Objective To explore the methods for coverage of the defect of great toe after the wrap-a- round flap transferand decrease the morbidity of donor site in great toes.Methods Twenty-five patients received three kinds of procedure for immediate resurfacing of donor defect of the great toes during wrap-around flap transferAmong them9 cases received the free flaps for coverage of defect in donor great toes12 cases was repaired by local pedieled dorsal or plamarpedis flapsand the other cases were treated by the nail-flap of second toe.Results All the flaps were survivalTwo patients received the flap thinning procedure in 6 months laterall patients were satisfied with cosmetic and functional outcomeThe appearance and sensory function of donor toe repaired by second toe nail-flap was best among three methods.Conclusion Accord- ing the detect situation of great toesthree kinds of flap were selected for immediate coverage of donor site, which can decrease the complication of donor great toe at the most.

Objective To study effect of excessive iodine intake on sodium-iodide symporter(NIS)mRNA and protein expression of breast in lactating rats.Methods60 Wistar rats,having been weaned for one month,were randomly divided into three groups according to their body weights,I.e,①normal iodine(NI,30 rats);②ten fold high iodine(10 HI,15 rats);③one hundred fold high iodine(100 HI,15 rats).Eating food containing iodine of 300μg/L and drinking water of iodine at 5,1845,20 295μg/L,respectively.After fed for 3 months,the rats mated and had offspring,and urine and milk iodine of lactating rats were determined by As-Ce-catalytic spectrophotometric method.Their marmnary glands were sampled at lactation day 10.Then NIS mRNA expression by RT-PCR was determined and NIS protein by immunohistochemistry(SABC)was observed.Results The urine iodine of 10 HI group(3597.5μg/L)and 100HI group(25 404.3μg/L)increased obviously compared with that of NI group(344.7μg/L).The milk iodine of 10HI group(27.1×103μg/L)and 100HI group(191.0×1μg/L)was higher than that of NI group(6.0×103μg/L),but the increased fold of milk iodine was not paralleled with that of urine iodine.Difference of NIS mRNA expression was significant(F=24.19,P<0.01)among the groups,and the NIS mRNA expression in 10HI(1.250±0.034)and 100HI(1.272±0.039)group were less than that in NI (1.532±0.044)group(P<0.01).The breast NIS mRNA expression in lactating rats(1.532±0.044)was significantly higher than that in unlactating rats(0.879±0.018,P<0.01).With the increasing iodine uptake,NIS protein expression decreased.Conclusions The NIS mRNA and protein in rat breasts is down-regulated by excessive iodine intake.So increasing extent of milk iodine concentration is inhibited,which is important to prevent off-spring from getting excessive iodine intake from parental generation.
6.The self-protective effect of low dosage of gentamicin.

AIMTo approach the protective effect of low dose gentamicin against high ototoxic dose of gentamicin.
METHODSThe guinea pigs were randomly divided into four groups: control group, low dose group, low dose protective group and high dose group. Each group received multiple intraperitoneal injections of gentamicin sulphate within different durations. Auditory brain stem response (ABR) was examined one day previous to the first and 24 h after the final injection respectively. The bulla was taken out so that the content of NO, MDA and the activity of LDH in cochlear were determined.
RESULTSThe threshold of ABR was significantly lower in low dose protective group compared with high dose group (P < 0.01). The content of NO (15.86 +/- 1.98 nmol/mg pro) and MDA (19.14 +/- 0.96 nmol/mg pro) in homogenate of high dose group was significantly higher than that of control group, low does group and low does protective group (P < 0.01). The increase of the content of NO and MDA induced by high dose GM could be significantly decreased by low dose GM administration previous to high dose injection (P < 0.01). The activity of LDH in homogenate of high dose group was significantly higher compared with control group, low dos group and low dos protective group (P < 0.01). There was no statistically significant difference of content of NO and MDA among control group, low does group and low does protective group.
CONCLUSIONThe protective effects resulting from previous low dose administration to high dose injection of GM may be related to the decrease of content of NO and MDA and activity of LDH both of which induced by high dose GM.

7.The correlative study on the occurrence of negative emotion of advanced cancer patients and the emotional changes of caregivers

Objective To explore whether the change of emotion of patients with malignant tumors affect their family member and other people around. Methods 62 cases of hospitalized cancer patients and their family member were respectively investigated by using self-rating anxiety scale (SAS) at the same time.According to the result of SAS, patients and their family member were divided into anxious group and nonanxious group. In incidence of negative emotion and abreaction status were compared between the patients group and the family member group. Results 88.71% patients occurred anxiety symptom and 96. 36% patients vented their negative emotion. 85.48% patients' family members occurred anxiety and 94.34% vented their negative emotion. Incidence of anxiety and abreaction status was no significant deference between patients and their family member. Conclusions The negative emotion of advanced cancer patients affected their family member and other people around. Those affected people vented their negative emotion to others. Therefore,psychological nursing should be taken in early stage both for patients and their family member.

9.Role of aerodynamic parameters in voice function assessment

Objective To investigate the application and significance of aerodynamic parameters in voice function assessment. Methods The phonatory aerodynamic system (PAS) was used to collect aerodynamic parameters from subjects with normal voice,vocal fold polyp,vocal fold cyst,and vocal fold immobility.Multivariate statistical analysis was used to compare measurements across groups. Results Phonation threshold flow ( PTF),mean flow rate (MFR),maximum phonation time ( MPT),and glottal resistance (GR) in one hundred normal subjects were significantly affected by sex ( P < 0.05 ),while phonation threshold pressure (PTP),subglottal pressure (SGP),and vocal efficiency (VE) were not (P >0.05). PTP,PTF,MFR,SGP,and MPT were significantly different between normal voice and voice disorders (P < 0.01 ),and there were no significant differences among the three disorders ( P > 0.05 ).Receiver operating characteristic (ROC) analysis found that PTP,PTF,SGP,MFR,MPT,and VE in one hundred thirteen voice dis orders had similar diagnostic utility (P <0.01 ),with PTP exhibiting the highest area under the curve.The aerodynamic parameters of the three degrees of voice dysfunction due to vocal cord polyps were compared and found to have no significant differences(P >0.05).PTP,PTF,MFR,SGP and MPT in forty one patients with vocal polyps were significantly different after surgical resection of vocal cord polyps (P < 0.01 ).Conclusion The aerodynamic parameters can objectively and effectively evaluate the variations of vocal function,and have good auxiliarv diagnostic value.
10.Expression of c-MPL in leukemic stem cells from acute myeloid leukemia patients.

This study was aimed to investigate the expression of c-MPL in acute myeloid leukemia (AML) and the correlation of the c-MPL expression with CD34 and CD38, so as to define the expression of c-MPL in leukemic stem cells. The expression levels of CD34, CD38 and c-MPL were detected by flow cytometry in bone marrow cells from 29 newly diagnosed AML patients. The relationship of c-MPL positive cell ratio with clinical parameters and correlation of c-MPL with CD34 and CD38 expression in AML patients were analyzed. The results showed that expression level of c-MPL in AML patients was significantly higher than that of normal controls (P < 0.05), and the expression level of c-MPL did not correlate with age, sex, white blood cell count, AML1-ETO fusion gene and remission after chemotherapy, but the expression of c-MPL in M2 and M5 patients was higher than that of normal control (P < 0.05). Expression of c-MPL in CD34 positive AML patients was obviously higher than that in CD34 negative AML patients (P < 0.01). c-MPL was significantly higher expressed in CD34(+) cells than that in CD34(-) cells (P < 0.001), while c-MPL expression was not significantly different between CD34(+)CD38(-) and CD34(+)CD38(-) cell groups. Positive correlation between c-MPL and CD34 expression was observed (r = 0.380, P = 0.042). It is concluded that expression of c-MPL is higher in AML patients, and positively correlates with the expression level of CD34. The c-MPL expresses in leukemic stem cells.
