1.Significance of recovering spinal motion and carrying ability by artificial lumbar intervertebral disc replacement

Aim To introduce and apply artificial lumbar interverte-brai disc replacement for the treatment of lumbar disc degenerative diseasesand lumbar disc herniation accompanying evident disc space narrowing andinvestigate the regulation of its recovering spinal motion and carrying a-bility. Methods Thirty-one cases (37 discs) of artificial lumbar disc re-placement were performed using SB Charite Ⅲ from April 1998 to April2000. Among them, disc degenerative diseases were in 16 cases (18discs), disc herniation accompanying evident disc space narrowing in 13cases ( 17 discs), rec urrent dise herniation in 2 cases. The rehabilitationtraining was done under postoperative instructions. Results All the caseswere followed up from 17 to 41 months (averagely 26 months) untilSeptember 2001. The clinical outcomes were excellent in 23 cases, goodin 6 cases, fair in 2 cases. The mobility of the operated level had 4.0°anterior flexion and 5. 1° posterior extension after operation and 9.1° ontotal mobility. Meanwhile, the operated intervertebral space got an average4. 2 mm higher than that before. Because of technical problem, a slightdisplacement of the core occurred in one case without any clinical symp-toms and signs. Conclusion Artificial lumbar disc replacement can re-covery spinal motion and carrying capacity and provides a new kind ofoperation for the treatment of lumbar disc degenerative diseases and discherniation accompanying evident disc space narrowing.
2.Imaging features of radiation-induced sarcoma in patients with nasopharyngeal carcinoma

Objective To summarize the CT and MRI features of radiation-induced sarcoma (RIS) after radiotherapy in patients with nasopharyngeal carcinoma (NPC).Methods From January 1997 to October 2012,a total of 73 NPC patients with RIS after radiotherapy were confirmed by pathology.The clinical data and imaging findings (CT and MRI findings) were retrospectively reviewed.Of the 73 patients,43 underwent CT examination,24 underwent MRI,and the remaining 6 underwent both CT and MRI scans.Results Fibrosarcoma [45.3% (33/73)] was the most frequently histologic type,followed by osteosarcoma[31.5% (23/73)] and malignant fibrous histiocytoma [9.6% (7/73)].The top three common sites were maxillary sinus [26.7% (20/73)],followed by the neck soft tissue [17.8% (13/73)] and mandible[13.7% (10/73)].The main characteristics of the RIS on CT and MRI were soft tissue masses[78.1% (57/73)] with an irregular shape and ill-defined margin,or rounded masses with welldefined margin [21.9% (16/73)].CT of 49 patients showed masses with isodensity or mixed density on precontrast CT.MRI of 30 patients showed lesions with isointensity signal on T1WI and intermediate signal intensity on T2WI.On post-contrast images,65.8% (48/73) tumors showed markedly homogeneous or inhomogeneous enhancement,23.2% (17/73) lesions showed moderate enhancement,and 11.0% (8/73) masses showed mild enhancement.Among the 23 patients with radiation-induced osteosarcomas,78.3% (18/23) presented tumor bone formation.Conclusions RIS has a characteristic imaging features.Clinical history,tumor sites and serial imaging follow-up are necessary for early detection of RIS in patients with NPC.
3.Research on genetic characteristics in the HA1 of influenza A(H1N1) viruses isolated in Taian City

Objective To analyze the data of influenza A(H1N1) viruses surveillance and genetic characteristics from Taian City during 2005-2008,so a scientific basis can be provided for the prevention and treatment of influenza.Methods The specimens from Influenza-Like Illness(ILI) were collected.The viruses were isolated with MDCK cell and identified with HAI and RT-PCR.The product of PCR were sequenced.Then the sequences were analyzed through biometric software.Results A total of 121 influenza strains were obtained from 615 specimens,and 4 of them were identified as A(H1N1) subtype.There were 3 strains mutated on several sites.Compared with strains isolated in 2005,there were 5 and 8 mutations in the amino acid sequences of virus strains isolated in 2007 and 2008 respectively.And there were a total of 22 amino acid mutations compared with A/Brisbane/59/2007(H1N1).Conclusions Influenza type A(H1N1) are detected in Taian City.There are several mutations in the amino acid sequences of virus strains isolated in Taian. The antigenic drift of virus strains is due to accumulation of amino acid substitutions
4.Wernicke's encephalopathy misdiagnosed as schizophrenia: a case report

This article presents a case report of Wernicke's encephalopathy misdiagnosed as schizophrenia. The patient was hospitalized and diagnosed with schizophrenia at a local psychiatric hospital over a month ago. Twelve days ago, the patient gradually developed symptoms such as salivation, difficulty in eating, trembling hands, unsteady walking, glossoptosis and so on. The above symptoms were interpreted as drug-induced adverse reactions and managed accordingly, whereas the patient showed a poor response to treatment, and began to experience intermittent consciousness disorder. After referral to our hospital, findings were diagnostic for Wernicke's encephalopathy and peripheral polyneuropathy. Due to the progression of the disease, the family members abandoned further treatment and requested discharge from the hospital, and the patient died outside the hospital. This case report aims to improve the prognosis of Wernicke's encephalopathy in patients with mental disorder by raising the awareness of the disease and enhancing the early prevention.
5.The clinical outcome of the treatment of isthmic spondylolysis with Smiley face rod fixation system

Objectives:To investigate the clinical outcome of Smiley face rod fixation system for lumbar isthmic spondylolysis.Methods:From January 2016 to June 2017,18 patients with isthmic spondylolysis were treated with smiley face screw-rod fixation system.There were 13 males and 5 females,with an average age of 28.2±3.2(25-32) years old.The average disease duration was 16.3±5.7(7-24) months.L5 isthmic spondylolysis was involved in all cases.All patients had been followed up at 3 months and 1 year after surgery.The preoperative and postoperative visual analogue scale(VAS) and Oswestry disability index(ODI) were compared.The X-ray and CT scan were evaluated to analyze the change of range of motion in the lumber spine and the bony fusion rate.Results:The mean operation time was 90.0±24.1 minutes.The average blood loss was 140±15ml.The average follow-up was 18.5±5.0 (12-24) months.The VAS at 3 months after operation was 3.0±1.2,and the ODI was (17.2±4.5)% respectively,which showed significant differences compared with the preoperative ones[7.3±2.5 and (67.0±15.1)%](P<0.05).The VAS and ODI at 1 year after operation were 1.0± 0.6 and (9.1±5.3)%,which showed significant differences compared with 3 months postoperation(P<0.05).The range of motion between L5 and S1 was 13.1°±2.1° and the incidence of lumbar instability was 83.3%(15/18) preoperatively;it was 9.3°±1.6° and 11.1%(2/18) postoperatively,and there was statistic difference(P<0.05).All patients achieved bone fusion in the isthmic defect 1 year after operation.Conclusions:The Smiley face rod fixation is a reliable treatment for lumbar spondylolysis with the advantage of minimal invasive,less nerve interference,and to restore normal anatomical structure,maintain the lumbar spine stability.
6.Comprehensive treatment for acute iliac femoral vein thrombosis complicating left iliac vein compression

Objective To explore the significance of comprehensive treatment including anticoagulation,inferior vena cava filter implantation,mechanical thrombectomy,iliac vein angioplasty and stent placement and catheter-directed thrombolysis (CDT) in the treatment of acute iliac femoral vein thrombosis on the basis of left iliac vein compression.Methods In this study 61 patients were enrolled at Beijing Luhe Hospital,Capital Medical University between Mar 2013 and Mar 2019 in anticoagulation combined with CDT group and comprehensive treatment group.Short-term venous recanalization rate scores and long-term proximal valvular function of left femoral vein were compared.The survival curve was drawn using kaplan-meier method,and incidence of PTS was compared with the Log rank test.Results The short-term venous recanalization rate scores of the patients in the comprehensive treatment group were higher than scores of the patients in the traditional treatment group(t =9.872,P < 0.001).16 patients developed PTS.The incidence of PTS in the comprehensive treatment group was lower than that in the traditional treatment group(x2 =7.146,P =0.008).Patients in comprehensive treatment group kept a better function of the proximal valvular of left femoral vein than patients in the traditional treatment group (x2 =23.834,P <0.001).Conclusions The comprehensive treatment can effectively reduce the occurrence of long-term PTS in patients with acute iliac femoral vein thrombosis complicated with left iliac vein compression,and improves the quality of patients' life.
7.Advances in post-operative thyroid scar control

Postoperative thyroid scar not only causes itching, pain, and pulling, but also seriously affects the patient’s aesthetics, so it is important to study the prevention and treatment of postoperative thyroid scar. In the past 20 years, clinicians have made many innovative researches to alleviate postoperative thyroid scar growth, and the following progresses have been made: firstly, thyroid surgery methods and surgical accesses have been improved to a certain extent, resulting in more concealed and less traumatic accesses; secondly, the use of lasers and silicone and other drugs has significantly improved the scar treatment effect; thirdly, the concept of "cosmetic" is gradually gaining popularity, and related technical means are used throughout the treatment of postoperative scarring of the thyroid gland, and multiple treatment modalities are used "early" and "in combination" to "prevent" scar growth after surgery. The use of multiple treatment modalities to prevent scar growth in the early postoperative period has achieved twice the result with half the effort.
