2.Surgical Site Infection after Open Cholecystectomy:A Prospective Study

OBJECTIVE To identify the possible risk factors for the development of surgical site infection (SSI). METHODS A total of 218 consecutive patients who received open cholecystectomy due to gallbladder disease and stone of common bile duct during from the Jun to Dec in 2007 were included in the study. The potential risk factors including clinical features,biochemical data,operative types and incision types were analyzed by univariate analysis. RESULTS The overall incidence of SSI was 5.04%.The incidence of SSI in cholecystectomy alone group was lower than in cholecystectomy with exploration of common bile duct group (10.9% vs 3.1%,P=0.022).The incidence of SSI in emergency group was higher than that in selective operation group (12.5% vs 3.8%,P=0.037). The incidence of SSI among patients with white blood cell count more than 10.0?109 befove surgery was higher (12.2% vs 3.0%,P=0.025). The incidence was 1.5%,6.1% and 26.3%,respectively,for patients with Ⅱ,Ⅲ and Ⅳ types incision (P=0.000). CONCLUSIONS These findings indicate that risk factors for the development of SSI after open cholecystectomy include operation manner,operation type,incision type and preoperative leucocyte count.
3.Correlation of diffusion tensor imaging between the cerebral cortex and speech discrimination in presbycusis.

OBJECTIVE:
To investigate the relationship between pure-tone average (PTA), the fractional anisotropy (FA) of the auditory pathway, cognitive cortex and auditory cortex in presbycusis.
METHOD:
Twenty-five elderly subjects with presbycusis were participated in the study. PTA, speech discrimination abilities were evaluated in each subject. Diffusion tensor imaging (DTI) was applied to access the FA of the IC, the superior frontal gyrus and the Heschl's gyrus. Compare the difference between two sides of the values of FA in the three areas. Bivariate correlation analysis was performed to evaluate the effects of PTA and FA of the inferior colliculus (IC), the superior frontal gyrus and the Heschl's gyrus on speech discrimination abilities.
RESULT:
There were no significant differences between the left and right side of the inferior colliculus (P > 0.05). Higher FA values were recorded at the left side of the Heschl's gyrus and the superior frontal gyrus (P < 0.05). Both PTA and the FA of the superior frontal gyrus have a negative association with speech discrimination abilities (P < 0.01, P < 0.05), while the FA of the Heschl's gyrus has a positive association with speech discrimination abilities (P < 0.05).
CONCLUSION
Our findings indicated that the speech discrimination abilities of the elderly is not only related to the peripheral auditory function, but also to the central auditory and cognitive function.

Sleep is a common physiological phenomenon of animals.We may have different kinds of sleep disorders in our daily life,such as difficulty falling asleep,insomnia,hypersomnia and changes of sleep patterns,etc.Pain is a subjective response of the body to stimuli of certain degrees.Sleep disorders are closely related to pain:pain can interfere with people's normal physiology and psychology conditions and affect individual's sleep;different kinds of sleep disorders can affect the body' s sensitivity to pain.The relationship between pain and sleep disorders has become one of the most common causality in clinical research.In this paper we summarizes the pathophysiological relation and mutual influence between pain and sleep disorders.
5.Change and significance of concentration of blood phosphorus and N-terminal pro-brain natriuretic peptide in patients with severe acute organophosphorns pesticide poisoning

ObjectiveTo observe the change of blood phosphorus and N-terminal pro-brain natriuretic peptide(NT-proBNP) in patients with severe acute organophosphorus pesticide poisoning (AOPP)and explore their clinical significance.MethodsSeventy-eight severe AOPP patients were selected and divided into dead group (54 cases) and survival group (24 cases).NT-proBNP and blood phosphorus were examined when the patients were just hospitalized,and after 1,2,4 d and when they were turn out from ICU or before the patients dead.ResultsComparing with survival group,blood phosphorus and NT-proBNP of dead group had no significant difference when the patients were just hospitalized(P> 0.05 ).After 1,2 and 4d NT-proBNP of dead group [ ( 1986.5 ± 24.9),(3568.2 ± 56.9),(7829.0 ± 64.3 ) ng/L ]was higher than that of survival group [ ( 1068.4 ± 20.2),(986.6 ± 16.4),(943.7 ± 14.6) ng/L,P < 0.05 or < 0.01 ],while blood phosphorus was significant lower than that in survival group [ ( 1.22 ± 0.13 ),(0.81 ± 0.10),(0.58 ±0.07 ) mmol/L vs.( 1.53 ± 0.16),( 1.48 ± 0.13 ),( 1.46 ± 0.14 ) mmol/L,P < 0.05 or < 0.01 ].NT-proBNP of dead group increased and blood phosphorusreduced gradually by time extending (P < 0.05).NT-proBNP was lower when patients were turn out from ICU than that when the patients were just hospitalized [ (327.5 ±12.3) ng/L vs. (1023.3 ± 18.8) ng/L,P < 0.05].Blood phosphorus had no difference at all time points in survival group (P > 0.05 ).NT-proBNP was higher [ ( 31 486.5 ± 120.7) ng/L vs.(327.5 ± 12.3) ng/L,P <0.01 ]and blood phosphorus was lower [ (0.24 ± 0.03 ) mmol/L vs.( 1.57 ± 0.15 ) mmol/L,P < 0.01 ]before death in dead group compared with those at the time turning out from ICU in survival group.Conclusions The value of NT-proBNP increased and blood phosphorus reduced gradually with disease progression.NT-proBNP and blood phosphorus were important factors reflecting the prognosis of blood AOPP patients.
6.Survey on the needle stick injuries and occupational protective education of nursing students in dif-ferent stages of clinical practice

Objective To investigate the incidence of needle stick injuries of nursing students in different stages of clinical practice and occupational protective education to provide evidence for developing education strategies. Methods One hundred and forty-one nursing students were surveyed retrospectively by a self-designed questionnaire. Results A total of 75.9%(107/141) nursing students had been injured in clinical practice. The incidence of needle stick injuries varied in different stages of clinical practice. Compared with the middle and late stage of clinical practice, the incidence of needle stick injuries was highest in the early stage of clinical practice:53.9%(76/141) vs. 38.3%(54/141),7.8%(11/141),and there was significant difference, χ2=216.14, P<0.05.About 64.2%(88/141)-78.0%(110/141) nursing students had not received protective education on the needle stick injuries. Only 48.9%(69/141)-55.3%(78/141) of nursing students were given protective training on needle stick injuries in their practice hospitals. Before clinical practice, 63.1%(89/141) nursing students had not been vaccinated to prevent infective diseases. Conclusions To reduce the incidence of needle stick injuries and related potential risks of infection caused by injuries, it is necessary to strengthen pre-practice education on occupational protection among nursing students. Perfect supervision system should also be established in practice hospitals and clinic wards.
7.Distribution characteristics of rs10975521 A/T and rs1929992 A/G polymorphism of IL-33 gene in Guangxi population

Objective: To study and explore the distribution characteristics of rs10975521A/T and rs1929992A/G polymorphism of the IL-33 gene in Chinese Guangxi population. To compare the frequency distribution differences of allele and genotype of two sites among different ethnic. Methods:The polymorphism of rs10975521A/T and rs1929992A/G of IL-33 gene in 283 subjects were analyzed with the methods of Single base extension (PCR-SEB) and DNA sequencing,and the distribution frequency and the differences between groups of that were analyzed statisticaly. Results:Three genotypes of AA,AT and TT were found in rs10975521A/T with the frequency distribution of 12. 7%,53. 0% and 34. 3% respectively and there was no significant difference between sexes of each genotype and allele frequency in the Guangxi population ( P>0. 05 ) . There were significant differences of the allele frequency of rs10975521A/T in the Guangxi population compared with that in the European (P< 0. 05), han Chinese in Bejing (P< 0. 05) and Japanese people (P< 0. 01). Three genotypes of AA,AG and GG were found in rs1929992A/G with the frequency distribution of 23. 9%,53. 7% and 13. 4% respectively and there was no significant difference between sexes of each genotype and allele frequency in the Guangxi population(P> 0. 05),but the differences of genotype frequency of rs1929992A/G was statistically significant compared with that in the European,han Chinese in Bejing and Japanese people ( P< 0. 05 ) . There were significant differences of the allele frequency between Guangxi population and European (P< 0. 01),han Chinese in Bejing(P< 0. 05). Conclusion: There are different degrees of discrepancy of rs10975521A/T and rs1929992A/G polymorphism of IL-33 gene among different race and region.
8.Ultrasonographic diagnosis of dermatofibrosarcoma protuberans

Objective To explore the ultrasonographic appearance of dermatofibrosarcoma protuberans. Methods Clinical data, sonographic features and color Doppler flow imaging characteristics of 12 patients surgically and pathologically confirmed dermatofibrosarcoma protuberans were retrospectively analyzed.Results Single mass was detected in 11 patients (11/12, 91.67%) with the diameter ranged 0.9-8.0 cm. The tumors distributed in the trunk (8/12, 66.67%), proximal limbs (2/12, 16.67%) or face and neck (2/12, 16.67%). Most of the tumors (10/12, 83.33%) developed from skin and subcutaneous fat layer of the trunk and limbs and were hypoechoic and heterogeneous (11/12, 91.67%), some (8/12,66.67%) with distinct boundary and regular shape without liquification, calcification nor regional lymph node metastasis. Affluent color Doppler flow signals of tumor were found in 11 patients (91.67%).Conclusion Ultrasonographic characteristics of dermatofibrosarcoma protuberans include distinct boundary, regular shape, heterogeneous hypoechogenicity and rich color Doppler flow signals without regional lymph node metastasis.
9.Effect of Electroacupuncture on the INS Level and CCK Content in a Rat Model of DGP

Objective To investigate the effect of electroacupuncture on serum insulin (INS) levels and gastric antral cholecystokinin (CCK) content in rats with diabetic gastroparesis (DGP).Methods Sixty SD rats were randomized into groups A, B, C, D and E, 12 rats each. Group A is a normal control. A DGP model was made by an intraperitoneal injection of 2% streptozocin (STZ) plus an irregular high sugar and fat diet in groups B, C, D and E. group B is a model one and did not receive treatment. Group C received electroacupuncture at points Zusanli, Sanyinjiao and Liangmen; group D, electroacupuncture at the control points of Zusanli, Sanyinjiao and Liangmen. Group C, an oral gavage of metoclopramide tablet solution. Blood sugar and urine sugar were determined using a OneTouch lood glucose meter and Tes-Tape, respectively. The gastric emptying rate and the intestinal migration rate were measured by an oral gavage of phenol red. Serum INS levels and gastric antral CCK content were measured by ELISA in every group of rats before and after treatment.Results There were statistically significant differences in blood and urine sugar values between group B, C, D or E rats and group A (P<0.01) and between group C rats and group B or E (P<0.05). There was a statistically significant difference in the gastric emptying rate between group B, C, D or E rats and group A (P<0.01) and between group C rats and group B or D (P<0.01,P<0.05). There was a statistically significant difference in the intestinal migration rate between group B or D rats and group A (P<0.01) and between group C rats and group B or D (P<0.01). There were statistically significant differences in serum INS levels and gastric antral CCK content between groups B and A rats (P<0.05,P<0.01). There was a statistically significant difference in gastric antral CCK content between groups D and A rats (P<0.01). There were statistically significant differences in serum INS levels and gastric antral CCK content between groups C or E rats and group B (P<0.05,P<0.01). There was a statistically significant difference in gastric antral CCK content between groups C and D rats (P<0.05).Conclusions Electroacupuncture can markedly improve gastrointestinal functions and promote gastric emptying in DGP rats. Its therapeutic effect may be related to electroacupuncture raising serum INS levels and reducing gastric antral CCK content.
10.Clinical application and value of dual-source spiral CT enhanced scan and multiplanar reformatting in diagnosis of gastric cancer

Objective To discuss the clinical application and value of dual-source spiral CT enhanced scan and multiplanar reformatting ( MPR) in the diagnosis of gastric cancer. Methods The 335 patients with gastric cancer who conducted dual-source spiral CT enhanced scan of upper abdomen under the hypotonic state before the definite pathological diagnosis were retrospectively analyzed,and had multi-di-mensional multiplanar reconstruction for the enhanced thin-section CT data by the multiplanar reformatting technique. Results Of 335 ca-ses,288 cases were detected by simple axial data,the detection rate was 86. 0%,320 cases were detected by MPR,the detection rate was 95. 5%,the differences were statistically significant (P<0. 05). The overall accuracy rate of Preoperative T staging by conventional axial and MPR images were 78. 3% and 89. 1% respectively, the difference was statistically significant. The overall accuracy rate of preoperative N staging by two methods were 73. 6% and 82. 3%,respectively,the difference was not significant. Conclusion Application of dual source spiral CT multiplanar reconstruction can significantly improve the detection rate of gastric cancer,and the average accuracy rate of preopera-tive TNM staging for gastric cancer is 84. 6%.
