1.Application of SOAP assessment note to rehabilitation therapy teaching

BACKGROUND:In rehabilitation therapy teaching, it is crucial to cultivate professional knowledge and clinical skil s of rehabilitation therapy for students. SOAP (Subjective, Objective, Assessment, Plan) is the international most commonly used and accepted in the medical records of problem oriented method. SOAP is used to develop clinical thinking ability, improve analysis, judgment and the ability of solving clinical problem for students. OBJECTIVE:To retrospectively analyze the composition, teaching benefit and problems of SOAP in China. METHODS:Published articles about SOAP note were searched from PubMed, Web of Science and China National Knowledge Infrastructure, and the articles were retrieved from database building to September 2014. The key words were“SOAP note, rehabilitation, teaching”in Chinese and English. We excluded the repetitive researches. RESULTS AND CONCLUSION:After screening, we included 20 articles in the review. SOAP note was helpful to promote students’ independent thinking, strengthened the standardization of the records, rehabilitation treatment, and developed the students’ clinical thinking and the ability to deal with the problem. SOAP note has several chal enges in the rehabilitation teaching, however, with the popularity of SOAP teaching concept, and deep research and gradual generalization of SOAP teaching methods, SOAP has a wide application prospect for the rehabilitation education in China.
2.Therapeutic observastion of the Combination of Concha meretricis Seu Cyclinae powder with furacilin solution in the treatment of dermatitis vulvae and eczema

OBJECTIVE: To observe therapeutic effect and adverse reaction of the combination of Concha merelricis Seu cyclinae powder with 0. 02% furacilin solution in treating dermatitis vulvae and eczema. METHODS: Thirty outpatients (9 men and 21 women with the average age of 47. 1 years old) were treated with Concha meretricis Seu cyclinae powder and 0. 02% furacilin solution by wet dressing or external use twice daily for three weeks. RESULTS: 21 cases were cured (70%); 6 cases were significant recovery (20%); 2 cases were effective (6. 66%) and 1 cases was ineffective (3. 33%), The total effective rate was 90. 0% and no adverse reaction was found. CONCLUSIONS: Concha meretricis Seu cyclinae powder combined with 0. 02% furacilin solution is one of the effective therapy in the treatment of dermatitis vulvae and eczema.
3.Factor analysis of hepatic cirrhosis combined with hospital infection in elderly patients

Objective To explore and analyze the related factors for cirrhosis patients with hospital infection.Methods Clinical data of 750 patients with cirrhosis in our hospital were retrospectively analyzed.Related factors for hospital infection including gender,age,length of hospital stay,Child-Pugh classification,accompanying invasive procedures,associating the underlying disease,using antibacterial drugs and the sites of hospital infection and pathogen distribution were analyzed.Results The infection rate was 20.7% (155/750).The infections were mainly spontaneous bacterial peritonitis [54.8% (85/155)],followed by respiratory,digestive,urinary tract and skin.Pathogenic bacteria were mainly Gram-negative bacteria [56.1% (101/1)],followed by Gram-positive bacteria and fungi.Patients aged over 70 years,with hospital stay over 4 weeks,with higher Child-Pugh classification,associated with invasive procedures,combined with underlying diseases,and treated with antimicrobial drugs were much easier to get hospital infection (x2=12.2,16.3,49.46,55.2,20.95,26.24,all P<0.05).Conclusions Patients aged over 70 years with long hospital stay and more severe liver damage,accompanied by invasive procedures,combined with basic diseases and treated with antimicrobial agents are the related factors for hospital infection in patients with cirrhosis.
5.Quantitative Real-time Detection of Spontaneous Perforin(PFR) Gene Expression in Peripheral Blood Mononuclear Cell(PBMC) by Polymerase Chain Reaction

Objective To develop a method for quantitative real-time detection of perforin(PFR) using polymerase chain reaction(PCR).Methods Gene expression of PFR in peripheral blood mononuclear cell(PBMC) from 25 healthy individuals was detected by PCR.An endogenous"housekeeping" gene GAPDH was used to normalize the results.Results and Conclusion The spontaneous PFR mRNA expression in PBMC in a given sample could be measured by PCR with relativity correlation coefficient of r=1.
7.Changes of T-helper1/T-helper2 Special Cytokine mRNA Expression during 5 Weeks Incremental Load Training

Objective To explore the exercise induced immunity changes during incremental load training from the view of the expression of helper T cells (Th) mRNA. Methods Using the technique of real-time quantified PCR to observe T-helper-special cytokine mRNA of peripheral blood mononuclear cell dynamically in healthy young male who have been trained with incremental exercise for five weeks. Results There was no obvious change in IL-2 mRNA during five weeks training. Meanwhile IL-4 mRNA increased greatly in the second, third and fourth week. IFN-? mRNA as well as IFN-? mRNA/IL-4 mRNA increased remarkably in the third, fourth and fifth week . Conclusion The expression of T-helper1/ T-helper2 cytokine mRNA were up-regulated after five weeks and the subjects were gradually adapt to the increased load which indicated the enhancement of immunity function .

Objective The purpose of this study was to compare the correlations of air displacement plethysmography (ADP, BOD-POD) with the bioelectrical impedance analysis (BIA, including TANITA and OMRON methods) and skinfold thickness (ST). Methods Six hundred and ninety-seven healthy Shanghai adults (471 male and 226 female), aged 20-to 59 year, were enrolled in this study. Measurement of body composition derived from ADP was used as the reference method. The validity of all of the other methods was assessed by comparison against the reference by one-way anova and bivariate correlations. 9.Spectral CT monochromatic imaging of hepatocellular carcinoma: effect of image fusion on image quality

Objective To assess the effects of image fusion of CT spectral monochromatic imaging on image quality in small hepatocellular carcinoma (HCC).Methods Thirty patients with 40 pathologically proven small HCCs (≤3 cm) underwent upper abdominal plain CT and dual-phase enhanced spectral CT scan were analyzed retrospectively.Conventional 140 kVp polychromatic images (group A) and monochromatic images with energy levels from 40 to 140 keV were reconstructed by using spectral imaging viewer.Monochromatic images with highest CNR (group B)and 70 keV images with lowest noise (group C) were fused to generate fused images (group D) with image fusion software.Objective evaluation of 40 HCCs [contrast-to-noise ratio (CNR) of lesion,image noise in HU] and subjective rating score of 30 patients (image noise score,overall image quality score,and lesion conspicuity score) among the four groups were compared by using One-way ANOVA and Kruskal-Wallis H test.Results CNR (1.3±0.6,2.1±0.6,1.5±0.6 and 2.4± 1.3 respectively) and image noise [(20±7),(32±9),(18±3) and (24±6) HU respectively] among group A,B,C and D all had statistical differences (F =5.724 and 13.619,both P values < 0.01).CNR in group D was higher as compared with group A and C (both P values < 0.05),but was similar to group B (P > 0.05).Image noise in group D was lower than group B and higher than group C,but showed no difference from group A (all P values > 0.05).Image noise scores [(3.5±0.5),(3.3±0.4),(3.6±0.5)and(3.5±0.4)point,respectively],overall image quality scores [(3.2 ± 0.4),(3.3 ± 0.3),(3.1 ± 0.3) and (3.7 ± 0.4) point respectively] and lesion conspicuity scores [(3.3 ±0.4),(3.9±0.4),(3.2±0.4) and (3.7 ±0.4) point,respectively] among the four groups all had statistical differences (Z =9.581,37.495 and 43.436,all P values < 0.05).Scores of the four groups were all greater than 3 and met the clinical diagnostic level.Group D was higher than group B in image noise score,higher than the other three groups in overall image quality score,higher than group A in lesion conspicuity score (all P values < 0.05).Conclusion Combined use of CT spectral monochromatic imaging and image fusion can improve overall image quality while maintaining or increasing CNR in small HCC.
10.Air-conducted ocular vestibular-evoked myogenic potential in patients with sudden sensorineural hearing loss.

OBJECTIVE:
To investigate the characteristics of air-conducted ocular vestibular-evoked myogenic potential(oVEMP) in patients with sudden sensorineural hearing loss (SSHL). So as to understanding the range and the degree of the vestibular system damage in patients with SSHL.
METHOD:
Thirty-five cases of unilateral SSHL were enrolled as the observing group from December in 2013 to December in 2014. 500 Hz air-conducted tone burst was employed as stimulation. Fifity-five healthy young subjects were recruited as the control group. The results were compared between the affected ears, the contralateral ears and the normal controls.
RESULT:
Thirty-five patients with SSHL, 31 of them in both ears lead to oVEMP Waveform, 4 sick ears did not elicit oVEMP waveform, extraction rate of 88%. 95 dBHL air-conducted get the latencies of P1 and N1, P1-N1 interval, peak-to-peak P1-N1 amplitude in healthy ear were (11.92 ± 0.85)ms; (17.07 ± 1.04)ms, (5.15 ± 0.69)ms, (5.44 ± 2.53) µv. 95 dBHL air-conducted get the latencies of P1 and N1, P1-N1 interval, peak-to-peak P1-N1 amplitude in sick ear were (12.16 ± 0.76) ms; (16.94 ± 2.57)ms, (5.16 ± 0.73)ms, (2.89 ± 1.66) v. The 55 cases (110 ears) were elicited P1-N1 complex typical, extraction rate was 100%. The threshold of oVEMP examination was (82.23 ± 2.92) dBHL, 95 dBHL air-conducted get the latencies of P1 and N1, P1-N1 interval, peak-to-peak P1-N1 amplitude in these healthy young people were (11.53 ± 0.80)ms, (16.61 ± 0.87) ms, (5.18 ± 1.04) ms, (5.96 ± 2.59) µv, there were no significant differences between the affected ears, the contralateral ears and the normal controls in the latencies P1 and N1, P1-N1 interval. The wave amplitude in SSHL were lower than the healthy ear and the healthy control group.
CONCLUSION
Through the detection of oVEMP in patients with SSHL,we observed that P1-N1 wave amplitude decreased, and it may be related to the damage of the utricle and vestibular nerve.
