2.Effect of the family-centered prenatal maternal health education on delivery methods and breast feeding

Objective To explore the effect of the family-centered prenatal maternal health education on delivery modes and breastfeeding.Methods Two hundred and thirty pregnant women who accepted prenatal maternal health education were divided into the observation group and the control group according to whether their families involved in the prenatal education.There were 108 cases in the observation group and 122 in the control group.The observation group was given prenatal health education involving the pregnant women and their caregivers or their husbands.The control group was given prenatal health education according to the previously-designed course without presence of their families.The two groups were compared in delivery mode,extent of pain during production and breast feeding.Results The rates of vaginal delivery and the breast feeding at discharge in the observation group were higher than the control group(P<0.05 for both).The rate of breast swelling in 72 hours was significantly lower than the control group(P<0.01). Conclusion Family-centered prenatal maternmal health education is good for the natural birth rate,improvement of breastfeeding rate and reduction the breast swelling.

Objective To explore the clinical value of serum Ang-1,Ang-2 and IL-8 in ARDS prediction.Methods Totally 283 critically ill patients admitted in EICU of the Hunan Provincial People's Hospital from January 2012 to July 2013 were enrolled in this study and divided into the non-ARDS group (n =251) and ARDS group (n =32) depending on the development of ARDS.According to the occurrence of death in the following 60 days,the non-ARDS group and the ARDS group were further subdivided into the non-ARDS survival group,the non-ARDS death group,thc ARDS survival group and the ARDS death group.The differences in serum Ang-1,Ang-2 and IL-8 concentrations between these four groups measured by ELISA on admission were analyzed by statistical methods and ROC curve.Results The EICU stays,duration of mechanical ventilation,APACHE Ⅱ score、serum Ang-2 and IL-8 levels in the ARDS group were significantly higher than those in the non-ARDS group,while the Ang-1 level in the ARDS group was significantly lower than that in the non-ARDS group.The serum Ang-2 and IL-8 concentrations in the ARDS death group were significantly higher than those in the non-ARDS survival group and the non-ARDS death group,and the serum Ang-2 concentrations in the ARDS death group were also significantly higher than those in the ARDS survival group.Further ROC curve analysis showed that the area under the curve of Ang-2 for ARDS diagnosis and ARDS death prediction were 0.907 and 0.899 respectively and their diagnostic sensitivity and specificity were 0.969 and 0.725,0.907 and 0.882 respectively,illustrating that Ang-2 possess the best diagnostic efficiency.Conclusions Ang-2 functions as a valuable biomarker for early diagnosis and prognosis of ARDS.

5.Application of association rules with constraints in medical data analysis

Objective To explore the application of association rules with Constraints in medical data analysis.Methods Constraints were applied in mining association rules of medical data by analyzing the characteristics of the data.An effective algorithm for mining association rules with constraints from medical data called CAMD was developed.The algorithm was applied to the analysis ofcoronary heart disease dataset.Results Assigning minimum support and minimum confidence as 0.20 and 0.60,respectively,with data from 303 cases of coronary heart dieses analyzed,8 association rules were tained.Crrelations between age,sex,electrocardiograph results,pathogeny and diagnosis of coronary heart disease were discovered.Conclusion Results from our experiments show that CAMD scans the database fewer times and has no need of pruning candidate itemsets.Thus,it greatly decrease the amount of useless rules produced and rapidly discover interesting association rules of medical data.
6.Relationship between Vascular Endothelial Growth Factor Receptor-3 and Occult Metastasis in Early-Stage Tongue Cancer

Objective:To detect the expression of vascular endothelial growth factor receptor-3(VEGFR-3)in earlystage tongue cancer and the relationship with occult metastasis thereof.Methods:The expression of VEGFR-3 was examined in paraffin-embedded specimens from 62 patients with early-stage tongue cancer by immunohistochemical staining.The relationship was analyzed between the expression of VEGFR-3 and size,occult metastasis and differentiation of early-stage tongue cancer.Results:The positive expression of VEGFR-3 mainly located in the lymphatic endothelial cells of peritumoral areas and negative expression in tumor area.The higher expression of VEGFR-3 was found in patients with occult metastasis.The expression of VEGFR-3 was significantly higher in patients of Ⅱ-Ⅲ grade than that of patients in I grade carcinoma(t=4.77,P < 0.05).The expression of VEGFR-3 was higher in patients with occult metastasis than that in patients with negative lymph node(t=6.33,P < 0.05).But there were no significant correlation between the expression of VEGFR-3 and the age,gender and size of tumor in patients.Conclusion:The value of VEGFR-3 can be a useful indicator to evaluate the occult metastasis of early-stage tongue cancer.
7.Advance in the treatment of senile unstable intertrochanteric fractures

BACKGROUND:With more and more research available,there are different classification systems for intertrochanteric fracture,from Evans and Boyd-Griffin classification to Evans-Jensen and AO/OTA classification,all providing guidance for choosing an appropriate treatment scheme.OBJECTIVE:To review the advance in the treatment of senile unstable intertrochanteric fractures.METHODS:PubMed,Web of Science,WanFang and CNKI databases were searched with the keywords of intertrochanteric fracture,intramedullary nail,hip arthroplasty in English and Chinese,respectively.1 535 literatures were retrieved firstly,and finally 47 articles were included in accordance with the inclusion and exclusion criteria.RESULTS AND CONCLUSION:The therapy conception of intertrochanteric fractures has been changed from conservative traction to early surgical treatment.With the development of technology,the internal fixators have been improved continuously.Screw-plate system has been developed from Jewett plate,AO/ASIF plate to dynamic hip screw.Intramedullary nails such as Gamma,proximal femoral and proximal femoral antirotation nails have been successfully applied in clinic.Senile patients with unstable intertrochanteric fractures also obtain favorable prognosis through active internal fixation,but the high incidence of complications appears because of osteoporosis,osteogenesis deficiency and accompanied by degenerative hip diseases.Thereafter,hip arthroplasty is recommended to treat senile unstable intertrochanteric fractures so as to achieve rapid functional recovery,desired efficacy and low incidence of complications.However,it still remains controversial to choose internal fixation or hip arthroplasty.

Objective:To explore the breakthrough points of pharmaceutical care performed by clinical pharmacists in the patients after aortic dissection surgery. Methods:Using the pharmaceutical care performed by clinical pharmacists in the patients after aortic dissection surgery as the example,the breakthrough points of pharmaceutical care performed by clinical pharmacists were discussed. Results:According to the specific clinical conditions,clinical pharmacists should find out such breakthrough points of pharmaceutical care as judging the indications of heart rate and blood pressure,focusing on drug interactions and guiding the discharge medication. Conclusion:Clinical pharmacists can participate in the aortic dissection postoperative treatment and carry out individual pharmaceutical care to ensure the safety and efficacy of drugs .

10.Effects of Onodera's prognostic nutrition index(PNI)on the survival of patients with gallbladder carcinoma

Objective:To retrospectively evaluate the prognostic risk factors of patients with gallbladder carcinoma and the effect of normal prognostic nutrition index (PNI) on the survival. Methods:We retrospectively analyzed 63 patients with gallbladder carcino-ma treated in our hospital between January 2008 and December 2012. The independent prognostic factors of the patients were calculat-ed by the Cox proportional hazards model. The benefit of normal PNI status on the prognosis was further explored in the patients using survival analysis. Results:Lymph node metastasis, PNI≤50, squamous carcinoma, liver invasion and choledoch invasion were the risk factors of overall survival (OS) in the univariate analysis, while only lymph node metastasis (HR=4.495, 95%CI 2.380~8.488, P<0.001) and PNI(HR=0.195, 95%CI 0.098~0.389, P<0.001) were the dependent influential factors of the prognosis and OS in the multi-variate analysis. The effect of PNI on OS was even more statistically significant different in the younger patients (≤65y) than in the el-der (>65y) (younger group:P=0.011;elder group:P=0.078). Conclusion:The gallbladder carcinoma patients with normal PNI status (>50) have better OS compared to the patients with PNI≤50, with statistical differences between the two. And it is more significant in the patients of the younger group (≤65y), comparing with the elder group. PNI could be taken as a potential prognostic evaluation factor for the gallbladder carcinoma patients and can provide a more comprehensive evaluation of patients' conditions to clinician.
