1.Effect of simvastatin on pulse pressure and blood uric acid in aged primary hypertension patients

Objective To discuss the relationship between pulse pressure and blood uric acid and the effect of simvastatin on pulse pressure and blood uric acid in aged primary hypertension (PH) patients with normal cholesterol.Methods Eighty-five patients with PH were divided into two groups randomly,routine group (using drugs only to lower blood pressure) and simvastatin group (simvastatin was added).Six months later,blood uric acid and pulse pressure was measured again.Results With the development of hypertension,there was statistical difference on pulse pressure and blood uric acid between hypertension grade 3 and grade 1 or grade 2 (P < 0.05).With the increase of pulse pressure,the trend of blood uric acid increase was distinct,and blood uric acid level was positively related with pulse pressure by correlation analyses(r = 0.87,P< 0.05).The pulse pressure and blood uric acid was decreased after treatment compared with that before treatment in simvastatin group (P < 0.05),but there was no significant difference before and dter treatment in routine group (P >0.05).There was significant difference after treatment between two groups (P< 0.05).Conclusions With the increasing of pulse pressure,there is a significant raise in blood uric acid.Blood uric acid is positively related with pulse pressure by correlation analyses.Simvastatin is a drug used to lower cholesterol,and it could also be used to reduce pulse pressure and blood uric acid in aged PH patients with normal cholesterol.
2.Experimental study of zinc finger protein A20 inhibiting oxidized low density lipoprotein induced migration in smooth muscle cell

Objective To investigate the possible mechanisms of ox-LDL on vascular smooth muscle cells (VSMC) with and without exogenous zinc finger protein A20 gene. Methods VSMC were trans-fected with plamid containing A20 gene and incubated with ox-LDL. TLR4 and LOX-1 mRNA expression were measured by RT-PCR. Results After VSMC incubated with ox-LDL, the expression of LOX-1 and TLR-4 mRNA reached a higher level. However, when VSMC were transfected with plasmid containing A20 gene, the NF-kB nuclear translocation and the ability of migration reduced to normal level. Conclusions ox-LDL can significantly activate NF-kB signaling system and enhance migration in VSMC. The activation may induce the inflammatory response in arterial wall, the migration of VSMC to the intima, and triggering the process of atherosclerosis. Transfecting plasmid containing A20 gene significantly inhibited the activation of NF-kB and the ability of migration of VSMC. A20 effects might be through inhibiting NF-kB signaling. Meanwhile, A20 blocked the development of atherosclerosis.

Objective To investigate the expression of nucleotide-binding oligomerization domain 2(NOD2),an intracelluar pathogen pattern recognition receptor,and Toll like receptor(TLR) 2,4 in human vascular smooth muscle cells(VSMC),and its effect on production of proinflammatory cytokines in VSMC.Methods Human coronary artery smooth muscle cells were in vitro stimulated with NOD2 agonist Muramyl dipeptide(MDP),TLR2 agonist Pam3CSK4(PAM3)and TLR4 agonist lipopolysaccharides(LPS) alone or MDP in cocultured with either PAM3 or LPS.The mRNA expression of NOD2 and fibroblast growth factor-2(FGF-2) were measured by real time RT-PCR.The concentration in the culture supernatants of interleukin-8(IL-8) and tumor necrosis factor-?(TNF-?) was determined by ELISA.VSMC proliferation was analyzed by the MTT assay.Results MDP up-regulate the expression of NOD2 mRNA in VSMC in a time-dependent manner(0 h:0.028?0.001;3 h:0.045?0.002;6 h:0.053?0.002;24 h:0.162?0.013).It up-regulate the expression of FGF-2 mRNA(MDP 9.3?0.4 vs control 7.4?0.2;P
4.Effect of family constellation therapy on the spouses of female tumor patients

Objective To explore the effect of family constellation therapy on the spouses of female tumor patients . Methods Two hundred and two spouses of 202 female tumor patients were divided to the control group ( n = 43 ) and experiment group according to their wills(n=159):the former were intervened with health education and skills training and the latter trained in a workshop with family constellation by psychology training specialists in addition to health intervention and skill training intervention. After the intervention, the two groups were compared in terms of quality of life, family function and marriage status. Result The total scores of the experiment group on quality of life , social relationship and environment were all higher than those of the control group (all P<0.05) and the experiment group got higher score in trusting their spouses than the control group (P<0.05). Conclusion The family constellation can improve female tumor patients'quality of life and promote the trust in their spouses.
5.Feasibility evaluation of pedicle screw fixation for repairing thoracolumbar fracture via fractured vertebrae

BACKGROUND:Traditionaly repairing single vertebral thoracolumbar fracture often uses of cross-vertebral 4 screw fixation. Because of “paralelogram effect” and the “suspension effect”, it is prone to have internal fixation failure, reset height and correction angle loss, and other complications. However, these complications can be avoided or reduced by fractured vertebra pedicle screw fixation. OBJECTIVE:To investigate the feasibility evaluation of pedicle screw fixation for repairing thoracolumbar fracturevia fractured vertebrae. METHODS:A total of 23 patients with single segment thoracolumbar fractures treated at the First Hospital of Lanzhou University between January 2013 and June 2014 were retrospectively analyzed. The fractured segments were between T11 to L2, the patient’s average age was 47.8 years old. Al the patients received the posterior pedicle screws and adjacent and up and down six pedicle screw internal fixation within 10 days after injury. The repair effect was observed, including short-term postoperative complications, vertebral height ratio, Cobb angle and the recovery of spinal neurological function. RESULTS AND CONCLUSION:Twenty-three cases were folowed up for 4 to 15 months. Al patients had no severe complications in the short term (4 to 15 months) after treatment. There was no screw rod breakage or internal fixation loosening at the last folow-up. The vertebral height ratios and Cobb angle which were measured in postoperative and final folow-up were significantly improved compared with the preoperative data (P < 0.05). The two indicators had a slight rebound in the last folow-up compared with that after treatment, but the difference was not significant (P> 0.05). The spinal neurological functions of most patients were partialy recovered at the last folow-up. These results suggest that pedicle screw embedding fixation repairing thoracolumbar fractureviafractured vertebrae can effectively restore vertebral height and correct kyphosis, and maintain correction effect. The advantages are obvious with a low occurrence rate of pedicle screw fracture.

7.Research of ankle chute tibia bone graft fusion in the treatment of late kaschin-beck disease

Objective To observe the clinical curative effect of ankle chute tibia bone graft fusion treatment of late kaschin-beck disease.Methods Thirty-three patients with late kaschin-beek disease performed ankle arthrodesis,the ankle surgery in 5 cases,simple right ankle surgery in 13 eases and 15 cases of left ankle surgery.Intraoperative used tibial chute hollow screw internal fixation and bone graft plaster external fixation postoperatively.Followed up 0.5-6.0 years,compared the American orthopaedic food and ankle society (AOFAS) score before surgery to determine the efficacy of last follow-up,X ray film fusion situation assessment in the postoperative period.Results Thirty-three cases did not occur postoperative incision infection,vascular or nerve complications.Last follow-up,30 cases were no obvious discomfort,3 cases of mild ankle pain,accompanied by walking or standing for a long time after the foot heel had mild acid bilges feeling,did not affect the daily life.The AOFAS score before surgery was significantly lower than that last follow-up [(25.84 ± 16.81) scores vs.(87.32 ± 4.68) scores] (P < 0.01).Clinical efficacy excellent in 25 cases,good in 5 cases,general in 2 cases,poor in 1 case.Follow-up X line piece was showed that in all cases to achieve bony fusion.Conclusion Using sliding kashin-beck terminal patients treated ankle bone graft fusion,simple operation,high bone graft fusion rate,less complications,curative effect is positive.
8.Role of nucleotide binding oligomerization domain receptor 1 as a new innate immune recognition receptor in activation of vascular smooth muscle cells

AIM: Nucleotide binding oligomerization domain receptor 1 (NOD1) is a recently identified intracellular pathogen pattern recognition receptor of innate immunity. This study was designed to explore the role of NOD1-mediated innate immune signal pathway in the activation of vascular smooth muscle cells (VSMCs), and investigate the effect of peptidoglycan (PGN) on the activation and expression of NOD1 in human VSMCs. METHODS: The experiment was performed in the central laboratory of the Second Affiliated Hospital of Dalian Medical University from June 2006 to March 2007.①Subject: Human coronary artery VSMCs were purchased from Cambrex company.② Methods: Human coronary artery VSMCs were cultured in vitro, and stimulated with NOD1 agonist PGN (10 mg/L) for 0, 3, 6 and 24 hours. ③ Evaluation: The mRNA expression of NOD1 in VSMCs was measured by real time quantitative reverse transcription-polymerase chain reaction. NODs are cytosolic proteins that contain a nucleotide-binding oligomerization domain(NOD).These intracellular proteins are a class of pattern recognition receptors with unique functions in the innate and the acquired immune systems.NOD like receptors,including NOD1 and NOD2,are associated with host responses to intracellular invasion by bacteria or the intracellular presence of specific bacterial products.Activation of NOD like receptors initiates proinflammatory signalling via NF-?B activation,which is necessary for clearance of infecting pathogens from the host.Several different mutations in the genes encoding NOD1 and NOD2 are associated with susceptibility to inflammatory disorders.
10.Relationship between low-grade albuminuria and unfavourable cardiac geometric adaptations and left ventricular diastolic dysfunction in essential hypertensive patients

Objective To investigate the relationship between low-grade albuminuria and unfavourable cardiac geometric adaptations and left ventricular diastolic dysfunction in non-treated essential hypertensive(EH ) patients. Methods Three hundred and one patients of EH were divided into three groups acording to morning urine albumin concentration ( MU AC ): normoalbuminuria (NAU) group ( 107 cases,MUAC < 10 mg/L) ;low-gradealbuminuria (LGAU) group ( 127 cases, MUAC 10-30 mg/L); microalbuminuria (MAU) group (67 cases, MUAC > 30 mg/L). The blood pressure,body height and weight was measured for all the patients. The serum creatinine (SCr),blood urea nitrogen (BUN), uric acid (UA),fasting plasma glucose (FBG), plasma glucose 2 hours after oral glucose load (2hPG), total cholesterol (TC), triglyceride (TG) and low density lipoprotein cholesterol (LDL-C) were detected by automatic biochemical analyzer.Echocardiogram exarmination was performed to detect left ventricular end-diastolic diameter (LVEDD), left ventricular end-diastolic posterior wall thickness (LVPWT) and interventricular septum thickness at end-diastole (IVST). Color Doppler ultrasound was used to measure indicators of left ventricular diastolic function, including peak E velocity of mitral diastolic flow (E) and peak A velocity of mitral diastolic flow ( A ), calculated E/A, measured the deceleration time of peak E ( DT ), and isovolumic relaxation time (IV RT ).Left ventricular mass index (LVMI) and relative wall thickness(RWT) were calculated by formula, and LVMI,RWT,left ventricular configuration diastolic dysfunction was compared. Results The level of MUAC had positive relationship with LVMI (r = 0.43 ,P < 0.01 ). Compared with that in NAU group, IVST, LVPWT,RWT, LVMI increased (P < 0.01 ), the ratio of left ventricular normal configuration decreased (P < 0.01 ), the incidence of concentric reconstitution, concentric hypertrophy increased (P < 0.05 ), A increased (P < 0.05 ),and E/A decreased (P < 0.01 ) in LGAU group. The total incidence of left ventricular diastolic dysfunction was 28.6%(86/301 ),12.1%(13/107),33.9%(43/127) and 44.8%(30/67)in NAU group,LGAU group and MAU group. The incidencerates of left ventricular diastolic dysfunction in LGAU group and MAU group were higher than that in NAU group (P < 0.01 ). Conclusion LGAU is positively related to incidence of unfavourable cardiac geometric adaptations and left ventricular diastolic dysfunction in EH patients.
