1.Experimental study on anti-atherosclerosis effects of Guizhi Fuling Capsule

Objective To observe the anti-atherosclerosis effects of Guizhi Fuling Capsule. Methods 50 healthy SD male rats were randomly divided into 2 groups: normal group (n=12) and AS model group(n=38). The model of experimental AS rats was established by feeding high cholesterol diet and intraperitoneal injecting VitD3. The normal control group was fed by ordinary diet. AS model group was further divided into 3 groups randomly: AS group(n=12), control group (n= 12), and therapy group(n= 12). The therapy group was given Guizhi Fuling Capsule by intragastric administration.The control group was given simvastatin and captopril by intragastric administration. 0.9% NaCl was given to the AS group instead of Guizhi Fuling Capsule. The rats were killed at the 16th weekend, and their aorta arch and breast aorta were observed.Blood fat and serum levels of Endothelin-1 (ET-1), Nitric oxide (NO), high-sensitivity C-reactive protein (hs-CRP) were detected respectively. Immunohistochemistry technology was used for observing the expression of intercellular adhesion molecule- 1 (ICAM- 1 ) in breast aorta. Results The values of ET- 1, NO, hs-CRP and the amount of ICAM- 1 were (142.61 ±25.67)ng/ml, (66.05 ± 8.63) μmol/L, (2.86±0.40)mg/L, (0.29±0.05) in the normal group; (182.38±22.96)ng/ml, (28.70±4.49)μmol/L, (5.60±0.49)mg/L, (0.58±0.06)in the AS group; (154.37±21.11)ng/ml, (45.88±11.36)μmol/L, (3.66±0.34)mg/L, (0.37±0.04)in the control group and (152.13±23.23)ng/ml, (57.67±8.96)μmol/L, (3.70±0.40)mg/L, (0.36±0.04) in the therapy group respectively. The levels of ET-1, NO, hs-CRP and the amount of ICAM-1 have significant difference between the AS group and the normal group(P=2.47E-4, 6.02E-19, 1.11E-12, 2.51E-18). There was also significant difference between the control group and the AS group(P=0.005, 1.59E-14, 1.87E-5, 3.87E-14). The level of NO in the therapy group was higher than that in the control group (P=0.002). The levels of ET-1, hs-CRP and the amount of ICAM-1 did not have significant difference between the therapy group and the control group(P=0.81, 0.81, 0.48) Conclusion Guizhi Fuling Capsule has effects of Anti-atherosclerosis on rats with AS.
3.Analysis on Volatile Components in Cuscuta chinensis from Different Habitats by HS-SPME-GC-MS

OBJECTIVE:To establish a method for analyzing the volatile components in Cuscuta chinensis,and compare the difference of the volatile components in C. chinensis. METHODS:HS-SPME-GC-MS was adopted:sampling amount was 1.0 g, extracting fibers was 65 μm PDMS/DVB,equilibrium temperature was 120 ℃,equilibrium time was 15 min,extraction time was 30 min,resolution time was 3 min;GC conditions:the column was HP-5MS quartz capillary column,programmed temperature, inlet temperature was 230 ℃,carrier gas was high purity helium,the flow rate was 1.0 ml/min,splitless injection;MS condi-tions:ion source was electron ionization,temperature was 230 ℃,quadrupole temperature was 150 ℃,electron energy was 70 eV,photomultiplier tube voltage was 1.2 kV,the interface temperature was 280 ℃,and scanning range was m/z 35-550. Com-bined with the qualitative analysis for volatile components of C. chinensis from different habitats by HP ChemStation,the relative content was calculated by peak area normalization,and the data was analyzed by principal component analysis and cluster analysis. RESULTS:Totally 52 components were identified,9 of which were the common components in C. chinensis,namely leaf alcohol, 1-octene-3-ol,3-octanol,malt alcohol,diethyl phthalate,caryophyllene,nonaldehyde,octanol and palmitic acid. sample 1,2,3 were clustered into a group,then clustered with 4,5,6 into a group,sample 7,8,9 was clustered into a group,then clustered with 10,11,12 into a group,and sample 13,14,15 clustered into a group individually. CONCLUSIONS:The method is stable and reliable,and suitable for the rapid analysis of volatile components in C. chinensis;and differences of volatile components in C. chinensis from diflerent habitats are discernible.
4.Antioxidafion functions of Guizhi-Fuling Capsule in rats with atherosclerosis

Objective To study antioxidation functions effect of Guizhi-Fuling Capsule in rats with atherosclerosis(AS). Methods 50 healthy SD male rats were randomly divided into 2 groups: normal group(n=12), and AS model group (n=36). The model of experimental AS rats was established by high cholesterol diet and intraperitoneal injecting Vit D3. The normal control group was fed by ordinary diet. AS model group was randomly divided into 3 groups: AS group (n= 12), control group (n= 12), therapy group (n=12). Therapy group was given Guizhi-Fuling Capsule (0.24 g/kg · d-1) , control group was given simvastafin (0.86 mg/kg · d-1 ) and captopril(4.3 mg/kg · d-1)by intragastric administration for four weeks. Oxidized-low density lipoprotein (oxLDL),malondialdehyde (MDA), superoxide dismutase(SOD) in serum were detected at the end of therapy. Results Compared with the normal group The oxLDL [ (83.46 ± 19.42)pg/dl], MDA [ (7.38 ±2.46)nmol/ml] in serum were increased significantly(both P<0.01 ), while SOD[ (135.70±23.49)nu/ml] was decreased significantly (P<0.01 )in AS model group. Compared with the model group, the oxLDL[(45.87± 1l.91)pg/di], MDA[ (5.27 ±1.95)nmol/ml] in serum in control group ,the oxLDL[(26.73±9.06)pg/dl], MDA[(4.05±1.83)nmol/ml] in serum in therapy group were lower significandy (all P<0.01 ), and the lowering effect in therapy group was better than that in control group. SOD in serum was higher significantly in the two treatment groups [in control group (185.87±37.36)nu/ml (P<0.01)、 in therapy group (158.65±31.4S) nu/ml (P<0.05) ], and the increasing effect in therapy group was better than that in control group. ConclusionGuizhi-Fuling Capsule has the effects of resisting atherosclerosis to rats with AS. Its mechanisms maybe relate to its antioxidation functions.
5.Pancreas-kidney transplantation in 5 cases

BACKGROUND: Pancreas-kidney transplantation is an effective treatment for diabetes combined with final stage renal disease. However, as the patients suffer diabetes for a long period of time, and cardiovascular system disease is complex, pre- and post-transplantation treatment is very important for successful pancreas-kidney transplantation.OBJECTIVE: To discuss immunosuppressant, coagulant, perioperative and postoperative treatment during pancreas-kidney transplantation to provide some clinical experience for pancreas-kidney transplantation.METHODS: Clinical data of 5 cases undergoing simultaneous pancreas-kidney transplantation in Department of Urinary Surgery, the 309 Hospital of Chinese PLA General Hospital between 2003 and 2008 were retrospectively analyzed to summarize the application of immunosuppressants and anticoagulant drugs and perioperative clinical monitoring focus. RESULTS AND CONCLUSION: There were 5 male patients with an average age of 43 years, and suffered type I diabetes mellitus complicated with final stage renal disease. The preoperative insulin dosage was 1.5-2.4 U/(kg·d). One case had diabetic retinopathy and fundus oculi hemorrhage for many times; two cases showed apparent coronary atherosclerotic heart disease with preoperative cardiac ejection fraction of 52% and 50%. Exocrine of transplanted pancreas had been considered by the intestinal fluid drainage. A total of 3 cases were complete rehabilitation. Of them, 1 case developed acute rejection in the first seven days after operation, but renal function restored with the hormones impact; 1 case had postoperative acute rejection of transplanted duodenum as well as intestinal fistula, eventually, transplanted pancreas was ectomized, but transplanted kidney was preserved; two cases succeeded in restoring and no complications occurred; 1 had postoperative gastrointestinal bleeding and died from multiple organ failure. Simultaneous pancreas-kidney transplantation is the most effective way to treat the diabetes mellitus with terminal nephropathy. Because of complications in the transplanted exocrine pancreas with bladder drainage, it has been replaced by the enteric drainage. Recovery of the transplanted kidney function is important for successful transplantation. After operation, oral FK should be taken when the serum creatinine returned to 300 umol/L. The application of clotting drug is one of the important factors for recovery of transplanted pancreatic function. Jejunostomy is an important therapeutic measure to prevent the reflux of intestinal juice to the transplanted pancreas in perioperative period. In the follow-up period cathartic drugs are recommended to prevent constipation and reduce the occurrence of acute pancreatitis caused by intestinal fluid reflux.
6.Medical assessment of living-relative kidney donors prior to transplantation in 77 cases

BACKGROUND: Living-donor kidney transplantation is not only associated to prognosis of recipients, but also donors' healthy. Complete medical and psychological assessment should be performed prior to transplantation to ensure the safety of donors. OBJECTIVE: To analyze and summarize the assessment experience of living-relative kidney donors prior to transplantation. METHODS: Totally 77 cases of living-relative kidney donors admitted at Organ Transplantation Center, Second Affiliated Hospital of General Hospital of Chinese PLA between January 2006 and March 2008 were reviewed. Among them, the analysis was carried out respectively according to the live donor nephrectomy guide of the United Kingdom (2005 Edition) before January 20, 2008, and live donor kidney transplantation consensus Boao meeting after January 20, 2008. In common practice, hypertension, diabetes mellitus, cardiovascular system, infectious disease study, age, obesity, proteinuria, renal artery, renal function, receptor for HLA typing and medical ethics, were systematically evaluated. RESULTS AND CONCLUSION: Of the 77 cases of assessed patients, 69 were qualified, successful donors, and completely cured, without complications. Totally 8 cases of non-donors were due to: 2 cases for hypertension combined with end organ damage; 2 for diabetes mellitus; 1 confirmed malignant tumor in kidney-donated surgery; 1 in the activity period of hepatitis B; 1 for resistance from his wife with medical ethics. The average age of donors was 45.3 years old, including and 7 cases above 60 years old, 24 of 50-59 years old, 29 of 30-49 years old, and 9 below 30 years old. There were 39 cases of parent child donation, 1 child parent donation, 19 siblings donation, 7 cases of three generations of collateral serum, and 3 cases of donation between husband and wife. Of successful donors, blood pressure was above 140/90 in 8 cases; Successful donors were without symptoms of myocardial ischemia but 14 cases had consciously ECG ST-T changes; 3 cases had abnormal fasting blood glucose. The successful donors' body mass index (BMI) reached the average of 23.05 kg/m~2, were below 30 kg/m~2; In assessment of infectious diseases, 3 cases were hepatitis B HBs and HBc-positive in a non-activity period, and the antibody titers were below 500 IU/L. In renography, glomerular filtration rate (GFR) was assessed to average (137.3±28.5) mL/min, and no significant statistical difference emerged (P < 0.05). Vascular three-dimensional CT prompted many left renal artery root in 3 cases, accounting for 4% in successful donors, 1 case did not match, accounting for 1%. It demonstrated that the primary purpose of assessment of -living-donor renal transplantation is to ensure that the adaptability, safety and health of donors. In order to avoid the omission of important medical information and unnecessary invasive inspection, as well as reducing the fees of medical assessment, the assessment should be based on the familiar, universally recognized, clinical evidence-based and reasonable procedures and the flexible assessment process.
7.Establishment of quality control evaluation system for 3D-ultrasound-based image-guided radiotherapy

Objective To analyze the stability and accuracy of the equipment for three-dimensional ultrasound-based image-guided radiation therapy (3DUS-IGRT) in daily practice, and to provide a basis for clinical application of radiotherapy for soft tissue tumors.Methods A specific calibration phantom was used for continuous calibration and quality control of the 3DUS-IGRT equipment in a year.The method for daily quality control of ultrasound-guided equipment was explored, and its stability and accuracy were monitored.Results The phantom position errors in both Sim and Guide stations of the 3DUS-IGRT equipment were within 1 mm.Conclusions The 3DUS-IGRT equipment has a stable performance with the support of a complete set of stringent and accurate calibration and quality control, which provides a new image-guided method for precise radiotherapy for soft tissue tumors.

9.Morphology analysis of bone marrow smear and clinical characteristics of 5 cases of blastic plasmacytoid dendritic cell leukemia

Objective To explore the diagnostic value of morphology in patients with blastic plasmacytoid dendritic cell leukemiafrom bone marrow cell.Methods Clinical data of 5 patients with BPDCN leukemia in Peking Union Medical College Hospital from 2011 to 2016were collected.The morphological characteristics of the cell size,nuclei,chromatin and cytoplasm of the BPDCNtumor cells in the bone marrow smears were observed under the microscope.Results The clinical manifestation of the 5 cases involved skin lesions (5/5,100%),lymphadenopathy (4/5,80%),hepatomegaly (3/5,60%),splenomegaly (4/5,80%).Immunophenotype showed all cases were positive for CD4,CD56 and CD123.The morphologiesof BPDCN cell characterized by heterogenous clls,were frequent,including,frequent mediumsized cells with a round or irregular nucleus,lacy chromatin,basophilic cytoplasm with lack of granules,marked large pseudopodia and vacuolation which may arranged as pearl necklace along the cytoplasmic outline.They might mimicking pseudomonoblast,Pseudolymphoblast or Pseudolymphoma cell.Conclusions Plasmacytoid dendritic cells had some peculiar morphological features,the patients suffered from the clinical manifestation of skin lesion may highly suggested the diagnosis of BPDCN,Flow cytometry and pathological biopsy are necessary for the final diagnosis of BPDCN.
10.Investigation on the Ethical Cognition of Clinical Research among Medical Staff in Hospitals of Qinghai Province

Objective:To investigate the ethical cognition of clinical research among medical staff working at different grades of hospitals in Qinghai Province and thus to provide references for the construction of medical ethics organization in hospitals.Methods:A questionnaire survey was conducted to investigate the knowledge of hospital ethics among medical staff from 11 hospitals of different grades in Qinghai Province.Results:Among the 611 med-ical staff,329 (53.8%) received medical ethics education in continuing education and 378 (61.9%) knew that the ethics committee had been set up in their hospitals.The medical staff in different grades of hospitals showed a statistical difference (P < 0.05) in the medical behavior cognition of ethical laws and regulations and the need for ethical review.Conclusion:Hospitals in Qinghai province need to strengthen the training of medical ethics knowl-edge,improve medical staff's awareness of ethics committee,and promote the efficient operation of hospital ethics.
