1.Expression of Th17-related transcription factor RORγt mRNA in peripheral blood mononuclear cells from patients with systemic lupus erythematosus

ObjectiveTo investigate the action mechanism of Th17 cells in immunoinflammatory response in systemic lupus erythematosus(SLE).MethodsReverse-transcription PCR was performed to measure the mRNA expre ssion of the retinoic acid receptor-related orphan nuclear receptor RORγt in peripheral blood mononuclear cells (PBMCs) from 12 patients with active SLE,9 patients with inactive SLE and 12 normal human controls.Data were statistically analyzed by approximate F test(Welch test) and Dunnett's T3 multiple comparison test (corrected).ResultsIn the case of RORγt mRNA expression in PBMCs,significant differences existed among the 3 groups(F =23.286,P < 0.01 ); in detail,the patients with active SLE were significantly higher than patients with inactive SLE and normal controls ( 1.06 ± 0.44 vs.0.65 ± 0.25,F =2.453,P < 0.05;1.06 ± 0.44 vs.0.22 ± 0.08,F =6.504,P < 0.05),and the patients with inactive SLE were significantly increased compared with the normal controls (F =3.343,P < 0.05).The expression level of RORγt mRN A was significantly positively correlated with SLE disease activity index (rp =0.623,P < 0.01 ).ConclusionsThere is a polarization of Th17 cells in patients with SLE.To antagonize the transcription factor RORγt,which plays an essential role in the regulation of Th17 cell differentiation,may facilitate the control of SLE via attenuating the immunoinflammatory response.
2.Clinical Observation of 50 Cases of Intracranial Infection after Neurosurgery

Objective To explore the clinical therapeutic effects of postoperative intracranial infection in neurosurgery.Methods Between January 2014 and January 2014,50 patients with postoperative intracranial infection in the department of neurosurgery in our hospital were selected in this study.Among the 50 patients,25 patients were treated by intravenous antibiotic therapy,15 patients were treated with intrathecal and intravenous antibiotics treatment,10 cases were treated by intrathecal injection,intravenous antibiotics and lumbar pool catheter drainage way for treatment,and then we observed the clinical therapeutic effect of three groups of patients.Results In 50 patients,37 patients were cured,3 cases had marked effect,8 patients had effect,2 patients had no effect.Conclusion For patients with neurosurgical operations,to maximally reduce the risk of intracranial infection,it is necessary to take antibiotics according to the actual situation of patients,and make different treatment schemes according to the difference of infection.
3.The analysis of the curative effect for the lipitor in curing essential hypercholesterolemia

Objective To evaluate the curative effect of the atorvastatin calcium tablet(trade name:lipitor) in curing essential hypercholesterolemia.Methods 76 patients with essential hypercholesterolemia were divided into two groups randomly:43 patients in the research group(lipitor tablet 10mg,qn);33 patients in the comparison group (simvastatin tablet 20mg,qn).The period of treatment for both two groups was 6 weeks.The changes of blood fat were detected before and after taking the medicine.The curative effect were compared at the same time.Resnlts The total cholesterol(TC)of the research groups decreased by 37.06%,the TC of the comparison group decreased by 22.05%.The triacylglyceride(TG)of the research group decreased by 24.81%,however,the TG of the compar- ison group decreased by just 14.49%.The low-density lipoprotein cholesterol(LDL-C)of the research group de- creased by 42.30% while the comparison group just decreased by 28.30%(P<0,01).Also,the high-density lipoprotein cholesterol(HDL-C)of the research group increased by 7.80%,and the comparison group increased by 7.13%(P>0.05),The effective rate of the research group was 95.35%.For the comparison group,the effective rate was 90.91%.Conclusion The lipitor has a good curative effect in curing essential hypereholesterolemia.Also, the lipitor has fewer side effect,This medicine is safe and reliable for patients.
4.Resection procedure for pancreatic carcinoma in patients with tumour invasion to the portal vein

Objective To discuss the resection technique for pancreatic carcinoma in patients with tumour(invasion) to the portal and/or superior mesentery vein,and the clinical significance of palliative resection of pancreatic carcinoma.Methods The clinical data of 71 cases of pancreatic carcinoma operated on within a period of 3 years were retrospectively reviewed.In patients with tumour invasion to the portal vein,a method of direc transection of pancreas was used in resection of the tumor,and was combined with local radiotherapy and chemotherapy.Results The resection rate was 57.75%.The complication rate was 22.54%.One patient died of multiple organ failure resulting from pneumonia.The survival rate at 6m,1-and 2-years was 100%,81.69%,and 40.85% respectively.Conclusions The direct transection method of pancreas(resection) is a simple and safe method to increase resection rate and prolong survival time for patients with(advanced) pancreatic carcinoma.
5.Degenarative necrosis and calcification of hepatic cycstic echinococcosis

Objective To study factors associated with necrosis and calcification of cyst echinococcosis. Methods This study retrospectively analyzed 191 cases of hepatic cycstic echinococcosis admitted from 2005 to 2008 in the First Affiliated Hospital of Xinjiang Medical University Department of General Surgery, possible relevant factors screened were the patients' gender, age, operation frequency, hepatitis, gall stones, calcium of blood serum, multiple hepatic hydatid cyst, biliary fistulae and hepatic cycstic echinococcosis degeneration. Single-factor analysis and multi-factors analysis were conducted. Results The single-factor independent samples t-test showed that age is significantly related to hepatic cycstic echinococcosis degeneration (t = - 2. 455, P = 0. 008). Single factor Chi-square test showed that factors impacting on hepatic cycstic echinococcosis degeneration were multiple hepatic hydatid cyst and the biliary fistulae (x2 = 6. 848, P = 0. 009; x2 = 7. 149, P = 0. 008). The multiple-factor Logistic regression analysis showed that the age, the multiple hepatic hydatid cyst, and biliary fistulae are the related risk factors(x2 =5. 306,P = 0. 021,OR = 1. 665;x2 = 10. 377,P =0. 001,OR =3. 300;x2 = 4.455,P =0. 035, OR =2. 164) of hepatic cycstic echinococcosis patients with cyst echinococcosis degeneration. Conclusions Age, multiple hepatic hydatid cyst and biliary fistulae are the risk factors associated with degenerative necrosis and calcification of hepatic echinococcosis.
6.Distribution features of stress-strain,principle stress vectors and displacements of normal femur during gait

BACKGROUND:Studies demonstrated that,the probability of middle and distal end of femur bone fatigue or fracture reaches maximum when the moving loads increase or under an impact load.OBJECTIVE:To analyze the stress-strain distribution,principle stress vectors and displacements of femur during gait.METHODS:The contour curves of femur and muscles attached to femur were extracted slice by slice based on the serial CT data from a healthy male volunteer,which were used to build musculoskeletal system models.And the straight-line muscle models of the lower extremities were constructed to evaluate muscles forces and joints forces by using a multi-objective optimization method based on gait analysis.The finite element models of femur during gait were built by taking muscle forces and joint forces as boundary conditions in ANSYS 10.0 to obtain the stress-strain distribution and displacements.RESULTS AND CONCLUSION:The maximal displacement occurred on the femur head,and the maximal stress occurred in the middle and distal end of femur during the whole gait.There exists bigger tensile stress in the location of the femur neck and middle and distal end of corpus ossis femoris.The result validates the fact that femur fracture often happens at the middle and below of femur stem from the viewpoint of biomechanics.
7.The relationship between MMP-1 genetic polymorphisms and dental implant early failure in Chinese Han nationality

Objective To make clear the relationship between the genetic polymorphism of MMP-1 and implant early failure.Methods We conducted a case control study,53 cases patients (no smoking history and without periodontitis) who have underwent dental implant were divided into experimental group(with dental implant failure) and control group(with dental implant success) according to early failure of dental implant (within one month).DNA of 53 patients in Han nationality were extracted by using Chelex-100 method and polymorphisms of the mmp-1-1607 gene were detected by restriction fragment length polymorphism after polymerase chain reactions.Results Patients with MMP-1-1607(1G/2G) genotype exhibited a significantly higher occurrence of implant early failure than those with MMP-1-1607(1G/1G) gene type(P<0.05).The OR of the 1G/2G versus the 1G/1G of the MMP-1-1607 genotype was 13.894(P<0.05).Conclusion MMP-1-1607(1G/2G) genotype is associated with implant early failure.
8.The Clinical Analysis of Microsurgical Treatment for Anterior Communicating Artery Aneurysm Via Pterional Approach

Objective To summarize treatment experiences of microsurgical clipping for anterior communicating artery aneurysm via pterional approach.Methods Clinical data of 82 cases undergoing microsurgical clipping for anterior communicating artery aneurysm via pterional approach in the first affiliated hospital of Kunming Medical University from October 2008 to December 2014 were collected and retrospectively analyzed.The patients were divided into different groups by Hunt-Hess illness grading scale,with 11 cases for level 0,7 cases for level Ⅰ,30 cases for level Ⅱ,25 cases for level Ⅲ,8 cases for level Ⅳ,and 1 cases for level Ⅴ.Twenty-one patients underwent operation at early stage of SAH (<3d),15 at late stage of SAH (4d~2W),and 35 at prolonged stage of SAH (>2W).The prognosis of patients was evaluated according to GOS classification criteria at discharge.Results A total of 85 ACoAA were found in 82 patients and all of them were clipped and,at the same time,3 aneurysms were resected and 11 thrombuses were punctured,cut and removed.In the operation,15 (17.6%) aneurysms ruptured again and temporary blocking happened for 73 times,with the shortest blocking time of 2 rmin,the longest of 40 rmin,and the average of 9.26min.According to GOS score,good recovery rate was 79.3% (65/82),moderate disability rate was 12.2% (10/82),severe disability rate was 3.7% (3/82),vegetative state rate was 0%,and death rate was 4.9% (4/82).Good recovery rates for the operations at early,late and prolonged stage of SAH were 85.7%,73.3% and 82.3% respectively and for level 0 to level Ⅴ were 90.9%,85.6%,86.7% and 84.0%,25.0% and 0.0% respectively.Results of DSA or CTA re-examination upon 55 patients followed-up at discharge or three months after discharge showed that tumor pedicle were clipped and aneurysm disappeared.Fifty cases were followed up from four months to seven years after the operation,with one case of aneurysm recurrence.Another thirty-two cases were lost to follow-up.ConclusiornS Microsurgical techniques and microdissection are keys to successful operation.Pterional approach can guarantee successful clipping of aneurysms with different directions,locations and sizes.It is also an easy,effective and reliable approach with less postoperative complications.Operation for anterior communicating artery aneurysm should be performed as early as possible.
9.A comparison of clinical outcome of radiofrequency modification and augmentation reconstruction in the treatment for partial anterior cruciate ligament tears

Objective To compare the outcomes between radiofrequency modification and augmentation reconstruction in the treatment for partial anterior cruciate ligament( ACL) tears. Methods Fifty?four patients with partial ACL tears were divided into radiofrequency modification group( n=16) and augmentation reconstruc?tion group( n=38) according to different treatment methods. Minimum follow?up was 2 years. Patients were eval?uated just after the injury and at the latest follow?up with several parameters, including the international knee documentation committee( IKDC) score,Lysholm knee score,Tegner activity rating,range of motion ( ROM) and the kneelax arthrometer. Results The kneelax was (4. 4±0. 9) mm of preoperative, (5. 0±1. 5) mm of post?operative in radiofrequency modification group,the difference was no signifiacnt( t=1. 372,P=0. 183);in aug?mentation reconstruction group was (4. 7±0. 8) mm and (1. 7±1. 0) mm respectively,the knee stability was in?crease significantly postoperative;knee stability was significantly improved in the augmentation reconstruction group compared with the radiofrequency modification group( t=9. 228,P=0. 000) . In radiofrequency modifica?tion group,ROM,Lysholm score and Tegner score were improved significantly between the results pre?and post?operatively((105. 4±22. 5)° vs. (123. 5±11. 4)°,t=59. 189,P=0. 000;65(51,90)) points vs. 86(59,100) points,Z=3. 852,P=0. 000;3(1,5) points vs. 6(1,9) points,Z=4. 637,P=0. 000). In augmentation recon? struction group, ROM,Lysholm score and Tegner score were improved significantly between the results pre?and post?operatively((99. 4±21. 9)° vs. (125. 3±10. 9)°,t=6. 527,P=0. 000;(65(51,89) points vs. (85 (59, 100) points,Z=4. 427,P=0. 000;3(1,5) points vs. 6(1,9) points,Z=3. 961,P=0. 000). Conclusion Augmentation reconstruction technique is more beneficial to the knee stability than radiofrequency modification in the treatment for the partial ACL tears.
10.Clinical observation on catgut implantation at acupoint combined sodium valproate in treatment of cerebrovascular epilepsy

Objective To observe the effect of catgut implantation at acupoint on treating cerebrovascular epilepsy and discuss the therapeutic mechanism. Methods 58 patients were divided into an observation group and a control group. The control group was treated with sodium valproate, and the observation group was treated with catgut implantation at acupoint on the basis of the control group.Epilepsy scoring, sodium valproate adverse reactions, the change of blood concentrations of sodium valproate and curative effect evaluation and so on were observed before and after the treatment. Results The basic control and excellent rate in the observation group were 16.7%and 73.3%, better than the control group of 10.7%and 53.6% respectively. The epilepsy scoring and sodium valproate adverse reactions after treatment in the observation group were superior to the control group [observation group is(5.05±1.5),(5.09±1.53),control group is(6.40±1.74),(10.85±1.91),P<0.05]. The number of patients with blood concentrations of sodium valproate in range 50-100 mg/L after treatment was increased in the observation group(Z=2.07, P=0.038). Conclusion Catgut implantation at acupoint combined sodium valproate was more effective than sodium valproate in treating cerebrovascular epilepsy, which can obviously alleviate the adverse reaction of drugs and increase blood concentrations of sodium valproate.
