and per month.In the end,through summary feedback and tracking ways,the quality of nursing was well controlled.Results By comparison,there were significant differences between first class nursing quality and third class nursing quality,nurses defect under standardized management mode improved to 59.60%,and it was significantly different compared to the result in 2009 ( 33.65% ) ( x2 =5.35,P < 0.05 ).The punishment rate decreased to 0.55%,it was obviously lower than the results surveyed in 2009 ( 11.22% ) ( x2 =53.49,P <0.05).Conclusions First class nursing standardized quality control methods is a seamless management that can help to continuously improve the quality of nursing and ensure the nursing safety.
10.The synergistic effect of FGF-21 and insulin on regulating glucose metabolism and its mechanism.

Previous studies proposed that the synergistic effect of fibroblast growth factor-21 (FGF-21) and insulin may be due to the improvement of insulin sensitivity by FGF-21. However, there is no experimental evidence to support this. This study was designed to elucidate the mechanism of synergistic effect of FGF-21 and insulin in the regulation of glucose metabolism. The synergistic effect of FGF-21 and insulin on regulating glucose metabolism was demonstrated by investigating the glucose absorption rate by insulin resistance HepG2 cell model and the blood glucose chances in type 2 diabetic db/db mice after treatments with different concentrations of FGF-21 or/and insulin; The synergistic metabolism was revealed through detecting GLUT1 and GLUT4 transcription levels in the liver by real-time PCR method. The experimental results showed that FGF-21 and insulin have a synergistic effect on the regulation of glucose metabolism. The results of real-time PCR showed that the effective dose of FGF-21 could up-regulate the transcription level of GLUT1 in a dose-dependent manner, but had no effect on the transcription level of GLUT4. Insulin (4 u) alone could up-regulate the transcription level of GLUT4, yet had no effect on that of GLUT1. Ineffective dose 0.1 mg kg(-1) FGF-21 alone could not change the transcription level of GLUT1 or GLUT4. However, when the ineffective dose 0.1 mg x kg(-1) FGF-21 was used in combination with insulin (4 u) significantly increased the transcription levels of both GLUT1 and GLUT4, the transcription level of GLUT1 was similar to that treated with 5 time concentration of FGF-21 alone; the transcription level of GLUT4 is higher than that treated with insulin (4 u) alone. In summary, in the presence of FGF-21, insulin increases the sensitivity of FGF-21 through enhancing GLUT1 transcription. Vice versa, FGF-21 increases the sensitivity of insulin by stimulating GLUT4 transcription in the presence of insulin. FGF-21 and insulin exert a synergistic effect on glucose metabolism through mutual sensitization.
