1.Use of Clamp-on Shapping Plate Implant for Comminuted Fracture

The clamp-on shapping plate implant, composed of a main rod with 3~4 pairs of curved short arms attached on both sides of the rod, possesses multifixing action to the fracture without hindering the movement of the adjacent joints. It is especially useful for the fixation of comminuted fracture of the short tubural bones which are difficult to be fixed by other methods. In the period of 1987.8~1990. 2,64 cases of such fractures had been treated with this device. Followup results in 52 cases showed that all the fractures thus treated healed nicely without any case of malunion. Total duration of external immobilization was relatively shorter and the functional recovery of the joints also much better.
2.Effects of different doses of fentanyl by target-controlled infusion on stress responses in patients undergoing abdominal surgery under general anesthesia

Objective To investigate the effects of two different doses of fentanyl given by target-controlled infusion (TCI) on stress responses during operation under general anesthesia and evaluate the accuracy of TCI of fentanyl.Methods Thirty ASA Ⅰ-Ⅱ patients (14 males, 16 females) aged 24-69 yr, weighing 48-77 kg scheduled for elective abdominal surgery under general anesthesia were randomly divided into three groups with 10 patients in each group : group Ⅰ and Ⅱ received fentanyl by TCI with the target effect-site concentration set at 2 ng Ⅲ ml 1 (Ⅰ) and 4 ng ? ml-1 ( Ⅱ ) during induction and maintenance of anesthesia, while group Ⅲ received a bolus of fentanyl 3 ?g?kg-1 during induction of anesthesia. In addition to fentanyl, anesthesia was induced with propofol 2mg?kg-1 and vecuromum 0.1 mg?kg-1 and maintained with propofol infusion at 6 mg?kg-1?h-1 and intermittent i. v. doses of vecuronium BP (SBP, DBP, MAP ) , HR, SpO2 and PET CO2 were recorded before induction of anesthesia (T0 ,baseline), at the loss of consciousness (T1), immediately after tracheal intubation (T2), 5 and 10 rnin after intubation (T3 , T4), during skin incision (T5) and exploration of abdominal cavity (T6). Blood samples were obtained for determination of plasma norepinephrine (NE) and epinephrine (E) concentrations and blood glucose ( BG) and fentanyl concentrations at T0, T2, T4 and T5 by HPLC. Results There were no significant differences in sex, age and body weight among the three groups. There were no significant changes in plasma NE, E and BG concentrations, which were all within normal range in the three groups. 3.Osteogenesis of human adipose-derived mesenchymal stem cells-biomaterial mixture in vivo after 3 D bio-printing

Objective:To construct human adipose-derived mesenchymal stem cells (hASCs)-biomate-rial mixture 3D bio-printing body and detect its osteogenesis in vivo,and to establish a guideline of osteogenesis in vivo by use of 3D bio-printing technology preliminarily.Methods:P4 hASCs were used as seed cells,whose osteogenic potential in vitro was tested by alkaline phosphatase (ALP)staining and alizarin red staining after 1 4 d of osteogenic induction.The cells were added into 20 g/L sodium alginate and 80 g/L gelatin mixture (cell density was 1 ×1 06/mL),and the cell-sodium alginate-gelatin mixture was printed by Bioplotter 3D bio-printer (Envision company,Germany),in which the cells’survival rate was detected by live-dead cell double fluorescence staining.Next,the printing body was osteogeni-cally induced for 1 week to gain the experimental group;and the sodium alginate-gelatin mixture without cells was also printed to gain the control group.Both the experimental group and the control group were implanted into the back of the nude mice.After 6 weeks of implantation,the samples were collected,HE staining,Masson staining,immunohistochemical staining and Inveon Micro CT test were preformed to analyze their osteogenic capability.Results:The cells’survival rate was 89%±2% after printing.Six weeks after implantation,the samples of the control group were mostly degraded,whose shape was irregu-lar and gel-like;the samples of the experimental group kept their original size and their texture was tough.HE staining and Masson staining showed that the bone-like tissue and vessel in-growth could be observed in the experimental group 6 weeks after implantation,immunohistochemical staining showed that the result of osteocalcin was positive,and Micro CT results showed that samples of the experimental group had a higher density and the new bone volume was 1 8%±1%.Conclusion:hASCs-biomaterial mixture 3D bio-printing body has capability of ectopic bone formation in nude mice,and it is feasible to apply cells-biomaterial mixture 3D bio-printing technology in the area of bone formation in vivo.
4.Determination of resveratrol glucoside and resveratrol in radix and rhizome of Polygonum cuspidatum yielded in Hanzhong Region

Object To determine the content of resveratrol glucoside and resveratrol in radix and rhizome of Polygonum cuspidatum Sieb. et Zucc. yielded in Hanzhong Region. Methods The HPLC method was used to assay resveratrol glucoside and resveratrol directly, using Eclipse XDB C 8 column and acetonitrile water as the mobile phase, the UV detection wavelength was 303 nm, with a flow rate of 1 mL/min. Results The content of resveratrol glucoside was 2 5% and resveratrol was 0 43% in the radix and rhizome. Conclusion The content of resveratrol glucoside is higher than that in the former refernece. The content of resveratrol in the radix and rhizome of P. cuspidatum in Hanzhong Region is first reported.
5.Intravenous immunoglobin for treating pneumonia following kidney transplantation

Objective To investigate the adjunctive therapeutic effects and safety of intravenous immunoglobin (IVIG) for treating pneumonia following kidney transplantation.Methods Sixteen cases of pulmonary infection after kidney transplantation were divided into two groups.Twenty-eight cases were subjected to IVIG therapy (0.2 g·kg-1 ·day-1) for 7-10 days besides the standard specific anti-bacterial,anti-fungal,and anti-virus treatment and regular immunosuppressive regimen with dose adjustment (IVIG group),and the control group was only treated with standard specific anti-pathogen therapy.The incidence and mortality ofsevere pulmonary infection,levels of serum IgG,T lymphocyte subsets,and creatinine in the two groups were observed.Results The effective power of IVIG group and control group was 100 % and 93.75 % (P<0.05).The incidence of severe pneumonia in IVIG and control groups was 0 and 12.5%,respectively (P<0.05),with the mortality being 0 and 6.25%,respectively (P< 0.05).The levels of serum IgG were significantly increased in IVIG group as compared with that before treatment and in control group.There were no significant adverse reactions associated with IVIG infusion.Conclusion As an adjunctive therapy,IVIG treatment for pulmonary infection can reduce the incidence of severe pulmonary infection and mortality after kidney transplantation,further increase the survival rate of patients after kidney transplantation.
6.Clinial application of free posterlateral leg perforator flap in the reconstruction of soft tissue defects at oral and maxillofacial region

Objective To investigate the outcome of free posterolateral leg perforator flap in the reconstruction of soft tissue defects of oral and maxillofacial area.Methods From February,2014 to August,2016,16 patients were performed defect reconstruction following oral cancer ablation with free posterolateral leg perforator flap including 3 cases of sequamous cell carcinoma of the tongue,6 cases of carcinoma of the buccal mucosa,7 cases of sequamouse cell carcinoma of the floor of mouth.The flap ranged from 5.0 cm×3.0 cm to 13.0 cm ×9.0 cm in size,and were adjected to the soft tissue defects of oral and maxillofacial area.The outcome of reconstruction was evaluated by considering the facial appearance,the swallowing and the speach function,the ranges of mouth opening and patient prognosis.Results All 15 transplanted flaps survived well and only 1 survived after disposing in time,because of vascular crisis.The donor sites were closed directly without donor-site morbidity.All patients were satisfactory with their facial appearance,the ranges of mouth opening and swallowing and speech function.Conclusion The free posterolateral leg perforator was an ideal free tissue for repairing functionally oral and maxillofacial defects,sinceit including some beneficial characteristic such as constant blood vessel,flexible and divese design,abundant and position-latent donor site tissues,small operation loss,flexible preparation and loss of major vascular nerves.
7.Effects of three-month rehabilitation on motor function in hemiplegics after stroke

ObjectiveIn order to study the effect of three-month rehabilitation on motor function in hemiplegics after stroke. Methods125 patients were randomly divided into two groups, rehabilitation group (72 cases)and control group(53 cases).Patients in the rehabilitation group were given clinical treatment,electric stimulation therapy(EST) and regularly physical therapy, while those in control group were given clinical treatment and unguided self-training, and the evaluation was done in pre-treatment and post-three-month respectively. Motor function was assessed in Fugl-Meyer Assessment(FMA).ResultsMotor scores in each group had a more significant difference(P<0.001)before and after treatment, the process of scores in the rehabilitation group was obviously superior to that in the control group(P<0.001). Conclusions Rehabilitation training on hemiplegics after stroke may obviously improve motor function, depressing the disability and increasing the living quality.
8.A study on imagination features of tumor thrombi in the portal vein of primary liver cancer

Objective To study on imagination features of tumor thrombi in the portal vein of primary liver cancer (PLC). Methods We established a new type system of tumor thrombi, with Ⅰ 0 to Ⅳ, a total of 5 types and 8 sub-types in the portal vein of PLC based on normal intrahepatic portal vein anatomy and growing features of the tumor thrombi. The relationship between the types and the imaging diagnosis of 130 PLC cases with tumor thrombi in the portal vein was studied. Results 85%(110/130) cases of PLC with tumor thrombi in the portal vein belongs to type Ⅱ or Ⅲ when being first diagnosed in the hospital. The resectability rates for type Ⅰ, Ⅱ, Ⅲ, Ⅳ were 62%(5/8), 16%(10/62), 10%(5/48), 0 (0/12), respectively, and chemoembolization therapy (TACE) was given in 38%(3/8), 40%(25/62), 27%(13/48), 16%(2/12), respectively. Conclusions The new types of tumor thrombi are helpful for diagnosis and treatment of PLC with tumor thrombi in the portal vein.
9.Effects of immunosuppressants on co-stimulated lymphocyte's Th1/Th2 cytokine production

Objective To analyze the effect of cyclosporin A(CsA), rapamycin(RPM) and macophenolic acid(MPA) on the co-stimulated lymphocytes, CD28 and CD40, and their production of Th1/Th2 cytokine, IL-2, IFN-γ, IL-4, IL-10 and IL-12. Methods The experimental groups were divided into ①mono-stimulating and co-stimulating groups: CD3 mAb mono-stimulating (group a),CD3 mAb+CD28 mAb co-stimulating (group b), CD3 mAb+CD28 mAb+CD40 L mAb co-stimulating(group c), CD3 mAb+CD28 mAb+CTLA4 mAb co-stimulating (group d). ②CsA groups: 300 ng/ml of CsA was added to group a, b, c and d. ③RPM groups: 300 ng/ml of RPM was added to group a,b, c and d. ④MPA groups:300 ng/ml of MPA was added to group a, b, c and d. The cytokine production was measured by ELISA.Results The co-stimulated CD28 and CD40 Th1/Th2 cytokines production of IFN-γ, IL-2, IL-4 and IL-10 were significantly increased. Compared with group a, IFN-γ increased from (248.91±11.20)ng/ml to (555.08±24.42)ng/ml and (548.19±33.06)ng/ml, IL-2 increased from (29.48±8.61)ng/ml to (1100.82±99.29)ng/ml and (842.23±29.31)ng/ml, IL-4 increased from (32.29±6.76)ng/ml to (116.02±15.03)ng/ml and (147.28±18.07)ng/ml, IL-10 increased from (147.01±10.47)ng/ml to (291.79±12.47)ng/ml and (302.52±35.18)ng/ml,respectively, P<0. 01. Compared group b with group c, the Th1 cytokines production was decreased.IL-2 and IL-12 decreased (P<0.05). The Th2 cytokine IL-4 production was increased (P<0. 05).CTLA4 mAb and three other immunosuppressants, CsA, RPM and MPA, inhibited co-stimulated lymphocyte's both cytokines Th1 and Th2 production. The inhibitory effect of CsA on Th1/Th2 cytokine production was more significant than RPM and MPA did. The co-inhibitory effect of CTLA4 mAb and CsA was observed as well. The increased co-stimulated CD28 and CD40 IL-12 production could be suppressed by MPA. CsA and RPM had no inhibitory effect on the IL-12 production.Conclusions CD28/CD40 co-stimulatory pathway plays the key role in lymphocyte activation and Th1/Th2 cytokine production. CsA, RPM and MPA can inhibit co-stimulated lymphocyte's Th1 and Th2 cytokine production. CsA and CTLA4 mAb have co-inhibitory effect on co-stimulated lymphocyte's Th1/Th2 cytokines production. CD40 L mAb decreases the Th1 cytokines production(including IL-12) and increases the Th2 (mainly IL-4) production, which may be the mechanism of its longevity effect on allograft.
10.Research on a rat model of female stress urinary incontinence

Objective To investigate the pathological changes and mechanism in the urethra by parturition-induced stress urinary incontinence.Methods Sprague Dawley female rats underwent vaginal balloon dilation for 4 hours immediately after delivery.One week later,the rats were anesthetized and both ovaries were excised.Then a rat model of stress urinary incontinence (SUI) was successfully established.One month after ovariectomy,conscious cystometry and Leak-Point Pressure (LPP) were measured by MP150.Histological examination and Western blotting were performed after functional assays.Results (1) 85% of rats presented astress urinary incontinence in the model group.(2) The urethras in SUI rats had decreased muscles,and striated muscles showed fragmentized and disorganized.(3) Elastic fibers were long,well organized and tightly connected to the muscle bundles in sham group,while elastic fibers showed fragmentation and disorganization in the model group.(4) The protein expression of vascular endothelial growth factor (VEGF) and blood vessels were reduced in SUI rats as compared with the sham rats.Conclusions Muscle and elastic fibers in the urethra are disrupted in SUI rat.VEGF may play an important role in regulation of pathological changes in urethra.
