Drug resistance is one of the major obstacles for chemotherapy and targeted therapy in lung cancer with complex mechanisms.The researches have demonstrated that epigenetic changes are closely related to drug resistance of tumor.DNA methylation is an important epigenetic modification.In this paper, the relationships between hMLH1 promoter aberrant methylation and platinum-resistance, and DAPK promoter aberrant methylation and EGFR-TKI resistance in lung cancer will be briefly reviewed.

OBJECTIVE:To study the chemical constituents in the fruiting bodies of armillariella tabescens for searching active leading compounds.METHODS:Compounds were isolated from methanol extract of armillariella tabescens by chromatography column.RESULTS:Six compounds were isolated and identified as methyl oleate(1),stearic acid(2),3?-hydroxyergosta-5,7,22-triene(3),armillarisin B(4),5-hydroxy-uracil(5) and cerebroside B(6),respectively.CONCLUSION:Compounds 1,2,3,5,6 were firstly isolated from the fruiting bodies of armillariella tabescens.

Objective To explore the effects of different swimming training intensities on the expression of glial fibrillary acidic protein (GFAP) and basic fibroblast growth factor (bFGF) around a cerebral infarct. Methods The intraluminal thread method was applied to establish left middle cerebral artery occlusion (MCAO) for 2 h in 180 rats.They were then reperfused for 3,7 and 14 days.The rats were divided into four groups ( Ⅰ -Ⅳ) according to the intensity of the swimming training they were required to do,plus a control group and a sham operation group.The rats in training group Ⅰ swam 5 min,once daily; group Ⅱ trained for 5 min twice daily; group Ⅲ swam for 10 min once daily and group Ⅳ trained for 10 min twice daily.Neurological function was evaluated using Bederson's test,the cerebral infarction volume was calculated by tripeny tetrazolium chloride (TTC) staining.The expression of GFAP and bFGF around the infract were detected using immunohistochemistry.ResultsThe average Bederson scores of the exercise training groups on the 7th and 14th days were significantly lower than that of the control group.The infarction volumes of each exercise training group were significantly lower than that of the control group,and cells positive for GFAP and bFGF in all training groups were significantly more numerous on the 3th,7th and 14th day after MCAO,especially in training group Ⅳ.Conclusion Intensive exercise can adjust the expression of GFAP and bFGF and promote the repair of brain damage after cerebral ischemia and reperfusion.

Objective To evaluate the effect of dexmedetomidine on inflammatory response and the changes of serum inflamma-tory mediator caused by cardiopulmonary bypass(CPB)in cardiac valve replacement surgery.Methods 30 patients scheduled for cardiac valve replacement were randomly divided into two groups:experimental group(n=15)and control group(n=15).For the experimental group,dexmedetomidine was infused intravenously with the dose of 2.0μg·kg-1 ·h-1 after induction of anesthesia. In control group,normal saline was given instead of dexmedetomidine.Blood samples were taken from radial artery in each of the following five time points:after induction of anesthesia(T1 ),the beginning of CPB(T2 ),after beginning of CPB 40 min(T3 ),after the termination of CPB 2 h(T4 )and 24 h(T5 )for determination of TNF-α、IL-6、IL-10.Results The concentration of TNF-α、IL-6、IL-10 levels showed no significantly difference between the two groups before CPB(P>0.05).But serum TNF-α、IL-6 levels of ex-perimental group were significantly lower and IL-10 were significantly higher than control group(P<0.05 or P<0.01)at the time points of T3~T5 .All biomakers rose significantly at the time of T3 and T4 in both groups(P<0.05 or P<0.01).At the time of T5 IL-6、IL-10 returned to preoperative levels(P>0.05)while TNF-αremained heigh level(P<0.05 or P<0.01)in both groups. Conclusion dexmedetomidine can regulate the release of the serum inflammatory mediator which suggest that dexmedetomidine may alleviate the systemic inflammatory reaction resulting from cardiopulmonary bypass in cardiac valve replacement surgery.

Objective To investigate the incidence of anxiety in patients with breast cancer,and further explore the surface-enhanced laser desorption/ionization time-of-flight mass spectrometry (SELDI) technology' s clinical value in the diagnosis of anxiety.Methods 121 breast cancer patients were selected as research objects by using convenience sampling method.Self-rating anxiety scale (SAS) was used to evaluate the anxiety on the first day of admission and SELDI was used on the next day to detect the serum from patients' fasting venous blood sample got in the morning after their consent,then anxiety-related protein fingerprints spectrums were selected.The SAS score and SELDI result were finally compared and analyzed.Results SAS score in 121 patients was 53.45±9.78,anxiety occurred in 63 cases (52.07 ％).Established the diagnostic model between 15 000+H and 16 800+H protein fingerprints abundance ≥5 ％,that was to say,abundance ≥5 ％ was judged as positive,otherwise negative.On this basis,patients can accurately be distinguished between anxious group and non-anxious group.Correct rate (total coincidence rate) was 91.74 ％,specificity and sensitivity respectively were 89.66 ％ (52/58) and 93.65 ％ (59/63),the positive predictive value was 90.77 ％ (59/65),and negative predictive value was 92.86 ％ (52/56).Conclusions Breast cancer patients have a high incidence of anxiety.SELDI technology has showed high sensitivity and specificity in anxiety detecting.As an objective assessment tool,it could have better prospects for clinical use.

Objective To investigate the modulation of affective pictures on experimental tonic cold and heat pain in a behavioral study. Methods Fifteen healthy female subjects went through the tonic cold and tonic heat stimulation when watching the affective pictures in a counter-balanced order after the pictures were grouped into the pleasant, the unpleasant and the neutral picture sets. The pain tolerance time ( PTT ) was recorded.Results The tolerance time was prolonged when watching the pleasant and unpleasant pictures contrast to thatwhen watching the neutral ( Unpleasant ( 146.20 ± 13.68 ) s, Neutral ( 86. 73 ± 12.93 ) s, Pleasant ( 145.27 ±15.99 ) s, F(2,42) = 5.718, P ＜ 0.01 ) ). However, no significant difference was observed in the heat pain test under the different experimental conditions ( Unpleasant ( 74.73 ± 15.78 ) s, Neutral ( 68.80 ± 15.85 ) s, Pleasant ( 78.80± 14.81 )s, F(2.42) =0. 109, P＞0.05) ). A correlation between the residual difference of arousal in pleasant and neutral picture sets and the residual difference of heat PTT in pleasant and neutral picture sets was investigated (r = 0.546, P = 0.035 ). Conclusion Besides the emotional valence,the arousal may be one of the key elements that modulate the tonic pain.

Objective To investigate the drug utilization of hospitalized children in psychiatric hospital and analyze its the rationality.Methods A retrospective study was conducted to survey the utilization of psychotropic drugs in hospitalized children between January 2016 and June 2016,and according to drug instruction,its the rational-ity was analyzed.Results The top three used psychotropic drugs were aripiprazole,sertraline and sodium valproic sustained -release tablets.The types of drugs without safety data were at most and most frequently used.Diagnosis was not consistent with indications for totally 12 kinds of drugs,aripiprazole involved the most cases.A total of 5 drugs existed usage not matching drug instruction,and sodium valproic sustained -release tablets accounted for the most. Conclusion Irrational use of drugs in psychiatric hospital is still common at present.Clinical pharmacists can improve rationality of drug utilization of hospitalized children in psychiatric hospital by creating a drug database and carrying out pharmaceutical care.

Objective To investigate the expression of Smac/DIABLO, XIAP mRNA in acute pancreatitis (AP) and the relationship with the severity in rats.Methods Fifty-four SD rats were randomly divided into three groups:sham-operation (SO) group, acute edematous pancreatitis (AEP) group and acute necrotizing pancreatitis (ANP) group.The models of AEP and ANP were induced by retrograde injection of 1％ and 3.5％ sodium deoxycholate into the pancreaticobiliary duct respectively.The specimens of pancreatic tissue at 3 h, 6 h, 12 h were collected, pathological changes of the pancreas were observed, apeptosis in pancreas were detected by TUNEL method and the expression of Smac/DIABLO, XIAP mRNA were analyzed by real-time PCR.Results Pathological changes of the pancreas confirmed the establishment of AEP and ANP.Apeptosis indexes in SO group, AEP group and ANP group were 0.67±0.82, 6.62 ±0.78 and 4.70 ±0.82, and the differences were significant (P< 0.05).The expression of Smac/DIABLO mRNA of AEP group increased with time, while the expression of ANP group decreased with time.Compared with SO group, Smac/DIABLO mRNA expressions at 6 h in AEP and ANP group were 2.41 ± 0.92 and 1.47± 0.53, and the differences were significant (P<0.05).By contrast, the expressions of XIAP mRNA in AEP group decreased with time,while the expressions in ANP group increased with time.The expressionsof XIAP mRNA at 6 h in AEP and ANP group were 5.51 ± 1.07 and 6.99 ± 1.00, and the differences were significant (P<0.05).Conclusions In acute pancreatitis, the expression of Smac/DIABLO mRNA was consistent with the apoptosis of pancreatic acinar cells, but not consistent with the severity of pancreatitis.The expression of XIAP mRNA was consistent with the severity of pancreatitis.Smac/DIABLO, XIAP mRNA is associated with regulation of apoptosis.

Objective To study the value of transfer constant(Ktrans)derived from dynamic contrast-enhanced MRI (DCE-MRI) for quantitative evaluation of Ki-67 labeling index (Ki-67 LI) in glioma. Methods Twenty patients with glioma who underwent DCE-MRI and operation were retrospectively reviewed. The Ktrans value and Ki-67 LI were acquired and correlated using the Spearman correlation test. Also, the Ktrans values were compared between high(larger than 10%)and low(no more than 10%)Ki-67 LI group with Mann-Whitney U test, receiver operating characteristic curves was performed to evaluate the diagnostic value. Results The Ktrans value(0.0165 to 0.8048, median 0.1252)was significantly associated with Ki-67 LI(5%to 50%, median 20%) (r=0.721,P<0.001), and the Ktrans value was significantly higher in high Ki-67 group(0.0810 to 0.8048, median 0.1810)than that in low Ki-67 LI group(0.0165 to 0.1456, median 0.0697)(Z=-3.209, P=0.001). The most predictive Ktrans value differentiated high Ki-67 LI and low Ki-67 LI with an area under the curve(AUC) of 0.945 at a sensitivity of 92.3% and specificity of 85.7%. Conclusion Ktrans value could be used for quantitative evaluation of Ki-67 LI in glioma.
