2.The clinical research on the screening,diagnosis and treatment of the left subclavian artery stenosis before the coronary artery bypass grafting

Objective To discuss the meanings and methods of the screening, diagnosis and treatment of the left subclavian artery stenosis before the coronary artery bypass grafting (CABG). Methods A total of 612 patients intend to perform coronary artery bypass grafting in Tianjin Thoracic Hospital,and who were with severe stenosis or occlusion with left subclavian artery were screened by non invasive arteriosclerosis before operation. The diagnosis was confirmed by computerized tomography angiography(CTA),then the stenting angioplasty to the left subclavian artery was performed.The off-pump bypass surgery with the left internal thoracic artery(LITA)-left anterior descending artery (LAD) bypass was performed one week after operation.Results The CTA and digital subtraction angiography(DSA)confirmed the results of the left subclavian stenosis according to the non-invasive artery testing.All the 5 patients were performed with the stenting angioplasty to the left subclavian artery successfully,and the residual stenosis rate was<10%.The clinical symptoms were markedly improved after operations in all symptomatic patients,and the systolic pressure difference was<20 mmHg in two upper extremities.The blood flow was enough in LITA during the LITA-LAD bypass.The angina pectoris was improved after the operation. No coronary-subclavian artery steal phenomenon occurred. Neither stroke, myocardial infraction nor death occurred during perioperative period.All the patients were followed up for a time of 6-12 months,and the average time was about 10 months.No posterior circulation,upper limbs or myocardial ischemia occurred,and the systolic pressure difference was<20 mmHg in two upper extremities.Conclusion The non-invasion testing is of unique value in the screening of the left subclavian artery stenosis.
3.The effect of the Mongolian medicine modified Tabusen-2 on kidney-yang deficiency in rats based on metabolomics

We used metabolomics to investigate the ability of a traditional Mongolian medicine called modified Tabusen-2 (MT-2) to improve kidney yang deficiency (KYD) in rats. All animal experiments were conducted under the guidance and standards of the Medical Ethics Committee of Inner Mongolia Medical University. SD rats were divided into 6 groups of six rats: a normal group, a model group, Jinkuishenqi pill administration group (1.26 g·kg-1), and MT-2 administration in high-, medium- and low-dose groups (1.512, 0.756, and 0.378 g·kg-1). KYD was established by intramuscular injection of hydrocortisone (HC) and biochemical indicators and clinical characterization was used to confirm that KYD was established. All groups received intragastrically administered drug (Jinkuishenqi pill or MT-2) or saline. Serum from each group was collected after 8 weeks and analyzed by UPLC-Q-exactive-MS to measure various biochemical indicators. The biomarkers affected by MT-2 were identified and the metabolic pathways of KYD regulated by MT-2 were analyzed by metabolomic analysis. Objective To investigate the Raman spectral characteristics of the pathological lip minor salivary glands affected in primary Sjtǒgren's syndrome.Methods Thirty pathological samples and 30 normal samples were collected in this study.The samples were examined by Raman microscope.Support vector machine (SVM) was employed to analyze the data and establish the classification model.Results The spectra of pathological tissues was different from the controls in proteins,nucleic acids,lipids and glycogen skeleton.The sensitivity,specificity and accuracy of the model established by SVM on the training sets were all 92.0% (92/100),but the sensitivity,specificity and accuracy of the model established by SVM on the testing sets were 69.2% (37/53),100.0% (37/37) and 82.0% (73/89) respectively.Conclusions There was significant difference in Raman spectra between the pathological and normal lip salivary glands,and the classification model established by SVM could discriminate the pathological glands from the normal ones.
5.Effect of excessive iodine intake on sodium-iodide symporter mRNA and protein expression of breast in lactating rats

Objective To study effect of excessive iodine intake on sodium-iodide symporter(NIS)mRNA and protein expression of breast in lactating rats.Methods60 Wistar rats,having been weaned for one month,were randomly divided into three groups according to their body weights,I.e,①normal iodine(NI,30 rats);②ten fold high iodine(10 HI,15 rats);③one hundred fold high iodine(100 HI,15 rats).Eating food containing iodine of 300μg/L and drinking water of iodine at 5,1845,20 295μg/L,respectively.After fed for 3 months,the rats mated and had offspring,and urine and milk iodine of lactating rats were determined by As-Ce-catalytic spectrophotometric method.Their marmnary glands were sampled at lactation day 10.Then NIS mRNA expression by RT-PCR was determined and NIS protein by immunohistochemistry(SABC)was observed.Results The urine iodine of 10 HI group(3597.5μg/L)and 100HI group(25 404.3μg/L)increased obviously compared with that of NI group(344.7μg/L).The milk iodine of 10HI group(27.1×103μg/L)and 100HI group(191.0×1μg/L)was higher than that of NI group(6.0×103μg/L),but the increased fold of milk iodine was not paralleled with that of urine iodine.Difference of NIS mRNA expression was significant(F=24.19,P<0.01)among the groups,and the NIS mRNA expression in 10HI(1.250±0.034)and 100HI(1.272±0.039)group were less than that in NI (1.532±0.044)group(P<0.01).The breast NIS mRNA expression in lactating rats(1.532±0.044)was significantly higher than that in unlactating rats(0.879±0.018,P<0.01).With the increasing iodine uptake,NIS protein expression decreased.Conclusions The NIS mRNA and protein in rat breasts is down-regulated by excessive iodine intake.So increasing extent of milk iodine concentration is inhibited,which is important to prevent off-spring from getting excessive iodine intake from parental generation.
6.The self-protective effect of low dosage of gentamicin.

AIMTo approach the protective effect of low dose gentamicin against high ototoxic dose of gentamicin.
METHODSThe guinea pigs were randomly divided into four groups: control group, low dose group, low dose protective group and high dose group. Each group received multiple intraperitoneal injections of gentamicin sulphate within different durations. Auditory brain stem response (ABR) was examined one day previous to the first and 24 h after the final injection respectively. The bulla was taken out so that the content of NO, MDA and the activity of LDH in cochlear were determined.
RESULTSThe threshold of ABR was significantly lower in low dose protective group compared with high dose group (P < 0.01). The content of NO (15.86 +/- 1.98 nmol/mg pro) and MDA (19.14 +/- 0.96 nmol/mg pro) in homogenate of high dose group was significantly higher than that of control group, low does group and low does protective group (P < 0.01). The increase of the content of NO and MDA induced by high dose GM could be significantly decreased by low dose GM administration previous to high dose injection (P < 0.01). The activity of LDH in homogenate of high dose group was significantly higher compared with control group, low dos group and low dos protective group (P < 0.01). There was no statistically significant difference of content of NO and MDA among control group, low does group and low does protective group.
CONCLUSIONThe protective effects resulting from previous low dose administration to high dose injection of GM may be related to the decrease of content of NO and MDA and activity of LDH both of which induced by high dose GM.

Objective To analyze and compare the MRI appearances of ovarian thecoma with pathologic findings in order to improve the knowledge of the disease.Methods Nineteen cases of ovarian thecoma confirmed by histopathology were analyzed.MRI morphological characteristics and signal intensity of the lesions were observed and compared with findings of pathomorphology.Correlation analysis between tumor size and amount of ascites was made.Results Ovarian thecoma displayed iso-or hypointense signal on T_1WI and significant hypointensity in the focal lesion on T_2WI.Hyperintensity occurred when cystic degeneration of the lesions existed.Fibrous septation was detected in some lesions.After enhancement,most lesions showed mild early enhancement with slight increase on the delayed phase.Pathological necrosis and cystic degeneration were seen in 9 cases which corresponded to the number and shape of the cystic lesions on MRI.A large amount of collagen hyperplastic was found between the oncocytes microscopically in 15 cases, which displayed significant hypointensity in the focal lesion on T_2WI;another 4 cases showed relatively less amounts of collagen hyperplastic and more oncocytes,which appeared moderate intensity in the focal leisom on T_2WI.The amount of ascites was not significantly correlated with the lesion size(r=0.43,P=0.10). Conclusions Hypointensity on T_2WI and mild enhancement pattern due to poor blood supply are the characteristics of ovarian thecoma.The MR findings can reflect the pathologic features of the tumor,which is helpful in the diagnosis and differential diagnosis.
10.Study on the clinical epidemiology of systemic lupus erythematosus among 1433 cases.

OBJECTIVETo explore factors that affecting the outcome of systemic lupus erythematosus (SLE) therapy.
METHODSFactors on the results of therapy were analysed through a case-control study.
RESULTSThe common symptoms of SLE were fever, joint pain and skin eruption on face while the common provocation factors of SLE were infection and birth. Through multivariate logistic regression analyses, factors that influencing SLE result of treatment were tachycardia, diastolic pressure step-up, complement C(3) reduction, anti-ds-DNA antibody, SLE relapse and brain syndrome with the OR values as 2.28, 2.34, 2.42, 2.47, 1.98 and 5.56, respectively.
CONCLUSIONThe symptom and clinical characteristics of SLE were complicated. SLE treatment result could be influenced by tachycardia, diastolic pressure step-up, complement C(3) reduction, anti-ds-DNA antibody, SLE relapse and brain syndrome suggesting that the prognosis of SLE patients should be comprehensively considered.
