1.Clinical analysis of phacoemulsification combined with goniosynechialysis for the treatment of angle-closure glaucoma with cataract

Objective To investigate the clinical effects of phacoemulsification combined with goniosynechialysis on the treatment of primary angle-closure glaucoma (PACG) with cataract. Methods The retrospective review included 71 patients (98 eyes) with PACG and cataract treated in Tianjin Huanghe Hospital. Patients were underwent phacoemulsification and intraocular lens implantation combined goniosynechialysis. Among these patients, there were 21 males (30 eyes) and 50 females (68 eyes). Patients ranged in age from 53 to 94 years old, everage (73.94 ± 6.43) years old. They were divided into acute angle-closure glaucoma with cataract group (47 patients, 64 eyes) and chronic angle-closure glaucoma with cataract group (24 patients, 34 eyes). The visual acuity, intraocular pressure (IOP), anterior chamber angle, anterior chamber depth (CCT), topical and oral medications and surgical complications were observed before and after surgery in two groups. All the patients were followed up for 6 months. Results At the end of follow-up, postoperative visual acuity, IOP, CCT, anterior chamber angle and IOP-lowing medicine (topical and oral medications) were all improved in the two groups compared with those of preoperative levels (all P < 0.01). There were no statistical differences in preoperative or postoperative average visual acuity, IOP and anterior chamber angle between two groups (all P > 0.05). Values of CCT were significantly higher before and after operation in chronic angle-closure glaucoma with cataract group than those of acute angle-closure glaucoma with cataract group (P < 0.05). No drug-control rate of intraocular pressure was 76.5% (26/34) for chronic angle-closure glaucoma with cataract group than that of acute angle-closure glaucoma with cataract group (100%, 64/64). There was no need on topical medication in acute angle-closure glaucoma with cataract group. There were 14 eyes had corneal edema and 8 eyes appeared iris fibrous exudation in two groups, while no other serious complications. Conclusion For patients with PACG and cataract, the combined surgery of phacoemulsification and goniosynechialysis is a safe and effective therapy that can improve visual acuity and reduce IOP with fewer complications.
2.Effects of piracetam on chronic epilepsy in childhood rats and quantitative effects on content of acetylcholine and activity of cholinacetyltranslase in hippocampus in learning-memory complex animal model

BACKGROUND: Cholinergic system projected in brain and hippocampal structure is relevant with learning and memory. Piracetam acts on protecting and repairing cerebral neural cell, resisting cerebral functional injury due to physical and chemical factors and improving learning-memory capacity.OBJECTIVE: Chronic epilepsy in childhood animal and learning-memory complex animal model were self-prepared to observe the changes in content of acetylcholine and activity of cholinacetyltranslase in cerebral hippocampus and the intervention of piracetam.DESIGN: Randomized control experiment and non-blind evaluation were designed.SETTING: Department of Pediatrics of Second Hospital of Hebei Medical University and Institute of Traditional Chinese Medicine of Hebei Medical UniversityMATERIALS: The experiment was performed in Hebei Medical University and College of Life Sciences of Hebei Normal University from July to December 2004, in which, 50 Wistar childhood rats of clean grade and either sex were employed.METHODS: Coriamyrthin injection was administrated muscularly to duplicated chronic epileptic grand mal model in rats. Muscular injection was repeated once every three days. During modeling, those with general paroxysmal convulsion with posterior extremities standing or falling with standing or general stiffness-paroxysmal attack continuously for 3 times, the injection was changed to be once every 14 days. Ten rats were selected to be in normal control without modeling. The rest 40 rats after 3 months of modeling were randomized into 4 groups, named piracetam of 2.4 g/'L group (Group A), piracetam of 4.8 g/L group (Group B), dilantin 6 g/L +piracetam 4.8 g/L group (Group C) and model group (Group D), 10 rats in each. In each group, gastric infusion was performed continuously in 3 months after modeling, once per day, 10 mL/kg. In Group A and Group B,piracetam mixed solution of 2.4 g/L and 4.8 g/L was administrated for infusion respectively. In Group C, dilantin 6 g/L and piracetam 4.8 g/L were infused. In group D and the control group, normal saline 10 mL/kg was administrated. Relevant index determination was done 1 month after medication. Morris water maze test was performed to discover platform time and searching distance of epileptic rats, continuously for 3 days, twice per day. After test, the rats were sacrificed to collect brains to determine the content of acetylcholine in bilateral hippocampus. In Group C and Group B, the searching time of 6 tests was decreased of various degrees compared with Group D [(44±9) s, (45±9) s;(43±9) s, (42±8) s; (42±7) s,(42±7) s; (40±9) s, (39±9) s; (38±7) s, (35±9) s; (35±6) s, (34±8) s,t=2.352-4.029, P < 0.05-0.01]. In every medication group, the average searching time was decreased gradually by the increased frequency of erage searching distance in Group D was remarkably increased compared with the control [(793±74) cm, (420±81) cm;(763±89) cm, (418±57) cm;(690±67) cm, (382±69) cm; (623±81) cm, (356±71) cm;(592±98) cm,(330±69) cm;(550±54) cm,(301±97) cm,P< 0.01]. In Group C and Group B, the average searching distance of 6 tests was decreased of various degrees compared with Group D [(586±91) cm, (510±89) cm;(566±70) cm,(497 ±76) cm; (521 ±84) cm, (455 ±56) cm; (480 ±74) cm, (421 ±63) cm;(437±51) cm, (396±79) cm;(392±79) cm, (385±48) cm, t=2.364-4.230, P < 0.05-0.01]. In every medication group, the average searching distance tent of acetylcholine in brain hippocampus and the activities of cholinacetyltranslase and acetylcholinesterase of rats in each group: those in Group D were all remarkably reduced compared with the control [(2.2±0.7) nmol/g,(3.8±0.9) nmol/g;(503.3±103.3) pkat/g, (778.3±125.0) pkat/g;(190.0±51.7) μkat/g, (368.3±86.7) μkat/g, P < 0.01]. In mixed group and Group B, the content of acetylcholine and activity of acetylcholinesterase were remarkably higher than the Group D [(2.7±0.6) nmol/g, (2.9±0.6) nmol/g;(256.7±58.3) μ kat/g, (306.7±88.3) μkat/g, t=3.445-4.148, P < 0.01]. In Group B, the activity of cholinacetyltranslase [(668.3±118.3) kat/g] was remarkably higher than those in the Group D(P < 0.01). Every index in group A was basically same as model group.CONCLUSION: Grand mal of chronic epileptic rat model is characterized as declined capacity of spatial learning and memory and associated with decreased content of acetylcholine and the activities of cholinacetyltranslase and acetylcholinesterase in brain hippocampus, explaining the successful complex model of learning and memory disturbance. Piracetam 4.8 g/L may increase content of acetylcholine and the activities of cholinacetyltranslase and acetylcholinesterase in brain hippocampus and improve learning-memory capacity, but its effect at 2.4 g/L was not remarkable.

4.Nursing care to patients with anastomotic leakage treated by open center vacuum aspiration

Objective To sum up the measures for nursing the patients with laparoscopic gastrectomy-induced anastomotic leakage treated by open center vacuum aspiration. Method We retrospectively reviewed the histories of 22 patients with laparoscopic gastrectomy-induced anastomotic leakage to look into the nursing measures to look into the management of open vacuum aspiration , observation of the drainage, leakage care, mental care. Results The time for open center vacuum aspiration was between 10 to 17 days, averaged (15.63 ± 3.22)d. The hospital stay was between 13 to 23 days, averaged (17.21 ± 5.47)d. No complications were observed. Conclusions The nursing measures for managing the anastomotic leakage can be effective in improving the effect of peritoneal irrigation. The mental care can be effective for the reduction of patients′pains so as to promote the heal of the leakages.
5.Effect of Tianbing Tiaodu Capsule on learning and memory,and expression of BDNF in hippocampus of epilepsy rats

AIM:To observe the effect of Tianbing Tiaodu Capsule on learning and memory,and expression of BDNF in brains of picrotoxin-kindled epilepsy rats. METHODS: The epilepsy rat model was established by picrotoxin in jection.The effects of Tianbing Tiaodu Capsule were observed on the model groups and compared with sodium valproate group and piracetam group.The behavior and emotion of epilepsy rats were observed by Action Observing Box.Its reactions were assessed by snatching rats with the gloves.The learning and memory abilities were tested after Morris water maze training.The brains were made into paraffin sections,processed for HE staining and Nissl staining.The expression levels of BDNF were measured by immunohistochemical S-P techniques. RESULTS: Taking Tianbing Tiaodu Capsule,the behavior and emotion of epilepsy rats were markedly improved.The escape latency of searching the platform were shortened and times of rats acrossing the original platform were increased.Tianbing Tiaodu Capsule could reduce neuron impairment and necrosis on the cordex and Hippocampus of epilepsy rats,improve the loss of Nissl bodies,increase BDNF positive remarks. CONCLUSION: Tianbing Tiaodu Capsule can markedly improve impairment of learning and memory in picrotoxin-kindle epilepsy rats,which might be the result of increaseing BDNF expression,the protection of Nissl bodies,making protein synthesize,improving neuron function.
6.Studies on coumarins from fruit of Cnidium monnieri and their cytotoxic activities.

This study is to study is to investigate the coumarins from Fruit of Cnidium monnieri and their cytotoxic activities. The constituents were separated by column chromatography, and their structures were elucidated by spectroscopic data analyses. The isolated compounds were evaluated for their cytoxic activities by MTT method. Eleven compounds were isolated and identified as osthole (1), bergaptan (2), xanthotoxol (3), xanthotoxin (4), imperatorin (5), isopimpinellin (6), osthenol (7), psoralen (8), 5,7-dimethoxycoumarin (9), oxypeucedaninhydrate (10), and swietenocoumarin F (11). Compounds 7, 9-11 were isolated from the Cnidium genus for the first time. Compounds 1,5,10 and 11 showed significant cytotoxic activities against L1210 cell lines at a concentration of 1 x 10(-5) mol x L(-1) with inhibitory rates of were 70.13, 63.10, 55.77, and 75.08% respectively.

7.Prenatal diagnosis of X-linked adrenoleukodystrophy in 20 pedigrees

Objective:To summarize the characteristics of genetic variation and prenatal diagnosis in pedigrees with X-linked adrenoleukodystrophy (X-ALD) and elucidate the value of prenatal diagnosis in preventing the birth of children with X-ALD.Methods:Twenty pedigrees, clinically diagnosed with X-ALD in Peking University First Hospital from November 2012 and March 2019, were included in this retrospective study. Genomic DNA was extracted from peripheral blood and amniotic fluid or chorionic villi samples of probands and their families for detecting variants in ATP-binding cassette subfamily D member 1 ( ABCD1) gene using polymerase chain reaction (PCR)-Sanger sequencing. Linkage analysis was also performed on five microsatellite markers near ABCD1 gene to exclude maternal contamination. Characteristics of ABCD1 gene variants and prenatal diagnosis of X-ALD pedigrees were summarized by descriptive statistics. Results:Twenty ABCD1 gene variants were identified in the 20 pedigrees. The variants in three probands that were not detected by next-generation sequencing were identified by PCR-Sanger sequencing. Among the mothers of the 20 probands, 17 carried ABCD1 variants and three did not. We performed 24 prenatal diagnoses on 20 pregnancies (24 fetuses) and identified eight fetuses with variants who were finally terminated. The 16 cases without variants were born alive. The validation results obtained after termination or delivery were consistent with those performed prenatally. Conclusions:No hotspot variants in ABCD1 gene are detected in these X-ALD patients and most variants are maternally inherited. PCR-Sanger sequencing is an effective method for detecting ABCD1 variants. Prenatal diagnosis for mothers who had a body with X-ALD could prevent another one from birth.
8.A descriptive study on the smoking pattern among general population in Xuzhou,2008

Objective To understand the prevalence of smoking in different sub-populations in Xuzhou area so as to develop effective tobacco control policies. Methods Through multi-stage randomized cluster sampling,a face-to-face study with standard questionnaire was carried out among residents aged above 15 years,from June to December 2008,with descriptive nature. Results Totally,44 686 people,with 21 524 males and 23 162 females at age 15 and over were investigated,including 34 391 of them from rural areas and 10 295 from the urban population. Data regarding rates on ever smoked and current smoking,regular smoking,heavy smoking,and average numbers of cigarettes smoked per day,types of tobacco products smoked,rates on quitting smoking successfully,relapse,passive smoke exposure etc.,were 22.45%,21.40%,15.49%,9.49%,15.09/d,4.68%,5.91% and 14.12% respectively. There were no significant differences in the rates of overall smoking,current smoking and regular smoking among urban and rural residents (P>0.05). However,rates of successfully quitting smoking and passive smoke exposure in the urban areas were higher than those in the rural areas (P<0.05). Rates regarding relapse,heavy smoking and average numbers of cigarettes smoked per day in the rural areas were higher than those in the urban areas(P<0.05). Rates on smoking,current smoking,regular smoking,heavy smoking,relapse and average numbers of cigarettes smoked per day were higher in males than those in females (P<0.05). Rates of quitting smoking successfully and passive smoke exposure were higher in females than those in males (P< 0.05). The major types of tobacco products smoked by people aged 15 years old and above were cigarettes (85.17%),and Chinese pipes (3.24%). Conclusion Smoking was quite common in people from Xuzhou. Our data suggested that health education should be strengthened and sustainable intervention measures be developed and implemented to control the tobacco use in the area.
9.Role of Salivary Immune Parameters in Patients With Primary Sjögren's Syndrome.

BACKGROUND: Several factors, including clinical manifestations and laboratory data, have been used to evaluate the disease activity of Sjögren's syndrome (SS). We investigated saliva indicators of disease activity in primary SS patients. METHODS: We enrolled 138 Taiwanese patients with primary SS and 100 Taiwanese normal controls. Interleukin (IL)-6, IL-17A, tumor necrosis factor-alpha (TNF-α), and rheumatoid factor (RF)-IgA levels in saliva samples were measured using ELISA or fluorescent enzyme-linked immunoassay. Serum IgG, IgA, and IgM levels were measured by nephelometry. Erythrocyte sedimentation rate (ESR) was measured with an automatic ESR analyzer. The t-test and Pearson correlation test were used. RESULTS: IL-6 level was higher in primary SS patients than in normal controls (14.23±14.77 vs 9.87±7.32, P=0.012), but there were no significant differences in IL-17A, TNF-α, and RF-IgA levels. In primary SS patients, IL-6 level correlated weakly with ESR and IgG levels (r=0.252, P=0.015, and r=0.248, P=0.017, respectively), and TNF-α level correlated weakly with IgG level (r=0.231, P=0.024). CONCLUSIONS: IL-6 may play a role in SS pathogenesis. Saliva IL-6 might be an indicator of disease activity in primary SS patients.

A new steroid with a long cross-conjugation structure, 15a-hydroxy-(22E, 24R)-ergosta-3, 5, 8 (14), 22-tetraen-7-one (1), was isolated from the marine-derived fungus Aspergillus aculeatus. Its structure was established by the extensive spectroscopic analyses, and its cytotoxicities against P388, HL-60, and PC-3 cell lines were measured in vitro.
