1.Comparison of catalpol and 5-hydroxymethyl-furfural in Radix Rehmanniae at different processing times

Objective:To approach the determination method of 5-hydroxymethyl-furfural in Radix Rehmanniae,and to investigate influence of steam processing on catalpol and 5-hydroxymethyl-furfural. Methods:Catalpol and 5-hydroxymethyl-furfural from Radix Rehmanniae at different processing time was determined by high performance liquid chromatography. Results:In the steam processing,content of 5-hydroxymethyl-furfural increased with the increase of time in the beginning,when steamed to 52h,the levels began to decline; content of catalpol reduced with increase of the processing time. Conclusion:Content of catalpol and 5-hydroxymethyl-furfural changed signifi cantly in the steam processing.
2.Total mesorectal excision in radical operation of rectal carcinoma:a report of 126 cases

Objective To evaluate the therapeutic effect of total mesorectal excision in radical operation of rectal carcinoma.Methods From 1997 to 2007,126 consecutive patients with middle and inferior rectal carcinoma underwent surgical operation according to the principle of total mesorectal excision.Results Two patients(1.6%) died of postoperative pulmonary infection,and postoperative anastomotic leakage occurred in 2 cases(1.6%).The postoperative follow-up of 1-5 years showed that local recurrence rate 2 years after surgical operation was 4%(5/126),the 3-,5-year survival rate was 93.6%(116/124) and 84.7%(105/124),respectively.Conclusions Total mesorectal excision can decrease the frequency of postoperative local recurrence of rectal cancer without increasing complications as long as its correct indications are selected.
3.Content Determination of Hypericin in Guanye Jinsitao Drop Pills by HPLC

Objective To build a method for content determination of Hypericin in Guanye Jinsitao drop pills by HPLC.Methods The column was Kromasil C18 (150 mm×4.6 mm, 5μm), mobile phase was acetonitrile-0.1% phosphoric acid (16∶84), UV detection was performed at 360 nm, the flow rate was 1.0 mL/min, and the detector drift tube temperature was set at 30℃.Results The calibration curves were linear in the range of 0.132-1.188μg for Hypericin (r=0.999 9). The average recovery was 99.55% (RSD=0.78%).Conclusion The method is simple, accurate, reliable, and can be used for the quality control of Hypericin in Guanye Jinsitao drop pills.
4.Perioperative intravenous nutritional treatment on the cellular immune function in patients with gastric carcinoma

5.Research Progress in Active Constituents of Polysaccharides and Flavones from Herba Hedyotis Diffusae

Herba Hedyotis Diffusae has many kinds of pharmacological activities such as anti-tumor, antioxidant, immune regulation and so on. This article collected and reported the related literature at home and abroad in recent years, found that pharmacological active ingredients of Herba Hedyotis Diffusae show consistency of polysaccharides and flavonoids to varying degrees, and summarized the pharmacological effects and extraction and purification process. The author believed that, the study about Herba Hedyotis Diffusae has made relatively great achievements, but the research mainly focused on polysaccharides or flavonoids, and efficacy of Herba Hedyotis Diffusae was not in enough in-depth exploration. The indicator of extraction and purification process is relatively single. This study estimated the mechanism of Herba Hedyotis Diffusae from the aspects of absorption and metabolism of intestinal flora and relationship between medicine structure and exert efficacy, hoping to optimize the extracting processes by multiple indexes, and provided references for further exploitation and utilization of Herba Hedyotis Diffusae.
6.Study on the Metabolism of Hedyotis Diffusa Flavonoid Extract by Intestinal Bacteria in Vitro

Objective To study and compare the metabolism of Hedyotis diffusa flavonoid extract in intestinal flora from human, normal rats and pseudo germ-free rats in vitro. Methods Intestinal flora of human, normal rats and pseudo germ-free rats were incubated with Hedyotis diffusa extract, rutosid, isoquercitrin and quercitrin under anaerobic conditions at 37℃.High-performance liquid chromatography with diode-array detection ( HPLC-DAD ) was used for the analysis of components and metabolic products.The trends of metabolic transformation of the various components were analyzed according to the changes in chromatographic peak areas at different incubation time points. Results The components of Hedyotis diffusa extract and its monomer were quickly metabolized by human and normal rat intestinal flora and the main metabolite was quercetin.However, they were hardly metabolized by intestinal flora from pseudo germ-free rats. The metabolism effect of intestinal flora was weaker on components than on the corresponding monomers. Conclusion By comparing flavonoids metabolism in intestinal flora from human, normal rats and pseudo germ-free rats, the important role of intestinal microflora in the metabolism of flavonoids is further confirmed.
7.Drug Counseling in Our Hospital:Comprehensive Analysis of 483 Cases

OBJECTIVE:To promote rational drug use and improve medical quality.METHODS:A comprehensive analysis was performed on the contents of drug counseling problems presented by 483 patients and medical staff.RESULTS:The use of cardio-cerebral-vascular drugs,anti-bacterial drugs and digestive system drugs were the most common drugs inquired by patients,and their questions focused on the action and clinical use,adverse reactions,administration and dosage etc of drugs.However,anti-bacterial drugs were the leading drugs inquired by medical staff,and their questions focused on drugs available for choice and combined use of drugs etc.CONCLUSIONS:Drug counseling can help promote rational drug use,improve medical quality and patients' awareness toward medical care,besides,it is conducive to the publicity for hospital pharmacists.
8.Arsenic trioxide induces multiple myeloma cell apoptosis viadisruption of mitochondrial transmembrane potentials and activation of caspase-3

Objective To investigate the response of multiple myeloma (MM) cells to arsenic trioxide (As2O3) and their possible mechanisms. Methods Two MM-derived cell lines RPMI8226 and U266 cells were used as in vitro models. Cell apoptosis was assessed by morphology, flow cytometry, and DNA gel electrophoresis. Mitochondrial transmembrane potentials (△Ψm) were evaluated by measuring cellular Rhodamine 123 staining intensity. Protein expression was analyzed using Western blot. Results Zero point one to 0.5?μmol/L As2O3 inhibited cell proliferation and 2.0?μmol/L As2O3 induced cell apoptosis, while 1.0?μmol/L As2O3 inhibited proliferation with a weak degree of apoptosis induction in RPMI8226 and U266 cell lines. As2O3-induced apoptosis was accompanied by mitochondrial transmembrane potentials (△Ψm) collapse and caspase-3 activation in the presence of intact membrane. Glutathione depleter buthionine sulfoximine enhanced, while disulfide bond-reducing agent dithiothreitol partially antagonized As2O3-induced △Ψm collapse and apoptosis in MM cells. All-trans retinoic acid (ATRA) could also induce apoptosis in RPMI8226 cells, but it did not show any cooperative effects with As2O3. Conclusion As2O3 exerts apoptosis-inducing and growth-inhibiting effects on MM cells, and mitochondrium is a pivotal and common target of As2O3 for apoptosis induction.
9.A prospective before-after self-control study of the scenario simulation teaching of junior nurses in a burns department on the fluid resuscitation care of massive burn patients during shock stage

Objective:To explore the effects of scenario simulation teaching of junior nurses in a burns department on the fluid resuscitation care of massive burn patients during shock stage.Methods:From March 2018 to July 2019, 25 junior nurses fixed in the Department of Burn of Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine met the inclusion criteria and were recruited in this prospective before-after self-control study, including 21 females and 4 males, aged 23 to 26 years. They had a work experience of 1.0 (1.0, 2.0) years in the Department of Burn. Three teaching scenarios were made according to the three major problems in the fluid resuscitation nursing of extensive burn patients during shock stage. The teaching plans were written based on the theoretical framework of Jeffrie's nursing simulation teaching. The high feasibility and practicability of the teaching plans was verified through expert consultation and evaluation by junior nurses. According to the three scenarios in the teaching plan, the junior nurses were performed with scenario simulation teaching, with 2 hours for each scenario, 6 hours in total. In one month before and after the teaching (hereinafter referred to as before and after teaching), the comprehensive ability of junior nurses was evaluated by the teachers using the self-designed junior nurse comprehensive ability evaluation scale, and their behaviors were evaluated by themselves with Performance Self-rating Scale for Nursing Students on High Simulation Training. Data were statistically analyzed with paired sample t test. Results:After teaching, the scores on communication skills, patient admissions, condition assessments, fluid resuscitation, emergency treatment, drug management, nursing skills, instrument use, teamwork, and humanitarian care and total score in the comprehensive ability evaluation of junior nurses as evaluated by teachers were significantly higher than those before teaching ( t=-8.060, -11.421, -17.232, -24.800, -4.670, -11.916, -12.131, -11.606, -14.632, -13.325, -25.068, P<0.01), with 4 items including fluid resuscitation, condition assessment, nursing skills, and equipment operation having higher scores. After teaching, the nurses' self-rating scores on communication, division of labor and cooperation, clinical decision-making, professional knowledge and skills, and professional development and critical thinking and total score were (4.6±0.4), (4.8±0.3), (4.6±0.3), (4.6±0.4), (4.6±0.4), and (23.2±1.5) points respectively, which were significantly higher than the corresponding scores of (4.1±0.7), (4.2±0.7), (3.9±0.5), (3.7±0.6), (3.9±0.5), and (18.8±2.6) points before teaching ( t=-3.361, -5.721, -6.987, -7.923, -7.922, -7.367, P<0.01). Conclusions:With solving the practical problems in nursing works of fluid resuscitation care of massive burn patients during shock stage as entry point, scenario simulation teaching aims to train the clinical practice ability and adaptability of junior nurses, thereby improving junior nurses' comprehensive ability of clinical practice in the fluid resuscitation care of massive burn patients during shock stage, enhancing their self-confidence and critical thinking ability.
10. Duration time and effectiveness of 0.25%ropivacaine for sciatic nerve block in patients with diabetes

AlM: To compare the duration of 0.25% ropivacaine used for sciatic nerve block between type 2 diabetic patients and non-diabetic patients, and to explore the factors affecting the duration of nerve block. METHODS: Sixty eight patients with unilateral lateral malleolus fracture who were to be treated with open reduction and internal fixation were selected from January 2021 to January 2022, aged 20-80 years old, ASA I-III, including 28 diabetic patients and 40 non-diabetic patients. All patients were given 0.25% ropivacaine 20 mL to the superior popliteal sciatic nerve under the guidance of ultrasound. The onset and duration of sensory block were evaluated by blunt needle stimulation. The onset and duration of motor block were evaluated by dorsiflexion and plantar flexion of the operated foot. The interval between the end of the operation and the patientls first request for analgesia was taken as the duration of nerve block analgesia. RESULTS: Compared with non-diabetic patients, the duration of sciatic nerve sensation, motor block and analgesia in diabetic patients were prolonged (P < 0.05). There was no significant difference in the onset time of sciatic nerve sensation and motor block between the two groups (P > 0.05). Linear regression analysis showed that diabetes mellitus, duration of diabetes mellitus, fasting blood glucose and glycosylated hemoglobin were factors affecting the duration of nerve block, and fasting blood glucose was not related to the duration of analgesia. CONCLUSlON: 0.25% ropivacaine can prolong the duration of sciatic nerve block in diabetic patients. The duration of diabetes, diabetes, fasting blood glucose and glycosylated hemoglobin are positively correlated with the duration of block.
