1.Effects of precautionary nursing on preventing adverse reaction of rhG-CSF injection in patients with femoral head necrosis
Chinese Journal of Modern Nursing 2017;23(28):3591-3593
Objective To explore the effects of precautionary nursing on preventing adverse effects (i.e. fever, headache, nausea/vomit) of recombinant human granulocyte colony stimulating factor (rhG-CSF) injections in patients with femoral head necrosis. Methods Eighty patients with femoral head necrosis who received autologous stem cell transplantation and rhG-CSF in Zhejiang Provincial Hospital of TCM from June 2015 to March 2016 were selected into this study using convenience sampling. According to the random number table, the participants were divided into the intervention group (n=40) and the control group (n=40). Based on the conventional nursing care, the intervention group received additional predictive and targeted nursing intervention. The incidence of adverse drug reaction was compared between two groups. Results The intervention group found significantly fewer cases that had headache (2 vs. 9 cases), fever (4 vs. 13 cases), and nausea/vomit (9 vs. 23 cases) compared with the control group (χ2=5.165, 6.050, 10.208;P<0.05).Conclusions The precautionary nursing intervention can significantly relieve the adverse drug reaction of rhG-CSF and may benefit the stem cell transplantation in patients with femoral head necrosis.

Objective To inves1tigate the needs and influencing factors of patients with cerebrovascular disease and their caregivers in rural areas. Methods An online questionnaire designed by World Stroke Organization for stroke patients and their caregivers was used, and modified appropriately according to China's national conditions.A questionnaire survey about general situation, and needs for psychological support,stroke-related knowledge, diagnosis and nursing, social support, and functional recovery in inpatients with cerebrovascular disease and their caregivers in rural areas was performed. Results A total of 514 patients with stroke and their caregivers (n =514) completed the questionnaires.93.0% of the patients had the needs for mental support, followed by clinical diagnosis and nursing (84.8%),stroke knowledge (74.7%),social support and functional recovery (53.5%);95.1% of the caregivers had the needs for mental support, followed by stroke knowledge (89.9%),clinical diagnosis and nursing (84.0%),social support, and functional recovery (66.3%).The degree of need for stroke knowledge, social support, and functional recovery in caregivers was higher than that in the patients (all P < 0.05).Multiple linear regression analysis showed that age, gender, degree of education, the National Institutes of Health Stroke Scale score, and the time from the first stroke onset were the influencing factors of needs for patients with cerebrovascular disease and their caregiver. Conclusions Many needs have not been met in patients with cerebrovascular disease and their caregivers, especially the psychological demand. The regional and individual services should be provided according to the different demands in patients with cerebrovascular disease and their caregivers, enhance the awareness of the prevention and treatment of stroke, and continuously improve the construction of rural health service system.

Objective:To explore the clinical efficacy of targeted interventional embolization therapy by a multidisciplinary team for the hemorrhagic shock caused by acute pelvic fracture or/and acetabular fracture. Methods:A retrospective study was conducted to analyze the data of 63 patients with hemorrhagic shock caused by pelvic fracture or/and acetabular fracture who had been admitted to Department of Orthopaedics, The Second Hospital of Shanxi Medical University from January 2015 to July 2022. There were 44 males and 19 females with an age of (39.6±15.6) years, and 23 pelvic fractures, 35 acetabular fractures, and 5 pelvic and acetabular fractures. The time from injury to targeted interventional embolization therapy was 2.67 (2.00, 3.33) hours. All the patients were treated with targeted interventional embolization therapy by a multidisciplinary team involving orthopedics, interventional medicine, general surgery, and urology. The shock index and lactate level within 12 hours after therapy, 24-hour urine output, and incidence of complications 3 weeks after therapy were recorded. Results:No bleeding was found again in the 63 patients after embolization. Within 12 hours after therapy, the shock index was ≤1.0, indicating the shock was corrected. Within 12 hours after targeted interventional embolization therapy, the shock index (0.70±0.46) and lactate value [(2.03±1.35) mmol/L] in the 63 patients were significantly lower than those before therapy [(1.76±0.56) and (4.53±1.74) mmol/L] ( P<0.05). The 24-hour urine output [(50.26±20.38) mL/h] was significantly higher than that before therapy [(21.56±1.27) mL/h] ( P<0.05). Two patients experienced poor blood circulation in the distal skin of the great toe, which was relieved after treatment with blood circulation promotion and anticoagulation. Three patients developed necrosis of the hip soft tissue, which was cured after multiple times of debridement and anti-infection treatments. One patient with severe injury died from multiple organ dysfunction. Conclusions:The targeted interventional embolization therapy can not only diagnose the bleeding location in patients with hemorrhagic shock caused by pelvic fracture or/and acetabular fracture, but also timely and accurately carry out hemostatic treatment to correct shock. Moreover, a multidisciplinary team can help patients avoid multiple surgeries and decrease their pain and financial loss.

Objective To investigate the correlation between the characteristics of peritumoral edema and the aggressiveness of breast invasive ductal carcinoma in preoperative magnetic resonance imaging(MRI).Methods A total of 79 patients(79 lesions)who underwent radical mastectomy in the First Affiliated Hospital of Ningbo University from January 2020 to May 2021 and were pathologically diagnosed as invasive ductal carcinoma were included in invasive ductal carcinoma group, and 45 patients(49 lesions)with benign breast lesions were included in benign lesion group during the same period. The difference of peritumoral edema between two groups and the relationship between different pathological features of invasive ductal carcinoma and peritumoral edema were compared. Results The peritumoral edema in benign lesion group was significantly less severe than that in invasive ductal carcinoma group(χ2=25.330,P<0.05).The tumor size of invasive ductal carcinoma group was positively correlated with the degree of peritumoral edema(r=0.381,P<0.05).There were significant differences in molecular type, histological grade,T stage, lymph node metastasis and Ki-67 expression level among patients with different peritumoral edema grades(P<0.05).Ki-67 expression level and the number of lymph node metastasis were positively correlated with the degree of peritumoral edema(r=0.348,0.273,P<0.05).Conclusion The degree of peritumoral edema in MRI correlates with the aggressiveness of breast invasive ductal carcinoma and can be used as one of the tools to evaluate breast carcinoma.

The p21 activated kinase 4 (PAK4) is serine/threonine protein kinase that is critical for cancer progression. Guided by X-ray crystallography and structure-based optimization, we report a novel subseries of C-3-substituted 6-ethynyl-1H-indole derivatives that display high potential and specificity towards group II PAKs. Among these inhibitors, compound 55 exhibited excellent inhibitory activity and kinase selectivity, displayed superior anti-migratory and anti-invasive properties against the lung cancer cell line A549 and the melanoma cell line B16. Compound 55 exhibited potent in vivo antitumor metastatic efficacy, with over 80% and 90% inhibition of lung metastasis in A549 or B16-BL6 lung metastasis models, respectively. Further mechanistic studies demonstrated that compound 55 mitigated TGF-β1-induced epithelial-mesenchymal transition (EMT).
