1.in vitro Modulation of the Invasive and Metastatic Potentials of Human Hepatocellular Carcinoma by Interlukin-4

Objective: To investigate the effects of Interlukin- 4(IL-4) on the invasiveness and the expression of several cell surface antigens related to invasive and metastatic potentials of human hepatocellular carcinoma QGY-7701 cell line in vitro. Methods: QGY-7701 cells were incubated with high concentration of IL-4 or low concentration of IL-4 in different time. The expression of ICAM-1, CD44 and HLA-I was determined by fluorescence-activated cell sorter (FACS) analysis, the tumor cell binding affinity to extracellular matrix (ECM) components was measured by cell attachment assay, the degree of homotypic aggregation was quantified by cell aggregation assay. Results: IL-4 pretreatment can enhance the expression of ICAM-1 and HLA-I, suppress the expression of CD44 on hepatocellular carcinoma cell line and decrease the binding affinity to ECM components and the degree of homotypic aggregation of hepatocellular carcinoma cells. Conclusoin: IL-4 can inhibit the invasive and metastatic potentials of hepatocellular carcinoma cells.
2.Experimental Study on the Effects of Immediate Administration of Inderal on Postinfarction Left Ventricular Remodeling of the Rabbit

The effects of immediate administration of inderal following acute myocardial infarction on left ventricular remodeling were studied in acute myocardial infarction model of the rabbit. The results showed that there was no significant difference in the infarct size and expansion index between the infarct-inderal group and the infarct-placebo group. As compared with sham surgery-inderal group,infarct-inderal group had a higher ventricular weight,left ventricular volume index and left ventricular wall stress [(2.65±0.17) g/kg vs (2.32±0.11) g/kg,P<0.05;(2.22±0.23) ml/kg vs (1.70±0.12) ml/kg,P<0.001;(0.84±0.07) vs (0.75±0.052),P<0.05,respectively],but there was no significant difference between the infarct-inderal group and infarct-placebo group. Heart rate in the infarct-inderal group was significantly decreased as compared with that of infarct-placebo group [(153±6.36) min-1 vs (225±4.08) min-1,P<0.001],but there was no significant difference in left ventricular end-diastolic pressure,+dp/dt max and cardiac index between the two groups. It was suggested that immediate administration of inderal postinfarction had no preventive effect on left ventricular remodeling.
3.Pregnancy outcome in women with lupus nephritis

Objective To study the outcome of pregnancy with lupus nephritis. Methods The clinical data of 9 pregnancies with lupus nephritis were retrospectively malyzed. Results Hypertension,proteinuria,injured renal function and abnormal serological marker were complicated with nine patients with lupus nephritis in pregnancy. The outcome of pregnancy was as following:abortion 2 cases,stillbirth 1 case,premature delivery 4 cases,term delivery 2 cases. All neonate survived without malformation. There was no postpartum hemorrhage or death. Patients with active SLE had very high fetal mortality( 100% ). And SLE got worsen in 50% of them. However,fetal mortality rate in pa-tients with controlled or quiescent SLE was 11.1% ,which was much lower than it in other patients(P<0.05). Con-clusion Lupus nephritis should be controlled before pregnancy. Pregnancy outcome could be improved by careful multidisciplinary monitoring and proper usage of adrenal cortical hormone.

Bronchogenic cysts( BC) is a congenital malformation of the lung which is common in chil-dren and adolescents. Because the disease is rare and not characteristic in symptom,sign and imaging,it can be easily misdiagnosed. It is still difficult to improve the rate of correct and early diagnosis. This article reviewes the research progress of the occurrence,symptom and management of BC.
5.Effect of the family-centered prenatal maternal health education on delivery methods and breast feeding

Objective To explore the effect of the family-centered prenatal maternal health education on delivery modes and breastfeeding.Methods Two hundred and thirty pregnant women who accepted prenatal maternal health education were divided into the observation group and the control group according to whether their families involved in the prenatal education.There were 108 cases in the observation group and 122 in the control group.The observation group was given prenatal health education involving the pregnant women and their caregivers or their husbands.The control group was given prenatal health education according to the previously-designed course without presence of their families.The two groups were compared in delivery mode,extent of pain during production and breast feeding.Results The rates of vaginal delivery and the breast feeding at discharge in the observation group were higher than the control group(P<0.05 for both).The rate of breast swelling in 72 hours was significantly lower than the control group(P<0.01). Conclusion Family-centered prenatal maternmal health education is good for the natural birth rate,improvement of breastfeeding rate and reduction the breast swelling.

7.Effect of Roxithromycin on Airway Inflammation in Patients with Chronic Obstructive Pulmonary Disease at Stable Phase

Objective:To investigate the effect of roxithromycin on airway inflammation in chronic obstructive pulmonary disease ( COPD) patients at stable phase to provide evidence for the clinical diagnosis and treatment .Methods:Totally 46 cases of outpatients with stable COPD were divided into the observation group and the control group .The control group received the conventional treatment , while the observation group was treated with roxithromycin capsules 150mg.d-1 additionally, and the treatment course was 10 months. The sputum, serum cytokines γ-IFN, IL-8 and TNF-αlevels, sputum leukocyte , neutrophil count and adverse reactions were observed and compared between the groups .Results:After the 5-month treatment, the levels of γ-IFN, IL-8 and TNF-αcytokines in serum and sputum in the observation group were significantly lower than those before the treatment (P<0.05) and in the control group (P<0.05).After the 10-month treatment, the above indices in the observation were further decreased when compared with those after the 5-month treatment (P<0.05), which were notably lower than those in the control group (P<0.05).There was only one case of ad-verse drug reactions in the observation group , and the patient was out of the study .Conclusion:Long-term use of roxithromycin at low dose can reduce cytokines in serum and sputum and inflammatory cell count in sputum in stable COPD patients , which exhibits the effect on airway inflammation to some extent .
8.Clinical efficacy and prognosis of nimotuzumab combined with intensity modulated radiotherapy for advanced local nasopharyngeal carcinoma

Objective To observe the clinical efficacy and prognosis of nimotuzumab combined with intensity modulated radiotherapy(IMRT) in the treatment of advanced local nasopharyngeal carcinoma.Methods 82 patients with advanced nasopharyngeal carcinoma were selected, and they were randomly divided into observation group(40 cases) and control group(42 cases) according to the digital table.The observation group was treated with nimotuzumab plus IMRT plus cisplatin, and the control group was treated with IMRT plus cisplatin.After 7 weeks of treatment, the clinical efficacy, adverse reactions and T cell subsets were observed.And the patients were followed up for 1 year, 2 years and 3 years to evaluate the survival rate.Results In the observation group, the total remission rate was 75.00%,which was higher than that in the control group(59.09%),the difference was statistically significant(x2=35.81,P<0.05).The CD+4,CD+8 and CD+4/CD+8 levels in the observation group were similar to those in the control group(tCD+4=0.59,P=0.56;tCD+8=1.70,P=0.01;tCD+4/CD+8=0.13,P=0.89).The incidence rates of Ⅰ degree and Ⅱ degree mucosal reaction and dermatitis in the observation group were 35.00%,40.00%,40.00%,35.00%,respectively,which were higher than 9.09%,9.09%,9.09%,9.09% of the control group,the differences were statistically significant(x2Ⅰmucous membrane=4.18,P=0.04;x2Ⅱmucous membrane=5.52,P=0.02;x2Ⅰdermatitis=5.52,P=0.02;x2Ⅱdermatitis=4.18,P=0.04).The 2-year and 3-year survival rates in the observation group were 65.50%,60.00%,respectively,which were significantly higher than 31.82%,27.27% in the control group, the differences were statistically significant(x2=4.11,P=0.04;x2=4.58,P=0.03).Conclusion Nimotuzumab combined with IMRT in the treatment of advanced nasopharyngeal carcinoma has good short-term clinical efficacy, good long-term efficacy and controllable adverse reactions.It is worthy of clinical application.

We have designed a new integrative assessing scheme which chiefly evaluates the practicalskills, that is: clinical skills account for 50%, oral test of professional knowledge accounts for 20% ,case analysis 20%,medical history writing 5%, and medical ethics and work discipline 5%. Through the nearly two years' implement,theproject greatly accelerates the cultivation of clinical practice abilities in medical students, enormously promotes thestudents' initiative and consciousness in clinical practice training, and roundly improves the teaching quality of clinicalpractice.
10.Reverse-flow axial skin flap of the delay by minimal invasive surgery

Objective The mechanism of delaying flap with the minimal invasive surgery was approached to understand its effects on the whole delayed survival of skin flaps thoroughly so as to provide the rationale for its clinical use.Methods 80 male or female Wistar rats were randomly subdivided into two groups:delayed by the minimal invasive surgery,and immediately transfer without delay.Flaps in each group delayed were cut at 1,2,3 and 4 weeks,respectively.The flap was designed at the lower back of rat,with the size of 5 cm × 1 cm,crossing the middle area for 2.5 cm and including stem of iliac branch from iliolumbar artery.2 weeks after second operation,the survival area,capillary density and content of lactic acid of the flaps in each group were examined,and the survival of the falps delayed by the minimal invasive surgery was compared.Results The longer delaying time,and the higher survival rate were observed in the experimental group.Delaying for 3 weeks and 4 weeks,the survival rate was (86.13 ±1.13) %,(93.49 ± 1.15) %,respectively,in the experimental group.While in control group,the survival rate was no more than 63%.The longer delaying time,the higher the capillary density were noted in two groups,but 3 weeks delayed group equally matched to the 4 weeks delayed group.In the experimental group,the content of lactic acid increased peaked in 1 week delayed group,then fell-down gradually,but kept steady in 3 and 4 weeks delayed group.The content of lactic acid in the control group kept steady.Conclusions The experimental model is selected as cross-area axial flap on the lower back of rats.The minimal invasive surgery plays the same role as in delaying flaps,which causes vasoconstriction,resulting in disorder of internal environment,ischemia and hypoxia,finally vasodilatation.The more ramus anastomosis,the more survival rate of the flap.
