1.Monitoring Indexes of Plasma CsA Concentration in Patients Receiving Hematopoietic Stem Cell Transplantation

OBJECTIVE:To study the optimal monitoring indexes of plasma concentration of cyclosporine A(CsA) in patients receiving hematopoietic stem cell transplantation.METHODS:The lowest CsA concentration(C0) and the highest CsA concentration(C2) in 23 hematopoietic stem cell transplant recipients were detected by FPIA.All the data were analyzed statistically.RESULTS:Within 6 months after hematopoietic stem cell transplanttion,C0,C2,C0+C2 and C2/C0 in the recipients were(228.84?142.48) ?g?L-1,(741.50?294.42) ?g?L-1,(970.34?391.18) ?g?L-1 and(3.88?1.94) ?g?L-1,respectively.CONCLUSION:As reasonable monitoring indexes for the plasma concentration of CsA,C0+C2 and C2/C0 can comprehensively reflect the exposure of drug in body and monitor the toxicity of CsA in liver and kidney.
2.Nursing care to patients with anastomotic leakage treated by open center vacuum aspiration

Objective To sum up the measures for nursing the patients with laparoscopic gastrectomy-induced anastomotic leakage treated by open center vacuum aspiration. Method We retrospectively reviewed the histories of 22 patients with laparoscopic gastrectomy-induced anastomotic leakage to look into the nursing measures to look into the management of open vacuum aspiration , observation of the drainage, leakage care, mental care. Results The time for open center vacuum aspiration was between 10 to 17 days, averaged (15.63 ± 3.22)d. The hospital stay was between 13 to 23 days, averaged (17.21 ± 5.47)d. No complications were observed. Conclusions The nursing measures for managing the anastomotic leakage can be effective in improving the effect of peritoneal irrigation. The mental care can be effective for the reduction of patients′pains so as to promote the heal of the leakages.
3.Application of vascularized free tissue transplantation in emergency repair of traumatized limbs:report of 86 cases

Objective To report the outcome of emergency repair traumatized limbs by vascularized free tissue transplantation.Methods From April 1988 to August 2004,86 patients,58 men and 28 women,had undergone emergency vascularized free tissue transplantation to have their injured limbs repaired in 54 cases and the missing thumbs reconstructed in 32.The patients aged from 5 to 55 (mean 27.9) years. The transplants included latissimus dorsi myocutaneons flap,anterolateral femoral skin flap,medial crural skin flaps,dorsal pedal flaps,medial plantar flap,composite tissue mass of the discarded limbs and big toe skin- nail flap.The operations were performed 1 to 5 days after injuries.Results Postoperative vascular crises occurred in 8 cases and were all followed by exploration with successes in 5 cases while failure in 3.The total survival rate of the transplants was 96.5% (83/86).In this series all the patients were followed up for 1 to 16 years with a mean of 7.5 years only to reveal satisfying functional recovery in all the repaired limbs and an ex- cellent and good rate of 87.5% in the reconstructed thumbs.Conclusion Emergency vascularized free tis- sue transplantation is an effective way to repair a traumatized limb and to reconstruct a traumatically missing thumb.
5.Effect comparison of statins on myocardial injure and inflammatory factors in patients with unstable angina pectoris undergoing percutaneous coronary intervention

Objective To compare the effect of atorvastatin and rosuvastatin on myocardial injure and inflammatory factors in patients with unstable angina pectoris(UAP)undergoing percutaneous coronary intervention(PCI).Methods Ninety six UAP pa-tients undergoing PCI were enrolled in the study and divided into two groups,patients were given atorvastatin(20 mg/d)or rosuv-astatin(20 mg/d)besides conventional treatment for 1 week before PCI.Datas were collected before medication,PCI and Twenty-four hours after PCI,including CK-MB,cTnI,hs-CRP,TNF-α,IL-6 and IL-10.Results Twenty-four hours after surgery,the PCI, CK-MB,cTnI,hs-CRP,TNF-α,IL-6 of rosuvastatin group were lower than those of atorvastatin group(P <0.05),but IL-10 of ro-suvastatin group was higher than that of atorvastatin group(P <0.05).Conclusion Rosuvastatin (20 mg/d)is better than atorvas-tatin (20 mg/d)in the efficacy of UAP patients undergoing PCI,which can reduce the level of myocardial injure and inflammatory factors effectively.

7.The role and mechanism of TRAP1 in the development of esophageal cancer

Objective The aim of this study was to investigate the role and mechanism of tumor necrosis factor receptor-re-lated protein 1(TRAP1)in the progression of human esophageal cancer. Methods Immunohistochemistry was used to detect the ex-pression of TRAP1 and S100A8 in human esophageal cancer tissues. A stably knocked-down TRAP1 cell line was established in the esophageal cancer KYSE150 cell line,the proliferation ability was detected by CCK-8,the transfer ability was detected by Transwell, and apoptosis was detected by flow cytometry. We conducted a gene profiling study to detect the expression of genes related to tumor progression. The expression of TRAP1 downstream genes-E-Cadherin,N-Cadherin and S100A8 was detected by Real-Time fluo-rescent quantitative PCR. Results The expression of TRAP1 in esophageal carcinoma was significantly higher than that in adjacent tissues and correlated with S100A8(χ2=4. 141,P<0. 001). The KYSE150 cell line with down-regulated of TRAP1(KYSE150-TRAP1)was established,and the expression of TRAP1 was down-regulated by 85% ,cell invaded ability was decreased by 46% ,no changes of cell proliferation and apoptosis were observed,when compared to the KYSE150 control cells. The expression level of E-cadherin was increased by 19% ,and the expression level of S100A8 was decreased by 39% in KYSE150-TRAP1 cells. Conclusion TRAP1 is overexpressed in esophageal carcinoma and promotes the metastasis of esophageal carcinoma by regulating S100A8 ex-pression.
8.Clinical Use of Antibiotics in General Hospitals

OBJECTIVE To evaluate the use of antibiotics in general hospitals of Ningbo. METHODS Totally 4 391 case history records in April,2004 of 12 hospitals were investigated on the use of antibiotics. RESULTS Medicines incomes accounted for 55.16% in total revenue in a hospital,and antibiotics accounted for 32.66% among medicines incomes.Antibiotics using rate was 62.11% in internal medicine departments.The percentage of antibiotics using without evidence was 23.92%,the combined antibiotics using rate was 49.89%.The average duration for antibiotics using was 10.83 days.Antibiotics using rate in surgery departments before operation was 71.00%,during operation was 20.48%,the combined antibiotics using rate during operation was 14.50%,antibiotics using rate after operation was 96.55%.Antibiotics using for treatment accounted for 62.37% and for prevention was 24.17%. CONCLUSIONS Antibiotics using rate is high in hospitals in Ningbo.Income of medicine is also an important part of total revenue in a hospital.We should pay more attention to management on clinical use of antibiotics.
9.Correlation between the atypical presentation of myasthenia gravis and radio-pathological classification of the thymus – A retrospective cohort study

Myasthenia gravis (MG) is a disease of neuromuscular junction and mainly autoimmune in aetiology.
The state of thymus is a critical determinant for the prognosis. In this retrospective review study, we
aimed at clarifying the relationship between the mode of clinical presentation of MG and the radiopathological classification of the thymus. We identified patients with MG from the database of our
medical center from 1988 – 2017. The patients were classified into two groups according to their
clinical presentation: those with a typical presentation with diurnal variation, and those with an atypical
presentation of persistent weakness or respiratory failure from the beginning. The underlying thymic
state was categorized into six groups: normal, abnormal by imaging (if no operation was performed),
hyperplasia, benign thymoma, cortical type thymoma, and malignant thymoma. In total, 227 patients
(133 females and 94 males) were included in the analysis, of whom 68% were classified into the
typical presentation group. The atypical presentation correlated significantly with thymic categories
(p = 0.014) and sex (p = 0.026) but not age at onset (p = 0.232). The atypical presentation was more
common in the male patients and in those with thymic carcinoma.
