Aim To introduce and apply artificial lumbar interverte-brai disc replacement for the treatment of lumbar disc degenerative diseasesand lumbar disc herniation accompanying evident disc space narrowing andinvestigate the regulation of its recovering spinal motion and carrying a-bility. Methods Thirty-one cases (37 discs) of artificial lumbar disc re-placement were performed using SB Charite Ⅲ from April 1998 to April2000. Among them, disc degenerative diseases were in 16 cases (18discs), disc herniation accompanying evident disc space narrowing in 13cases ( 17 discs), rec urrent dise herniation in 2 cases. The rehabilitationtraining was done under postoperative instructions. Results All the caseswere followed up from 17 to 41 months (averagely 26 months) untilSeptember 2001. The clinical outcomes were excellent in 23 cases, goodin 6 cases, fair in 2 cases. The mobility of the operated level had 4.0°anterior flexion and 5. 1° posterior extension after operation and 9.1° ontotal mobility. Meanwhile, the operated intervertebral space got an average4. 2 mm higher than that before. Because of technical problem, a slightdisplacement of the core occurred in one case without any clinical symp-toms and signs. Conclusion Artificial lumbar disc replacement can re-covery spinal motion and carrying capacity and provides a new kind ofoperation for the treatment of lumbar disc degenerative diseases and discherniation accompanying evident disc space narrowing.

Objective To study the accuracy in placement pediele screws with different method in the selection of entrance point in surgery of scoliosis.Method 27 cases with Lenke Ⅰ type of Adolescent Idiopathic Seoliosis(AIS)from Mar 2006 to September 2008,were retrospectively analyzed.14 eases were randomly divided into standard group(group A)and the entrance point was the same one as described in literatures.13 cases were randomly divided into individually group(group B)and the entran~point wag designed by surgeons on CT scans before surgery. There was no statistical difference in the mean age at time of surgery,Cobb angle on coronal and saggital(T5T12)plane,flexibility and rotational degree of apical vertebra preoperatively between two groups.Result There was no statistical difference in the time of surgery, blood losing, fixation segments, correction rate in Cobb angle and rotational degree of apical vertebra between two groups. The accuracy in placement pedicle screws in individually group was higher than that in control group(P<0.01).Conclusion The accuracy in placement pedicle screws in the surgery of scoliesis can be improved by individual selection of entrance point.

Abstract: Objective This study aimed to investigate the expression of CD73 in colonic tissues in Crohn's disease (CD) and its significance and possible mechanism of action. Methods Thirty male BALB/c mice were randomly divided into normal control group, model group and intervention group. The control group was fed normally, and the model group was treated with TNBS+40% alcohol enema to establish a mouse model of Crohn's disease induced by chronic inflammation. The intervention group was treated with AB-680 intraperitoneally on the second day of each enema based on the model group. Mice body weight, fecal traits and fecal occult blood were recorded for disease activity index (DAI) score of inflammatory bowel disease. The animals were sacrificed at 7th week, their colonic tissues were removed, weighed and measured. The tissue inflammation was observed by standard hematoxylin-eosin (HE) staining. Masson staining was used to measure the area of collagen in colon tissue of mice. CD73 was detected by fluorescence quantitative PCR and immunohistochemistry. The expression levels of TGF-β, TNF-α and IL-6 in colon tissue of mice were determined by ELISA. Results The DAI score was (0.10±0.16) in the control group, (2.80±0.79) in the model group, and (3.07±0.34) in the intervention group. Compared with the control group, the DAI scores of the model and intervention groups were increased significantly (P<0.05). Compared with the model group, the DAI score of the intervention group was significantly increased (P<0.05). HE staining showed that there was no inflammation in the colon of the control group, while the colon of the model group and the intervention group showed typical inflammatory manifestations such as edema and congestion, inflammatory cell infiltration, and mucosal ulcer. The area ratio of collagen in the control group was (4.95±0.82)%, in the model group was (24.62±1.46)%, and in the intervention group was (54.47±2.75)%. Compared with the control group, the area ratio of collagen in the model group and the intervention group was significantly increased (P<0.05). Compared with the model group, the area ratio of collagen in the intervention group was significantly increased (P<0.05). Compared with the control group, the expression of CD73 in colon tissue of the model group and the intervention group was significantly increased (P<0.05). Compared with the model group, the expression of CD73 in colon tissue of the intervention group was decreased (P<0.05). Compared with the control group, the expressions of TGF-β, TNF-α and IL-6 in the model group and the intervention group were significantly increased (P<0.05). Compared with the model group, the expression of TGF-β, TNF-α and IL-6 in the intervention group decreased (P<0.05). Conclusions CD73 is upregulated in colon tissue of CD mice, it can inhibit inflammatory reaction and improve fibrosis by up-regulating TGF-β expression. On the other hand, CD73 can aggravate the inflammatory response in CD intestinal inflammation and fibrosis by up-regulating the expression of IL-6 and TNF-α. Therefore, CD73 may play a bidirectional regulatory role in intestinal inflammation and fibrosis of CD.

Objectives:To investigate the clinical outcome of Smiley face rod fixation system for lumbar isthmic spondylolysis. Methods:From January 2016 to June 2017,18 patients with isthmic spondylolysis were treated with smiley face screw-rod fixation system. There were 13 males and 5 females, with an average age of 28.2±3.2(25-32) years old.The average disease duration was 16.3±5.7(7-24) months.L5 isthmic spondylolysis was involved in all cases. All patients had been followed up at 3 months and 1 year after surgery. The preoperative and postoperative visual analogue scale(VAS) and Oswestry disability index(ODI) were compared. The X-ray and CT scan were evaluated to analyze the change of range of motion in the lumber spine and the bony fusion rate. Results:The mean operation time was 90.0±24.1 minutes. The average blood loss was 140±15ml.The average follow-up was 18.5±5.0 (12-24) months. The VAS at 3 months after operation was 3.0±1.2,and the ODI was (17.2±4.5)% respectively, which showed significant differences compared with the preoperative ones[7.3±2.5 and (67.0±15.1)%](P<0.05).The VAS and ODI at 1 year after operation were 1.0± 0.6 and (9.1±5.3)%,which showed significant differences compared with 3 months postoperation(P<0.05).The range of motion between L5 and S1 was 13.1°±2.1° and the incidence of lumbar instability was 83.3%(15/18) preoperatively;it was 9.3°±1.6° and 11.1%(2/18) postoperatively, and there was statistic difference(P<0.05).All patients achieved bone fusion in the isthmic defect 1 year after operation. Conclusions:The Smiley face rod fixation is a reliable treatment for lumbar spondylolysis with the advantage of minimal invasive, less nerve interference, and to restore normal anatomical structure, maintain the lumbar spine stability.

Online courses are an indispensable part of medical teaching in the new era. Online courses have good prospects, although also with certain problems in practice. As an important basic medical course, medical genetics has both basic theoretical knowledge and clinical cases, involving basic principles and the latest developments. A single online course or offline teaching model cannot meet the needs of subject development and training a new generation of medical professionals. Therefore, actively exploring the online and offline hybrid teaching model is one of the important topics in the current medical teaching reform.
