1.Diffusion Tensor Imaging Study on Cervical Spinal Cord Injury without Radiographic Abnormality

Objective To apply MR diffusion tensor imaging (DTI) to quantitatively analyze cervical spinal cord injury without radiographic abnormality (CSCIWORA). Methods 15 patients with CSCIWORA and 20 healthy controls were scanned with MRI of conventional scans and DTI. The fractional anisotropy (FA) and apparent diffusion coefficient (ADC) were measured. Results FA and ADC of the patients were (0.475±0.109) and (1.438±0.252)×10-3 mm2/s, respectively. Whereas, they were (0.604±0.096) and (1.371±0.280)×10-3 mm2/s in the controls. Compared with the controls, the FA was less (P<0.05) in the patients, but the ADC was not significantly different (P=0.267). The fiber tracking (FT) showed the abnormality of white matter fiber tracts of cervical spinal cord in the patients. Conclusion DTI can detect the CSCIWORA, and FT can directly display the injuries of white matter fiber tracts of cervical spinal cord, which provide more information to evaluate the clinical severity of CSCIWORA.
2.Application of infrahyoid myocutaneous flap in the reconstruction for early stage mouth floor carcinoma

ObjectiveTo evaluate the clinical value of infrahyoid myocutaneous flap in the reconstruction of mouth floor tissue defects. MethodsInfrahyoid myocutaneous flap was made including stenohyoid,thyrohyoid,omohyoid and the affiliated skin based on the axis of superior thyroid artery,vein and ansa cervicalis. The clinical data of 11 patients with reconstruction of mouth floor tissue defects using infrahyoid myocutaneous flap (maximum area was 8 cm × 3 cm) from Match 1993 to June 2009 were retrospectively reviewed.ResultsSeven infrahyoid myocutaneous flaps which reserved platysma branch of superior thyroid artery were all alive, while 2 patients presented epidermal necrosis of 4 un-protected patients. The follow-up period was range from 24 months to 195 months, one patient received salvage surgery for local regional lymphatic recurrence in level Ⅰ,which survived without diseases until now.The rest cases had turned out a success.No dysphagia,dysphonia,and tongue extension difficulty was reported in any patient.ConclusionInfrahyoid myocutaneous flap is one of the valuable donor sites for the reconstruction of mouth floor tissue defects after radical oncologic operations,especially for those with early stage carcinoma of the mouth floor.
3.In vitro inhibition of carnosic acid against influenza A virus infections

Carnosic acid (CA) is the main phenolic diterpenoid active ingredient in plants such as rosemary and sage, and has antiviral, antioxidant, anti-inflammatory effects and so on, however, its antiviral activity against influenza virus infections was not reported. In this study, antiviral activities against influenza A virus infections of three main bioactive ingredients from rosemary, including rosmarinic acid, CA and ursolic acid, were evaluated using virus titer titration assay, and CA showed remarkable inhibition on influenza H5N1 replication in A549 cells. The antiviral activity of CA was further confirmed and its mechanism of action was investigated using the indirect immunofluorescence assay (IFA), Western blot and real-time fluorescence quantification polymerase chain reaction (qRT-PCR). The results showed that the 50% effective concentration (EC50) of CA against influenza H5N1 in A549 cells and MDCK cells were 4.30 and 3.64 μmol·L-1, respectively. Meanwhile, CA also showed inhibition on influenza virus 2009panH1N1 (EC50: 10.1 μmol·L-1) and H3N2 (EC50: 12.8 μmol·L-1) replications in A549 cells. Mechanistic studies showed that antiviral activity of CA is related to its induction of heme oxygenase-1 (HO-1) in A549 cells and suppression on production of reactive oxygen in H5N1-infected cells.
4.Effects of Electroacupuncture at Baihui and Shenting Acupoints on Learning and Memory Ability and Expression of 5-hydroxytryptamine 1A Receptor in Rats with Cerebral Ischemia-Reperfusion

Objective To explore the effect of electroacupuncture at Baihui (DU20) and Shenting (DU24) acupoints on learning and memory ability in rats with middle cerebral artery occlusion and reperfusion (MCAO/R), and the potential mechanisms. Methods A total of 90 healthy male Sprague-Dawley rats were randomly divided into sham group (n=18) and operation group (n=72). The MCAO/R model was established by suture method in the operation group. Finally, 54 qualified rats of the operation group were randomly divided into ischemia group (n=18), electroacupuncture group (n=18) and non-acupoint group (n=18). The electroacupuncture group received electroacupuncture at Baihui (DU20) and Shenting (DU24) for 14 days. The cerebral infarction volume was measured by magnetic resonance imaging (MRI). The learning and memory ability was tested by Morris water maze. The protein expression of 5-hydroxytryptamine 1A (5-HT1A) receptor was detected by immunohistochemistry. Results There was no significant difference in the cerebral infarction volume among three groups before intervention (F=1.678, P>0.05). Compared with the ischemia group and the non-acupoint group, the cerebral infarction volume signif-icantly reduced (P<0.001);the latency significantly shortened (P<0.001) and the times crossing the platform decreased (P<0.05);the expres-sion of 5-HT1A receptor decreased in the left hippocampus (P<0.05) in the electroacupuncture group after intervention. There was no signifi-cant difference in all the indices between the non-acupoint group and the ischemia group after intervention (P>0.05). Conclusion Electroacu-puncture at Baihui (DU20) and Shenting (DU24) could effectively increase the learning and memory ability of MCAO/R rats, which might relate with inhibiting the expression of 5-HT1A receptor in hippocampus.
5.Effect between felodipine plus irbesartan and felodipine plus metoprolol regimen on the sexual function in young and middle-aged women with hypertension

Objective To compare the effects between felodipine plus irbesartan and felodipine plus metoprolol regimen on blood pressure and the sexual function in young and middle-aged hypertensive women. Methods In this prospective, randomized, parallelized, controlled and fixed combined therapy trial, 99 female patients(aged 18 to 60)with grade 1 and grade 2 hypertension(BP ≥ 140/90 mm Hg and < 179/109 mm Hg, 1 mm Hg =0.133 kPa)were assigned to felodipine 5 mg q.d + irbesartan 150 mg q.d(F + I group, n = 49)and felodipine 5 mg q. d + metoprolol 47.5 mg q. d(F + M group, n = 50)group. Target blood pressure was < 140/90 mm Hg. The female sexual function index(FSFI)questionnaire, levels of serum estradiol and testosterone were assessed. Female sexual dysfunction was defined as a FSFI score of less than 25.5. Patients were followed up for 24 weeks. Results The rate of achieving blood pressure goal between 2 groups was similar at the 4th, 8th, 12th and 24th weeks respectively (42.9% vs. 62.0% at 4th week, 89.8% vs. 90.0% at 8th week, 93.9% vs. 94.0% at 12th week,98.0% vs. 96.0% at 24th week, P > 0.05). Compared to baseline, scores for the items related to"desire" and "arousal" were significantly improved(P < 0.05), the level of the serum estradiol was significantly elevated[(50.3 ± 37.4)pg/L vs.(54.4 ± 10.8)pg/L before menopause,(18.4 ± 2.9)pg/L vs.(20.2 ±3.1)pg/L after menopause, P <0.05]and the level of the serum testosterone was significantly decreased[(722.8 ± 277.1)ng/L vs.(650.0 ± 156.0)ng/L before menopause,(841.2 ± 279.3)ng/L vs.(761.9 ± 197.8)ng/L after menopause, P < 0.05]in the F + I group, while scores for the items related to" sexual desire" and "lubrication" were statistically reduced(P < 0.01), the concentration of the serum estradiol was significantly reduced[(57.4 ± 9.7)pg/L vs.(51.1 ± 12.1)pg/L before menopause,(19.8±2.3)pg/L vs.(17.8 ± 3.3)pg/L after menopause, P <0.01]and the level of the serum testosterone was significantly increased[(775.6 ± 217.8)ng/L vs.(886.0 ± 186.4)ng/L before menopause,(812.5 ± 311.3)ng/L vs.(914.4 ± 300.2)ng/L after menopause, P<0.01]in the F+M group. FSFI score was negatively correlated with age and systolic blood pressure levels. Conclusion felodipine plus irbesartan or metoprolol for 24 weeks equally reduced blood pressure and the former regimen is superior to the latter on sexual function improvement in this patient cohort.
6.The molecular epidemiologic investigation of hantavirus carried by rodent hosts in Wenzhou, Zhejiang province

Objective To study the epidemiological features of hantavirus in rodents in Wenzhou,Zhejiang province.Methods Rodents were captured in Wenzhou,where hemorrhagic fever with renal syndrome (HFRS) had been endemic. Hantavirus antigens in the rat lungs were detected by immunofluorescence assay (IFA).Partial S segment (nt 620-999) and partial M segment (nt 2001-2301)sequences were amplified by RT-PCR,and then sequenced.Neighbor-joining method was used to construct for phylogenetic analysis.Results A total of 96 rodents were trapped in the epidemic areas,and 6 hantavirus antigens were identified from these lung samples (6.3%).Partial S and partial M segment sequences were successfully recovered from 5 samples and determined. Phylogenetic analysis of these sequences indicated that all viruses belonged to Seoul virus (SEOV),regardless of the sources (Rattus norvegicus,Rattus tanezumi and Rattus rattoide) that they were derived. However,the clustering pattern in the partial S-tree was different from that in the partial M-tree,suggesting that the re-assortment between SEOVs had occurred.Conclusion All Rattus rats carried SEOV in Wenzhou and the genetic reassortment with SEOV had occurred naturally.
7.Evaluation of carotid atherosclerotic plaques by vascular plaque quantification (VPQ) technology of three-dimensional ultrasonography.

OBJECTIVE:
To analyze the feasibility of using vascular plaque quantification (VPQ) to evaluate carotid atherosclerotic plaques and to observe the effect of statins on carotid atherosclerotic plaques.
METHODS:
Patients with carotid plaques from January 2016 to September 2018 in Peking University First Hospital Neurology Department were recruited and underwent three-dimonsional ultrasound (3DUS). Their gray scale median (GSM) and other parameters of carotid plaques were measured with VPQ. The patients were divided into low GSM group (GSM < 40) and high GSM group (GSM≥40). The clinical characteristics and plaque characteristics of the patients in the two groups were compared to analyze the stability of plaques. According to whether taking statins or not, the patients were further divided into statin group and non-statin group, plaque GSM and other parameters of their carotid plaques were measured and the changes of carotid plaques at the end of 3 months and 2 years were observed.
RESULTS:
A total of 120 patients were enrolled, including 79 males and 41 females, with an average age of (65.39±9.11) years. The patients were divided into low GSM group (31 cases) (GSM < 40) and high GSM group (89 cases) (GSM≥40). The stenosis of the lumen in the low GSM group was more severe (the area stenosis rate was 41.32%±21.37% vs. 29.79%±17.16%, P < 0.05). The nor-malized wall index (NWI) of plaque in low GSM group was significantly higher than that in high GSM group (0.61 ±0.14 vs. 0.52±0.12, P < 0.01). A total of 77 patients, including 51 males and 26 females, aged (64.96±9.58) years, were enrolled to observe the statin effects on carotid plaque. They were divided into statin group (n=56) and non-statin group (n=21) according to whether taking statins or not. At the baseline and 3-month follow-up, there were no significant differences in carotid plaque volume, area, degree of luminal stenosis and GSM between the two groups (P>0.05). At the end of the 2-year follow-up, GSM increased in the statin group [median 10.00 (2.00, 28.00)] but decreased in the non-statin group [median -7.00 (-11.00, 5.50)], with a statistically significant difference between the two groups (P < 0.01). There was no significant increase in carotid plaque volume in the statin group, while there was a slight increase in the non-statin group, but there was no significant difference between the two groups [median increase in plaque volume was 0.00 (-30.00, 40.00) mm3 in the statin group and 30.00 (10.00, 70.00) mm3 in the non-statin group, P>0.05].
CONCLUSION
The VPQ technology of 3DUS can be used to evaluate carotid atherosclerotic plaques. Patients with low GSM (GSM < 40) have more severe vascular stenosis and higher normalized wall index. VPQ technology can also be used to observe the effect of statins on carotid plaque, the GSM of plaques increase in patients who are taking moderate-intensity statin treatment for two years.

8.Chemical constituents from the methanol fraction of Premna fulva leaves and their anti-inflammatory activities

AIM To study the chemical constituents from the methanol fraction of Premna fulva Craib leaves and their anti-inflammatory activities.METHODS The methanol fraction from P.fluva were isolated and purified by TLC,column chromatography,and HSCCC,then the structures of obtained compounds were identified by physicochemical properties and spectral data.Their anti-inflammatory activities in vitro were evaluated by RAW264.7 model.RESULTS Twelve compounds were isolated and identified as vitexin(1),balanophonin(2),inotodisaccharide(3),4-allyl-2,6-dimethoxyphenol-β-D-glucopyranoside(4),dehydrovomifoliol(5),loliolide(6),(E)-4-((1S,3R,4R)-1-hydroxy-4,5,5-trimethyl-7-oxabicyclo[4.1.0]heptan-1-yl)but-1-en-3-o-ne(7),(E)-4-hydroxyphenylprop-7-ene 4-O-β-D-glucopyranoside(8),4-O-β-D-glucopyranosylbenzoic acid(9),vicenin(10),oleanicacid(11),sesamin(12),respectively.Compounds 1,2,5,7,10,and 12 showed good inhibitory activities against NO,and the IC50 values were(26.42±2.5)、(21.24±2.2)、(25.88±1.9)、(29.72±2.1)、(8.90±1.1)、(9.73±0.7)μmol/L,respectively.CONCLUSION Compounds 2,4-8 are isolated from Premna genus plants for the first time.Compounds 1,2,5,7,10,12 have anti-inflammatory activities.
9.Observation on therapeutic effect of " acupuncture" on premature ovarian insufficiency of kidney deficiency.

OBJECTIVE:
To observe the clinical efficacy of " acupuncture" and oral estradiol and dydrogesterone tablets (femoston) on premature ovarian insufficiency of kidney deficiency.
METHODS:
A total of 50 patients with premature ovarian insufficiency of kidney deficiency were randomized into an observation group and a control group, 25 cases in each one.In the observation group, " acupuncture" was applied at Baihui (GV 20), Zhongwan (CV 12), Guanyuan (CV 4), Qihai (CV 6), Zhongji (CV 3), Yaoyangguan (GV 3), Yaoshu (GV 2), Mingmen (GV 4), etc. once every 2 days, 1 month as a course. In the control group, femoston was prescribed for oral administration, one tablet per time, once a day, 1 month as a course. Both of the two groups were given consecutive treatment for 3 courses. Before and after treatment, the clinical symptoms, menstrual improvement as well as the changes of estradiol (E), luteotrophic hormone (LH) and follicle-stimulating hormone (FSH) in serum were observed in the two groups.
RESULTS:
After treatment, the clinical symptoms and menstrual conditions were improved (<0.01), the levels of FSH and LH were significantly reduced (<0.01), and the levels of E were significantly increased in the two groups (<0.01). There were no significant difference in menstrual improvement rate and menstrual improvement time between the observation group and the control group (<0.05), the recurrence rate of menopause and clinical symptom score improvement in the observation group were superior to the control group (<0.05). In the observation group, the level of E in serum was lower and the levels of FSH and LH in serum were significantly lower than those in the control group (<0.05, <0.01). In the observation group, the rate of adverse reaction was 4.0% (1/25), which was lower than 36.0% (9/25) in the control group (<0.05).
CONCLUSION
" acupuncture" has better therapeutic effect for premature ovarian insufficiency of kidney deficiency. It is superior to femoston in improving clinical symptoms and recurrence rate of menopause as well as reducing the levels of FSH and LH.
