1.Effect of eradication of Helicobacter pylori with quadruple therapy on posthepatitic cirrhosis accompanied with peptic ulcer

Objective To explore the clinical characteristics of posthepatitic cirrhosis accompanied with peptic ulcer (called as hepatogenic ulcer, HU) and the effect of quadruple therapy in this event Methods A total of 105 cases of PU were subjected in this study, and 126 patients with simple peptic ulcer (PU) served as control Endoscopy was carried out to observe the peptic ulcer, bleeding, and healing after treatment Urease testing was employed to detect Helicobacter pylori (Hp) in gastric mucosa All patients were followed up for 2 years Results Most cases of PU were confirmed to have duodenal ulcer The positive rate of Hp was 72 4% in HU group before treatment Ulcer healing rates of quadruple therapy were 65 7% and 88 5% respectively in HU group and PU group During the follow up, the rates of recurrence and rebleeding were 68 4% and 30 2% in HU group, and 48 9% and 16 6% in PU group,with very significant difference between them ( P
2.Reconstructing coracoclavicular ligament in treating Rockwood - III Acromioclavicular dislocation by palmaris longus muscle with polyester suture.

OBJECTIVETo explore therapeutic effects of reconstructing coracoclavicular ligament for the treatment of Rockwood-III Acromioclavicular dislocation by palmaris longus muscle with polyester suture.
METHODSFrom August 2011 to November 2013,37 cases with Rockwood-III acromioclavicular dislocation were treated with reconstructing coracoclavicular ligament by palmaris longus muscle with polyester suture. Among patients, 24 were males and 13 were females, ranging the age from 19 to 46 years old, with an average of 32 years old. There were 11 cases on the left side and 26 cases on the right side. Twenty-nine cases were fresh dislocation and 8 cases were old dislocation. Blood loss, operative time were observed, and Karlsson evaltae standard were applied for assessing postoperative recovery of shoulder joint function.
RESULTSAll patients were followed up from 2.5 to 5 months with an average of 3.5 months. Operative time ranged from 52 to 98 (meaned 72) min, blood loss ranged from 50 to 180 (meaned 75) ml. All operative incision were healed at the satge I . According to Karlsson standard, 32 cases obtained excellent results and 5 cases were moderate.
CONCLUSIONFor Rockwood-III acromioclavicular dislocation,reconstructing coracoclavicular ligament by palmaris longus muscle with polyester suture has advantages of simple operation, and rapid recovery of shoulder joint function.

3.Comparison of Therapeutic Efficacy of Different Administration Routes of Ambroxol in the Treatment of Pediatric Respiratory Disease

OBJECTIVE:To investigate the optimal administration route of ambroxol in the treatment of pediatric respiratory disease. METHODS:Totally 120 children with respiratory disease in pediatric department of our hospital during Jun. 2014-Jun. 2016 were divided into intravenous dripping group and atomization inhalation group according to even and odd-numbered admission order,with 60 cases in each group. Intravenous drip group was given Ambroxol hydrochloride injection 7.5 mg dissolved in 5%glucose solution 50 mL,ivgtt,bid;aerosol inhalation group was given aerosol inhalation of Ambroxol hydrochloride injection 7.5 mg,for 15 min each time,bid. The two groups were treated with 7 d. Clinical efficacies,p(O2)and p(CO2)level,the times of sputum absorption,clinical indexes and the occurrence of ADR were compared between 2 groups. RESULTS:Total response rate of atomization inhalation group(96.67%)was significantly higher than intravenous dripping group(78.33%);p(O2)level was sig-nificantly higher than intravenous dripping group,while the times of sputum absorption,fever disappearance time,asthma disap-pearance time,oxygen therapy time,pulmonary rales disappearance time,cough disappearance time and average hospitalization time were significantly less or shorter intravenous dripping group,with statistical significance (P<0.05). There was no statistical significance in p(CO2) level and the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Ambroxol is effective in the treatment of pediatric respiratory disease,and clinical efficacy of atomization inhalation is better than intravenous drop.
4.Comparative study on the effects of suspension and pillow elevation on limb swelling after elbow surgery

Objective To compare the clinic effects of suspension and pillow elevation on limb swelling after elbow surgery. Methods One hundred patients receiving operations on elbow fracture were randomly divided into experiment group(suspension elevation)and the control group(pillow elevation)to eliminate limb swelling,50 cases in each group.The forearm circumference(10 cm below elbow)were measured 1-5d after operation,the circumferences on the first day and the fifth day recorded for comparing effects of the limb swelling elimination.Results The circumferences on the 1st day and 5th day of the suspension elevation group and the pillow elevation group were(2?53±0?76)cm,and(4?19±2?36)cm,respectively.The differences in circumference between the two groups were statistically significant(P<0?05).Conclusion The suspension elevation is more effective in promoting venous circulation and eliminating swelling,so it is conducive for the elbow fracture patients in early functional exercise?

The microRNA let-7 family,functioning as a tumor suppressor,has emerged as a regulator of stem cell differentiation and tumor repression.The RNA-binding protein lin28 is a stem cell pluripotency factor,which contributes to the maintenance of stem cell characteristics and promoting cell malignant transformation.lin28 can block the maturity of let-7 by combining with the precursors of let-7 family miRNAs,which is itself able to repress lin28 by binding to the 3'-UTR of lin28 transcripts,thus forming a feedback loop.The double-negative feedback loop comprising lin28 and let-7 is involved in several biological processes such as differentiation of stem cell,tumorigenesis,invasion and metastasis,drug resistance and relapse,which couldbe a potential target for cancer therapy.
6.Analysis of antibiotic resistance genes in Neisseria gonorrhoeae strains with decreased sensitivity to ceftriaxone from Shenzhen city

Objective To analyze the relationship of penA, ponA, porB and mtrR gene mutations with the reduced sensitivity to ceftriaxone in N.gonorrhoeae isolates from Shenzhen city.Methods A total of 296 clinical isolates of N.gonorrhoeae were collected in Shenzhen city from 2009 to 2011.The agar dilution method was used to estimate the sensitivity of these N.gonorrhoeae to ceftriaxone.Totally, 53 strains with reduced sensitivity to ceftriaxone (minimum inhibitory concentration (MIC): 0.06-0.50 μg/ml) were identified, and 53 strains with high sensitivity to ceftriaxone were randomly selected from the remaining strains and served as the control group.PCR was performed to amplify the penA, ponA,porB and mtrR genes from the 106 isolates followed DNA sequencing.Results The mosaic structure of the penicillinbinding protein 2 (PBP2) gene (penA gene) was found in only one isolate with a ceftriaxone MIC of 0.125 0 μg/ml.Amino acid sequence analysis of the remaining 105 isolates yielded 16 different amino acid patterns.The MICs of ceftriaxone were relatively high (0.062 5 μg/ml) in N.gonorrhoeae strains harboring the amino acid patterns ⅩⅢ, ⅩⅧ or ⅩⅩⅩⅧ,but relatively low (0.008 0 μg/ml) in those harboring the amino acid pattern Ⅱ.No significant differences were observed in the frequency of mtrR, porB or ponA gene mutations between N.gonorrhoeae isolates with reduced sensitivity to ceftriaxone and those with high sensitivity (all P > 0.05).Conclusions The mosaic structure of PBP2 may be not the primary reason for reduced sensitivity of Neisseria gonorrhoeae to ceftriaxone in Shenzhen, while different amino acid patterns produced by various mutations in amino acid residues at positions 500-580 in the non-mosaic PBP2, together with mtrR, porB and ponA mutations, may play more important roles in the reduced sensitivity.
7.Evaluation of operative risk and suvival of colorectal cancer patients 80 years of age and older

Objective To estimate the operative risk and outcomes of colorectal cancer patients 80 years of age and older. Methods Colorectal cancer resection was performed in 99 patients 80 years of age and older between Dec. 1987 and June 2005. The informations about clinical data, co-morbidity,complications, operative mortality and survival were retrospectively analyzed. The patients were followed-up for 45.12 months (range 1-136) months. Results Of 99 patients, co-morbidity was found in 43 patients (43.4% ) and complication in 10 patients (10.1%). No patient died of operation.Eighty patients completed the follow-up study. The overall 3-year survival rate and disease-free survival rate were 64.3% and 61.1%, respectively. Whereas the overall 5-year survival rate and disease-free survival rate were 52.8% and 52.1%, respectively. In univariate analysis, curative or palliative operation, tumor differentiation, cancer embolism in the vasculature, tumor staging and complications were proved to be significant prognostic factors. Multivariate survival analysis,however, showed that only the curative or palliative operation was independent factor for survival.Conclusions The high risk of co-morbidity for patients 80 years of age and older is not the obstacle to cancer resection.These patients will have satisfactory outcomes via optimal treatment and operation.
8.Analysis of 378 Cases of Pediatric ADR Report in Our Hospital

OBJECTIVE:To investigate the characteristics and regularity of pediatric adverse drug reactions(ADR)in our hos-pital in order to promote rational drug use of children. METHODS:In retrospective study,378 pediatric ADR cases collected from our hospital during 2010-2014 were analyzed statistically. RESULTS:Among 378 pediatric ADR cases,255 cases were male (67.46%),and 123 cases were female (32.54%);the incidence of ADR in patients aged from one year to three years were the highest (27.51%);most of the cases were induced by intravenous injection,accounting for 87.57%(331 cases);sterile powder for injection was main dosage form (236 cases,62.43%) anti-infective agents were the major cause of ADR in respect of drug types (50.26%),among which cephalothin ranked the first place(43.68%);the lesion of skin and its appendants were the most common clinical manifestation of ADR(56.35%),such as erythra and pruritus. And most of the cases were cured(64.29%)or im-proved(34.66%)after treatment. CONCLUSIONS:The occurrence of ADR are related to multiple factors,such as administration route and drug varieties. Great importance should be attached to indication,route of adiminstration,method,speed and so on,in order to promote rational drug use and reduce the occurrence of ADR.
10.Human bone marrow mesenchymal stem cells into vascular endothelial cells before and after differentiation induced differential expression of eNOS/activity and its metabolites

Objective To investigate the differences in eNOS gene expression,activity and its metabolites before and after human bone marrow mesenchymal stem cells (hBMSCs) are induced into vascular endothelial cells.Methods hBMSCs were induced into vascular endothelial cells.The morphological changes of the cells were observed under inverted microscope.Transwell assay was used to detect the cells' migration ability.The protein expression of eNOS was detected by immunofluorescence and Western blot.The activity of eNOS was detected by ELISA and the content of NO in cell culture supernatant was determined by nitrate reduction method.Results Compared with those in the undifferentiated group,the morphological changes of the differentiated cells were obvious.Cell migration ability increased by 238.10% (73.000±7.002 vs.21.000±4.359,P<0.05).The expression of eNOS protein increased by 114.72% (0.423±0.011 vs.0.197±0.079,P<0.05).The activity of eNOS was enhanced by 157.49% (4.967±0.073 vs.1.929±±0.103,P<0.05).The synthesis and release of NO increased by 155.67% (184.909±1.853 vs.72.323±0.426,P<0.05).Conclusion After hBMSCs are induced into endothelial cells,the expression of eNOS gene increases,their activities increase,synthesis and release of the metabolite NO increase.It may provide a basis for prevention and treatment of cardiovascular diseases with stem cells.
