1.Clinical Evaluation of Recombinant Human Interferon Alpha-2 b Combined with LEEP in the Treatment of Cervical Ectopic Columnar Epithelium with Chronic Cervicitis

Objective:To investigate the clinical efficacy of recombinant human interferon alpha -2b combined with Loop electro-sugical excision procedure( LEEP)in the treatment of cervical ectopic columnar epithelium with chronic cervicitis. Methods:The med-ical records of 68 cases of cervical ectopic columnar epithelium with chronic cervicitis in our hospital from April 2012 to April 2014 treated with LEEP were analyzed retrospectively. The control group(30 cases)was treated with LEEP combined with 5-day postopera-tive oral administration of antibiotics to prevent infection,and the treatment group(38 cases)was given recombinant human interferon alpha -2b through vagina additionally,every other day for 20 days every month after menstrua. After 6 courses of treatment,the clinical efficacy,vaginal bleeding time,vaginal discharge duration,wound healing time and HPV persistent infection were compared between the two groups. Results:The cure rate of the control group was 66. 67%,while that of the treatment group was 92. 11%,and the difference was significant(P<0. 05). Totally 19 cases in the control group were with wound healing(accounted for 63. 33%),while that in the treatment group was 32 cases(accounted for 84. 21%),the difference was significant(P<0. 05). And the vaginal bleeding time,vaginal discharge duration,and wound healing time of the treatment group were all significantly shorter than those of the control group(P<0. 05). After the treatment,the HPV persistent infection rate in the control group was 16. 67%,while that in the treatment group was 0%,and the difference was statistically significant(P<0. 05). The two groups during the treatment process had no obvious adverse reactions,indicating high drug safety. Conclusion:Recombinant human interferon alpha-2b combined with LEEP in the treat-ment of cervical ectopic columnar epithelium is safe and effective,which can effectively shorten the wound healing time,vaginal bleed-ing time and vaginal discharge duration,and is worthy of promotion in clinics.
2.Determination of protocatechuic acid and protocatechualdehyde in Compound Sijiqing Tablets by HPLC

AIM: To establish an external standard HPLC method for the determination of protocatechuic acid and protocatechualdehyde in Compound Shijiqing Tablets(Folium Ilicis Chinensis, etc.). METHODS:Alltima C 18(5?m, 4.6mm?250mm)was used. The mobile phase was consisted of methanol-1% acetate(18∶82). The flow rate was 1.0mL?min -1. The UV detection wavelength was at 280nm. RESULTS: The two components has good resolution. The liner ranges were in the range of 7.6~127.1?g?mL -1 for protocatechuic acid and in the range of 2.62~43.6?g?mL -1 for protocatechualdehyde with the correlation coefficients 0.9997 and 0.9994, respectively. The average recoveries were 99.9% and 97.5% with RSD 2.1%,2.2%(n=5), respectively CONCLUSION: The method is accurative, reproducible and can be used to control the quality of Compound Sijiqing Tablets.
3.Isolation and Sequencing the Differential Gene Fragments Expressed in Human Stomach Cancer Tissue

Objective: Identification of the genes specially expressed in tumor cell but not in normal cell is important for understanding the molecular mechanisms of carcinogencsis. This study will focus on identification of differentially expressed gene fragments in human stomach cancer. Methods: By using the new developing mRNA differential display (DD) technique, genes fragments differentially expressed in stomach cancer tissues from a patient and the adjacent normal tissues beyond the tumor mass were studied. Results: Two differentially displayed complementary DNA fragments from stomach cancer tissues, scgl and scg2 (stomach cancer-associated gene, scg), cofirmed by Northern Blot, were cloned and sequenced. The nucleotide length of scgl is 194 base pairs and that of scg2 is 343 base pairs. After searching against GenBank databases by BLASTN, neither scgl nor scg2 had significant homological gene sequences with the known genes. Conclusion: These results suggested scgl and scg2 might be complementary DNA fragments of novel genes expressed in stomach cancer tissues, but not in normal tissues and may play a role in the occurrence and development of stomach cancer. Further characterization of full-length of these two complementary DNA fragments will be continued.
4.Clinical effect of cognitive behavioral Intervention on the cognitive level and quality of life in patients with venous thromboembolism

Objective To explore the effectof cognitive behavioral Intervention on the quality of life and cognitive lever of patients with venous thrombus embolism. Methods 164 patients with Venous Thrombus Embolism were randomly divided into control group and research group, the control group received routine care, The study group received cognitive behavior intervention. The cognitive lever and quality of life score was measured between before and after the intervention. Results Before the intervention, the difference score of living quality and cognitive lever in the two groups has no statistical significance (P > 0.05), and after intervention, the cognition lever and the three aspects of risk factors, anticoagulant knowledge and treatment methods of research group scored (28.47 ± 7.63), (7.38 ± 2.48), (6.82±2.73), (3.64±1.59) points which were significantly higher than the control group scored (20.54±5.49), (4.31±2.14), (3.72±1.62), (2.18±1.36) points (t=7.635, 8.469, 8.822, 6.299, P<0.01),the quality of life and four dimensions of daily life ability, pain, social activities, mental scored ( 37.04 ± 7.08), [9.00 (5.00, 11.00)],(9.04 ± 1.55),(5.52 ± 1.59),(14.00 ± 3.06) points which showed great improvement than the control group scored [49.50(39.75-59.25)], [11.50 (6.75, 16.00)], (10.05 ± 1.65), (7.95 ± 2.19), (19.18 ± 3.88) points. There were significant difference in the quality of life and four dimensions between the two groups. (Z=-3.558,-2.143, t=2.102, 4.274, 4.991, P<0.05 or 0.01). Conclusions The quality of life for the venous thrombus embolism patients were in lower level. The cognitive behavioral intervention could correcting their bad behavior and negative emotions, eventually improve the quality of life of venous thrombus embolism patients.
5.Experimental study on enhancement of tissue expansion with papaverine cream.

OBJECTIVETo evaluate the effect of external use of papaverine cream on enhancement of tissue expansion.
METHODS8 mini pigs were randomly divided into 4 groups. During inflation, papaverine was given externally in group A, through a delivery system in group B, though intramuscular injection in group C. Papaverine was not given in group D. The time of inflation, the expansion rate, the surviving length of expanded flaps and the papaverine concentration in the fibrous capsule in each group were examined. Histologic and electroscope examinations of the expanded tissue were made.
RESULTSThe average inflation time was 28.9 +/- 4.5 days in group A, 34.0 +/- 2.6 days in group B, 37.6 +/- 4.8 days in group C and 38.5 +/- 3.5 days in group D. There was significant difference between each group(P < 0.05). The average surviving length of expanded flaps in group A was 13.67 +/- 1.28 cm, 11.07 +/- 0.88 cm in group B, 10.79 +/- 0.49 cm in group C, and 9.49 +/- 0.77 cm in group D. There was significant difference between each group (P < 0.01). Histologic examinations of the expanded tissue suggested that there were no significant differences in thickness of epidermis, derma and fibrous capsule among the four groups. Electroscope examinations of the myofibroblast in fibrous capsule showed that there were smaller nuclei, fewer microfilaments and fewer mitochondrias and rougher endoplastic reticulume in group A than those in other groups. The concentration of papaverine in fibrous capsule of group A was 3.5 +/- 1.2 micrograms/g. Papaverine was detected neither in the skin of other groups nor in the blood of all groups.
CONCLUSIONS1. Papaverine cream can permeate through skin and maintain high concentration and continuous effect in local tissue. 2. External use of papaverine cream can inhabit the function of myofibroblast in fibrous capsule, increase inflation rate, accelerate expansion process, and improve microcirculation of the expanded skin with a result of prolongation of surviving length of expanded flaps. 3. The technique of enhancing tissue expansion with external use of papaverine has the advantages of convenient delivery, cheapness, no more injury and infection, and no side effects etc. It is a safe, effective, simple, and reliable method for accelerating tissue expansion.

6. Effect of individual exercise prescription on walking ability of patients with arteriosclerosis obliterans of lower limbs

Objective:
To explore whether individualized exercise prescription can improve the walking ability of patients with lower extremity arteriosclerosis occlusion (ASO).
Methods:
A total of 64 ASO patients with ankle brachial index (ABI) between 0.6 and 0.9 were randomly divided into the observation group (32 cases) and the control group (32 cases), Patients in the control group received routine nursing intervention, and patients in the observation group received nursing intervention based on individualized exercise prescription, ABI and walking impaired questionnaire (WIQ) scales were measured at admission and 12 weeks after intervention.

ObjectiveTo search, appraise and summarize the best evidence of the diabetic foot assessment and provide a reference for the standardization of the clinical diabetic foot evaluation in China. MethodsOn the computer, the following websites as well as databases were searched: National Guideline Clearinghouse(NGC) of the USA, Registered Nurses' Association of Ontario(RNAO), National Institute for Health and Care Excellence(NICE) of UK, the Scottish Intercollegiate Guidelines Network(SIGN), New England Guidelines Group(NEGG), International Guidelines Network(IGN), American Center for Disease Control and Prevention(ACDC), World Council of Enterostomal therapists(WCET), Wound Ostomy Continence Nurses Society(WOCN), the International Working Group on Diabetic Foot(IWGDF), American Society for Wound Healing, American Diabetes Association(ADA), American Society for Vascular Surgery(ASVS), Society of Endocrinology of Chinese Medical Association, Clinical evidence, PubMed, ProQuest, Web of Science, Clinical Key, CINAHL, Best Practice, CNKI, VIP, Wanfang Data, Medlive from January of 2010 to July of 2018 about all evidence-based clinical practice guidelines and systematic reviews on assessment of diabetic foot. Two researchers evaluated the quality of the articles and performed materials extraction, and extracted the evidence from the qualified articles. ResultsA total of 7 articles were included among which 6 were guidelines and 1 was systemic review. After analyzing them, 9 categories and 32 items of the best evidence contents about diabetic foot assessment were concluded. ConclusionsThis study summarizes the best evidence for assessment of diabetic foot and provide evidence basis for medical institutions to improve clinical practice of diabetic foot assessment. When applying the evidence into clinical practice, it is necessary to evaluate specific situations, consider patients' values and willingness and select targeted evidence accordingly.
8.Feasibility and efficacy of preserving internal branch of superior laryngeal nerve in endoscopic surgery for hypopharyngeal squamous cancer: an observational study

Objective:This study was performed to investigate the feasibility of preservation of internal branch of superior laryngeal nerve(ibSLN) during transoral endoscopic surgery for hypopharyngeal squamous cancer(HSCC) and the influence on patient′s swallowing function after operation.Methods:From May 2020 to June 2021, the data of 29 HSCC patients who required for transoral endoscopic surgery in the Department of Otorhinolaryngology Head and Neck Surgery, the Second Xiangya Hospital of Central South University were prospectively included, and the included patients were divided into two groups randomly by lottery. According to whether ibSLN was actively dissected during operation, they were divided into ibSLN preservation group ( n=15) and control group ( n=14, without ibSLN preservation). Operation time, intraoperative hemorrhage, intraoperative neck dissection, postoperative radiotherapy, postoperative recurrence within 1 year, retention and swallowing function, the recovery of oral soft diet and the quality of life were compared between two groups. SPSS 25.0 software was used for statistical analysis. Results:The study included 29 eligible patients, including 25 males and 4 females.The age ranged from 42 to 67 (56.07±5.93) years. There were no significant differences( P>0.05) between 2 groups in the following data,including age( t=-0.56), gender( χ2=0.01), TNM stage(T stage χ2=0.29, N stage χ2=0.02), pathological diagnosis( χ2=0.03), preoperative swallowing function( χ2=0.00) and M. D. Anderson Dysphagia Inventory(MDADI) score(global t=0.55, emotional t=0.16, functional t=0.60, physical t=0.64), operation time( t=1.62) and intraoperative hemorrhage( t=-1.46), intraoperative neck dissection( χ2=0.01), postoperative radiotherapy( χ2=0.32), postoperative recurrence within 1 year( P>0.050). The swallowing function was evaluated by water swallowing test after operation. The swallowing function of ibSLN preservation group was better than control group, and the difference between two groups was statistically significant on the 1st ( χ2=4.44, P=0.035), 5th ( χ2=4.24, P=0.039) and 7th ( χ2=4.55, P=0.033) day after operation. On the 14th day after operation, the MDADI scores of patients in the ibSLN preservation group were higher than those in the control group in global ( t=2.45, P=0.021), functional ( t=2.54, P=0.017) and physical ( t=2.24, P=0.034) dimensions, except for emotional dimension ( t=1.89, P=0.070). The median time of oral soft diet( U=23.00, P<0.001), normal oral diet( U=21.00, P<0.001) and the nasogastric tube removal time ( U=18.50, P<0.001) in ibSLN preservation group was 2 days, 5 days and 6 days respectively, earlier than that in control group, which had statistically significant difference. Conclusion:Our results show that it is feasible to preserve the ibSLN during HSCC transoral endoscopic surgery, which can achieve rapid recovery of postoperative swallowing function.

With completing a baseline survey of a large natural population cohort, conducting regular follow-up has become a key factor in further improving the quality of cohort construction and ensuring its sustainable development. Typical cohort follow-up methods include repeat surveys, routine monitoring, and community-oriented surveillance. However, in practical applications, there are often issues such as high costs, difficulty, and high error rates. Telephone follow-up is an important supplementary method to the methods mentioned above, as it has the characteristics of low cost, fast response, and high quality. However, the with difficult organization, quality control is challenging, response rates are low, and management levels vary widely, which limits its widespread use in large-scale population cohort studies. Given the above problems, this study draws on customer relationship management based on the actual needs of the China Northwest Cohort follow-up. It relies on the REDCap electronic data collection platform to build a telephone follow-up management and quality control system. Targeted solutions are provided for key issues in telephone follow-up implementation, including organizational structure, project management, data collection, and process quality control, to improve the quality control level of telephone follow-up comprehensively and thereby enhance the quality and efficiency of follow-up. We hope to provide standardized follow-up programs and efficient quality control tools for newly established and existing cohort studies.
