1.Operating treatment for fracture and dislocation of lower cervical spine with articular process interlocking.

OBJECTIVETo study the operative methods for fracture and dislocation of lower cervical spine with articular process interlocking.
METHODSFrom Feb. 2002 to Feb. 2008, 57 patients with fracture and dislocation of the lower cervical spine with articular process interlocking were treated by operation involving interior, posterior and combining anterior and posterior approach, of whom there were 45 males and 12 females aged from 18 to 73 years old with average of 39.5 years. There were 35 patients caused by traffic accident, 17 patients by falling injury and 5 patienst by crash injury of heavy object. Injured cervical segment: there were 4 patienst with C3-C4, 13 patients with C4-C5, 32 patientst with C5-C6 and 8 patients with C6-C7. Fracture types of cervical dislocation: there were 32 patients with unilateral articular interlocking and 25 patients with bilateral articular interlocking. According to ASIA criteria the level of nerve injury was grade A in 9 patients,grade B in 17 patients, grade C in 19 patients and grade D in 12 patients. OPERATIVE METHODS: 14 patients were selected by anterior operation, 11 by posterior operation,and 32 by combining anterior and posterior operation.
RESULTSAll patients were followed up for 3 to 26 months averaged 11 months. According to ASIA criteria, all patients improved 1 to 2 grade. Patients with cervical fracture and dislocation recovered completely with normal cervical sequence and cervical curvature. There was no patients with cervical unstability. Bony fusion were found in the patients with anterior interbody fusion at 12 weeks after operation.
CONCLUSIONThe anterior operation was selected when one segment fractures on fresh and dislocation, or the cervical fracture and dislocation were reduction with traction, or the angulation deformity was severe. The posterior operation was selected when the cervical vertebral plate of articular facet fractures and the compression were from posterior, or multi-segment of cervical injured. The combining anterior and posterior operation was selected when the compression of spinal cord was from both side.

Objective To establish an animal model by placing one end of PICC in the hepatic portal vein of a beagle dog and leaving the other end out of its body.Methods Six Beagle dogs were given respiration anesthesia through orotracheal intubation.An incision was made through the right rectus abdominalis to locate the superior mesenteric vein (SMA) and the main hepatic portal vein.The left branch of SMA was separated and cut to put PICC into the main hepatic portal vein before being ligated and fixed.The other end of PICC was elicited through the right abdominal wall and passed beneath the skin to the back neck and fastened in case of movement.Results The anesthetic effect was good and all the operations were successful.The mean operation time was about an hour and the mean blood loss was about 15 ml.The incision healed 5-7 d after operation.Conclusion The establishment of the model can improve the effects of liver-targeting drugs,which can cut down the dosage,lower the cost of treatment and experiment and reduce the adverse effect of medicines.Through PICC,we can directly draw blood from the hepatic portal vein to measure the blood concentration before the first pass elimination.Then according to the concentration,we can calculate the absorption rate in the gastrointestinal tract,which can facilitate related experimental studies.
3.Factors affecting clinical application of sex therapy for erectile dysfunction.

OBJECTIVETo discuss the factors that affect the clinical application of sex therapy for erectile dysfunction (ED).
METHODSUrological researchers recommended free sex therapy to ED patients and their partners in the outpatient department and studied the factors affecting the patients of Medicaid their partnersology, acceptance of sex therapy by interview and observation.
RESULTSMany patients refused sex therapy. Only 11 pairs of volunteers underwent it, of whom 5 pairs gave up halfway, 1 pair were stopped by the therapist due to the breakup of their marriage, and merely 5 pairs accomplished the whole course. Among the 5 pairs of quitters, 2 pairs thought of their problem as organic and turned to drug therapy, 1 pair withdrew because the patient's girlfriend broke up with him, 1 pair gave up because they lived in another city too far away from the clinic, and the other pair discontinued because they could not spare the time from their work.
CONCLUSIONSMany factors affect the clinical application of sex therapy for ED in China. Apart from the shortage of qualified professional sex therapists, some factors from the patients and their partners are at work, such as stress of work, lack of time, home location, education background, relationship between the patient and his partner, attitude to sex therapy, and so on.

4.Sex therapy is effective for erectile dysfunction.

OBJECTIVETo evaluate the sex therapy for erectile dysfunction (ED).
METHODSWe recommended the sex therapy to ED patients and their partners (11 couples in all) in the outpatient department. Of the total number of volunteers, 2 males were accompanied by ejaculation disorder, another 2 with hyposexuality, and 1 female had vaginismus. The effect of the therapy was assessed and the problems with it analyzed by interviews with the subjects and the results of International Index of Erectile Function (IIEF) before and after the treatment.
RESULTSOf the 11 pairs of volunteers, 5 quitted halfway and 1 discontinued at the therapists' decision. The 5 couples who accomplished the whole therapy felt more satisfied with their sexual life and general relationship, with improved scores on all the items of IIEF, particularly on erectile function (EF), with the only exception of sexual desire (SD).
CONCLUSIONThe sex therapy is effective not only for ED but also for other accompanying sexual dysfunctions, the sexual dysfunctions of the patients' partners and the improvement of the general relationship of the couples.

5.The risk factors of periprosthetic fracture after hip arthroplasty:a meta-analysis.

OBJECTIVE:
To explore risk factors of the periprosthetic fracture after hip arthroplasty.
METHODS:
Potential studies were searched in databases including Pubmed, Embase, Cochrane Library, CNKI as well as Wanfang Database up to November 2018 and references in related literatures. The methodological quality of literature was estimated by Newcastle-Ottawa Scale. Raw data were merged and tested mainly by Revmain 5.3.
RESULTS:
Seventeen studies in total were appropriate with 90 632 patients. The results revealed that it increased the risk of periprosthetic fracture after hip arthroplasty, including female (=1.62, 95%CI:1.44 to 1.82, <0.01), revision(=3.78, 95%CI:1.88 to 7.58, <0.01), preoperative diagnosis of rheumatoid arthritis(=1.60, 95%CI:1.07 to 2.37, =0.02). Conversely, patients involved with cemented prosthesis fixation(=0.43, 95%CI:0.27 to 0.68, <0.01) were less likely to suffer periprosthetic fracture after hip arthroplasty. Other factors were not significantly relevant to periprosthetic fracture after hip arthroplasty, including the age, preoperative diagnosis(femoral head necrosis, osteoarthritis, developmental dysplasia of the hip, femoral fracture, concomitant heart diseases) and American Society of Anesthesiologists >=3.
CONCLUSIONS
Orthopedics doctors should constantly be cantious about the risk factors including female, revision and diagnosis of rheumatoid arthritis. They are supposed to prevent the periprosthetic fracture by gentle operation during hip arthroplasty and monitoring the functional exercise after operations when the above risk factors occur.

7.A new 3, 4-seco-lanostane triterpenoid from a marine-derived fungus Ascotricha sp. ZJ-M-5.

To study the secondary metabolites of a marine-derived fungus Ascotricha sp. ZJ-M-5, several chromatographic methods including macroporous resin, silica gel, ODS and Sephadex LH-20 were used to isolate the compounds, and their structures were elucidated on the basis of physicochemical properties and spectroscopic methods. Ten compounds were obtained and identified as ascotrichic acid B (1), (3R)-6-hydroxymellein (2), beta-carboline (3), (22E, 24R)-ergosta-7, 22-diene-3beta, 5alpha, 6beta-triol (4), (22E, 24R)-ergosta-7, 22-diene-3beta, 5alpha, 6beta, 9alpha-tetraol (5), cyclo (Leu-Pro) (6), cyclo (Ile-Leu) (7), cyclo (Pro-Val) (8), cyclo (Pro-Gly) (9), and cyclo (Hpro-Ala) (10). Among them, compound 1 is a new 3, 4-seco-lanostane triterpenoid which has been isolated from the filamentous fungi for the first time, and compounds 2-10 are firstly isolated from Ascotricha genus.

Acute ST-segment elevation myocardial infarction with multivessel disease is one of the high-risk types of coronary heart disease.Early opening of infarct-related artery and reperfusion of myocardium could significantly reduce the mortality in acute phase.However,the presence of non-culprit lesions in non-infarct-related arteries is still at risk and has an important impact on the long-term prognosis of patients.It remains controversial on how to precisely evaluate the clinical significance and revascularization value of non-culprit lesions.This article aims to review the research status and progress of guidance strategies of non-culprit lesion revascularization in patients with ST-segment elevation myocardial infarction and multivessel disease.

Tubular dentin is of great significance in the process of tooth tissue and tooth regeneration, because it is not only the structural feature of primary dentin, but also can affect the tooth sensory function, affect the differentiation of dental pulp cells and provide strong mechanical support for teeth. Scaffold is one of the three elements of tissue engineering dentin regeneration. Most experiments on dentin regeneration involve the study of the microstructure and mechanical properties of the scaffold. The microstructure and mechanical characteristics of scaffold materials have important effects on the differentiation and adhesion of odontoblast, it can directly affect the tissue structure of regenerated dentin.

As a topical plaster developed by modern pharmaceutical technology based on traditional Tibetan medicine,Cheezheng Xiaotong Tiegao has functions of promoting blood circulation,relieving swelling and relieving pain. Since its introduction in 1993,it has been widely used in the treatment of various types of acute and chronic musculoskeletal pain and various types of spinal,joint and soft tissue diseases. In order to better standardize the clinical application and improve the clinical efficacy of Cheezheng Xiaotong Tiegao,the research and development work of the Experts consensus statement on Cheezheng Xiaotong Tiegao in clinical practice was officially launched on October 19,2017,upon approval from China Association of Chinese Medicine. In this paper,main R&D process and related technical links for the experts consensus on Cheezheng Xiaotong Tiegao would be summarized,which will help the various medical workers understand,master and apply more accurately,and also provide reference for the development of experts consensus on clinical application of other topical Chinese medicines.
