1.Development of infection and drug-resistance of Acinetobacter baumannii

ObjectiveTo investigate the recent development of Acinetobacter baumannii infection and its drug-resistance.MethodsThe Acinetobacter baumannii infections of inpatients during the year 2001 to June,2005 were analyzed retrospectively. ResultsThe infection of Acinetobacter baumannii increased from 15 strains in 2001 to 68 strains in the first half of 2005.The rate of high drug-resistance strains also raised from 20% to 77%.ConclusionThe incident of infection of Acinetobacter baumannii continuously increased in the past 5 years and became more difficult to treat.
2.Clinical features of chronic lung diseases patients combined with lung fungal infection

ObjectiveTo investigate the clinical features of chronic lung diseases patients combined with lung fungal infection.MethodsThe data of 216 hospitalized cases with chronic lung diseases were retrospectively analyzed.ResultsThe rate of lung fungal infection of chronic lung diseases patients was 28.1%, and the main pathologic fungal was Candida albicans, about 67.2 %.ConclusionThe chronic lung diseases patient has a higher rate of lung fungal infections compared with other diseases. The measures of preventing, diagnosing and treating lung fungal infection at early stage should be taken.
3.Effects of preoperative parecoxib on interleukin 6 and postoperative analgesia for mastectomy

Objective To investigate the effects of preoperative parecoxib on the levels of inter-leukin 6 (IL-6)and postoperative analgesia for breast surgery.Methods Sixty breast cancer patients undergoing mastectomy were randomly divided into two groups:parecoxib group (group P)and con-trol group (group C),n=30 in each group.All patients received sevoflurane and fentanyl anesthesia. Group P was injected parecoxib 40 mg at 10 minutes before induction of anesthesia,meanwhile group C was injected saline 5 ml.All patients received postoperative patient-controlled intravenous analgesia (PCIA)with fentanyl.VAS scores for pain were assessed at postoperative 2,4,8,12,24 hours. The serum levels of IL-6 were measured by ELISA at 10 minutes before induction,4 h,8 h,and 24 h after surgery.Results Group P had lower VAS scores than group C at 2-12 h after surgery (P <0.05).Compared with 10 minutes before induction,the levels of IL-6 increased significantly at post-operative 4,8,24 h in two groups (P<0.01),while group P had lower levels of IL-6 than group C (P <0.01).Conclusion Preoperative administration of parecoxib has a stronger analgesic effect in breast cancer patients after mastectomy,and decreases the levels of IL-6.
4.Development of multifunctional anorectal therapeutic apparatus

This article introduces the structure, principle and characteristics of a new anorectal therapeutic apparatus. The adoption of new technologies, new components and new materials lays the foundation for the compatibility for electric energy, magnetic energy and thermal energy as well as the safety and comfortability of the intracavitary electrode. The intracavitary electrode possesses the functions of electrotherapy, thermotherapy, magnetotherapy and massage, and thus can be applied to the physical therapies of colitis, irritable bowel syndrome and prostatitis. The apparatus has such advantages as reliable curative effects, no toxic and side effects, simple manipulations, low cost, etc, and thus should be popularized.
5.Changes of brain gangliosides in rats transient global cerebral ischemia/reperfusion models after hyperbaric oxygen treatment

BACKGROUND: Ganglioside (Gls) is a kind of N-acetylneuraminatecontaining glycosphingolipid, which is abundant in neural tissues and exerts neuroprotective roles, and has been found its changes in content and composition pattern after cerebral ischemia or hypoxia diseases.OBJECTIVE: To investigate the effect of hyperbaric oxygen on brain gangliosides after cerebral ischemia/reperfusion and explore the possible mechanism of hyperbaric oxygen treatment to ameliorate cerebral ischemia/reperfusion damage.DESIGN: Randomized-control observation.SETTING: Department of Hyperbaric Oxygen, Chaoyang Hospital Affiliated to Capital University of Medical Sciences and Department of Biochemistry and Molecular Biology, Basic College, Capital University of Medical Sciences.MATERIALS: Animal models were established at Department of Hyperbaric Oxygen, Chaoyang Hospital Affiliated to Capital University of Medical Sciences (Key Laboratory of Beijing) from March to April 2002. All indexes were determined at the Department of Biochemistry and Molecular Biology, Basic College, Capital University of Medical Sciences. A total of 54 SD male rats were selected and randomly divided into 9 groups: shamoperated group; ischemia/reperfusion 6, 24, 48 and 96 hours group; and hyperbaric oxygen 6, 24, 48 and 96 hours group with 6 rats in each group.METHODS: Animal models with cerebral ischemic/reperfusion were established in the other 8 groups, except the sham-operation group. Global cerebral ischemia was induced by a four-vessel occlusion and reperfusion allowed after 20-minute ischemia. The rats in sham-operation group were operated in the same way except of arterial occlusion. The rats in the HBO group were placed in experimental animal chamber. After 5-minute wash by pure oxygen, the pressure of oxygen cabins was increased to 0.1 MPa in 5 minutes and kept stable for 45 minutes, then decreased to ambient level within 10 minutes. The rats in the HBO groups were treated once by hyperbaric oxygen at reperfusion 3, 24, 48 and 96 hours respectively; while the rats in the ischemia/reperfusion group and sham-operation group were placed in normal atmospheric environment. The rats of HBO and ischemia/reperfusion groups were killed by decapitation at the 6th, 24th, 48th and 96th hours respectively, and the brains removed quickly. Gls and its percentage content in each group were detected with high performance thin-layer chromatography plate.MAIN OUTCOME MEASURES: Total content of gangliosde in the whole brain tissue of rats and its percentage content.RESULTS: Totally 54 rats were involved in the result analysis without drop out. ①The contents of Gangliosides in HBO groups at 24 and 48 hours was significantly higher than those in sham-operation group and ischemia/reperfusion group at corresponding time phase (F=12.730,122.246,P < 0.01), but there was no significant difference between ischemia/reperfusion and sham-operation groups (P > 0.05). ② N-acetylneuraminosylgalactosyl-N-acetylgalactosaminyl- (N-acetylneuraminosylα2→8- N-acetylneuraminosyl) -galactosylglucosylceramide (GT1b) proportion in the ischemia/reperfusion 24 hours group was lower markedly than that in the S-O (F=13.575,P < 0.01); Both galactosyl-N-acetylgalactosaminyl- (N-acetylneuraminosyl- α2→8-N-acetylneuraminosyl) - galactosylglucosylceramide (GD1b) and galactosyl- N-acetylgalactosaminyl-(N-acetylneuraminosyl)galactosylglucosylceramide (GM1) in the reperfusion 48 hours group were lower than those in the sham-operation group (F= 4.015,3.979,P < 0.05); (N-acetylneuraminosyl)galactosylglucosylceramide (GM3)in ischemia/reperfusion 24 hours group was much higher than that in sham-operation and any other ischemia/reperfusion groups (F=21.450,P< 0.01 ); An unknown increase of GM3 was found again at the 96th hour;③GM1and GM3 proportion in the hyperbaric oxygen group was higher than that of sham-operation group at the 24th hour (F=3.970,21.450,P< 0.05, < 0.01), and all of GD1a, GD1b and GT1b were lower than that in the sham-operation group at the same times (F= 13.575,5.745,8.783, P< 0.05-0.01), but GT1b was remarkably higher than that in ischemia/reperfusion group (F=8.783 ,P < 0.05).CONCLUSION: The pattern of brain gangliosides changed after transient whole cerebral ischemia/reperfusion. The new mechanism of neuron damage after transient whole cerebral ischemia/reperfusion might be the decreasing of GT1b, GD1b and GM1 with increasing of GM3 proportion. The hyperbaric oxygen treatment could ameliorate cerebral ischemia/reperfusion damage by increasing total gangliosides content and GM1 proportion and accelerating GT1b restoration to normal level. It is unknown that the effect of percentage content of GD1a and GD1b decreased.
6.Early diagnosis and treatment on suspected SARS patients in general hospital

ObjectiveTo investigate the diagnosis and correlative factors of suspected severe acute respiratory synd rome (SARS) cases, and finally, complete the diagnosis and treatment.MethodsTo analyze the clinical, laboratory data,chest radiog raph and treatment of 100 suspected SARS cases.ResultsAmong 100 suspected SARS cases,66 were diagnosed as SARS,4 were mycoplasma l pneumonia, 2 were pulmonary tuberculosis,the others were general bacteria pne umonia.Conclusions SARS hasn't characteristic clin ical manifestation.Because there is not a gold diagnosis standard up to now, the disinfection and isolation of suspected SARS cases should be treated as SARS c ases. In order to prevent missed and misdiagnosis, the diagnosis and treatment s hould be synthetically thought.
7.Investigation on the tolerance of ibandronate by a single intravenous infusion

AIM: To evaluate the safety and tolerance of ibandronate in Chinese healthy volunteers. METHODS: The trial protocol was designed according to the Good Clinical Practice(GCP). After physical examination and laboratory tests were performed, 36 healthy volunteers were divided randomly into 6 dose groups, including 1 mg , 2 mg , 3 mg , 4 mg , 5 mg and 6 mg , with 6 subjects in each group(3 male and 3 female). Clinical symptoms, vital signs, routine blood tests, routine urine tests, hepatic function, renal function, blood electrolytes, electrocardiogram, and electroencephalogram were observed or examined before and after a single intravenous infusion of ibandronate. RESULTS: After single intravenous infusion doses of 1- 6 mg , the vital signs, clinical symptoms and laboratory tests were all in the normal range, but there were some slight ADRs concerned with the drug, such as hypophosphataemia, increased body temperature, perspiring,pain of bone or muscle and hypocalcaemia. But the ADRs were found vanishing in one or two weeks. CONCLUSION: Single intravenous infusion (up to 6 mg ) of domestic ibandronate in 36 chinese healthy volunteers is safe and tolerable.
8.Localization of hepatitis C virus core protein in the nucleus of peripheral blood mononuclear cells of hepatitis C patients.

BACKGROUNDTo study the significance of the expression of HCV core protein in PBMC of patients with chronic hepatitis C and to evaluate the relationship between HCV core protein expression and clinical states.
METHODSIdentification of HCV protein antigen (Ag) in PBMC of 66 hepatitis C patients by immunohistochemical method and clinical status of the patients with HCV protein positive expression were investigated. In 27 out off 66 patients the HCV RNA and HCV Ag in PBMC were detected with RT-PCR and immunohistochemical method.
RESULTSThe HCV Ag (core+NS3) was identified in PBMC in 51 out of 66 patients (77.27%). It was also demonstrated that HCV core protein in nucleus showed strong expression and NS3 protein in cytoplasm showed weak expression. The expression of core protein in nucleus of PBMC were 35.29% in advanced chronic hepatitis patients, which was significantly higher than that from moderate cases (5.88%).
CONCLUSIONSThis study suggested that the expression of PMBC-HCV core protein may be related to the clinical state of the patients. The nuclear expression of HCV core protein in PBMC of patients with hepatitis C may be related to the persistence and activity of the chronic hepatitis C virus and play an important role in the pathogenesis of cirrhrosis and hepatocellular carcinoma.

9.Application of walking combined with Buerger exercise in patients with grade 0 diabetic foot

Objective To study the application of walking combined with Buerger exercise in the treatment of grade 0 diabetic foot.Methods Eighty patients with grade 0 diabetic foot were randomly divided into observation group and control group.The control group was trained by walking exercise .The observation group was trained by walking combined with Buerger exercise through 12 months of training.The changes of self-sensory symptoms,ABI and DAWV were observed.Results The self-sensory symptoms of the two groups were significantly decreased (observation group: markedly effective 15 cases,effective 19 cases;control group: markedly effective 5 cases,effective 23 cases).The effective rate of the observation group was higher than that of the control group (85% vs.70%,χ2 =7.831,P <0.05).The ABI ratio of the two groups were higher than those before treatment ,and the ABI ratio of the observation group was higher than that of the control group [(0.889 ±0.113) vs.(0.842 ±0.124),t =1.772 P <0.05].The ratio of DAWV in the observation group was higher than that in the control group [(6.772 ±0.435)cm/s vs.(6.543 ±0.552)cm/s,t =2.061,P <0.05].Conclusion Walking combined with Buerger exercise can improve the blood flow velocity and blood flow of the collateral circulation of the lower limbs of patients with grade 0 diabetic foot,and improve the self-sensory symptoms of peripheral neuropathy .
10.Surveillance and control of infectious disease in primary and middle schools in Hebei Province from 2019 to 2021

Objective:
To understand the infectious disease prevention and control among primary and secondary schools in Hebei Province from 2019 to 2021 and to provide a scientific basis for promoting epidemic prevention and control in schools.
Methods:
Relevant indicators of infectious disease prevention and control in primary and secondary schools were collected and screened from the on site supervision and inspection data uploaded from various places, and analyzed with SPSS 22.0 software.
Results:
The qualified rates of infectious disease prevention and control in primary and secondary schools in Hebei Province from 2019 to 2021 were 77.11%, 89.74% and 96.24%, respectively, the difference was statistically significant( χ 2=455.45, P <0.01). The qualified rates of infectious disease prevention and control in primary schools, middle schools and high schools from 2019 to 2021 increased by year, the difference was statistically significant( χ 2=319.49, 118.74, 25.73, P <0.05). The qualified rates of six infectious disease prevention and control indicators such as morning inspection record, special person responsible for epidemic report, registration record of absence due to illness increased by year, the difference was statistically significant( χ 2=140.34, 9.10, 113.55 , 163.71 , 286.74, 329.18, P <0.05).
Conclusion
Steady improvement in school infectious disease prevention and control has been observed, while qualification rate in primary school and rural area still need to be improved, with missing or late report. Government support and talent policy, hardware and sofeware construction, as management level should be strengthened.
