1.Urinary calculi composition analysis and individualized prevention instruction

Objective To investigate the calculi composition of patients with urinary calculi in Henan area and the clinical significance of preventing calculi recurrence with individualized method.Methods From August 2009 to July 2010,1050 patients in our hospital underwent extracorporeal shock wave lithotripsy (ESWL) and ureteroscopy and percutaneous nephrolithotomy were set as the experimental group,all stone specimens were detected with the BRUKER TENSOR27 infrared spectroscopy for analysis of stone composition,and nurse on duty gave instructions according to the stone composition to prevent recurrence.From July 2008 to July 2009 1010 patients in our hospital underwent extracorporeal shock wave lithotripsy,ureteroscope and percutaneous nephrolithotomy were set as the control group,patients in the control group were not given calculi component analysis and these patients received general prevention guidance.The calculi recurrence was compared between two groups.Results Among 1050 cases in the experimental group,urinary calculi with single component accounted for 46.29%,of which calcium oxalate stones accounted for 44.95%.Calculi with mixed components accounted for 53.71%,mainly were calcium oxalate and carbonate apatite mixture components (30.48%).57 cases (5.43%) occurred urinary stone recurrence in the experimental group,while 177 cases(17.52%) in the control group.The difference had statistical significance.Conclusions Urinary calculi analysis has important clinical significance for understanding the causes and treatment of calculi as well as prevention of recurrence of calculi.
2.Associations of single nucleotide polymorphisms of UCP genes with diabetic retinopathy in Chinese Han population

Background Researches showed that elevatory blood glucose level results in long-term damage of cells and tissue,or metabolic memory phenomenon,and manipulation of hyperglycemic memory is a good approach in the prevention of diabetic complications.However,its mechanism is not clear.It is speculated that the pathogenesis of diabetic retinopathy (DR) in diabetic patients may be associated to related mechanisms.Uncoupling proteins (UCPs) can decrease the production of reactive oxygen species (ROS),which may be related to DR.Objective This study was to explore the association between DR and the single nucleotide polymorphisms (SNPs) of UCP genes in Chinese Han population with type 2 diabetes.Methods A cross-sectional study was performed.This study was approved by Ethic Committee of Affiliated First Hospital of Shanghai Jiao Tong University and complied with Declaration of Helsinki,and written informed consent was obtained from each subject prior to any medical examination.One thousand eight hundreds and seventy-five patients with type 2 diabetes mellitus were enrolled in Xinjing district of Shanghai city by cluster sampling from November 2014 to January 2015.The demographic and medical baseline characteristics,ocular examination and laboratory tests were obtained and periphery blood of 2 ml was collected for extraction of DNA.Eight tag SNPs of UCP1,three tag SNPs of UCP2,and seven tag SNPs of UCP3 were selected as marker locus for the detection of genotype by Sequenom Mass ARRAY.Matrix-assisted laser desorption ionization-time of flight (MALDI-TOF) mass spectrometry platform were used for genotyping.Hardy-Weinberg equilibrium (HWE) analysis,allele and genotype frequencies,haplotype analysis,and association tests for DR and SNPs were performed by SAS and SHEsis software.Results A total of 530 DR patients were checked out from 1 875 subjects with type 2 diabetes mellitus,with the detection rate of 28.27%.rs660339 locn of UCP2 gene and rs1626521,rs668514 locus of UCP3 gene appeared to have low detectable rates,and the secondary allele base frequency of rs632862 in UCP2 gene was <0.01 and rs15763 of UCP3 gene was unmatched with HWE,therefore,these locus analysis was not included.In 13 SNPs locus included in the analysis,only 2 SNPs of UCP1 gene were related to DR.Compared with the non-diabetic retinopathy (NDR) patients,the G allele frequency of rs10011540 was increased (P =0.03,OR =1.31,95 % confidence interval[CI] =1.03-1.67,and T allele frequency of rs3811787 was decreased (P=0.04,OR=0.86,95% CI=0.75-0.99) in DR patients.Genotyping detection showed that the C/C and A/A frequencies of rs3811790 in UCP1 gene were significantly more and C/A frequency was less in DR patients than those in NDR patients (all at P<0.01).The logistic regression analysis indicated an association of SNPs of rs10011540 and rs3811787 with DR independent from glucose and disease duration.Conclusions The SNPs of rs10011540 and rs3811787 locus in UCP1 gene are associated with DR in Chinese type 2 diabetes patients.
3.A comparative evaluation of the role of musculoskeletal ultrasound in diagnosing symptomatic joint lesions in patients with rheumatoid arthritis and gouty arthritis

Objective To compare the ultrasound signs of symptomatic joint lesions in rheumatoid arthritis (RA) and gouty arthritis (GA), musculoskeletal ultrasound (MSUS) was utilized.Methods A retrospective analysis was performed for 85 hospitalized patients with RA and 55 hospitalized patients with GA in the same period, and the differences in general data, diseased joints and ultrasound signs between the two groups were compared.Re-sults The gender, age and diseased joints of the two groups were statistically significant (all P<0.05).The de-tection rate of knee joint lesions was the highest;the RA group had high sensitivity, high specificity of meniscal in-jury, and high diagnostic efficiency of bone erosion, while the diagnostic performance of the three combined ultra-sound signs of punctate strong echo, double track sign and tophi in the GA group was higher than that of any indi-vidual diagnosis, and the sensitivity and specificity were also higher.The course of disease in the RA group was positively correlated with bone erosion (P<0.05) , and the course in the GA group was positively correlated with tophi (P<0.05).Conclusion The ultrasound signs of RA and GA are different, and MSUS has good value in the diagnosis and differential diagnosis of the two.
4.Technical guideline for intra-prepontine cisternal drug delivery via spinal puncture through subarachnoid catheterization

Objective:The distribution characteristics of intrathecal drugs and the limitation of current catheterization techniques make traditional intrathecal analgesic treatment nearly useless for refractory craniofacial pain,such as trigemina neuralgia.This technical guideline aims to promote the widespread and standardize the application of intra-prepontine cisternal drug delivery via spinal puncture and catheterization. Methods:A modified Delphi approach was used to work for this guideline.On the issues related to the intra-prepontine cisternal targeted drug delivery technique,the working group consulted 10 experts from the field with 3 rounds of email feedback and 3 rounds of conference discussion. Results:For the efficacy and safety of the intra-prepontine cisternal targeted drug delivery technique,a consensus was formed on 7 topics(with an agreement rate of more than 80%),including the principles of the technique,indications and contraindications,patient preparation,surgical specifications for intra-prepontine cisternal catheter placement,analgesic dosage coordination,analgesic management,and prevention and treatment of complications. Conclusion:Utilizing the intra-prepontine cisternal drug infusion system to manage refractory craniofacial pain could provide advantages in terms of minimally invasive,secure,and effective treatment.This application can not only alleviate the suffering of individuals experiencing the prolonged pain but also support the maintenance of quality of life and dignity in their final moments,justifiing its widespread dissemination and standardized adoption in domestic and international professional fields.
5.GPR120 mediates mechanism of protective effect of NF-κB and MAPK in regula-ting LTA-induced Mac-T cells

Lipophosphatidic acid(LTA)was used to stimulate Mac-T cells,and the expression lev-els and phosphorylation levels of key proteins of nuclear factor-κB(NF-κB)and mitogen-activated protein kinase(MAPK)signaling pathway and the expression levels of upstream key action factors TLR4 and MyD88 proteins were detected by Western blot,and EDU assay was used to detect cell proliferation levels and flow cytometry was used to detect apoptosis.The results showed that acti-vation of GPR120 significantly decreased the phosphorylation levels of LTA-induced NF-κB(P65 and IκBα)(P<0.01)and MAPK(JNK,ERK,p38)(P<0.01)in Mac-T cells;inhibition of GPR120 was able to upregulate LTA-induced NF-κB(p65 and IκBα)in Mac-T cells(P<0.01)and MAPK(JNK,ERK,p38)phosphorylation levels(P<0.01);and activation of GPR120 significantly allevia-ted LTA-induced upregulation of TLR4 and MyD88(P<0.01);inhibition of GPR120 significantly exacerbated LTA-induced upregulation of TLR4 and MyD88(P<0.05);LTA stimulation led to a trend of diminished Mac-T cell proliferation and significantly increased apoptosis,whereas activa-tion of the GPR120 gene significantly increased cell activity(P<0.01),promoted cell proliferation and significantly reduced apoptosis(P<0.05)thereby alleviating the damage to Mac-T cells by LTA;LTA stimulation led to a highly significant increase in apoptosis(P<0.01).In contrast,acti-vation of the GPR120 gene significantly reversed the increase in the apoptosis rate of Mac-T cells induced by LTA(P<0.01),while inhibition of the GPR120 gene enhanced the apoptosis-promo-ting effect of LTA(P<0.05),indicating that activation of the GPR120 gene attenuated the in-crease of apoptosis rate caused by LTA-induced inflammatory Mac-T cells.The results suggest that GPR120 can regulate inflammation by mediating TLR4 and MyD88 expression to inhibit NF-κB/MAPK inflammatory pathway activation and can promote cell proliferation.
6.Sealing effect of GuttaFlow2 in curved root canals

Objective:To evaluate the sealing ability of GuttaFlow2 single cone obturation in curved root canals.Methods:(1)Slight,moderate and severe curved 3D printed root canals were made.After instrumentation,these samples were randomly divided into 2 groups(n=10)according to different root canal filling techniques:GuttaFlow2 single cone group(GF2)and AH-Plus warm vertical compation group(WVC).All the samples were sectioned at 2 mm to apex(the apical of the root),4 mm,6 mm to apex(the middle of the root)and 8 mm to apex(the coronal of the root).The filling void areas were ob-served by stereomicroscope,and the percentages of void areas in each section were calculated by ImageJ 1.52a.(2)Forty-eight roots of extracted adult molars were selected.After instrumentation,they were randomly divided into 2 groups(n=8):GF2 group and WVC group.Before root canal filling,the sea-lers were mixed with Rhodamine B dye for visualization under confocal laser scanning microscope.All samples were sectioned at 2 mm to apex(the apical of the root),5 mm to apex(the middle of the root)and 8 mm to apex(the coronal of the root).Then,the percentages of sealer penetration into dentinal tu-bules in each section were calculated.Results:At the apical of the root,there were no obvious voids in slight,moderate and severe curved canals in the GF2 group and the WVC group.There was no signifi-cant difference between the two groups(P>0.05).At the middle of the root,there was no significant difference in the percentages of void areas between the two filling methods,but at the coronal of the root,there were more voids in the slight curved root canal in the GF2 group than in the WVC group,and the difference was significant(P=0.009).The percentages of sealer penetration into dentinal tubules of slight,moderate and severe curved root canals in the GF2 group were 36.10%,55.80%,65.08%,re-spectively.And they were all higher than those in the WVC group(15.78%,20.70%,15.61%),re-spectively,the difference between the two groups was significant(P=0.001).At the middle of the root,the percentages of sealer penetration into dentinal tubules of slight and moderate curved root canals in GF2 group were also significantly higher than those in the WVC group(P=0.001).At the coronal of the root,there was no significant difference between the two groups(P>0.05).Conclusion:Gutta-Flow2 filling technique has the same sealing effect on the root canal at the apical and middle of the root as the warm vertical technique,but has a better sealing effect on the dentin tubules,especially for severe curved root canal.
