Objective:To explore the effects of multidimensional pain intervention model on postoperative pain in patients with perianal abscess. Methods:Totally 106 patients with perianal abscess who were admitted to the China-Japan Friendship Hospital from March 2020 to March 2022 were selected by convenience sampling and divided into observation group and routine group according to the admission time, with 53 patients in each group. Patients in the routine group received routine nursing care, while patients in the observation group were treated with a multidimensional pain intervention model based on the care provided to the routine group. The postoperative Numerical Rating Scale (NRS) and Depression-Anxiety-Stress Scale 21 (DASS-21) scores were compared between the two groups. Results:The NRS scores of the observation group at 1, 3, 5 and 7 days after surgery were all lower than those of the routine group, and the differences were statistically significant ( P<0.05) ; the DASS-21 scores of the observation group were lower than those of the routine group at 1 and 7 days after surgery, with statistically significant differences ( P<0.05) . Conclusions:The multidimensional pain intervention model can ameliorate postoperative pain and negative emotions of patients with perianal abscess.

Objective:To analyze the evolution path and diffusion mechanism of the unified drug management system of countywide medical communities in China, and provide references for the deepening implementation of the system. Methods:The policy documents of the central and provincial governments were retrieved with the keywords of " medical community" " drug management" " county-township-village integration" and " central pharmacy". By means of the policy diffusion theory, the evolution path of the diffusion of the unified drug management system for the countywide medical communities was identified from such dimensions as time, space and hierarchy. On the other hand, the action mechanism of the diffusion of the system was summarized from such aspects as competition, administrative instruction, learning and imitation. Results:A total of 36 effective policy documents were collected. The time diffusion of the drug unified management system of countywide medical communities was characterized by an " S" curve. By the end of 2022, there were 30 provinces implementing the countywide medical community drug unified management system, and the policy diffusion has entered a saturation and stagnation stage; spatial diffusion showed " proximity effect" ; hierarchy diffusion embodied the " leader follower" mode. In the process of policy diffusion, competition mechanism, administrative instruction mechanism, learning mechanism, and imitation mechanism coexisted, but there were difference in the dominant mechanism at different stages of policy diffusion. Conclusions:The unified drug management system of the countywide medical communities has been widely disseminated. It is recommended to promote the introduction of supporting policies, optimize the system evaluation system, and comprehensively use various diffusion mechanisms to promote the optimization of the system, so as to promote the deepening and sustainable operation of the system.

Objective:
To analyze the spatio-temporal distribution characteristics of pulmonary tuberculosis incidence in Xinjiang, and to provide reference for the prevention and control of pulmonary tuberculosis.
Methods:
The reported incidence numbers of tuberculosis and population data from various counties, cities(prefectures), and districts in Xinjiang from 2010 to 2022 were collected. Based on spatial-temporal scan statistics, standard deviational ellipse analysis, and centroid migration models, the clustering and migration trends of tuberculosis incidence were evaluated. Utilizing Kriging interpolation techniques, an interpolation analysis of the 2022 incidence rate was conducted on an annual scale, with the reported incidence rate in 2018 serving as a temporal control, to identify hotspots of the spatio-temporal distribution. ArcGIS software was employed to visualize the continuous spatial trends of incidence rate changes.
Results:
The annual reported incidence rate of tuberculosis in Xinjiang from 2010 to 2022 varied year by year, with fluctuations and increases prior to 2018, peaking in 2018, and then declining annually thereafter. The spatial distribution of the incidence rate exhibited a trend of initial clustering followed by diffusion, with the centroid of incidence shifting towards the northeast, yet the epicenter of the epidemic remained in Aksu Prefecture. The results of spatiotemporal scan statistics analysis revealed that the three-level aggregated areas of the epidemic encompassed a total of 41 prefectures, counties, and cities, with the tuberculosis incidence risk in the primary and secondary aggregated areas being significantly higher than that in other regions(P<0.01). The Kriging interpolation prediction map suggested that the four prefectures in southern Xinjiang continued to be high-risk regions for tuberculosis(incidence rate>300/100 000). The overall incidence rate in the northern region was relatively low, with the lowest rate observed in Urumqi, radiating outwards.
Conclusion
The incidence rate of tuberculosis in Xinjiang shows an upward trend before 2018, followed by a year-on-year decrease. The centroid of the incidence rate shifts towards the northeast. From 2010 to 2022, the tuberculosis epidemic in Xinjiang exhibits a notable spatiotemporal clustering, particularly prominent in the southwestern region, where the four prefectures constitute high-risk areas for tuberculosis. The prevention and control efforts of tuberculosis in Xinjiang should prioritize the regions with high tuberculosis incidence, intensifying prevention and control measures as well as policy support.

Objective:To share related knowledge and experiences with countries along the line, literature regarding current cohort studies was summarized. Distribution, establishment and development of cohort studies among large prospective general population were analyzed in 17 countries of Western Asia and the 16 countries of Central and Eastern Europe. Methods:Literature review was conducted to collect basic information on cohort studies, with descriptive study used to analyze the characteristics of these cohort studies. Results:There were 562 cohort studies with sample size as more than 1 000 stated in Western Asia and Central and Eastern Europe, including 468 (83.27 %) carried out in the nation itself and 94 (16.73 %) with international multicentered collaboration. According to the nature of cohort studies, 347 (61.74 %) were etiologically based. As for the contents involved, 310 (55.16 %) of them targeted on chronic/non-communicable diseases, 125 (22.24 %) concentrated on maternal and child health. Among those on chronic/non-communicable diseases, 51 (16.45 %) were on cancers and 83 (26.77 %) on cardiovascular disease studies. There appeared 10 large prospective cohort studies targeting on general population, mainly ongoing in Iran and European countries, with a duration of 8-29 years, including 4 of them with sample size as more than 50 000. In terms of the contents, epidemiological investigation, physical examination and biological samples collection took the major parts. Few papers were published in 9 out of the 10 cohort studies at the early stage of those projects but the number of papers increased annually and stabilized to certain extent. Conclusions:The regional distribution of cohort studies carried out in countries from the Western Asia and Central and Eastern European areas appeared unbalanced. Contents of these designs would mainly involve etiological studies, with focus on non-communicable diseases as cancer, cardiovascular disease, diabetes, respiratory diseases, mental and psychological diseases, and maternal and infant health etc.. However, only few large prospective cohort studies would base on general population.
