Objective:To investigate the changes of intestinal flora and serum amino acid levels in elderly patients with Parkinson's disease(PD)at different stages, and to provide evidence for diagnosis, prevention and treatment of PD.Methods:A total of 140 elderly PD patients diagnosed and treated in our hospital from January 2018 to August 2019 were enrolled and divided into PD course <5 years group(n=70)and PD course≥5 years group(n=70). Ninety subjects without PD confirmed by medical examination in our hospital were included as the control group.Real-time fluorescence quantitative PCR of 16S rDNA gene was used to detect levels of Bifidobacterium, Lactobacillus, Clostridium prasus, Enterococcus, Enterobacter, Prevotella copri and Akkermansia muciniphila bacteria in the gut, and the high-performance liquid chromatography was used to detect the serum amino acids in the three groups. Results:Compared with the control group, the levels of Bifidobacterium, Lactobacillus, Clostridium prasalis and Prevotella copri in the intestines were decreased and Enterobacter level was increased in PD course <5 years group and PD course≥5 year group( F=20.863, 32.251, 23.166, 24.683 and 10.136, all P<0.001). The serum levels of methionine, tryptophan, lysine and glutamate in PD course <5 years and PD course≥5 year group were decreased as compared with the control group( F=5.858, 5.877, 4.183 and 25.462, all P<0.05). Spearman correlation analysis showed that serum glutamate level was positively correlated with Clostridium prastia in the intestine( r=0.647, P=0.002)and negatively correlated with Prevotella copri( r=-0.559, P=0.010). Multivariate Logistic regression analysis showed that the protective factors in the intestines of elderly PD patients were Bifidobacterium( OR=0.186, 95% CI: 0.054-0.637, P=0.007), Lactobacillus( OR=0.283, 95% CI: 0.098-0.816, P=0.020), Clostridium prastia( OR=0.232, 95% CI: 0.063-0.851, P=0.028), and Prevotella copri( OR=0.222, 95% CI: 0.058-0.851, P=0.028), and the risk factor was Enterobacter( OR=5.119, 95% CI: 1.406-18.636, P=0.013). Conclusions:The decreases of Bifidobacterium, Lactobacillus, Clostridium, Prevotella copri and Enterobacter levels in the intestinal tract of PD patients, and the decrease of serum glutamate level may be correlated to the progression of PD.

OBJECTIVETo compare the clinical outcomes of anterior cervical decompression plus sublevel fusion and posterior cervical laminoplasty in treating multilevel cervical spondylotic myelopathy.
METHODSThe clinical data of 56 patients with multilevel cervical spondylotic myelopathy were retrospectively analyzed from July 2009 to June 2012. There were 32 males and 24 females, aged from 42 to 79 years old with an average of (56.9 +/- 12.8) years. All patients had the typical clinical features of cervical spondylotic myelopathy, radiological evidences, and courses of disease were from 2 months to 16 years with an average of (10.6 +/- 3.2)years. Of them,34 patients were treated with anterior cervical decompression plus sublevel fusion (anterior fusion group) and 22 patients with posterior cervical laminoplasty (posterior laminoplasty group). JOA score and radiological data were used to evaluate the clinical results:
RESULTSNo complications about nerve and blood vessel was found and the patients were followed up from 24 to 36 months with an average of 28.6 months. In anterior fusion group, the cervical anterior column height was significantly increased and the anterior cervical curvature angle was significantly decreased at 2 weeks after surgery (P < 0.05). In posterior laminoplasty group, there was no significant difference in above items between preoperative and postoperative at 2 weeks, final follow-up. Postoperative at 2 weeks and final follow-up, there was significant difference in anterior cervical curvature angle between two groups (P<0.05). Postoperative JOA score had obviously improved in all patients, at 3 months after operation and final follow-up, anterior fusion group was better than that of posterior laminoplasty group (P < .05).
CONCLUSIONThe anterior sublevel fusion can effectively restore cervical anterior column height, and compared with the posterior cervical laminoplasty, it can obviously improve the spinal cord function. It is an effective method for the multilevel cervical spondylotic myelopathv.

AIM:To observe the expression of Toll like receptor 4 (TLR4) in the left ventricle of Goldblatt rats and to explore the role and mechanism of TLR4 in left ventricular remodeling of hypertension.METHODS:Goldblatt model of Two-kidney,one-clip (2K1 C) renovascular hypertension was induced in twenty-five rats(H group),and twenty rats served the sham-operated group(sham group). The tail cuff blood pressure was detected every week and echocardiogram was observed every other week. After eight weeks of operation, the rats were killed and the samples of the left ventricle were collected. The concentration of Ang Ⅱ in left ventricle was assessed by tadioimmunoassay.Western blotting and RT-PCR were used to exam the mRNA and protein expression of TLR4 in the left ventricle.Immunohistochemistry was adopted to exam the location of TLR4 in the myocardium.RESULTS:TLR4 mRNA and protein expression were consistently upregulated in the left ventricle of H group compared with sham group. In H group, predominantly sarcolemmal staining was observed, especially focal areas of intense TLR4 staining were found in juxtaposed regions of two or more adjacent myocytes; However,in sham group,TLR4 expression was diffuse and presumably cytoplasmic.Considerable correlation was found between blood pressure,MESS,LVMI,RWT,the concentration of Ang Ⅱ in left ventricle and the protein expression of TLR4 in myoeytes.CONCLUSION:During the development of left ventricular remodeling of Goldblatt rats, expression of TLR4 increases significantly.Enhanced expression of TLR4 locates in the sarcolemma, especially in juxtaposed regions of two or more adjacent myocytes. These indicate that TLR4 transmembrane receptor which is closely relative with inflammation and immunity probably contributes to the development of ventricular remodeling.[KEY WORDS]Hypertension;Ventricular remodeling;Receptors,Toll-like;Goldblatt rats

Objective To study the angiogenesis in serous ovarian neoplasms. Methods Immunohistochemical method was used to examine microvessei density (MVD) and the expressions of vascular endothelial growth factor (VEGF) in 76 cases of serous ovarian neoplasm. The relationship between MVD and VEGF were investigated. Results MVD in the tissues of cystadenoma, bordline cystadenoma and cystadenocarcinoma were 7.8±2.4, 17.5±3.3 and 27.6±7.5; VEGF expressions were (16.1±3.9)%, (36.7±14.2)%, (52.3±22.8)%; the differences were significant respectively. Of cystadenocarcinoma grade 1, grade 2 and grade 3, MVD were 18.3+3.4, 24.2±3.1, and 32.3±6.4, VEGF expressions were (30.9±16.2)%, (48.0±18.4) % and (62.1±21.3) %, the differences were significant respectively. Of FIGO stage Ⅲ~Ⅳ and stage Ⅰ~Ⅱ, MVD were 28.9±4.7 and 22.0±4.7, VEGF expressions were (55.1±22.9) % and (40.18±18.0) %, the differences were significant respectively. There was significant positive correlation between VEGF expression and MVD. Conclusion Angiogenesis is very important in the development of serous ovarian neoplasms.

Objective To investigate the expression profile and biological function of lncRNAs in urothelial cancer stem cells and explore whether they can be biomarkers for prediction of clinical characteristics for bladder cancer patients. Methods Microarray analysis was performed to identify differentially expressed lncRNAs in cancer stem cells and parental cancer cells.Expression profiles were validated by Coding Potential Caculator analysis,real-time polymerase chain reaction.By performing in vitro sphere formation assays,J82 cells with lentivirus-based knockdown of lncRNA-BCSC (bladder cancer stem cells) were used to confirmed its function.Samples were obtained from patients who underwent TURBT between January 2009 and December 2012.All tissues were initially confirmed by pathologists. The association of the clinicpathologic of bladder cancer and lncRNA-BCSC expression was analyzed by x2 analysis. Results 750 lncRNAs were highly expressed from microarray analysis in bladder cancer stem cells.Among these,lncRNA-BCSC was identified as potentially maintaining self-renewal of cancer stem cells.Knockdown of this transcript in J82 cells inhibited spheroid formation.lncRNA-BCSC expression were higher in 54.8% (57/104) bladder cancer tissues. Moreover, using x2 analysis,lncRNA-BCSC expression in primary tumors was found to be a predictor for recurrence following transurethral resection in patients with nonmuscle-invasive bladder cancer (P =0.009).Conclusions Our study provides strong evidence that lncRNA-BCSC are indispensable modulators of self-renewal ability of bladder cancer stem cells.Overexpression of lncRNA-BCSC may have a predictive value for early recurrence in patients suffering from nonmuscle-invasive bladder cancer.
