1.Efficiency and safety of non-nasogastric tube on fast-track surgery patients with colorectal cancer:Meta- analysis and systematic review

Objective To evaluate the efficiency and safety of non- nasogastric tube on fast-track surgery patients with colorectal cancer. Methods We searched PubMed, CBM, Cochrane Library, CNKI, Wanfang and Vip data. Controlled trials that met the criteria were selected. All reports were collected and analyzed by RevMan5.3. Results 8 reports were enrolled. The non-nasogastric tube group had a shorter anus exhausting time and lower morbidity of pulmonary and throat complications , and it also had a shorter postoperative length of hospital stays. Significant differences were found between the non-nasogastric tube group and the control group (P< 0.01). There are no significant difference in the morbidity of gastrointestinal complications (P > 0.05). Conclusion Current evidence shows that non-nasgastric tubes are recommended for the patients with colorectal cancer surgery.
2.Practice and reflection on the performance evaluation index system of scientific research in a general hospital

Objective Implementing the scientific research performance evaluation indicator system constructed to evaluate the situation of scientific research performance conducted by two basic Departments during the 12th Five-Year period in a general hospital.To understand the practical applicability of the scientific evaluation index system,and also propose comments and suggestions based on the comparison between assessment score and actual research situation in the departments.Methods Based on the database of scientific research management information platform,collect related research data from two basic departments of the hospital in latest five years.Using SPSS software to conduct statistical analysis on the total score of the first and two indicators,as well as the specific indicators of the three indicators,according to the performance evaluation index system and the weight built by the hospital.Results The index system can basically reflect the general information of scientific research input and output in the department,however,some indicators cannot truly reflect the index importance and the differences between the index weight is not significant,so it is necessary to make amendment based on demonstration.Conclusions Hospital should dynamically update the index system to make them match the developing strategy of the hospital,also important to combine them with research stimulation to guarantee more scientific management and better service to the research development of hospitals.
3.Degradation kinetics of chlorogenic acid, cryptochlorogenic acid, and neochlorogenic acid at neutral and alkaline pH values.

The degradation kinetics of chlorogenic acid (5-CQA), cryptochlorogenic acid (4-CQA), and neochlorogenic acid (3-CQA) in aqueous solution at 37 degrees C and different pH values (7.05, 7.96, 9.25) were investigated in the present work. The results indicated that 3-, 4- and 5-CQA tended to remain stable in acidic pH circumstance, and unstable in neutral and alkaline pH circumstance. With the increase of the alkalinity, the degradation of 3-, 4- and 5-CQA was increased leading to a less amount of total CQA and was satisfactorily described by the Weibull equation. Meanwhile, caffeic acid was not detected after the degradation of CQA. Moreover, the degradation of 3-CQA and 5-CQA tended to be converted to 4-CQA, and the degradation of 4-CQA tended to be converted to 3-CQA rather than 5-CQA. The comparison of the degradation kinetics parameters of 3-, 4- and 5-CQA at neutral and alkaline pH values showed that the orders of the rate constant (k) values were 4-CQA > 3-CQA > 5-CQA, while the orders of the degradation half life (t½) values were 4-CQA < 3-CQA < 5-CQA, indicating the orders of the stabilities of 3-, 4- and 5-CQA at 37 degrees C and neutral and alkaline pH values were 4-CQA < 3-CQA < 5-CQA.

7.Clinical analysis of 18 cases of pituitary apoplexy

Objective To analyze the etiology and clinical feature of pi t uitary apoplexy.Methods The clinical data from 18 cases of pitui tary apoplexy were analyzed retrospectively.Results Main symptom s were headache in 12 cases and decreased visual acuity in 12 cases.Positive si ghs were temporal hemianopisia in 6 cases,unequal pupil in 3 cases,oculomotor ne rve(Ⅲ) paralysis in 2 cases and abducent nerve (Ⅵ) paralysis in 2 cases.Imagin g examinations showed CT positive finding in 4 cases and MRI positive finding in 6 cases.Tumor diameter was longer than 2 cm in 13 cases and less than 2 cm in 5 cases.Chromophobe pituitary tumor was found in 11 cases.All cases received surg ical operation.Conclusion Clinical features of pituitary tumor a poplexy are atypical,of which headache and visual acuity decrease are the main s ymptoms.Most cases occur suddenly or primary symptoms are deteriorated suddenly. CT and MRI positive findings are helpful for diagnosis.
8.Aspirin Improves Survival of Colorectal Cancer Patients:A Meta-analysis and Systematic Evaluation

Objective To systematically assess the relationship between oral administration of aspirin and survival benefit for patients with colorec?tal cancer(CRC)by meta?analysis. Methods Relevant studies were identified through searching PubMed and EMBASE. Random?effects model was derived to composite the pooled hazard ratio for overall mortality and CRC?specific mortality. The subgroup analysis was conducted for included data,and the bias analysis was reported. Results Thirteen studies on aspirin therapy were finally included in this meta?analysis. The overall surviv?al benefit associated with oral administration of aspirin represented an HR of 0.83(95%CI:0.71?0.96). Oral administration of aspirin was also asso?ciated with CRC?specific survival(HR=0.77,95%CI:0.64?0.93). No evidence was observed of an association between prediagnostic aspirin use and CRC overall survival(HR=1.00,95%CI:0.85?1.18)or CRC?specific survival(HR=0.97,95%CI:0.83?1.13). Conclusion These findings provide further indication that post?diagnostic aspirin therapy can improve CRC prognosis.
9.Chaotic medicine, homeostasis and sudden death

BACKGROUND: Clinical studies have discovered heart rate variability (HRV) is closely correlated with autonomic nervous system, sub-health and sudden death. Medical studies have summarized some chaotic medical science and homeostasis relevance theory. But so far the combination could still not systematically explain the clinical manifestation or summarize the diagnostic standards for sub-health and effective index to prevent sudden death. OBJECTIVE: To elucidate the correlation among autonomic nervous system, sub-health and sudden death by using chaotic medicine and homeostasis, so as to provide theoretical basis for evaluating sub-healthy state using HRV and preventing sudden death HRV. RETRIEVAL STRATEGY: A computer-based online search of Pubmed was undertaken to identify the articles published in English between January 1995 and June 2007 with the keywords of "chaotic medicine, heart rate variability (HRV), Sudden death, Sub-health, homeostasis". Meanwhile, the Weblog and CNKI database were searched for relevant articles dated from January 1995 and June 2007 with the same keywords in Chinese. Fifty-six papers were searched. After the first trial, only articles ①closely correlated with chaotic medical science, sudden death and sub-health, and ②published recently or in authority magazine in same field were selected. Repetitive studies were excluded. LITERATURE EVALUATION: The review articles, basic and clinical experiments about chaotic medical science, HRV, sudden death, sub-health, and homeostasis as well as articles published in related weblogs were selected. DATA SYNTHESIS: ①In any individual composed of multi-cells and multi-system, there are at least three-level chaotic system: overall chaotic system, cardiovascular chaotic system maintaining the blood pressure and cell chaotic system. Healthy state is that the three chaotic systems are in normal condition. Disease and sub-healthy state is the sub-stable state of chaotic system caused by the overstress of central nervous system, overexcitation of autonomic nerves and HRV decrease when there is some cell or cardiovascular system malfunction, and the information is feedback to central nervous system. ②Based on the theory about chaotic medicine and homeostasis, there are four states in individual chaotic motion: health, disease, sub-health, and homeostasis border before sudden death. The health is the homeostasis of body; disease and sub-health are the sub-homeostasis and may develop from healthy state because various factors; they may transform each other and both of them may lead to sudden death for unsteady. ③The extreme tension of central nervous system and over-excitation of autonomic nerves result in the destabilizing overall chaotic motion. Stimulation induces system destabilizing and sudden death because the patient cannot maintain arterial blood pressure. CONCLUSION: Long-term healthy state is unpredictable, but we can forecast 1-2 days situations by short-term HRV testing for 512 times in the early morning of every day, and judge the healthy state after sleep, and the change tendency of healthy state. Accordingly, we could predict person is in healthy state, sub-healthy state or unsteady border state that could cause sudden death.
10.Strengthen the management of outside projects of general hospital and improve the level of scientific research management

Objective The number of awarded provincial-level research projects and their budget had been declining year by year.We analyzed the data of provincial-level research projects in a general hospital in last ten years to be able to describe the distribution of research projects,the subject crossing situation,the distributions of subject and age structure,degree,title of the project leaders.We tried to find out the problems of them and to provide evidence for policy making.Methods Based on the scientific research management software,the data of the provincial-level research projects can be collected from 2005 to 2014.To Analyze the distributions of subject and structure of age,degree,professional title of the projects leaders by SPSS software.Results In general,the number and funds of provincial-level projects were unstable in the nearly ten years.Many of them focused mainly on basic departments and key subject.Young and middle-aged professionals were the backbone of the hospital team and talents with a high degree and title were the main force in the successful provincial-level projects.Conclusions According to the distribution trend of the projects,the hospital should deliver a carefully targeted set of actions.According to the distribution trend of the subject departments,the hospital should strengthen cooperation among departments and promote the integration of subjects.Strengthen the construction of talent team,pay attention to the cultivation of young talents.Strengthen scientific research management,To provide services for provincial level projects management.
