2.Clinical Effect of arterial chemotherapy and operation for bladder carcinoma

Objective To evaluate the clinical efficacy of arterial chemotherapy and partial cystotomy for treatment of bladder carcinoma. Methods A total of 171 cases of the bladder carcinoma who would undergo partial cystotomy were randomly diviede into 2 groups:(1)78 patients received arterial chemotherapy before the operation;(2) 93 cases were treated with arterial chemotherapy after the operation.In addition,35 cases who underwent operation alone were selected for the DNA content detection by FCM as control group.All these cases were followed up for 5 years. Results Among the 78 cases with the cystoscopy,the tumors disappeared in 9 cases and the tumors shrank by half or more in 49 cases.The total effective rate was 74.3%(58/78).The pathological examination showed the effective rate was 75.9%(44/58).There was a remarkable change in the carcinoma cells by the electronic microscopy.DNA index,proliferation index,SFR and aneuploid rate were detected by FCM.In the cases of grades Ⅱ and Ⅲ bladder carcinoma there was a statistically significant difference ( P

4.Expression of tissue factor in pulmonary artery after experimental acute pulmonary embolism

Objective To investigate the expression of tissue factor and explore its clinical significances in pulmonary artery after acute pulmonary thromboembolism.Methods Thirty-four Japanese white rabbits (Level Ⅱ animals) were randomly (random number) assigned into four groups:group A (specimen of pulmonary artery was taken 3 hours after pulmonary embolism,n =8),group B (specimen of pulmonary artery was taken 8 hours after pulmonary embolism,n =8),group C (specimen of pulmonary artery was taken 24 hours after pulmonary embolism,n =8) and control group (pseudo-operations were carried out without injecting autologous blood clots,n =10).The animal model of pulmonary thromboembolism was established by injecting autologous blood clots into jugular vein through a 5F catheter and confirmed by digital subtraction angiography.The mRNA expression of TF in different parts of pulmonary artery was assayed by RT-PCR.The q test was utilized if there was a significant difference in a given continuous variable among three groups analyzed by ANOVA.Results The TF expression in the specimen adjacentto emboli was stable at 3 h,8 h or 24 hours after embolism.The mRNA expression of TF at 3 h and 8 h after embolism was lower in specimen taken from distal-end of morbid pulmonary artery than those adjacent to emboli.While at 24 hours after embolism,there were similar mRNA expressions in specimen either adjacent or distal to emboli.Conclusions The high expression of tissue factor in pulmonary artery tissue adjacent to emboli could lead to locally increased coagulation activity,indicating the necessity of initiating anti-coagulation treatment as soon as possible after acute pulmonary embolism.
5.Research on age-related changes in cardiac structure and function using transthoracic high-frequency ultrasonography in Wistar rats

Objective To determine the cardiac structure and function by transthoracic highfrequency ultrasonography in Wistar rats, and to explore the patterns of age-related changes.Methods Male Wistar rats aged 1, 2, 5, 12, and 20 months (n= 12 each group) underwent transthoracic echocardiographic analysis to examine the parameters of cardiac structure and function.Finally, the rats were sacrificed and the left ventricles were weighed. Results The left atrial dimension (LAD), left ventricular end-diastolic diameter (LVEDD), interventricular septum thickness at diastole (IVSD), left ventricular posterior wall thickness (LVPWd) and left ventricular mass (LVM) increased with age (all P < 0.05 ) . There was a positive relationship between echocardiographic value and the autopsy weight LVM (r=0. 78, P<0.01). There were no statistical significances in ejection fraction and fractional shortening among groups (all P>0.05). Isovolumic relaxation time (IVRT) prolonged with age (P<0. 01). After 2 month-old, tissue Doppler imaging Ea gradually decreased with age, Aa tended to increase with age. Ea/Aa ratio was more than 1 value in 1-, 2- and 5-months-old group, and it was less than 1 value in 12-, and 20-month-old group.Multivariate analysis showed that age was the influence factor of LAD, LVEDd and Ea. Conclusions Transthoracic high-frequency ultrasonography can be used to evaluate cardiac structure and function in rats; In aged rats, the wall-thickness, LAD and LVEDd are significantly increased. There is no significant change in systolic function, but diastolic function is decreased.
6.Effect of Chronic Alcohol Consumption on Left Ventricular Myocardial Collagen Content and Diastolic Function in Rats

Objective:To investigate the effect of chronic alcohol consumption on both left ventricular myocardial collagen and diastolic function in rats,and their relationship thereof.Methods:Twenty-four male Wistar rats were randomly divided into 2 groups:control group(n=12)and ethanol group(n=12).The changes in cardiac diastolic function were evaluated by echocardiography and tissue Doppler imaging(TDI).The value of myocardial hydroxyproline content was determined by hydroxyproline reagent kit.The expressions of collagen Ⅰ and collagen Ⅲ mRNA were detected by RT-PCR analysis.Results:It was found that mitral E and mitral annulus Ea were decreased,mitral annulus Aa was increased,and isovolumic relaxation time(IVRT)was prolonged in the ethanol group compared with those in control group(P<0.05).The value of Ea/Aa ratio was greater than 1 in control group and less than 1 in ethanol group(P<0.01).It was found that myocardial hydroxyproline content,collagen Ⅰ,collagen Ⅲ mRNA expression and their ratio significantly increased in ethanol group compared with those in the control group(P<0.01).There was positive correlation between hydroxyproline content,collagen Ⅰ,collagen Ⅲ mRNA expression,and collagen Ⅰ /collagen Ⅲ mRNA ratio with IVRT(P<0.05),and negative correlation between hydroxyproline content,collagen Ⅰ,collagen Ⅲ mRNA expression,and collagen Ⅰ /collagen Ⅲ mRNA ratio with the Ea/Aa ratio(P<0.01).Conclusion:Chronic ethanol consumption can induce increase in left ventricular myocardial collagen synthesis and impairment in diastolic function in rats.Left ventricular diastolic dysfunction correlates with increase in myocardial collagen synthesis positively.
7.Percutaneous mechanical thrombectomy for the treatment of acute limb ischemia

Objective:To analyze the application value of the mechanical thrombectomy system in the treatment of acute limb ischemia.Methods:The clinical data of 50 patients with lower limb ischemia who were treated with the Rotarex mechanical thrombectomy system from Jun 2017 to Sep 2019 were retrospectively analyzed.Results:In 4 cases of popliteal artery rupture occurred during the operation. The success rate of the operation was 92%. Catheter-directed thrombolysis was used in 7 cases, percutaneous transluminal angioplasty was used in 4 cases and percutaneous transluminal angioplasty combined with stent implantation was used in 39 cases. The ankle-brachial index of these 50 patients before and after operation was 0.18±0.24 and 0.64±0.28 respectively ( t=12.87, P<0.001). Treatment was successful in 43 cases. Follow-up ranged from 1 to 24 months, 5 cases were amputated, 2 cases had no improvement of toe ulcer gangrene, 9 cases had thrombus recurrence, and no complications such as bleeding were observed. The primary patency rates at 3, 6 and 12 months were 92%, 84% and 74%, respectively. Conclusion:The mechanical thrombectomy system is safe and effective in the treatment of acute lower limb ischemia with ideal short-term patency.

Objective:To analyze the detection and influencing factors of work-related musculoskeletal disorders (WMSDs) of workers in an automobile manufacturing enterprise in Guangzhou City.Methods:In April 2020, a total of 7065 workers of an automobile manufacturing enterprise in Guangzhou City were selected as research subjects using cluster sampling method. The Nordic Musculoskeletal Questionnaire (NMQ) was used to investigate the detection of WMSDs. Multiple logistic regression analysis was used to analyze the influencing factors of WMSDs.Results:The detection rate of WMSDs was 43.9% (3102/7065) , among which the detection rate of multiple WMSDs was 31.4%. The detection rates of WMSDs in different parts from high to low were as follows: neck (24.5%) , shoulder (21.1%) and ankle (20.1%) . Multivariate logistic regression analysis showed that working experience of 3-8 years, >8 years, age>30 years old, repetitive work every day, multiple repetitions per minute, uncomfortable work posture, frequent overtime work and lack of staff in the department were risk factors for multi-site WMSDs ( P<0.05) . Adequate rest time was a protective factor for multi-site WMSDs ( P<0.05) . Conclusion:The detection rates of WMSDs in neck, shoulder, ankle, and multisite in automobile manufacturing enterprise are relatively high. The primary risk factors include adverse ergonomic factors and unreasonable organization of labour factors, optimize the work breaks can effectively reduce the risk of disease, effective ergonomic interventions should be carried out to prevent the occurrence of WMSDs among workers in the automobile manufacturing industry.

10.Upright Integrated Relaxation Pressure Predicts Symptom Outcome for Esophagogastric Junction Outflow Obstruction

Background/Aims:
Esophagogastric junction outflow obstruction (EGJOO) is characterized by elevated integrated relaxation pressure (IRP) and preserved esophageal peristalsis. The clinical significance of EGJOO is uncertain. This study aim to describe the clinical characteristics of these patients and to find out potential parameters to predict patients’ symptom outcome.
Methods:
Consecutive patients who received high-resolution manometry examination in our hospital in 2013-2019 and met the diagnostic criteria of EGJOO were retrospectively included. Motility and reflux parameters as well as endoscopy and barium esophagogram results were studied and compared. Patients were also followed up to record their treatment methods and symptom outcomes.
Results:
A total of 138 EGJOO (accounting for 5.2% of total patients taking high-resolution manometry examination in our hospital) patients were included. Only 2.9% of these patients had persistent dysphagia. A total of 81.8% of EGJOO patients had symptom resolution during follow-up. Patients with persistent dysphagia had significantly higher upright IRP (16.6 [10.3, 19.8] vs 7.8 [3.2, 11.5]; P = 0.026) than those without. Upright IRP can effectively distinguished patients with persistent dysphagia (area under curve: 0.826; P = 0.026) using optimal cut-off value of 9.05 mmHg.
Conclusion
EGJOO patients with persistent dysphagia and higher upright IRP (median > 9.05 mmHg) needs further evaluation and aggressive management.
