1.Surgical treatment for severe cervical disc herniation of C_(3、4) by anterior and posterior approach

[Objective]To observe the surgical treatment for the huge disc herniation of C3、4.[Method]Sixteen patients with huge disc herniation of C3、4 were operated upon with combined posterior decompression of expansive open-door laminoplasty with anterior decompression,bone graft and plate fixation.After operation,neurological findings,height of the interbody space and fusion rate of the space were observed.[Result]All the patients were followed up for an average of more than six months,all of them did not get secondary nerve injuries in operation,neurological function were improved obviously and all the affected interbody space got solid fusion.Based on the standard of Odom,50% of the patients were very good,25 % good,25 % general,and the effective rate was about 100%,excellent rate was about 75%.[Conclusion]The combined posterior with anterior decopression and interbody fusion for treatment of huge disc herniation of C3、4 has advantage of more security in removing the prolapsed disc,improvement of the interbody fusion rate,keeping the height ofinterbody space.This procedure is one of the effective operative methods in the treatment of huge disc herniation of C3、4.
2.Tissue repairing and functional reconstruction in limb salvage operation of bone malignant tumors

[Objective]To discuss the significance of tissue repairing and functional reconstruction in treatment of extremity malignant tumors by allogeneic bone graft and replanting devitalized bone after malignant bone tumor resection. [Method]A retrospective study of 56 patients with malignant bone tumor was conducted, including 35 male patients and 21 female patients. The age was between 9 and 60, with an average of 18.Tumor in 9 cases was in the proximal hunerus, 30 in the distal femur, and 17 in the proximal cnemis.Thirty-two cases were osteogenic sarcoma,17 cases were chondroma sarcomatosum,3 cases were maligant giant cell tumor of bone and 4 cases were metastatic.Soft tissue was repaired and extremity function was reconstructed by adopting allogeneic bone graft and replanting devitalized bone. Extremity functions, bone healing conditions, and complication were estimated according to Mankin metheds.[Result]Among these 56 patients,results in 20(35.7%) were excellent,11(19.6%) were good,8(14.3%) were fair,17 (30%) were poor. The satisfactory rate was 70%.Thirty-eight patients survived for two years without tumor.Thirty patients survived for 5 years with the survival rate of 55%. Except local relapsed, the major complications were infection (4 cases), fracture (3 cases),collapse of articular facet (1 cases), and arthrocleisis (5 cases),with the infection rate of 7%.No internal fixation was broken.[Conclusion]For extremity malignant tumor, effective tissue repairing and functional reconstruction not only offer a limb-sparing effect, but also keep the limb in good function. This is an effective method for bone malignant tumor.
3.Comparison of Allergic Rhinitic Models Induced by Various Allergens

Objective:To compare the models of guinea pig allergic rhinitis induced by different allergens. Methods: Ovalbumin (OVA), 2,4-tolylene diisocyanate (TDI) and alternariaalternata was respectively used as the allergens to establish the model of guinea pigs allergic rhinitis. The conformity of the models and human allergic rhinitis was studied through the behavioral indices, such as the times of nose itches, nasal discharge flow, histological properties and serum HA and IgE indices. Results:The times of sneezing and scratching nose, serum HA and IgE in OVA group was significantly different from those in the control group (P<0. 001 or P<0. 01). Conclusion:The models of allergic rhinitis induced by OVA are the same as allergic rhinitis in typical symptoms and pathological changes.
4.Biological characteristics of synovial mesenchymal stem cells after isolation and purification

BACKGROUND:A large amount of studies have confirmed that synovial mesenchymal stem cells have the similarity in cellmorphology, immune phenotype, colony forming ability and differentiation potential with bone marrow mesenchymal stem cells. But bone marrow mesenchymal stem cells are better than synovial mesenchymal stem cells in the ability to differentiate into cartilages. OBJECTIVE:To discuss the possibility of using synovial mesenchymal stem cells as seed cells for meniscal tissue engineering. METHODS:The synovial mesenchymal stem cells were isolated from rabbit synovial tissues with limiting dilution monoclonal culture method, and then the cells were purified. The morphology, ultrastructure, molecular phenotype, proliferation kinetics, karyotype and tumorigenicity of the in vitro cultured cells were analyzed. RESULTS AND CONCLUSION:The synovial mesenchymal stem cells isolated from the rabbit synovial cells had high proliferation capacity during in vitro monolayer culture. The synovial mesenchymal stem cells grew to peak at 6 days, and the doubling time was (30.2±2.4) hours. Flow cytometry results showed the synovial mesenchymal stem cells could express some molecular makers of mesenchymal stem cells, such as CD44 and CD90. DNA contents check, karyotype test and oncogenicity test confirmed isolated and purified synovial mesenchymal stem cells were the normal diploid cells without tumorigenicity, so the cells can be used as seed cells for meniscal tissue engineering.
5.Histocompatibility between small intestinal submucosa and synovial mesenchymal stem cells

BACKGROUND:Smal intestinal submucosa is characterized as antimicrobial activity, good biocompatibility, bio-mechanical properties, and rapid degradation in vivo, similar to the extracellular matrix of meniscal
fibrochondrocytes.
OBJECTIVE:To observe whether there exists a good histocompatibility between smal intestinal submucosa and synovial mesenchymal stem cells.
METHODS:Smal intestinal submucosa was treated with physical and chemical treatment. And hematoxylin-eosin staining and scanning electron microscopy observation were performed. Then, smal intestinal submucosa extracts were prepared for the fol owing experiments. (1) Pyrogenic test:smal intestinal submucosa extracts were injected into the ear vein of New Zealand white rabbits. (2) Skin sensitization test:smal intestinal submucosa extracts, paraformaldehyde solution and normal saline were respectively injected intradermal y into New Zealand white rabbits. (3) General toxicity test:smal intestinal submucosa extracts and normal saline were respectively injected into the ear vein of New Zealand white rabbits. Smal intestinal submucosa was co-cultured with osteogenic rabbit synovial mesenchymal stem cells, and smal intestinal submucosa cultured alone served as control. RESULTS AND CONCLUSION:There were some intestinal mucosal epithelial cells, fat cells and other cells adhered onto the surface of smal intestinal submucosa after physical treatment. While, the amount of residual cells decreased sharply after chemical treatment. But the main structure and the component had not been changed. The surface of smal intestinal submucosa was smooth and no cells remained, and there was a three-dimension network spatial structure. The porosity was 80%. Smal intestinal submucosa is a non-toxic, nonirritating, non-immunogenic biomaterial with very good biocompatibility, which has a good histocompatibility with rabbit synovial mesenchymal stem cells.
6.Effect of three-dimensional conformal intensity-modulated radiotherapy combined with capecitabine and oxaliplatin chemotherapy on postoperative survival in patients with advanced gastric cancer

Objective To investigate the effect of three-dimensional conformal intensity-modulated radiotherapy combined with oxaliplatin plus capecitabine chemotherapy upon the postoperative median survival time of patients diagnosed with advanced gastric cancer. Methods A total of 74 patients with advanced gastric cancer admitted to Linyi Central Hospital from February 2010 to January 2012 were recruited and divided into the postoperative chemotherapy group (n=37) and postoperative radiotherapy and chemotherapy group (n=37) according to the treatment plan. All patients in two groups were treated with laparoscopic D2radical operation. In the postoperative chemotherapy group, patients were treated with oxaliplatin combined with capecitabine. In the postoperative radiotherapy and chemotherapy group, patients were treated with oxaliplatin combined with capecitabine plus three-dimensional conformal radiotherapy.In both groups, 4 cycles of chemotherapy were delivered. The incidence of adverse reactions, median progression-free survival, median overall survival, and 1-, 3-and 5-year recurrence rate and mortality rate were statistically compared between two groups. Results In the postoperative radiotherapy and chemotherapy group, the incidence rate of bone marrow suppression ( 41%), abnormal liver function ( 30%), nausea and vomiting (30%) and neutropenia ( 46%) did not significantly differ from 35%, 35%, 24% and 41% in the postoperative chemotherapy group ( all P>0. 05).In the postoperative radiotherapy and chemotherapy group, the median progression-free survival and median overall survival were significantly longer compared with those in the postoperative chemotherapy group (both P<0. 05).The 1-, 3-and 5-year recurrence rates were 8%, 14%and 16% in the postoperative radiotherapy and chemotherapy group, significantly lower than 32%, 41% and 46% in the postoperative chemotherapy group ( all P<0. 05).The mortality rate was 11%( 4/37) in the postoperative radiotherapy and chemotherapy group, which was significantly lower than 30%(11/37) in the postoperative chemotherapy group ( P<0. 05 ). Conclusions Three-dimensional conformal radiotherapy combined with oxaliplatin plus capecitabine chemotherapy can effectively prolong the median survival, reduce the recurrence rate and does not enhance the risk of adverse events for patients with advanced gastric cancer.
7.Ten-year trends of mortality from cerebrovascular diseases in two resident areas in Henan

The cluster sampling method was used and a retrospective survey on mortality trends of cerebrovascular diseases from 1989 to 2008 was conducted among residents from Erqi District in Zhengzhou city and Xigong District in Luoyang city. The average mortality rate of cerebrovascular diseases in these two districts was 69. 5/100 000 in 1989 to 2008. The standardized mortality rate for men was 118. 67/100 000 in 1989 to 1993, and dropped to 44. 23/100 000 in 2004 to 2008. The standardized mortality rate for women was 68.21/100 000 in 1989 to 1993, and dropped to 30. 2/100 000 in 2004 to 2008. The declining trends of cerebrovascular disease mortality rates might be related to early diagnosis and early treatment of the disease, and the extensive health education and prevention programs.
8.Analysis of results of endoscopic screening of esophageal, gastric cardia and gastric cancers in high risk population.

OBJECTIVETo summarize the results of endoscopic screening of esophageal, gastric cardiac and gastric cancers in the high-risk population, and analyze the influencing factors such as age, gender and biopsy rate on their detection and early diagnosis rates.
METHODSNine high incidence cities and counties of esophageal cancer in Henan province were included in this study. People aged 40-69 years were set to the target population. Excluding contraindications for gastroscopy, in accordance with the national technical scheme of early cancer diagnosis and treatment, gastroscopic screening and biopsy pathology for human esophageal, cardiac and gastric cancers were carried out.
RESULTSDuring the 3-year period, a total of 40 156 subjects were screened. Among them, 18 459 cases of various precancerous lesions (46.0%) were detected. The cancer detection rate was 2.3% (916 cases), including 763 cases of early cancers. The diagnosis rate of early cancers was 83.3%. Precancerous lesions were detected in 9297 cases (23.2%) for esophagus and 9162 cases (22.8%) for gastric cardia as well as stomach, respectively.
CONCLUSIONSThe results of this study demonstrate that endoscopic screening is feasible for early detection, diagnosis and treatment of esophageal, gastric cardia and gastric cancers among high risk population in high incidence area. Exploration analysis of relevant affecting factors may help to further improve the screening project for early diagnosis and treatment of those cancers.

9.Cancer incidence and mortality in Henan 2013

Objective:To describe the cancer incidences and mortalities in Henan cancer registries in 2013.Methods:Registration data were evaluated according to the criteria of quality control of cancer registry,and the qualified data were analyzed based on areas(urban/rural), gender,age and cancer sites.The age-standardized rates were applied according to Segi's population and the fifth Chinese population census in 2000.Results:The total coverage of population from 19 qualified cancer registries data was 16,225,815(15.13%),Among which,8,370, 772(51.59%)were males and 7,855,043(48.41%)were females;and 2,819,817(17.38%)live in urban areas and 13,405,998(82.62%) live in rural areas.The crude incidence rate in Henan was 250.34/105(males:264.35/105,females:235.42/105),whereas the age-standardized incidence rates by Chinese standard population and by world standard population were 208.66/105and 207.25/105,respectively.The cumulative incidence rate(0-74 years old)was 24.31%.The cancer mortality in Henan was 161.05/105(males:185.50/105,females:134.98/105),whereas the age-standardized mortality by Chinese standard population and by world standard population were 131.18/105and 131.52/105,respectively.The cumulative mortality rate(0-74 years old)was 15.05%.Cancer incidence and mortality rates were increasing by age, reaching the peak values at 80 age group and 85 plus age group,respectively.Lung cancer,gastric cancer,esophageal cancer,liver cancer and colorectal cancer were the most common cancers ranked by the incidence rate,and the most common cause of cancer death was lung cancer,followed by gastric cancer,esophageal cancer,liver cancer and colorectal cancer.Incidence rate and mortality rate of breast cancer ranked the first and the fifth among the females,respectively.Conclusion:Cancer incidence and mortality rates were higher in the rural areas and in males than in the urban areas and in females.Lung cancer,digestive system cancers,and female breast cancer are the major cancer types that require attention for the prevention and control in Henan.
10.Intelligent-assisted coding technology in review and entry of ICD codes on medical record's first pages

Objective The ICD intelligent-assisted coding system for the first pages of medical records enhances the in-telligent review and entry of ICD codes on medical records through data collection of medical records information,and improving accuracy and efficiency.Methods The coding rule library for the first page of medical records was constructed,clinical informa-tion across systems was obtained,the operation steps of manual coding review and entry was simulated through coding intelligent analysis,and an intelligent system was introduced into coding review and entry.Three coding methods,namely coding manually by coders,by the intelligent coding system,and by a man-machine collaboration,were adopted to conduct coding review and en-try.The coding results were compared between the three methods.Results Compared to manual coding method,the ICD intelli-gent-assisted coding system exhibited the same accuracy and completeness of coding but a significant increase in coding efficien-cy.The method of human-machine collaborative coding had a better improvement in coding accuracy and efficiency as well com-pared with the manual coding.Conclusion Artificial intelligence technology has superiority in coding entry on the first page,through the ICD Intelligent Assisted Coding Review and Entry System.for it can increase the accuracy of review and entry.The method of human-machine collaboration enhance both the efficiency and accuracy in coding review and entry.
