1.Monochromatic energy images from spectral CT in diagnosing X-ray negative biliary stones

Objective To study the role of monochromatic energy images from spectral CT in diagnosing X-ray negative biliary stones.Methods 32 patients who were diagnosed to have X-ray negative biliary stones were retrospectively studied.They were examined by spectral CT scanning and the spectral CT data were loaded into a spectrum analysis software.The optimal contrast-to-noise ratio (CNR) of the monochromatic energy images were exposed and the contrast ratio between the X-ray negative stones and bile on the optimal monochromatic and hybrid energy CT images were compared,respectively.The monochromatic and hybrid energy CT images for diagnosing X-ray negative biliary stones were validated by two senior radiologists based on postoperative histology.Results The corresponding KeV of optimal CNR for X-ray negative stones were not quite consistent.They were 140KeV in 19 patients,40KeV in 8 patients,53KeV,57KeV,62KeV,64KeV and 73KeV in one patient each.The contrast between the negative stones and the adjacent bile were (6.4 ±5.6) HU on hybrid energy CT images and (50.4 ±24.4) HU on optimal monochromatic energy CT images.The Eff-Z of negative stones and bile were 6.6 ± 0.6 and 7.9 ± 0.2,respectively.In our study,the diagnostic accuracy of hybrid energy CT images was 34.38% and that of optimal monochromatic energy CT images was 78.13%.Conclusion The optimal monochromatic energy CT images were more valuable in diagnosing X-ray negative bile duct stones,which were obviously better than the hybrid energy images from traditional CT.
2.Effect of Dabigatran Etexilate on Coagulation Indexes and Safety of Elderly Patients with Persistent Atrial Fi-brillation

OBJECTIVE:To explore the effect of dabigatran etexilate on coagulation indexes and safety of elderly patients with persistent atrial fibrillation. METHODS:52 elderly patients with persistent atrial fibrillation were randomly divided into obser-vation group and control group by random number table,26 cases in each group. Creatinine clearance rate in observation group was higher than 30 ml/min,110 mg Dabigatran etexilate capsule was orally given,bid;when 15-30 ml/min,55 mg capsule was orally given,bid. Control group orally received Warfarin sodium tablet with initial dose of 2.5 mg,qd,according to international normal-ized ratio (INR) after 3-5 days till maintained in 2.0-3.0. After 3 months,coagulation indexes [prothrombin time (PT),activated partial thromboplastin time (APTT),thrombin time (TT),INR,platelet count (PLT),fibrinogen (FIB)] before and after treat-ment,incidences of clinical events (cerebral embolism,cerebral hemorrhage,fatal bleeding,the remaining parts of embolism, death)and adverse reactions in 2 groups were compared. RESULTS:There was no significant difference in PT,APTT,TT,PLT and FIB between 2 groups before treatment(P>0.05);after treatment,PT and INR in 2 groups significantly increased,and obser-vation group was obviously better than control group,the difference was statistically significant(P<0.05). No cerebral hemorrhage or fatal hemorrhage occurred in the two groups. There was no significant difference in the incidence of cerebral embolism between 2 groups (P>0.05);the incidence of remaining parts of embolism,death in observation group significantly lower than control group,the difference was statistically significant(P<0.05). The incidence of adverse reactions in observation group was significant-ly lower than control group,the difference was statistically significant (P<0.05). CONCLUSIONS:Dabigatran etexilate shows good coagulation effect on elderly patients with persistent atrial fibrillation,which can effectively improve PT and INR levels and reduce the incidences of clinical events,with good safety.

4.Analysis of corticomuscular coherence during rehabilitation exercises after stroke.

To better evaluate neuromuscular function of patients with stroke related motor dysfunction, we proposed an effective corticomuscular coherence analysis and coherent significant judgment method. Firstly, the related functional frequency bands in the electroencephalogram (EEG) were extracted via wavelet decomposition. Secondly, coherence were analysed between surface electromyography (sEMG) and sub-bands extracted from EEG. Further more, a coherent significant indicator was defined to quantitatively describe the similarity in certain frequency domain and phase lock activity between EEG and sEMG. Through the analysis of corticomuscular coherence during knee flexion-extension of stroke patients and healthy controls, we found that the stroke patients exhibited significantly lower gamma-band corticomuscular coherence in performing the task with their affected leg, and there was no statistically significant difference between their unaffected lag and the healthy controls, but with the rehabilitation training, the bilateral difference of corticomuscular coherence in patients decreased gradually.

5.Optimization of high pressure machine decocting process for Dachengqi Tang using HPLC fingerprints combined with the Box-Behnken experimental design

Using Dachengqi Tang (DCQT) as a model, high performance liquid chromatography (HPLC) fingerprints were applied to optimize machine extracting process with the Box–Behnken experimental design. HPLC fingerprints were carried out to investigate the chemical ingredients of DCQT; synthetic weighing method based on analytic hierarchy process (AHP) and criteria importance through intercriteria correlation (CRITIC) was performed to calculate synthetic scores of fingerprints; using the mark ingredients contents and synthetic scores as indicators, the Box–Behnken design was carried out to optimize the process parameters of machine decocting process under high pressure for DCQT. Results of optimal process showed that the herb materials were soaked for 45 min and extracted with 9 folds volume of water in the decocting machine under the temperature of 140 1C till the pressure arrived at 0.25 MPa;then hot decoction was excreted to soak Dahuang and Mangxiao for 5 min. Finally, obtained solutions were mixed, filtrated and packed. It concluded that HPLC fingerprints combined with the Box–Behnken experimental design could be used to optimize extracting process of traditional Chinese medicine (TCM).
6.The impact of hyperthyroidism on erythrocyte indices and serum iron of rats

Objective To study the change of erythrocyte indices and serum iron in hyperthyroidism rats .Methods SD female rats were randomly divided into hyperthyroidism group and control group .Hyperthyroidism rats were induced by intragastric ad‐ministration of Euthyrox (Levothyroxine) and their blood and serum were obtained after administration of 1-8 weeks ,respective‐ly .And the control rats were given normal saline .Levels of T3 ,T4 and TSH were determined by a radioimmunoassay .Erythrocyte index were measured by automatic blood cell analyzer .Serum iron concentration and total iron binding capacity were determined by chromatometry .Results The concentration of serum T3 and T4 increased in hyperthyroidism rats ,whereas the TSH decreased . RBC was higher in hyperthyroidism rats at 2-8 weeks compared with the control rats (P<0 .05) .HCT was higher in hyperthy‐roidism rats at 1-8 weeks compared with the control rats (P<0 .01) .MCV was lower in hyperthyroidism rats at 2-8 weeks com‐pared with the control rats (P<0 .05) .MCHC was lower in hyperthyroidism rats at 1 -8 weeks compared with the control rats (P<0 .01) .RDW SD and RDW CV was higher hyperthyroidism rats at 1-2 weeks compared with the control rats(P<0 .01) .Ser‐um iron concentration and total iron binding capacity increased in hyperthyroidism rats at 4 -8 weeks compared with the control rats(P<0 .01) .Conclusion The hyperthyroidism rats in the present study is absent significant manifestation of anemia ,whereas the hypochromic changes of erythrocyte is significant .Serum iron concentration and total iron binding capacity increase in hyperthy‐roidism rats ,which demonstrate the iron utilization disorder .
7.The quality of cleanliness and safety on the bowel preparation with different program of oral polyethylene glycol 4000

Objective To investigate the effect and safety of different polyethylene glycol 4000 (PEG)oral regimen on cleanliness of bowel preparation.Methods 280 patients received painness colonoscopy examination were randomly divided into A,B and C groups:group A which drank 1 L of PEG solution the day before colonoscopy and the rest 3 L in the morning at the day of colonoscopy;group B drank 2 L of PEG solution the day before colonoscopy and the rest 2 L in the morning at the day of colonoscopy;group C drank 4 L of PEG solution in the morning at the day of colonoscopy.Bowel cleanliness after taking the drug,stool frequency and adverse events before colonoscopy were observed.Results Group A had better bowel preparation compare with group B (P<0.05),which was similar to that of group C(P>0.05);But group A had lower incidence of abdominal distension(abdominal pain),nausea(vomiting) than group C(P<0.05);and group B had worse sleep quality and more defecate frequency than the other two groups(P<0.05).Conclusion Drink 1 L of PEG solution the day before colonoscopy and the rest 3 L in the morning at the day of colonoscopy provided a better quality preparation and with less adverse reaction,especially suitable for patients with poor quality of sleep,poor health and severe constipation.
8.Three-dimensional DSA in displaying origin of uterine artery

Objective To explore the value of three-dimensional DSA (3D-DSA) in displaying the location of the origin of uterine artery.Methods A total of 90 female patients underwent uterine artery (UA) embolization were enrolled.The bilateral internal iliac artery catheterization were performed by 3D-DSA,then the images were reconstructed in every 5 degree interval to choose the optimum range of viewing angle.The origination and the degree of the origin artery and UA were calculated.The distance between the origin of UA and superior glutea artery which was identified as the locating point was measured.Results Bilateral and contralateral oblique position of >30°-60°were the optimal projection positions of UA.Totally 64.44% (116/180) of UA originated from the anterior trunk of internal iliac artery,18.33% (33/180) originated from the inferior gluteal trunk,9.44 % (17 / 180) originated from the internal pundenal artery,5.56 % (10 / 180) originated from internal iliac artery,and 2.22% (4/180) originated from the superior gluteal artery;10.56% (19/180) of the angle of the origin artery and UA were 0-30°,38.89% (70/180) were >30°-60°,41.11% (74/180) were >60°-90°,4.44% (8/180) were>90°-120°,2.78% (5/180) were>120°-150°,2.22% (4/180) were>150°-180°.Distance between the origin of UA and superior gluteal artery was 3.04-18.31 mm,average was (11.71±4.28)mm.Conclusion 3D-DSA can clearly display the origination,viewing angle and the distance away from superior gluteal artery.
9.Protective effect of astaxanthin against cognitive impairment in offspring prenatally exposed to maternal epilepsy

Objective To investigate the protective effect of prenatal astaxanthin treatment against cognitive impairment in adult offspring induced by exposure to maternal seizures in utero.Methods Female adult Sprague-Dawley rats were randomly divided into four groups:control group,astaxanthin group,kindling group and kindling+astaxanthin group.Each rat was implanted with electrodes.Those in the kindling and kindling+astaxanthin groups were kindled once a day by electrical stimulation of the amygdala.All rats were allowed to mate after one week's amygdala kindling.Rats in the kindling and kindling+astaxanthin groups continued to be treated with electrical stimulation every 48 hours from gestational day 1 to 20,and those in the astaxanthin and kindling+astaxanthin groups were injected intraperitoneally with 30 mg/(kg · d) of astaxanthin simultaneously.Naturally delivered offspring were raised till 12 weeks of age.Morris water maze test was performed to assess the cognitive function of adult offspring.Changes in the morphology of hippocampus were observed with Nissal's staining and transmission electron microscope.Expression of cyclic adenosine monophosphate response element binding protein (CREB) and brain-derived neurotrophic factor (BDNF) in adult offsprings' hippocampus tissues at protein and mRNA levels were determined using Western-blotting and reverse transcription-polymerase chain reaction.Analysis of variance and LSD test were used as statistical methods.Results Morris water maze test showed that from the 3rd day to the 5th day,the kindling group had significantly longer escape latency [(36.33 ±7.85),(28.80± 8.41),(29.50± 11.72) s] than the control [(28.90±7.46),(17.59±9.12),(10.40±3.69) s] and kindling+astaxanthin groups [(28.30±5.75),(18.37±3.39),(15.23±6.63) s] (F=3.601,9.811 and 14.226,all P<0.05).In probe trials,the kindling group had significantly fewer platform crossings as compared with the control and kindling+astaxanthin groups [(4.40± 1.71) vs (7.20± 1.62) and (6.50±1.84) times,F=6.586,P=0.001].The kindling group spent dramatically less time in the target quadrant than the control and kindling+astaxanthin groups [(27.35±7.63) vs (58.29± 10.48) and (40.41 ± 7.06) s,F=25.825,P<0.001].Nissl staining showed that hippocampal neurons of offspring in the control group were normal,but there was hippocampal damage in the kindling group and the damage was more severe than that in the kindling+astaxanthin group.Electron microscope observation showed that neurons and synapses in the hippocampal CA1 area of offspring in the control group were normal.However,obvious damage to neurons and synapses was induced in the kindling group and that was worse than the damage induced in the kindling+astaxanthin group.Expression of CREB and BDNF protein in the kindling group (0.19±0.06and 0.32 ±0.04,respectively) were significantly lower than those in the control (0.81 ±0.11 and 0.93 ± 0.04,respectively) and kindling+astaxanthin groups (0.60± 0.07 and 0.80±0.06,respectively) (F were 34.015and 71.074,both P<0.001).Moreover,the kindling group showed decreased expression of CREB and BDNF mRNA (0.48 ± 0.11 and 0.43± 0.08,respectively) as compared with the control (1.02± 0.65 and 0.99± 0.09,respectively) and kindling+astaxanthin groups (0.89±0.15 and 0.96±0.13,respectively) (F were 13.447 and 21.912,both P<0.01).Conclusion Treatment with astaxanthin could ameliorate the cognitive impairment and pathological damage in hippocampus of adult offspring induced by exposure to maternal seizures in utero through regulating the CREB-BDNF signal pathway.
10.Expression and diagnostic value of plasma exosomal hsa_circ_0064910 in gastric cancer

Objective:This study aimed to investigate the expression of plasma exosomal hsa_circ_0064910 in gastric cancer (GC) patients, explore its correlation with the clinical pathological characteristics and evaluate its diagnostic efficacy in GC.Methods:Sixty patients with GC and 30 patients with Chronic Gastritis (disease control group) admitted to The First Affiliated Hospital of Henan University from October 2019 to December 2020 were selected. Meanwhile, 30 healthy subjects (healthy control group) who underwent physical examination were also enrolled. General data of GC patients were collected, including tumor size, degree of differentiation, TNM stage, lymph node metastasis, etc. Blood samples were collected before treatment and the expression levels of plasma exosomal hsa_circ_0064910 were detected via quantitative reverse transcription PCR (qRT-PCR). The serum concentrations of traditional biomarker (CEA and CA19-9) were measured via a chemiluminescent detection system. The receiver operating characteristic (ROC) curve and area under curve (AUC) were used to estimate the diagnostic capacity of different index in GC. Then, the expression difference of plasma exosomal hsa_circ_0064910 in GC patients before and after operation was analyzed, and its relationship with clinicopathological features of GC patients was also investigated.Results:RT-PCR results revealed that compared with Chronic gastritis patients and healthy control, the expression of plasma exosomal hsa_circ_0064910 was upregulated in the gastric cancer group(0.47±0.06, 0.43±0.05, 0.97±0.12, all P<0.001). The area under the ROC curve was 0.778, and AUC of the combination of CEA and CA19-9 for the diagnosis of gastric cancer was 0.841. which was higher than the diagnostic accuracies of CEA (AUC=0.673)and CA 19-9(AUC=0.653). The expression level of exosomal hsa_circ_0064910 was also significantly correlated with tumor size( χ2=7.545, P<0.01), TNM stage( χ2=4.571, P<0.05)and lymph node metastasis( χ2=6.907, P<0.01). The postoperative expression levels of exosomal hsa_circ_0064910 were lower compared with those of preoperative levels(1.21±0.21 vs 0.62±0.11, P<0.01). Conclusion:Our data demonstrated that exosomal hsa_circ_0064910 is highly expressed in GC patients and might be a potential noninvasive biomarker for the auxiliary diagnosis of GC.
