1.Analysis of Seasonal Variations in The Incidence of Primary Acute Cerebral Infarction

Objective Investigating the relationship between the incidence of Primary Acute Cerebral Infarction (PACI) and seasonal variation to employ measures to prevent PACI with climate change. Methods A retrospective analysis of patients diagnosed with PACI between 2009 and 2013 in the department of Neurology of Second hospital of Tian Jin Medi-cal University (n=1 198 patients) was carried out. Combined with the general clinical data, we analyzed seasonal, monthly and weekly variation among PACI incidents. Results The incidence of PACI increases over years between 2009 to 2013 (P < 0.01). Significant difference of incidents of PACI was observed between each season (P=0.047). Incidence of PACI peaked in winter(30.33 ± 9.63/month), while bottomed in spring(21.83 ± 5.36/month). Significant difference of incidents of PACI was also observed between each months(P=0.010). The monthly incidence was highest in January and February (33.25 ± 9.62/month)and lowest in March and April(20.75 ± 4.89/month). The seasonal variation was only found in the pa-tients who are complicated with pulmonary infection (P<0.01) regardless of the presence or absence of other risk factors, such as smoking, drinking ,hypertension, coronary heart disease, and diabetes mellitus. The weekly variation of PACI was on-ly significant in patients younger than 65 years old(P=0.043). The peak incidence among a week was Monday(17.86%),and incidence bottomed on Friday (13.36%). Conclusion Our study revealed that the incidence of PACI increase over year be-tween 2009 to 2013 and it shew a characteristic variation with respect to season, month and week. Based on these results, we can formulate prevention measures accordingly.
2.Therapeutic effect and safety of tirofiban combined large dose statin in patients with acute myocardial infarction

Objective:To observe and analyze the therapeutic effect and safety of tirofiban combined large dose statin in patients with acute myocardial infarction (AMI),and evaluate its influence on myocardial perfusion and cardiac function.Methods:A total of 97 consecutive AMI patients visited to our hospital from May 2011 to January 2013, who were beyond the emergency PCI time window (≥12h),their chest pain was remissive or not further aggravated were studied.All patients were pumped with tirofiban for 48h continuously;according to combined rosuvastatin dose,they were divided into large dose group (n = 52,20mg,once/d,until one month after infarction,then changed to routine dose of 10mg,once/d)and routine dose group (n=45,10mg,once/d).All patients received se-lective PCI after 7 ~ 10d conservative treatment.Myocardial perfusion level,left ventricular ejection fraction (LVEF)on one week after PCI and 30d after AMI,enzymology changes [creatine (CK),lactate dehydrogenase (LDH)]and major adverse cardiovascular events (MACE)during hospitalization were compared between two groups.Results:Compared with routine dose group,there were significant reductions in thrombus scores in infarct related artery (IRA)[(1.32±1.01)scores vs.(0.81±0.78)scores]and corrected TIMI frame [(32.4±4.73)vs. (26.8±2.34)]in large dose group (P =0.021,P <0.001);after selective PCI,TIMI flow of large dose group was significantly better than that of routine dose group (P =0.024).On one week after PCI,LVEF:(51.4±8.9)% of large dose group was significantly higher than that of routine dose group (47.7±8.7)%,P =0.021;there were no significant difference in levels of CK and LDH between two groups on 7d and 30d after PCI (P >0.05).There was no MACE in both groups during hospitalization and 30d after PCI.Conclusion:Tirofiban combined large dose statin is safe and effective in patients with acute myocardial infarction,it can reduce intra-coronary thrombus burden,im-prove myocardial tissue perfusion and cardiac function without increasing MACE.
3.Determination of uric dopamine content by HPLC-RF for patients with Parkinsons disease treated with laser acupuncture

OBJECTIVE To determine the concentrations of dopamine (DA) in human urine,studying the change of uric dopamine content before and after laser acupuncture treated Parkinsons disease patients.To estabilsh attemptly the standard of apprasing the treatment method which is combined traditional medical theory with modern technology.METHODS The urine samples were extracted with organic solvent and determined by HPLC-RF.RESULTS A good correlation was obtained between dopamine over 20~400 ng*mL-1 and peak height,r=0.9996.The recoveries of DA were (89.69±3.95)%~(92.99±6.5)% for urine samples.There was a close correlation between DA content and the severity of parkinsons disease.The DA content in urine of patients before acupuncture was less than healthy ones.DA content in urine increased most at 4 h and content still maintained high as normal scope at 12 after acupuncture.CONCLUSION This HPLC-RF assay was sensitive,accurate stable and convenient.It is an important method to evaluate the effects of treated Parkinsons disease by laser acupunctrue.
4.A possible anti-apoptosis mechanism of hyperbaric oxygen in rats with memory impairments induced by Aβ25-35

Objective To explore the possible protective effect of hyperbaric oxygen (HBO) on cognitive deficits induced by amyloid β25-35 (Aβ25-35) and neuronal apoptosis in the hippocampi of rats with Alzheimer's disease (AD).Methods The animal AD model was established in 24 Sprague-Dawley rats by bilateral hippocampal injection of Aβ25-35.Twelve rats were injected with normal saline as controls,and another 12 served as normal controls.After the injection,the model rats were further divided into a model group and a treatment group.All the rats were housed with normal feeding for 2 weeks and then those in the treatment groups received a total of 2 courses of HBO treatment (10 days each with an interval of 3 days in between).The other groups were left with no treatment.After the treatment,the rats' learning and memory ability were tested using Morris' water maze test,and any neuronal changes were observed using TUNEL staining.The expression of mRNA and Bcl-2 and Bax proteins in the hippocampus were detected using a RT-PCR and Western blotting.Results HBO significantly improved the learning and memory impairment and alleviated neuronal apoptosis in the hippocampus compared against the control group.In addition,HBO treatment significantly increased the mRNA and protein expression of Bcl-2 and down-regulated the expression of Bax.Conclusion HBO treatment can prevent learning and memory impairment induced by Aβ25-35 peptides,which might be mediated by inhibiting neuronal apoptosis in the hippocampus.
5.Effect of history of open nephrolithotomy on percutaneous nephrolithotomy

Objective To investigate the effects of previous open nephrolithotomy on the technical features, outcomes and morbidities of subsequent percutaneous nephrolithotomy (PCNL). Methods Ninety-eight patients who underwent PCNL from January 2006 to January 2011 were selected in this study. The 34 patients of them who had previous open nephrolithotomy on the same kidney were assigned as group A, and the other 64 patients who had no previous open surgery as group B. The data of operation time, blood transfusion quantity, residual stones rate, hospitalization time and time of tube evulsion were collected and compared between the two groups. Results There were no significant differences between the group A and B with respect to the mean operative time [(84.0±24.6) min vs. (94.0±22.7) min, t=1.372, P=0.177], hospitalization time [(6.5±1.1)days vs. (6.3±1.8)days, t=0.49, P=0.261], blood transfusion quantity [(82.9±10.6) ml vs. (85.0±11.8) ml, t=0.415, P=0.682], kidney and colostomy channels [single channel(70.6% vs. 75.0%), double channel (29.4% vs. 25.0%), χ2 =0.22, P=0.638] and residual stones rate (5% vs. 3%,χ2=0.42, P=0.282). Conclusions When PCNL is performed after previous open nephrolithotomy, there is no difference in success rate and morbidities.
6.Relationship between Acute Cerebral Infarction and Seasonal Variations in Tianjin

Objective To explore the influence of seasonal variations on acute cerebral infarction outcome. Meth-ods We collected data from acute cerebral infarction patients in the Second Hospital of Tianjin Medical University from January 2009 to February 2012. All cases were classified by months and seasons into different groups to analyze the season-al variations retrospectively. Results (1) The percentage of good outcome of acute cerebral infarction show significant dif-ference between each month(P<0.01). The percentage of good outcome was highest in August(92.1%), lowest in Decem-ber(78.1%), second lowest in April(81.9%). The percentage of good outcome in summer(91.6%)was higher than spring (87.2%), autumn(86.4%)and winter(83.9%)(P<0.05). (2) Among those patients converted to bad outcome more pa-tients was attacked by cerebrovascular accident in winter(68.3%)than in summer(43.2%), and more patients was combined with a pulmonary infection in winter(69.5%)than in summer(48.6%)and in autumn(49.3%)(P<0.05). (3) Seasonal effect in acute cerebral infarction outcome was obvious in those≥65 years of age(P<0.05). Conclusion Seasonal changes are associated with the outcome and severity of acute cerebral infarction.
7.Comparative Study on 7 Indicative Constituents of Wild and Cultivated Gentiana officinalis

OBJECTIVE:To compare the contents of loganic acid,swertiamarin,6′-O-β-D-glucosyl gentiopicroside,gentiopi-croside,sweroside,isoorientin and isovitexin in wild and cultivated Gentiana officinalis,and to provide basis for rational use of G. officinalis. METHODS:UPLC method was adopted. The separation was performed on ACQUITY UPLC? BEH C18 column (50 mm × 2.1 mm,1.7 μm) with mobile phase consisted of methanol-0.04% phosphoric acid (gradient elution) at the flow rate of 0.3 ml/min. The detection wavelength was set at 242 nm,and column temperature was 30 ℃. RESULTS:For loganic acid,swertiama-rin,6′-O-β- D-glucosyl gentiopicroside,gentiopicroside,sweroside,isoorientin and isovitexin,a good linearity was obtained in the range of 2.1-537.1 μg,1.05-270 μg,0.92-236 μg,11.1-2 830 μg,0.75-192 μg,0.167-102 μg,0.216-52.80 μg(r≥0.999 5), respectively. Their average recoveries were 97.72%-99.84%(RSD≤3.39%,n=6). The contents of loganic acid,swertiamarin, 6′-O-β-D-glucosyl gentiopicroside,gentiopicroside,sweroside and isoorientin in the wild sample were higher than in the cultivat-ed;the content of isovitexin was lower than the cultivated,but there was no statistical significance(P>0.05). The sum of gentiopi-croside and loganin acid content were all higher than 2.5% in both wild and cultivated samples,and met the requirements of 2015 edition of Chinese Pharmacopoeia(first part). CONCLUSIONS:The content difference of 7 indicative constituents in wild and cul-tivated G. officinalis is not statistically significant,and the indicative constituents of the pharmacopoeia is qualified.

9.Effects of thrombopoietin and thrombopoietinⅡ on human platelet activation

AIM:To investigate effects of thrombopoietin(TPO) and TPOⅡ on human platelet activation in vitro. METHODS:Human platelets were incubated in the phosphate-buffered saline containing rhTPO or TPOⅡ at the concentration of 100 μg/L for five minutes. In order to determine the rate of platelet activation. The CD62P and CD41 expressions on platelets were analysed by flow cytometry using fluorescence labelled monoclonal antibody to CD62P and CD41. RESULTS:The results demonstrated that expression of CD62P on platelets which were incubated with rhTPO or TPOⅡ didn't increase compared with that of contrast group. CONCLUSION:Both rhTPO and TPOⅡdidn't cause the disorder of platelet activation.
10.Clinical and genetic analysis of a newborn with hypoparathyroidism, sensorineural hearing loss, and renal dysplasia syndrome.

OBJECTIVE:
To analyze the clinical phenotype and genetic basis for a male neonate featuring hypoparathyroidism, sensorineural hearing loss, and renal dysplasia (HDR) syndrome.
METHODS:
The child was subjected to genome-wide copy number variation (CNVs) analysis and whole exome sequencing (WES). Clinical data of the patient was analyzed. A literature review was also carried out.
RESULTS:
The patient, a male neonate, had presented with peculiar facial appearance, simian crease and sacrococcygeal mass. Blood test revealed hypocalcemia, hypoparathyroidism. Hearing test suggested bilateral sensorineural deafness. Doppler ultrasound showed absence of right kidney. Copy number variation sequencing revealed a 12.71 Mb deletion at 10p15.3-p13 (chr10: 105 001_12 815 001) region. WES confirmed haploinsufficiency of the GATA3 gene. With supplement of calcium and vitamin D, the condition of the child has improved.
CONCLUSION
The deletion of 10p15.3p13 probably underlay the HDR syndrome in this patient.
