1.Relationship between Prognosis of Cyclic Vomiting Syndrome and Migraine

Objective To study the proportion of cyclic vomiting syndrome(CVS) developing to migraine by the medium term prognosis,and explore the relationship between CVS and migraine.Methods Twenty-eight of 38 cases who had identified in our clinical records were traced ,each child was matched to a control,and they all conducted a telephone interview by a standardized question.Results Ninteen(46%) of the subject had continued CVS and(or) migraine,the prevalence of past or present migraine in subjects(46%)was significantly higher than that in control group(10.7%)(P=0.003).Conclusion The progonosis of CVS is closely related to migraine,many of the suffers of CVS tend to develop migraine.
2.Clinical observation of indwelling transanal tube on prevention of anastomotic leakage after congenital megacolon radical correction

Objective To observe the effect of indwelling transanal tube on prevention of anastomotic leakage after congenital megacolon radical correction. Methods One hundred and three children who had underwent congenital megacolon radical correction were divided into indwelling group (53 cases) and no indwelling group (50 cases) according to the operation method, and the operation condition and incidence of complications was compared between 2 groups. Results There were no statistical differences in operation time, amount of bleeding during operation and incidence of other complications (pelvic infection and disruption of wound) (P>0.05). The incidence of anastomotic leakage in indwelling group was significantly lower than that in no indwelling group: 0 vs. 12.0% (6/50), and there was statistical difference (P<0.05). Conclusions Indwelling transanal tube for drainage and decompression is effective in preventing the anastomotic leakage after congenital megacolon radical correction, and it has high effectivity and safety.
3.The protection of carbamylated erythropoietin on focal ischemic brain injury

Objective To study the protection and its mechanism of carbamylated erythropoietin (CEPO) on ischemic brain injury and to compare its function with erythropoietin (EPO).Methods Focal cerebral ischemia/reperfusion was induced by occlusion of the middle cerebral artery (MCAO) using the intraluminal filament technique.The expression of endothelial NO synthase (eNOS) and activated caspase-3 were detected with Western blot.The inducible NO synthase (iNOS) positive cells were detected by immunohistochemistry staining.The apoptotic cell was detected by TUNEL staining.Results The expression of eNOS, iNOS and activated caspase-3 in cerebral cortex significantly increased after MCAO.The influence of CEPO and EPO on eNOS in ischemic cortex were not significantly different.However, the expression of activated caspase-3 markedly dropped from 95.4%±16.7% in group NS to 43.5%±13.1% in group CEPO and 45.1%±11.2% in group EPO (t=5.99 and 6.13,P<0.01).Immunohistochemistry staining revealed iNOS positive cells in ischemic cortex was (3.1±1.9) cells/square, CEPO and EPO remarkably reduced them to (0.7±0.2) cells/square and (0.8±0.2) cells/square, respectively (t=3.08 and 2.95, P < 0.05).The apoptotic cells in ischemic cortex fell from (94.2±15.2) cells/square in group NS to (40.5±9.8) cells/square in group CEPO (t=7.27, P < 0.01), the anti-apoptosis by EPO was similar to CEPO.Conclusion CEPO and EPO have the similar function of anti-apoptosis by inhibiting expression of activated caspase-3 and iNOS.
4.Blood pressure changes post liver transplantation in 206 recipients.

OBJECTIVETo study the blood pressure (BP) changes in the liver transplant recipients.
METHODSA total of 206 patients without preoperation hypertension received liver transplantation in our hospital from February 2001 to July 2005. The BP level and serum immunosuppressant concentration at preoperation and various time points post operation were determined.
RESULTSCompared with the preoperation, the average systolic and diastolic pressure was significantly increased at the 2 week, 1, 2, 4 and 6 months post operation. The mobility of hypertension increased significantly after liver transplantation, with the highest mobility (46.49%) at the 1st month post operation. There was no linear correlation between the immunosuppressant (FK506) concentration and the BP level at any time point.
CONCLUSIONThere was a high hypertension incidence after liver transplantation. Although the use of immunosuppressive drugs accompanied with the BP increase, there was no linear correlation between the immunosuppressant concentration and the BP level post operation.

5.Analysis of The Efficacy of Continuous Blood Purfication in the Treatment of Severe Acute Pancreatitis

Objective:To investigate the efficacy and safety of continuous blood purification in the treatment of severe acute pancreatitis.Methods:60 cases of severe acute pancreatitis were selected and divided into two groups.The control group (29 cases) was given routine treatment and the observation group (31 cases) was given continuous blood purification.The efficacy of continuous blood purification in the treatment of severe acute pancreatitis was evaluated by CRP,ALT,PaO2/FiO2,HCO3-,Scr,APACHE Ⅱ and MODS scores before and after treatment,complications and survival situation during treatment.Results:Before treatment,there was no statistical significance in the CRP,ALT,PaO2/FiO2,HCO3-,Scr levels between two groups (P>0.05).After treatment,the CRP,ALT,HCO3-,Scr levels of two groups were decreased.These indexes of observation group were lower than those of the control group (P<0.05).The PaO2/FiO2 of both groups were increased.The PaO2/FiO2 of observation group was high than that of the control group (P<0.05).Before treatment,there was no statistical significance in the APACHE Ⅱ and MODS scores between two groups (P>0.05).After treatment,the APACHE Ⅱ and MODS scores were lower than those before treatment.The APACHE Ⅱ and MODS scores in the observation group were lower than those of the control group (P<0.05).During treatment,there was no statistical significance in the complications and survival rate between two groups (P>0.05).Conclusion:Continuous blood purification had a good therapeutic effect on the severe acute pancreatitis.It could improve the organ function,reduce inflammation and regulate the balance of water and electrolyte with high safety.
6.Exon deletions of parkin gene in patients with Parkinson disease.

Mutations in the parkin gene have recently been identified in familial and isolated patients with early-onset Parkinson disease (PD) and that subregions between exon 2 and 4 of the parkin gene are hot spots of deletive mutations. To study the distribution of deletions in the parkin gene among variant subset patients with PD in China, and to explore the role of parkin gene in the pathogenesis of PD, 63 patients were divided into early onset and later onset groups. Exons 1-12 were amplified by PCR, templated by the genomic DNA of patients, and then the deletion distribution detected by agarose electrophoresis. Four patients were found to be carrier of exon deletions in 63 patients with PD. The location of the deletion was on exon 2 (1 case), exon 3 (2 cases) and exon 4 (1 case). All patients were belong to the group of early onset PD. The results showed that parkin gene deletion on exon 2, exon 3 and exon 4 found in Chinese population contributes partly to early onset PD.

7.Value of three-dimensional color power angiography in the differential diagnosis of benign and malignant breast tumors

Objective To evaluate the value of three-dimensional color power angiography(3D-CPA) in the differential diagnosis of benign and malignant breast tumors.Methods 3D-CPA vascular parameters were collected and analyzed in 121 breast solid lesions (63 malignant,58 benign) in the 114 female patients.The diagnostic performances of each parameter were compared by receiver operating characteristic (ROC)cure.Results The parameters of intra-tumor flow index (FI1),flow index of shells with outside thickness of 3mm surrounding the breast tumors(FI2 ) and the ratio of FI2 and FI1 (G2) in malignant group were significantly higher than those in benign group( P <0.05,respectively).Analysis of the areas under ROC curve showed that the areas under FI2 curve was the largest.No significant difference( P >0.05) was found between the area under FI2 curve and the area under G2 curve.Comparing the area under FI2 curve to that under FI1,there was significant differences ( P < 0.05).Taking FI2 ≥ 27.385 as the critical value,the sensitivity and specificity for the diagnosis of malignant breast tumors were 88.9% and 81.0%,and the positive and negative predictive values were 83.6% and 87.0% respectively.Conclusions The sensitivity and specificity of 3D-CPA parameter FI2 in the diagnosis of malignant breast tumors were higher than others,FI2 with the optimal critical value of ≥27.385 could help to identify malignant from benign breast tumors.
8.Significance of detection of Tau protein and β amyloid protein levels in cerebrospinal fluid in patients with mild cognitive impairment

ObjectiveTo study the levels of Tau protein and β amyloid protein(Aβ) in cerebrospinal fluid(CSF) in patients with mild cognitive impairment(MCI) and their significance of diagnosis. MethodsSensitive enzyme linked immunosorbent assay were performed to detect the CSF levels of Tau protein and Aβ in 32 patients with MCI and 15 healthy controls. ResultsSignificant elevation of the level of Tau protein and decrease of the level of the Aβ were observed in patients with MCI compared with healthy controls.ConclusionThe assays of the CSF Tau and Aβprotein levels can provide valuable biological marker for the diagnoses and treatment of MCI.
9.Establishment of a high expressing system of human coagulant factor VIII in vitro.

OBJECTIVETo construct a recombinant lentiviral vector (pXZ208-BDDhFVIII) mediating B-domain-deleted human coagulation factor VIII (BDDhFVIII) gene and investigate its expression in HLF, Chang-Liver and MSC cells.
METHODSBDDhFVIII gene fragment was separated by endonuclease digestion and was cloned into the multiple cloning sites of pXZ208 to construct a recombinant lentiviral vector pXZ208-BDDhFVIII. Viral particles were prepared by means of three-plasmid cotransfection of 293T package cells by calcium phosphate precipitation. After infection, the coagulant activity of human FVIII in the culture medium of 293T, HLF, Chang-Liver and MSC cells was assayed by one-stage method. The gene transduction efficiency was assayed by flow cytometry (FCM). Furthermore, PCR was performed to test the integration of BDDhFVIII.
RESULTSThe infection rates of HLF, Chang-Liver and MSC were (74.52 +/- 7.57)%, (27.24 +/- 6.53)% and (42.34 +/- 5.84)% respectively. The activities of FVIII in supernatants of HLF, Chang-Liver and MSC were (54.1 +/- 5.6)%, (22.5 +/- 2.9)% and (12.5 +/- 2.7)% respectively. BDDhFVIII gene integration was detected in all the infected cells.
CONCLUSIONThe recombinant lentiviral vector pXZ208-BDDhFVIII was successfully constructed and efficiently integrated into target cells to express human FVIII activity in vitro.

10.Modified Shengma Biejia Decoction Combined with CAG Program for Elderly Acute Myeloid Leuke- mia Patients with Yin Deficiency Toxin Stasis Syndrome.

OBJECTIVETo observe the efficacy and safety of modified Shengma Biejia Decoction (MSBD) combined with CAG program in treating elderly acute myeloid leukemia (AML) patients with yin deficiency toxin stasis syndrome (YDTSS).
METHODSTotally 46 elderly AML patients were assigned to the treatment group (24 cases; treated with MSBD + CAG) and the control group (22 cases; treated with CAG + placebos of Chinese medicine) according to random digit table. The therapeutic course of CM placebo or MSBD was 21 days. The clinical efficacy and adverse reactions were observed. Meanwhile, physical state (ECOG Score), transfusion dependency, and TCM syndrome score were compared before and after treatment.
RESULTS(1) The complete remission rate was 54% (13/24) and the objective response rate (ORR) was 71% (17/24) in the treatment group, obviously higher than those of the control group [36% (8/22); 54% (13/24)], with statistical difference (P = 0.036, 0.042). When comparing the efficacy based on risk level, the moderate and poor ORR was 71% (10/14) and 67% (6/9) in the treatment group, and 57% (8/14) and 33% (2/6) in the control group, with statistical difference between the two groups (P = 0.048; P = 0.010). (2) Compared with before treatment in the same group, the ECOG score significantly decreased, the average infusion time of red cells and platelets were markedly prolonged in the treatment group after treatment (P < 0.05). ECOG score, the average infusion time of red cells and platelets were significantly better in the treatment group than in the control group after treatment (P < 0.05). (3) Compared with before treatment in the same group, scores of fever, hemorrhage, and bone pain were markedly reduced in the control group (P < 0.05); scores of fever, fatigue, hemorrhage, dry mouth, and bone pain were markedly reduced in the treatment group (P < 0.05). Better effect in relief of fever, fatigue, hemorrhage, dry mouth, and so on was obtained in the treatment group than in the control group (P < 0.05). (4) In aspect of hematotoxicity, the incidence of neutropenia, anemia, thrombocytopenia was obviously lower in the treatment group than in the control group [29.2% (7/24) vs 54.5% (12/22); 16.7% (4/ 24) vs 45.5% (10/22); 33.3% (8/24) vs 63.6% (14/22); P < 0.05]. The incidence of fatigue and anorexia was obviously lower in the treatment group than in the control group [37.5% (9/24) vs 63.6% (14/22), 37.5% (9/24) vs 81.8% (18/22); P < 0.05].
CONCLUSIONMSBD combined with CAG program in treating elderly AML patients with YDTSS, with efficacy enhancing toxicity reducing effect, had distinct advantages in improving physical condition and clinical symptoms, and reducing transfusion dependency.
