1.Survey on replacement care demands of family caregivers of the aged in Zhengzhou city

Objective To discuss the demands of replacement care service for family caregivers of the aged in Zhengzhou City,so as to provide the reference for replacement care service of the aged.Methods A total of 145 family caregivers of the aged were collected in Zhengzhou city and were investigated with a self-designed questionnaire.Results In the 145 questionnaires,106 were valid.81 (76.4%) of them expressed needing to be replaced in the process of care,and 68 (64.1%) were willing to accept replacement care service.Order of demands of replacement service content was daily life nursing (77,72.6%),disease (66,62.3%) and psychological care (60,56.6%),etc.Influencing factors of selecting replacement care service for family caregivers included quality of service (63,59.4%),service price (38,35.8%) and professional skill (37,34.9%).The degree of demand of different ages,different careers,as well as different duration of daily care of family caregivers was statistically significant for replacement care service.Conclusions The degree of demand of family caregivers of the aged for replacement care service was higher.The government needs to establish replacement care service mechanism which is suitable for China's national conditions through policy guidance and support.
2.Application effect of de-escalation thinking in craniocerebral injury complicated with cervical injury in emergency nursing

Objective The aim of the study was to investigate the application effect of de-escalation thinking in first aid nursing of craniocerebral injury complicated with cervical spine injury.Methods 23 cases of craniocerebral injury and cervical spine injury patients received first aid rescue in emergency department of our hospital during the period from September 2012 to August 2014 were set as the observation group,and the de-escalation thinking way was used during rescue process.19 cases of craniocerebral injury and cervical spine injury patients during the period from September 2010 to August 2012 were set as the control group,traditional way of clinical experience thinking way was used.The early warning time,emergency treatment time and missed diagnosis cases were compared between the two groups.Results The early warning time of diagnosed as traumatic brain injury complicated with cervical spine injury (t=18.035) and emergency treatment time (t=7.650) in the observation group were shorter than those of the control group,the difference was significant.The missed diagnosis rate of the observation group [4.35%(1/23)] was significantly lower than that of the control group (9/19),the difference was significant (x2=8.38).Conclusions Application of de-escalation thinking mode in rescue by emergency nurses can shorten the early warning and emergency treatment time,decrease the missed diagnosis rate and improve the timeliness of the rescue for craniocerebral injury complicated with cervical spine injury.
3.Development of training curriculum in sexual health of breast cancer patients for oncology nurse

Objective To develop a curriculum of a training course for oncology nurses in regard to patient education in sexuality for breast cancer patients. Methods Delphi Method was used in the study. Two rounds of questionnaire investigation were conducted among 21 specialists in breast nursing care, sexology, and psychology. The curriculum was developed after revised based on the comments and suggestions brought up in the experts. Results The training curriculum in sexual health of the breast cancer patients for oncology nurse consisted of 5 sections, professional education, sexology, medicine, psychology and practice, which were divided into 45 teaching topics included in 13 courses. The comments of the specialists on the training curriculum were consistent. The coefficients of variation ranged from 0.07 to 0.20. The coordination coefficient was 0.344. Conclusions The current study developed an integral curriculum for oncology nurses in regard to patient education in sexuality for breast cancer patients, which will benefit oncology nurses for their nursing practice.

5.The impact of hyperthyroidism on erythrocyte indices and serum iron of rats

Objective To study the change of erythrocyte indices and serum iron in hyperthyroidism rats .Methods SD female rats were randomly divided into hyperthyroidism group and control group .Hyperthyroidism rats were induced by intragastric ad‐ministration of Euthyrox (Levothyroxine) and their blood and serum were obtained after administration of 1-8 weeks ,respective‐ly .And the control rats were given normal saline .Levels of T3 ,T4 and TSH were determined by a radioimmunoassay .Erythrocyte index were measured by automatic blood cell analyzer .Serum iron concentration and total iron binding capacity were determined by chromatometry .Results The concentration of serum T3 and T4 increased in hyperthyroidism rats ,whereas the TSH decreased . RBC was higher in hyperthyroidism rats at 2-8 weeks compared with the control rats (P<0 .05) .HCT was higher in hyperthy‐roidism rats at 1-8 weeks compared with the control rats (P<0 .01) .MCV was lower in hyperthyroidism rats at 2-8 weeks com‐pared with the control rats (P<0 .05) .MCHC was lower in hyperthyroidism rats at 1 -8 weeks compared with the control rats (P<0 .01) .RDW SD and RDW CV was higher hyperthyroidism rats at 1-2 weeks compared with the control rats(P<0 .01) .Ser‐um iron concentration and total iron binding capacity increased in hyperthyroidism rats at 4 -8 weeks compared with the control rats(P<0 .01) .Conclusion The hyperthyroidism rats in the present study is absent significant manifestation of anemia ,whereas the hypochromic changes of erythrocyte is significant .Serum iron concentration and total iron binding capacity increase in hyperthy‐roidism rats ,which demonstrate the iron utilization disorder .
6.Analysis of the safety of medication and relevant influence factors among the elderly chronic diseases

Objective To investigate the medication safety and relevant influence factors among the elderly with chronic diseases,to improve the safety of the elderly medication,and provide the theory basis for the rational use of drugs.Methods Questionnaire survey was conducted on 452 elderly outpatients with chronic diseases using self-designed questionnaire.Results 21.7% (98/452) of the elderly took seven or more drugs.The incidence of adverse drug reactions was 38.4% (174/452).The prevalence in the top three were cardiovascular disease [61.3% (277/452)],diabetes [53.5% (242/452)],and lung disease [42.0% (190/452)].Old people had unsafe medication behaviors including missing medication,stopping medication on their own,increased or reduced dosages of medicine and taking overdue medication,and so on.Logistic regression analysis showed that the main influence factors of medication safety were age,previous occupation,educational level,monthly household income per capita,medication time,the source of drugs and whether received medication safety health education (P<0.05).Conclusions Relative lack of safety knowledge,wrong cognition and behaviors exist in the elderly.It is worthy of paying attention to adverse drug reactions and hidden dangers of the elderly.
7.Isolated culture and functional identification of mouse bone marrow derived tolerogenic dendritic cells

Objective:To establish the methods of isolated culture and functional identification of mice bone marrow derived tolerogenic dendritic cells (CD11b+F4/80 +TDCs) in vitro.Methods: Mice bone marrow cells were isolated and cultured to obtain iDCs with the simulation of mouse rmGM-CSF and rmIL-4.CD11b+F4/80 +TDCs were purified by fluorescence-activated cell sorting on day 6.The morphological changes of TDCs were observed with the inverted microscope dynamically .The expression of CD11b+F4/80 +TDCs were analyzed by the flow cytometry .Tolerogenic function of CD11b+F4/80 +TDCs was evaluated by the expression of MHCⅡ, CD83, IDO, TLR-2, IL-10 and TGF-β1.The expression of MHCⅡ was analyzed by the flow cytometry , and the expression of CD83, IDO and TLR-2 were analyzed by immune-histochemistry.The levels of IL-10 and TGF-β1 in the supernatant of CD11b+F4/80 +TDC were analyzed by ELISA .Meanwhile mature DCs ( mDCs) induced by LPS were used as control .Results:The fresh isolated bone marrow cells look like round and small under microscope .After two days of culture , cells became big and formed into clusters . Five or six days later, cells clusters increased, and the morphology of cells became irregular .At the same time, more dendrite ap-peared on the surface of cells .The percentage of CD11b+F4/80 +TDCs induced by rmGM-CSF and rmIL-4 was about 23%, and the purity of the purified BM CD11b+F4/80 +iDC was about 99%.Compared with mDCs, CD11b+F4/80 +TDCs expressed low levels of MHCⅡand CD83 and high levels of IDO, TLR-2, IL-10 and TGF-β1.Conclusion:CD11b+F4/80 +TDCs derived from mouse bone marrow could be induced successfully by rmGM-CSF and rmIL-4 in vitro.CD11b+F4/80 +TDCs showed tolerogenic function by the expressions of IL-10, TGF-β1, IDO and TLR-2.
8.Universal mental intervention for children in school based on mental resilience forced enhancement:a randomized controlled trial study protocol

Abstract
Mental disorders represent 45% of the global disease burden among children and adolescents aged 10-24 years old. It is crucial to develop effective interventions that can be integrated into daily school activities in order to provide convenient and accessible mental health services for students and faculty members. The study conducts a randomized controlled trial to examine universal mental health promotion among children. The current protocol aims to develop a universal resilience focused program, which focuses on emotional regulation, coping, and problem solving. A total of 20 classes from two primary schools in grades 4-5 in Anhui, Tianchang were randomly assigned to either the intervention or waitlist group. This training consisted of 14 weekly 40 minute sessions of manualized interventions that were compatible with regular curricular activities. It is expected that this current intervention will enhance the psychological resilience of children and evaluate the effectiveness of promoting mental health among children followed up on indicators of depressive and anxiety symptoms, psychological resilience, oppositional defiant and conduct problems by questionnaires at baseline and after the intervention.
9.Value analysis of chlamydia trachomatis antigen detection in patients with iubal infertility and its effect on prevention of infection

Objective To investigate the value of Chlamydia trachomatis antigen in patients with tubal infertility and its effect on the prevention of infection. Methods 80 patients with tubal infertility who were admitted in our hospital from January 2016 to October 2017 were selected as the study group and 80 patients with tubal patency by hysterosalpingography in our hospital were selected as the control group. The study group underwent Chlamydia trachomatis antigen detection. The relevant indicators were observed and clinical diagnostic effect and the effect of infection prevention of patients was recorded in detail. Results The diagnostic results showed that there were 77 cases diagnosed in the study group, and the diagnostic coincidence rate was 96. 2%, of which the diagnostic coincidence rates of tubal blockage, genital tract infection, tubal effusion, acute salpingitis were 100. 0%, 96. 1%, 96. 6%, 93. 8%. The positive rate of Chlamydia trachomatis antigen in the study group was 82. 5% (66/80). The positive rate of Chlamydia trachomatis antigen in the control group was 15. 0% (12/80). The positive rate of the study group was significantly higher than that of the control group (P<0. 05). The test results showed that the incidence of abortion, preterm birth, dead fetus, stillbirth, ectopic pregnancy and other symptoms was significantly higher in the positive patients' study group. In the aspect of infection prevention, the number of patients infected at each time period in the study group gradually reduced with the growth of time and the increase of number of infected people. Conclusion The detection of Chlamydia trachomatis antigen in patients with tubal infertility can achieve high coincidence rate of clinical diagnosis, earlier detection of tubal disease in patients and timely treatment measures, reduce the incidence of postoperative infection in patients, which should be further applied in clinical.
10.The establishment of a cell strain expressing HIV Vpr and the study on Vpr induced apoptosis

Objective To establish cell strain expressing the genes of HIV vpr and mutant HIV vpr-FS, and to explore cell apoptosis ability by HIV Vpr and Vpr-FS. Methods The recombinant plasmids were constructed by cloning HIV vpr and HIV vpr-FS genes into the eukaryotic expression vector pcDNA3.1respectively. To determine the primary structures of HIV vpr and HIV vpr-FS, plasmids were cleaved by restriction enzymes. After the plasmids were transfected into HeLa cells by liposome, the HeLa cells were selected with G418 selective medium, mRNA expression of HIV vpr or HIV vpr-FS of transfected cells was detected by RT-PCR, and Vpr and Vpr-FS protein expression were detected by Western blot assay respectively. The DNA content and the percentage of apoptosis in HeLa HIV vpr cell, HeLa HIV vpr-FS cell and HeLa pcDNA3.1 cell were monitored by flow cytometry and the DNA fragmentation was analyzed by agarose gel electrophoresis. Results BamH Ⅰ and Hind Ⅲ cleavaged products of pcDNA3.1-vpr and pcDNA3.1-vpr-Fincluded 342 bp length fragments suggesting that the length of DNA sequence containing HIV vpr (HIV vpr-FS) within pcDNA3.1 was the same as theoretical length. The HeLa cells transfected by pcDNA3.1-vpr or pcDNA3, l-vpr-FS and selected with G418 could express HIV vpr or HIV vpr-FS by RT-PCR, and express HIV Vpr or HIV Vpr-FS protein by Western blot. The results of flow cytometry and DNA fragmentation showed that there was significant different in the number of apoptotic cells between HeLa HIV vpr cell and HeLa HIV vpr-FS cell, but the difference between HeLa HIV vpr-FS cell and control group was not obvious. Conclusion Recombinant plasmids pcDNA3.1-vpr and pcDNA3. 1-vpr-FS were constructed successfully, and the cell strain expressing HIV Vpr and HIV Vpr-FS proteins was established. The HIV Vpr could induce host cell apoptosis, while the mutant of Vpr did not or weakened this ability. This study provides foundation for further study on HIV vpr gene.
