1.Comparison of dosimetry in radiotherapy for craniospinal irradiation

Objective To explore the dosimetry of simplified intensity modulated radiotherapy (sIMRT)in the spinal component of craniospinal irradiation(CSI)compared with conventional plan(3D-CRT).Methods Five previously treated patients were adopted to generate 3D-CRT,3-and 5-field slMRT plans.The prescribed dose was 36 Gy to the planning target volume(PTV)in 20 fractions.The dose distributions of target volume and normal tissues,and homogeneity index(HI)were ananlyzed using the dose volume histogram.The total monitor units(MUs)were also analyzed to compare the delivery time indirectly.Results For 3D-CRT plans,the region of the neighbouring fields only received 70% of the prescribed dose,and the maximum dose delivered in the normal tissues was increased to approximately 140% of the prescribed dose.The homogeneity index(HI)of 3-and 5-field sIMRT were 0.09 ± 0.01 and 0.08 ±0.01,respectively,superior to that of 3D-CRT(0.18 ± 0.02,t=7.80,7.65,P<0.05).The values of V10 for the heart of 3-and 5-field sIMRT were(8.4 ± 1.9)% and(8.4 ± 2.0)%,respectively,lower than that of 3D-CRT((36.0 ± 6.0)%(t=13.3,13.0,P<0.05).V20 for the thyroid gland were (12.4±1.5)% and(12.4±1.6)%,respectively,lower than(69.4±5.7)% of 3D-CRT(t=26.3,26.4,P<0.05).V20 for the larynx were(17.2 ± 1.2)% and(17.9 ± 1.5)%,respectively,lower than that of 3D-CRT[(89.4±7.0)%(t=25.5,26.5,P<0.05)].V30for the NTwas(4.4±1.4)%,(4.9± 1.9)%,lower than that of 3 D-CRT[(31.9 ± 6.1)%(t=8.5,10.1,P<0.05)].The averaged values of total MUs for 3-and 5-field sIMRT were 1100 ± 177 and 1160 ±204,respectively,higher than that of 3D-CRT(640 ± 151).Conclusions Compared with the 3D-CRT CSI technique,3-and 5-field sIMRT have better dose distribution and can improve target dose uniformity and protect the organs at risk.
2.Locking calcaneal plate in the treatment of complicated calcaneal fractures

Objective To evaluate the effects of locking calcaneal plate in the treatment of SandersⅢ~Ⅳcalcaneal fractures.Methods 15 feet of 12 patients of SandersⅢ~Ⅳcalcaneal fractures received the treatment of internal fixation reduction with locking calcaneal plate and hone graft.Results All the patients were followed up for 4 to 21 months(average 11 months) with the results that according to Maryland foot score,4 feet were reported ex- cellent,6 good,1 average and 1 poor,with total excellent and good rate of 83.3 %.Conclusion Using locking cal- caneat plate in the treatment of complicated calcaneal fractures was proved to be a good method and had satisfactory effects.
3.Pathological changes of diabetic rat thyroid ultrastructure and intervention effects of insulin and aminoguanidine

Objective To investigate the thyroid ultrastructural pathological changes of diabetes mellitus (DM) rats as well as the intervention effects of insulin and aminoguanidine. Methods Totally 87 rats were treated with streptozotocin to establish DM animal models and divided into DM group(n=27),insulin intervention group(n=32) and aminoguanidine intervention group(n=28),25 rats were taken as normal controls. Twelve and 20 weeks after the animal model establishment, animals were sacrificed, thyroid tissue was taken and ultrastructure was observed. Results In the thyroid of DM rats, follicular epithelial cells present as applanate shape, microvilli were depleted, rough endoplasmic reticulum dilated to irregular vesicular. None pinocytotic vacuole and casual primary or secondary lysosome were seen. Follicular cavity was dilated, colloid in the cavity had higher electronic-density. Interstitial edema, capillary base lamian was thickened at different stage. Proteo-substance deposition with granulo-shape, cloud shape or homogeneity appeared. The number of thyroid parafollicular cells increased. But endocrine granule in parafollicular cells was few. When compared with DM group, the thyroid tissue injury of insulin intervention group and aminoguanidine intervention group were lessened to different degree. Conclusion The hypofunctional thyroid follicular cells, large quantity of proteo-substance deposition in the interstitium and increased parafollicular cells of DM rats may be related with hyperglycemia toxicity. Insulin and aminoguanidine treatment have some protection effects.
4.VEGF pathway-targeted therapy for advanced renal cell carcinoma: A meta-analysis of randomized controlled trials.

Immunotherapy which has been in practice for more than 20 years proves effective for the treatment of metastatic renal cell carcinoma (mRCC). Anti-angiogenesis-targeted therapy has recently been identified as a promising therapeutic strategy for mRCC. This study was aimed to evaluate the effectiveness of vascular endothelial growth factor (VEGF) pathway-targeted therapy for mRCC by comparing its effectiveness with that of immunotherapy. The electronic databases were searched. Randomized controlled trials (RCTs) on comparison of VEGF inhibiting drugs (sorafenib, sunitinib and bevacizumab) with interferon (IFN) or placebo for mRCC treatment were included. Data were pooled to meta-analyze. A total of 7 RCTs with 3451 patients were involved. The results showed that anti-VEGF agents improved progression-free survival (PFS) and offered substantial clinical benefits to patients with mRCC. Among them, sunitinib had a higher overall response rate (ORR) than IFN (47% versus 12%, P<0.000001). Bevacizumab plus IFN produced a superior PFS [risk ratio (RR): 0.86, 95% confidence interval (CI): 0.76-0.97; P=0.01] and ORR (RR: 2.19; 95% CI: 1.72-2.78; P<0.00001) in patients with mRCC over IFN, but it yielded an increase by 31% in the risk of serious toxic effects (RR: 1.31; 95% CI: 1.20-1.43; P<0.00001) as compared with IFN. The overall survival (OS) was extended by sorafenib (17.8 months) and sunitinib (26.4 months) as compared with IFN (13 months). It was concluded that compared with IFN therapy, VEGF pathway-targeted therapies improved PFS and achieved significant therapeutic benefits in mRCC. However, the risk to benefit ratio of these agents needs to be further evaluated.
5.Characteristics of astrovirus infection and typing characteristics analysis among children hospitalized with diarrhea in Tianjin

Objective To study the pathogenic prevalence and genotypes of astrovirus among children under 5 years old hospitalized with diarrhea in Tianjin. Methods A total 837 stool specimens were collected from children with diarrhea hospitalized in Tianjin children's hospital from May 2008 to April 2009. Astrovirus antigens were detected using ELISA and the postive specimens were inoculated in CaCo-2cells. After the CPE caused by virus were observed, the total RNA of virus was extracted, then the genomc fragments of the strains were amplified by using RT-PCR and confirmed by sequencing of the RT-PCR products. Detection of rotavirus was employed by Colloidal Gold Device. Results Astrovirus antigen was found positive in 3.0% of the patients. The coinfection rate of astrovirus and rotavirus was 0. 7% (6/837).Ninety-six persent of children with astrovirus diarrhea were younger than 2 years of age, Forty-eight persent of children with astrovirus diarrhea were younger than 6 months. The astrovirus infections occurred mainly between August 2008 and April 2009. Of the 21 astrovirus positive specimens, 11 cases were successfully identified by RT-PCR and they were all serotype 1. Conclusion Astrovirus is a major cause of nonbacterical diarrhea between 2008 and 2009 in Tianjin, and the predominant serotype is type 1.
6.Paracrine effect of chondrocytes on gene expression of matrix metalloproteinases and tissue inhibitors of metalloproteinases in osteoblasts

BACKGROUND:Cel co-culture can maximize the simulation ofin vivomicroenvironment. Cel scratch test and interleukin-1β can destroy the balance between matrix metaloproteinases (MMPs) and matrix metaloproteinase inhibitors (TIMPs), resulting in extracelular matrix degradation of the articular cartilage, functional disorders of chondrocytes and articular cartilage degeneration. OBJECTIVE:To study the effect of interleukin-1β on migration, MMP and TIMP expression of chondrocytes co-cultured with osteoblast supernatantin vitro. METHODS:There were three groups: chondrocyte monoculture group, osteoblast+chondrocyte group (co-culture group), osteoblast+chondrocyte+interleukin-1β group (interleukin-1β group). Cel scratch test was conducted to observe the migration of chondrocytes within 24 hours. Semi-quantitative PCR test was used to detect the changes in expressions of MMP-1, MMP-2, MMP-3, MMP-9, TIMP-1, TIMP-2, TIMP-3, TIMP-9 in chondrocytes within 24 hours. RESULTS AND CONCLUSION:Compared with the monoculture group, cel migration rate of the other two groups were increased significantly (P< 0.01). Compared with the monoculture group, the gene expressions of MMP-1, MMP-2, MMP-3 and MMP-9 were increased significantly in the coculture group (P < 0. 05); the gene expressions of MMP-1, MMP-3, MMP-9 were increased significantly in the interleukin-1β group (P< 0. 01). Compared with monoculture group, the gene expression of TIMP-1 was increased significantly (P < 0. 01), but the gene expressions of TIMP-3 and TIMP-4 were declined significantly (P < 0. 05) in the other two groups. These findings indicate that co-culture of chondrocytes with osteoblasts can promote chondrocytes migration, enhance gene expression of chondrocytes MMP-1, MMP-2, MMP-3, MMP-9 and regulate gene expression of TIMPs family. Interleukin-1β inhibitsthe migration of chondrocytes co-cultured with osteoblasts and gene expression of TIMPs family.
7.Advances in the research on the stature estimation of forensic anthropology

Stature is one of the important criteria to individual recognition. And it is always a focus in forensic anthropology. There are some new progresses studies in stature estimation in recent years. We review some studies in stature estimation published in domestic and foreign in the past six years, and find some progresses in stature estimation: (1) As the increasing average human stature, the stature estimation equation is needed to be corrected; (2) Gender, age, race and territory have a great influence to stature estimation. Thus when the study protocol designed, these influences should be considered; (3)The medical imaging technology, especially Computed Tomography, is becoming a new tool to infer stature estimation in forensic anthropology.
8.Vasculitic lesion in idiopathic IgA nephropathy

Objective To analyze the pathological and clinical characteristics of patients with idiopathic IgA nephropathy accompanied by vasculitic/crescentic lesion (IgA-V/C). Methods Data of 222 patients diagnosed as idiopathic IgA nephropathy by renal biopsy, among them 87 cases with vasculitic/crescenlic (V/C)lesion, from our department in 2004 were analyzed retrospectively. Clinical and pathological data from patients with IgA-V/C were compared to those of non-IgA-V/C patients and of lupus nephritis (LN) patients with V/C lesion. Results Vasculitic/crescentic lesion was found in 39.19% (87/222) patients with idiopathic IgA nephropathy.And about( 14.08?12.75)% of the glomeruli was affected. It should be taken into account that there was no significant differences of clinical manifestations including blood pressure, urinary protein excretion between IgA-V/C group and non-IgA-V/C group .except serum creatinine(Scr)level which was significantly higher in IgA-V/C group. In addition, only 37.9% of IgA-V/C patients presented high Scr level,thus the lesion of V/C in idiopathic IgA nephropathy was easily overlooked. Patients with idiopathic IgA nephropathy were found to have higher percentage of glomerular sclerosis (64.86% vs 40.00%) and ratio of sclerostic glomeruli to total glomeruli [ (26.98 ?24.68)% vs (16.10 ?18.80)% ]as compared to LN group, which further predicated the progressing characteristics of IgA nephropathy. Conclusions Vasculitic/crescentic lesion is a quite common finding in idiopathic IgA nephropathy and often associated with no dramatically symptoms. It is possible for vasculitic/crescentic lesion to induce unmarked lose of nephron slowly and continually, so as to accelerate IgA nephropathy progression to end-stage renal failure.
9.A study on the time-dependent expression of caspase-6 during repair of skin contusion in rats.

OBJECTIVE:
To investigate the expression of caspase-6 in rat skin contusion and its surrounding areas during repairment.
METHODS:
Immunohistochemical SP method and Western blot technique were used to study the expression and activation of caspase-6 in rat skin contusion and its surrounding areas.
RESULTS:
Weak expression of caspase-6 was detected in cytoplasms of polymorphonuchear cells (PMNs) infiltrated in the injured area at 3 hours post-contusion. The ratio of the caspase-6 positive cells was low (25.78 +/- 1.38)%. The expression of caspase-6 was increased prominently (47.70 +/- 5.14)% at 12 hours post-contusion. Almost all of the PMNs, mononuclear cells (MNCs) and fibroblastic cells (FBCs) were caspase-6 positive with both cytoplasm and nucleus staining (54.58 +/- 5.64)% on post-contusion day 3. The expression of caspase-6 decreased gradually thereafter. The expression of the 34-kDa pro-caspase-6 was detected by Western blot in both control and the post-contusion groups with time dependent dynamics.
CONCLUSION
These results suggest that caspase-6 may play a major role in trauma-induced inflammatory response. Since caspase-6 shows a timely dependent expression in PMNs, MNCs and FBCs during skin injury repair in rat, it may be used as a marker for the contusion age determination,

10.A Scale of Vocational Rehabilitation for Adult with Intellectual Disability

Objective To establish a scale of vocational rehabilitation for adult with intellectual disability.MethodsDelphi method was applied to comply the scale which was divided into four dimensions including life satisfaction, life skills, social interaction ability and personality to evaluate vocational rehabilitation for adult with intellectual disability.ResultsFor the test-retest reliability, the coefficient was 0.735 for the total scale, 0.869 for life satisfaction, 0.621 for life skills, 0.611 for social skills, and 0.505 for ability to work. The coefficien of the scale (all versions) in all dimensions and total scores were between 0.635~0.938.ConclusionThe scale has good reliability and validity to evaluate the effect of vocational rehabilitation of adult with intellectual disability.
