1.Clinical thinking residents should possess in standardized training in respiratory medicine depart-ment and its training strategies

Standardized training of resident doctors is the golden stage for clinicians to establish good clinical thinking. Although respiratory disease is common, there are serious and critical diseases, which need to develop clinical thinking and improve the analytical ability. The resident physicians' instructor should help residents to establish the correct diagnostic thinking through the following means: clinical demonstration teaching show, guidance of reading the relevant literature and books, strengthening the basic skills of collecting patients' history and physical examination, and guidance of appropriate use of auxiliary examination and analysis of the results. For the treatment thinking, solid theoretical knowledge and life-long learning ideas and habits can improve the successful rate of initial treatment. Furthermore, being familiar with the efficacy evaluation system and analyzing the reasons for the success and failure of the treatment will help the residents to develop a good therapeutic thinking. The good clinical thinking established by the residents in the department of respiration will be conducive to their own follow-up specialist training and career development.
2.Comparative analysis between chest pain center management model and emergency green channel in pre-hospital treatment of patients with acute ST-segment elevation myocardial infarction

Objective:To observe the effect of chest pain center management model and emergency green channel on pre-hospital rescue rate of patients with acute ST-segment elevation myocardial infarction (STEMI).Methods:Patients with STEMI admitted in Yunnan Provincial Emergency Center between January 2017 and June 2019 were selected as the study subjects. 522 patients with STEMI sent by emergency center were included in the observation group (chest pain center management model of first-aid), while 326 patients with STEMI who came to the hospital by themselves were included in the control group (emergency green channel for first-aid). The pre-hospital and nosocomial first-aid related indexes, cardiac function (assessed by Killip grade), rescue rate, hospital stay and incidence of complications were compared between the two groups.Results:The response time of visit, time of initial electrocardiogram (ECG) completion and total time of first-aid in observation group [(1.04±0.11)min, (1.56±0.25)min, (10.63±2.26)min] were significantly shorter than those in control group [(2.82±0.26)min, (5.99±1.06)min, (18.65±2.98)min, P<0.05]. The grade of cardiac function in observation group was significantly better than that in control group ( P<0.05). Compared with the control group, the observation group had higher successful rescue rate, shorter hospital stay , lower total incidence of complications [94.25% vs 42.02%, (6.09±1.02)d vs (8.92±1.65)d, 13.01% vs 32.12%, P<0.05]. Conclusions:Compared with emergency green channel, chest pain center management model can not only shorten first-aid related time of STEMI patients, but also improve their successful rescue rate, reduce incidence of complications and improve prognosis.

4.Effect of recombinant human growth hormone on the immune function and prognosis in patients with liver neoplasms

Objective To investigate the long-term effect of recombinate human growth hormone(rhGH) on the immune function in patients with liver cancer. Methods Fifty patients with hepatoma who underwent hepatectomy were randomly divided into two groups:(1)Control group;(2)rhGH treatment(GH) group. All patients were treated with TPN for five days postoperatively,and additionally the patients in the GH group received rhGH(8U/d IH) in the evening for seven days. Results IgG,IgM,CD3,CD4 in the control group and CD8 in the GH group were significantly lower on day 10 postoperatively than that on preoperative days. IgG in the GH group significantly increased on day 15. The 1 year and 3 year survival rate and recurrent rate were not different between the two groups. Conclusions rhGH therapy can improve the immune function of patients with hepatoma who underwent hepatectomy and has no side effect on the long term survival and recurrence.
5.The relationship between extraocular muscle thickness and exophthalmos degree in patients with thyroid associated ophthalmopathy

Objective To study the relationship between extraocular muscle thickness and exophthalmos degree in patients with thyroid-associated ophthalmopathy (TAO),so as to provide a scientific basis for guiding clinical treatment.Methods Forty seven patients diagnosed with TAO from May 2005 to May 2011,in the 451 Hospital of the People's Liberation Army were chosen in this study,and divided into TAO group(23 cases) and treatment group (24 cases).Meanwhile,another 32 healthy people were selected as control.Extraocular rectus muscles (including the medial rectus muscle,lateral rectus muscle,superior rectus muscle and inferior rectus muscle) of all participants were measured with B-scan ultrasonography,and exophthalmos degree with Hertel exophthalmometer.Results The thicknesses of medial rectus in control group,TAO group and treatment group were (3.35 ± 0.25),(4.38 ± 0.85),(3.58± 0.52)mm,respectively; the thicknesses of lateral rectus muscle were (2.95 ± 0.61),(4.24 ± 0.75),(3.65 ± 0.42)mm,respectively; the thicknesses of superior rectus muscle were (3.15 ± 0.20),(4.65± 0.68),(4.01 ± 0.15)mm,respectively; the thicknesses of inferior rectus muscle were (3.17 ± 0.72),(4.75 ± 0.52),(3.71 ± 0.91)mm,respectively; the sums of binocular rectus thickness in these three groups were (21.95 ± 0.65),(28.02 ± 1.51),(22.95 ± 0.65)mm,respectively; the difference between the three groups was statistically significant(F=2.32,2.43,2.25,2.17,5.63,all P < 0.05),and TAO group was significantly higher than that in the control group and in the treatment group(all P < 0.05),while there was no statistically significant between the control group and the treatment group(all P > 0.05).The eyes exophthalmos degree in TAO group and treatment group were (16.82 ± 2.46),(14.52 ± 1.83)mm,respectively.The sum of the rectus muscle thickness and eyes exophthalmos was positively correlated(r =0.65,0.58,all P < 0.05).Conclusions Extraocular muscle thickness in patients with TAO is positively correlated with exophthalmos degree,and using B-scan ultrasonography to detect the changes is practical for clinical diagnosis and treatment.
6.Distribution and antimicrobial resistance profile of the bacterial strains from lower respiratory tract

Objective To examine the distribution and antibiotic resistance proifle of the bacterial isolates from lower respiratory tract of hospitalized patients for better empirical therapy.Methods A total of 3 155 lower respiratory tract specimens were collected from Respiratory Department of the First Afifliated Hospital of Chongqing Medical University in 2013 to identify bacterial pathogens for susceptibility testing. The results were compared with the data of 2011 and 2012.Results The top three bacterial species from lower respiratory tract in the period from 2011 to 2013 wereA. baumannii,P. aeruginosa, andK. pneumoniae. The prevalence ofS. aureusincreased, while the prevalence ofS. maltophilia andS. pneumoniae was still low.A. baumannii isolates were highly resistant to antibiotics. Not more than 40% of these strains were susceptible to the antimicrobial agents except minocycline, which was active against more than 60% of the ICU and non-ICU strains. More than 70% of theP. aeruginosa strains from non-ICU were susceptible to cefepime, meropenem, imipenem, levofloxacin, and ciprofloxacin in the three years, 50%-70% of the strains from ICU were susceptible to the antimicrobial agents tested. Carbapenem-resistantA. baumannii andP. aeruginosa strains were identiifed.S. maltophilia strains were relatively susceptible to minocycline and levolfoxacin. All the S. aureus isolates were susceptible to vancomycin, linezolid, and tigecycline. The prevalence of MRSA increased year by
year. More than 80% of theK. pneumoniae andE. coli strains were susceptible to imipenem, piperacillin-tazobactam, and cefepime. ESBLs-producingK. pneumoniae was identified. More than 85% of theH. influenzae strains were susceptible to meropenem, ceftazidime, cefuroxime, ciprolfoxacin, levolfoxacin, and amoxicillin-clavulanate potassium. All theS. pneumoniae isolates were susceptible to vancomycin, and 80%-96% of these strains were susceptible to levofloxacin and penicillin.ConclusionsA. baumannii,P. aeruginosa, andK. pneumoniae are the top three bacterial species isolated from lower respiratory tract. Carbapenem-resistantA. baumannii,P. aeruginosa and ESBLs-producingK. pneumoniaeare emerging. The prevalence of MRSA is increasing year by year, which poses a challenge to antimicrobial therapy. It is urgent to contain and control the growing antibiotic resistance.
7.Totally laparoscopic associating liver partition and portal vein ligation for staged hepatectomy for the treatment of cirrhotic hepatocellular carcinoma

Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) is a breakthrough in the field of hepatobiliary surgery.The remnant liver regeneration was stimulated during the first stage of the operation,and the radical resection of the tumor could be successfully carried out during the second stage of the operation.ALPPS is a new approach for patients with tumor which is previously considered unresectable during one hospital stay.In April 2014,a patient with hepatocellular carcinoma (HCC) in the right liver and liver cirrhosis was admitted to the Southwest Hospital.Preoperative examination confirmed that the ratio of the remnant liver volume to the standard liver volume was 26.9%,which indicated that the patient was inappropriate to receive radical resection of HCC.Therefore,totally laparoscopic ALPPS was applied.In the first stage of the operation,the portal vein ligation associated with liver hanging tape placement and in situ splitting of liver were carried out after hanging the Glisson's pedicle under the laparoscope.Thirteen days after the first stage of the operation,the ratio of the remnant liver volume to the standard liver volume was 40.6%.The second stage of the operation was carried out 14 days after the first stage of the operation.The right hepatic pedicle and right hepatic vein were transected with a stapler.The tumor was removed after full mobilization of the right liver.The distance between the resection margin to the tumor was 1.5 cm.No complications were detected after the first stage of the operation,while the patient was complicated with pleural effusion after the second stage of the operation and was cured by pleural puncture and drainage.The patient was discharged at postoperative day 9 and followed up at postoperative month 1.The results of follow-up confirmed that the hepatic function was normal,and no mass occupying lesions and pleural effusion were detected.Totally laparoscopic ALPPS is safe and feasible with satisfactory short-term efficacy.
8.Status and prospect of argon-helium cryosurgery system for the treatment of gliomas

Glioma is one of the most refractory tumors affecting the central nervous system. As a new technique, cryoablation has been considered for the treatment of glioma. Considering previous studies, we summarized the mechanism of cryoablation to treat glioma based on pathology and molecular biology perspectives. We also analyzed the indications, effects, and problems of cryoablation in clinical practice. Furthermore, we proposed that this technique may be used in future trends and applications. The mechanism of eradicating tumor cells by cryoablation involves the following:(1) using an ultralow temperature to induce necrosis and apoptosis;(2) changing the local microcirculation of tumors;and (3) adjusting anti-tumor immunity. As an alternative surgical treatment, cryoablation could improve the therapeutic effects by real-time medical imaging. Cryoablation combined with chemotherapy, radiotherapy, immunotherapy, and other methods could provide a more effective treatment for gliomas. Therefore, cryotherapy could be a possible treatment for glioma and become a sustained research field.
9.Effect of nerve invasion on long term survival of middle and distal cholangiocarcinoma

Objective To analyze the long-term survival of patients with middle and distal cholangiocarcinoma,and to explore the influence of nerve invasion on the survival time.Methods We retrospectively analyzed the clinical data of 161 patients with middle or distal cholangiocarcinoma in General Hospital of Fushun Mining Bureau between January 2012 and June 2016.All the patients were divided into nerve group and control group according to the nerve invasion.And then the perioperative and prognosis data of the two groups were analyzed and compared.Results In our research,all patients were successfully completed surgery,the incidence of neural invasion was 47.2% (76/161),more common of the nerve invasion in distal cholangiocarcinoma and phase Ⅱ cholangiocarcinoma.The perioperative mortality rate was 3.7% (6/161),and the incidence of complications was 27.3% (44/161).The 1 years,2 years and 3 years survival rates of middle and distal cholangiocarcinoma were 78.7%,42.1%,20.0%,the average survival time was(23.8 ±1.2) months.The 1 years,2 years and 3 years survival rates of nerve group and control group were 67.1%,29.8%,9.4% and 89.0%,52.6%,26.8%,the average survival time were (18.6 ± 1.4) months and (27.3 ± 1.6) months.The prognosis of nerve group was significantly poor than the control group (P < 0.05).Conclusions Nerve invasion is a common way of invasion and metastasis of cholangiocarcinoma.And it has a poor prognosis in cholangiocarcinoma patients with nerve invasion.
10.Effect of peroral endoscopic myotomy on esophageal manometry in patients with achalasia

ObjectiveTo determine the effect of peroral endoscopic myotomy (POEM) on esophageal manometry in patients with achalasia.MethodsEsophageal manometric data of 3 patients with achalasin before and after POEM was retrospectively analyzed.ResultsThe post-POEM lower esophageal sphincter pressure (LESP) and upper esophageal sphincter residual pressure (UESRP) were significantly lower than those of before ( P < 0.05 ).The average post-POEM lower esophageal sphincter residual pressure (LESRP) decreased (P >0.05),with significant decrease to normal level in 2 patients and almost unchanged (above normal) in the other.The esophageal aperistalsis remained after POEM in all 3 patients.The lower esophageal sphincter relaxing rate (LESRR) remained lower than normal after POEM as before.The average upper esophgeal sphincter pressure (UESP) decreased after POEM without statistical significance.ConclusionPOEM can improve the outcomes of esophageal manometry in patients with achalasia in the short term.
