1.Factors influence elderly patients with hip fracture sitting early away from bed after operation and countermeasures

Objective To explore the factors influencing the sitting early away from bed after operation in elderly patients having hip fractures.Method Forty-six elderly patients with hip fractures who were taking postoperative functional exercises of sitting away from bed and 32 nurses were investigated on the influential factors by a questionnaire.Results More than 52.1%of the patients believed the factors influencing their sitting early away from bed including lack of family support,worry about insufficient physical strength and fear of pains.65.6%of the nurses thought the factor was their inadequate assessment.Conclusion Such measures as enhancing family support,nutritional support,relief of pains from sitting from bed and improving nurse’s ability in rehabilitative training instruction are important for the patients to sit early away from bed after operation on hip fractures.
2.Influence of evidence-based nursing on the compliance of sitting at early stage on post-operative elders with hip fracture

Objective To investigate the compliance of evidence-based nursing on post-operative elders with hip fracture sitting at early stage. Methods A total of 108 post-operative elders with hip fracture were divided into the observation group and the control group with 54 cases in each group with random digit table. The patients in the control group sit from bed based on their state of vital signs and mental status on the 2nd or 3rd postoperative day. And those in the observation group employ evidence-based nursing on the basis of the control group. The degree of anxiety, postoperative fatigue and compliance of patients were compared. Results Two weeks after the operation, the degree of anxiety scored 40.96±5.13, postoperative fatigue scored 1.89±0.65 in the observation group, which were lower than those of the control group, 56.40±4.28, 4.55±0.71, the difference was significant (t=288.41, 412.35, both P<0.01). The rate of compliance in the observation group was 94.44% (51/54), and the rate of the control group was 77.77%(42/54). The rate of compliance in the observation group was higher than that of the control group(χ2=8.57, P<0.05). Conclusions Using evidence-based nursing on post-operative elders with hip fracture sitting at early stage is able to improve the compliance of patients. It is good for reducing the postoperative complications, improving activities of daily life and mental state, promoting rehabilitation in patients as well.
3.Effect of restriction decision wheel and grade on ICU patients with critical consciousness disorder

Objective To investigate the effect of restriction decision wheel and grade ICU patients with critical consciousness disorder. Methods Two hundred and fifty four patients in ICU with critical consciousness disorder from July to December 2012, who were intubated and constrained with their limbs due to the risk of unexpected extubation, were divided into the control group (n=125) and the experiment group (n=129), according to registration sequence. The former were constrained to their limbs after evaluation referring to the constraint nursing form and the latter was constrained by using the constraint decision-making wheel and grade. The two groups were compared in terms of restraint rate, unplanned extubation rate, and incidence of skin abnormality. Results The restraint rate of the observation group was significantly lower than that of the control group (P<0.01). There was no incidence of abnormal skin in the two groups. There was no significance in the rate of unplanned extubation between the two groups (P>0.05). Conclusion Constraints decision making wheel and grade is effective in ensuring patients'safety and improving the quality of nursing safety management by reducing unnecessary constraints or the incidence of delayed constraints.
4.Application effects of new restraint tools in critical patients

Objective To investigate the application effects of the new restraint tools in the critical patient care. Methods A total of 354 critical patients were averagely divided into observation group and control group according to the order number with random number table. The patients in the control group were routinely used ordinary restraint belt with restraint upper limbs or limbs, while the patients underwent the new restraint tools and implemented personalized restraint that emphasize personalized and humanized on restraint of method, site, time and tools selecting. Results In the observation group, binding site skin abnormalities ( swelling, bruising or injury), unplanned extubation rates were lower than these of the control group (P <0. 05). Conclusions Personalized application of the new restraint tool can reduce restraint adverse events, ensure patient safety and improve effectiveness of care and safety management.
5.Application of eardrum message on barotrauma of middle ear after hyperbaric oxygenation therapy

Objective To explore the eardrum massage to reduce the effect of middle ear barotrauma after hyperbaric oxygenation therapy. Methods A total of 160 hospitalized patients with middle ear barotrauma after hyperbaric oxygenation therapy at first time were averagely divided into experimental group and control group by random number table. Valsalva maneuver had been used in the control group while eardrum message were used in the experimental group guided by pneumatic otoscope. Two groups were observed in patients with ear plug, earswelling, earache, the time of above symptom lighten and the time of eardrum improvement. Results In the observation group, the alleviated time of ear plugging time, earache and ear swelling were shorter than that of control group (t = 11. 304,8. 270,8. 640;P < 0. 01). For the I grade tympanic membrane injury, the healing time was shorter in the experimental group compared with that of the control group ( t =2. 095,P < 0. 05). Conclusions After hyperbaric oxygenation therapy, the symptom of ear plug, ear swelling, earache and tympanic hyperaemia are used eardrum message to promote blood circulation and negative pressure of middle and external ear, so as to minimize the discomfort of ear.
