1.The comparison of cerebral oximetry and somatosensory evoked potential for monitoring carotid endarterectomy

Objective To compare cerebral oximetry(rSO 2 %)and somatosensory evoked potential (N 35 ) in their accuracy in monitoring cerebral perfusion during carotid endarterectomy procedure under general anesthesia. Methods Ten patients (6 males, 4 females), scheduled for selective carotid endarterectomy, were enrolled in the study. Their rSO 2 %and N 35 were also continuously monitored and recorded during perioperative periods. The patients' cognitive function was evaluated at the same time. The MAP, HR, SpO 2 were continuously monitored during operation. Results There was a significant difference in rSO 2 % between that at the period of elamping and that at the period of declamping of the carotid artery. No patients suffered from cognition dysfunction during perioperative period. Conclusion With comparison to the amplitude of N 35 it was rSO 2 % which was earlier to show alteration in cerebral perfusion in case of cerebral oxygen deficit. In monitoring cerebral perfusion, rSO 2 % was better than N 35
2.Diagnosis and treatment of occult indirect inguinal hernia (A report of 60 cases)

Ojective To enhance the ability of diagnosis and therapy for occult indirect inguinal hernia. Methods Surgical exploration was performed for patients highly suspected with occult indirect inguinal hernia based on clinical manifestations and physical examinations in the initial stage. At present, herniography was used to confirm the diagnosis. High ligation of the hernial sac, non-mesh methods of open groin hernia repair or tension-free inguinal hernia repair was performed for the patients. Results Forty-four cases were given surgical exploration in the initial stage and 41 cases were confirmed and treated by operation. Misdiagnosis was found in 3 cases and surgical exploration revealed no sign of disease. Relapse occurred in two cases within 5 years. Nineteen out of 30 patients were confirmed by herniography in the last five years and operated on. There was no false-positive herniogram in this series. Follow-up evaluation of the patients showed disappearance of preoperative symptoms. Groin hernia has not recurred so far. Conclusions The ability of diagnosis for occult indirect inguinal hernia can be enhanced by herniography. Blind surgical exploration can be reduced. Herniography is a safe, reliable and practical diagnostic procedure.

4.7-Nitro indazole as a selective inhibitor of neuronal NO synthase decreases sevoflurane MAC and cerebellar cMGP content

Objective:To examine the effects of acute or chronic administration of 7-nitro indazole (7-NI) on the MAC of sevoflurane and cerebellar cGMP levels in mice. Method:In acute experiment,sevoflurane MAC was determined in mice after 7-NI 120mg/kg, 180mg/kg or 120mg/kg plus L-arginine 600mg/kg was given intraperitoneally. During chronic experiment, MAC was measured on 1st, 4th and 7th day during week-long gavage feeding of 7-NI. The levels of cerebellar cGMP also were measured after acute and chronic administration of 7-NI. Result:Acute administration of 7-NI 120mg/kg decreased sevoflurane MAC by about 20% and cerebellar cGMP level by 96%. 7-NI 180mg/kg did decrease MAC not more than 7-NI 120mg/kg. No significant reduction of MAC was observed in mice treated by intraperitoneal 7-NI 120mg/kg+L-arginine 600mg/kg. In chronic experiment,sevoflurane MAC and cerebellar cGMP were decreased significantly on 1st 4th and 7th day to the similar extend, with 15%-20% reduction in MAC and 46%-60% in cGMP levels. Conclusion:Acute and chronic selective inhibition of nNOS decreases sevoflurane MAC and cerebellar cGMP levels in mice. The maximal reduction (only about 20%) of MAC can be obtained when cGMP levesl are decreased about 50% and further inhibition of generation of cGMP does not produce more enhancement to the potency of sevoflurane anesthesia. No compensatory mechanism for the inhibition of L-Arg-NO-cGMP pathway appears during week-long treatment with 7-NI.
5.Correlation between expression of CD44V6,E-cadherin and biological behavior in breast cancer

Purpose:To investigate the relationship of expression of CD44v6, E cadherin (E Cad) with clinic biologicl behavior in breast cancer and its correlation. Methods:Catalyzed signal amplification immunohistochemical method was used to detect the expression of CD44v6 and E Cad in 94 cases of breast cancer.Results:The positive expression of CD44v6 and the negative expression of E Cad were positively correlated with the grading, clinical stage, lymph node metastasis recurrence and prognosis of breast cancer. In breast cancer, the expression of CD44v6 had negative correlation to that of E Cad.Conclusions:Several genes were involved in the occurrence and development of breast cancer in which the expression of CD44v6 had negative regulative synergistic action with that of E Cad. CD44v6 and E Cad expression, as a clinical valuable marker, has a great significance in assessing node metastasis and survival.
6.Comparison of the effects of propofol total intravenous anesthesia vs higher dosages of fentanyl anesthesia on myocardial function in CABG operation

Objective To evaluate the effects of different anesthetic methods on myocardial function after the CABG operation and compare the hemodynamic variables under the propofol total intravenous anesthesia with high dosages fentanyl anesthesia. Methods 12 patients (ASA Ⅲ) undergoing selective CABG operation under CPB were randomly divided into two groups (with 6 in each): Group Ⅰ (large dosages of fentanyle anesthesia), and Group Ⅱ (propofol total intravenous anesthesia). The hemodynamic parameters were recorded after induction of anesthesia, after sternotomy, after CPB, and after closure of thoracostomy. Results There were no significant differences of MBP, HR, SVRI, CI, LVEF, and A/E, Tei index between the two groups. Conclusion The method of propofol total intravenous anesthesia (TIVA) was safe, effective and praticable for the CABG operations under CPB.
7.INNER-STENT TREATMENT FOR MALIGNANT COLORECTAL OBSTRUCTION

To evaluate initial experience with shape memory alloy stent as an alterative to colostomy in patients with intestinal obstruction by colorectal cancer, forty four patients diagnosed with acute and chronic colorectal obstructions from malignant causes were subjected to stent placement. Among them 41 patients achieved clinical decompression. Thirty three cases died within 51~720 days and the other 5 cases were still alive without intestinal obstruction for 1~8 months. Our conclusions are shape memory alloy stent may be useful in the management of terminal or high risk surgical patients for palliative purposes instead of colostomy. Palliative management by stent placement combined with chemotherapy and immunotherapy might prolong the survival time of these patients. Placement of stent for acute colorectal bowel obstructions facilitates preoperative bowel preparation for surgical resection and obviates the need for fecal diversion or on table lavage.
8.Herniography in the diagnosis of occult or recurrent inguinal indirect hernia

ObjectiveTo improve the diagnosis of occult or recurrent inguinal indirect hernias. Methods 47 patients with obscure groin or incisional pain with normal physical examinations were undergone herniography. Results26 patients had hernias confirmed by herniography, including 22 indirect and 4 recurrent hernias. The symptoms disappeared after herniorrhaphy in the 26 patients. Eighteen of the other 21 patients with normal herniography were asymptomatic via other treatment 3 months after herniography. Conclusions Herniography is useful to diagnose the occult or recurrent inguinal indirect hernias in patients with inguinal region pain of unclear causes.
9.Study on The Expressions of Matrix Metalloproteinase-26 and Sialyl Lewis-X in Breast Carcinoma

Objective To investigate the expressions of matrix metalloproteinase-26(MMP-26) and sialyl Lewis-X(SLeX) in breast carcinoma and their relation with lymphaden metastasis.Methods The expressions of MMP-26 and SLeX protein in breast carcinoma(n=120),metastatic lymph node(n=71) and normal mammary gland tissue(n=30) were studied by catalyzed signal amplification(CSA) immunohistochemical method.Results The positive rate of MMP-26 and SLeX protein expression were 16.7%(5/30) and 20.0%(6/30) in normal breast tissues,and 63.3%(76/120) and 86.7%(104/120) in breast carcinoma,resepectively.The expressions of MMP-26 and SLeX protein in breast carcinoma tissues were significantly higher than those in remote normal breast tissues(P0.05).The positive rate of MMP-26 and SLeX protein expression were 23.9%(17/71) and 88.7%(63/71) in metastatic lymph node,resepectively.The expression of SLeX protein in metastatic lymph node was correlated with that in tissues of breast carcinoma(r=0.874,P
10.Cause and management of difficult laparoscopic appendectomy

Objective To study the cause and management of difficult laparoscopic appendectomy.Method A retrospective analysis of 102 difficult laparoscopic appendectomies was performed. Results The associating factors leading to operative difficulty were as follows:adhesion of peri-appendix in 44 cases,gangrene at root of appendix in 21 cases,special location of appendix in 17 cases,abdominal adhesion in 12 cases,obesity and inflatable intestine in 8 cases. Laparoscopic appendectomy was successfully performed in 96 cases(94.1%,96/102) ,and no intestinal leakage or massive hemorrahge occurred. Conclusions Adhesion of peri-appendix is the most common cause in difficult laparoscopic appendectomy,and gangrene at root of appendix is the most difficult one. Reasonable management for appendix stump contributes to successful operation.
