1.Clinical value of contrast - enhanced ultrasound for intra - ocular tumor diagnosis

AIM: To investigate the application value of contrast -enhanced ultrasound ( CEUS) and analysis software for intra-ocular tumor diagnosis.
METHODS:Retrospective analysis for 90 cases (90 eyes) with intra- ocular tumor with CEUS examination, and all cases were confirmed by pathology after surgery or clinical comprehensive diagnosis and follow-up.
RESULTS: Choroidal hemangioma, choroidal melanoma, choroidal metastatic carcinoma had obvious different CEUS enhancement pattern, quantitative analysis indexes of those tumors by imaging analysis software were significantly different (P<0. 05).
CONCLUSION: The CEUS can provide quantitative analysis for intra - ocular tumor with preferable clinical application value.

4.Detection on effect of different processed Cibotium barometz on osteoblasts by CCK-8.

The pancreatic enzyme-II type collagenase digestion method was adopted for primary culture of osteoblasts, inoculation and passage. They were identified by alkaline phosphatase dye-liquor. N-butanol extract fractions from different processed products of Cibotium barometz were prepared. The above osteoblasts were jointly cultured with protocatechuic acid, protocatechuic aldehyde, kojic acid and the mixed control liquid of the above three substances, and their proliferation was detected by CCK-8. Various n-butanol extract fractions from different processed products of C. barometz showed a significant proliferative effect on osteoblasts in the order of the wined > the heated > the salted > the sand-heated and wined system > the alcohol-processed > the steamed > the crude. The q test showed no significant difference among sand-heated, alcohol-processed and steamed C. barometz, no significant difference between heated and salted C. barometz. Various control substances also showed a certain proliferative effect on osteoblasts in the order of the mixed control > protocatechuic aldehyde > protocatechuic acid > kojic acid. The q test showed no significant difference between protocatechuic aldehyde and protocatechuic acid. All of n-butanol extract fractions from different processed products of C. barometz showed a significant effect on osteoblast proliferation, of which wined C. barometz showed the best effect. All of phenolic compounds such as protocatechuic aldehyde, protocatechuic acid and kojic acid showed a significant proliferative effect on osteoblasts.

5.Influencing factors analysis of the therapeutic efficacy of targeted therapy in patients with epidermal growth factor receptor mutant advanced non-small cell lung cancer

Objective:To investigate the influencing factors of the therapeutic effect of epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI) in patients with epidermal growth factor receptor (EGFR) mutant advanced non-small cell lung cancer (NSCLC).Methods:The clinical data of 104 EGFR mutant advanced NSCLC patients who received EGFR-TKI treatment in Wuxi No.2 Hospital Affiliated to Nanjing Medical University from January 2015 to October 2019 were collected. The correlation of different types of EGFR mutation with the clinicopathological characteristics, the hematological examination results and the treatment mode of patients was analyzed. Cox proportional hazard model was used to analyze the association of the progression-free survival (PFS) time of patients receiving EGFR-TKI treatment with the different types of EGFR mutation, the clinicopathological characteristics, hematological related indexes and treatment mode. Kaplan-Meier method was used to analyze the independent influencing factors for the PFS of the stratified patients.Results:The overall disease control rate (DCR) of patients receiving EGFR-TKI treatment was 92.3% (96/104). Cox univariate analysis showed that the levels of carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), D-dimer, and previous surgical treatment history of patients receiving EGFR-TKI treatment were associated with PFS of patients (all P < 0.05). Cox multi-factor analysis showed that EGFR mutation type ( HR =2.371, 95% CI 1.298-4.332, P = 0.005), combination therapy ( HR = 0.489, 95% CI 0.245-0.978, P = 0.043) and choice of therapeutic drugs ( HR = 0.261, 95% CI 0.113-0.606, P = 0.002) were independent influencing factors for the PFS of patients receiving EGFR-TKI treatment. The PFS of EGFR 19 exon-mutant patients with advanced NSCLC was longer than that of those with EGFR 21 exon-mutant (median PFS time: 14.0 months vs.9.5 months, P<0.05); the PFS of combination of radiotherapy or chemotherapy was longer than that of EGFR-TKI single therapy (median PFS time: 15.0 months vs. 9.0 months, P<0.05), the PFS of patients receiving erlotinib was better than that of those receiving gefitinib ( P<0.05). According to EGFR mutation types, it was found that EGFR 19 exon-mutant patients receiving EGFR-TKI in first-line treatment could obtain better PFS than those who receiving EGFR-TKI in second-line and above treatment (median PFS time: 14.0 months vs. 9.5 months, P<0.05). When receiving EGFR-TKI, EGFR 19 exon-mutant patients with CA125 <85 U/ml could obtain longer PFS time than those with CA125 ≥85 U/ml (median PFS time: 14.0 months vs. 6.5 months, P<0.05). Conclusions:The therapeutic effect of EGFR-TKI in EGFR-mutant patients with advanced NSCLC is positive. EGFR 19 exon-mutant NSCLC patients with low-level CA125 receiving EGFR-TKI in first-line treatment can obtain better PFS.
6.Effects of lamotrigine on cognitive function and quality of life in epilepsy patients

Objective To explore the effects of lamotrigine on the cognitive function and the quality of life in epilepsy patients.Methods This was a prospective study and 91 newly diagnosed epilepsy patients were enrolled.The neuropsychological tests score and the quality of life in epilepsy inventory(QOLIE-31) were obtained before and after the treatment with lamotrigine.A battery of neuropsychological tests comprised the auditory verbal learning test(AVLT), the logical memory test(LMT), the digital symbol test(DST), the stroop color word test(SCWT), the trail making test(TMT), the verbal fluency test(VFT), the WAIS block design test(WBDT), the WAIS digital span test(WDST)and the Boston naming test(BNT). Results The repeated assessments in the patients taking lamotrigine were associated with significant improvements in many domains.The greatest changes were observed in the immediate and delayed recall of AVLT, DST, the time consuming of SCWT card C and TMT test A and B, the immediate and delayed recall of LMT, VFT, WBDT and BNT.For the quality of life, significant improvements were recorded in the fields of the seizure worry(38.81?16.06 vs 45.68?15.18), the overall quality of life(59.12?13.50 vs 64.99?13.33), the social function(64.59?25.14 vs 69.41?22.70)and the self-health evaluation (71.18?13.73 vs 76.75?11.30).Conclusion Improvements of the cognitive function and the quality of life can be observed in the initial period of medication with lamotrigine in epilepsy patients.
7.Bilateral maxillary bisphosphonate-related osteonecrosis of the jaw: a case report

Bisphosphonate-related osteonecrosis of the jaw (BRONJ),as one serious side-effect of bisphosphonate therapy,has been known for more than ten years since it was first reported in 2003.In the majority of the cases,BRONJ occurs more commonly in the mandible.Those involving the maxilla are relatively few.This paper reported a case that a patient with multiple myeloma developed bilateral maxillary BRONJ after tooth extraction.The patient had used bisphosphonates for more than three years,meanwhile with uncontrolled diabetes mellitus.The patient recovered completely after surgical treatment,in combination with diabetes disease control and antibiotics application.Two key factors to ensure the success of surgical treatment are as follows:sufficient removal of infected and necrotic tissue,and good blood-supply for the local flap to help completely close the wound.The literature was reviewed to analyze the reasons why bone necrosis related to bisphosphonates was most likely to occur in the jaw,especially in the mandible,according to the pathogenesis of this disease.Furthermore,the related risk factors of BRONJ presented in this case were discussed,such as tooth extraction,oral infection and diabetes mellitus,etc.We summarized adjuvant prophylaxes for prevention of BRONJ after tooth extraction,for example,drug holiday that could be used in the dental clinic.This case report reminds us that it's of great importance to establish the awareness that the osteonecrosis of the jaw may be related to the use of some bone-stabilizers.As for patients with a history of exposure to antiresorptive or antiangiogenic agents,dentists are supposed to be cautious.It's recommended to take appropriate measures in perioperative period of oral surgical treatment to prevent BRONJ.
8.Autophagy Activation and Mitochondrial Quality Control in Sarcopenia.

Sarcopenia,an age-related disease caused by the imbalance in protein synthesis and degradation,can result in significant decreases in skeletal muscle mass and strength.Skeletal muscle loss during aging is inevitable and can affect the life quality of the elderly.Moreover,it may increase the risks of other age-related diseases in the elderly.However,the underlying molecular mechanism remains unclear in age-related skeletal muscle loss.Autophagy is a degradation pathway for the removal of dysfunctional organelles and damaged macromolecules during aging.Mitochondria also play a key role in skeletal muscle function.To maintain skeletal muscle mass,we should pay attention to autophagy and improve mitochondrial homeostasis through autophagy or other means.This paper summarizes the research progress of autophagy and mitochondrial quality control in sarcopenia,aiming to provide reference for exploring new therapies.

9.Functional MRI study of mild Alzheimer's disease using amplitude of low frequency fluctuation analysis.

BACKGROUNDPrevious studies have shown that the functional brain activity in the resting state is impaired in Alzheimer's disease (AD) patients. However, most studies focused on the relationship between different brain areas, rather than the amplitude or strength of the regional brain activity. The purpose of this study was to explore the functional brain changes in AD patients by measuring the amplitude of the blood oxygenation level dependent (BOLD) functional MRI (fMRI) signals.
METHODSTwenty mild AD patients and twenty healthy elderly subjects participated in the fMRI scan. The amplitude of low frequency fluctuation (ALFF) was calculated using REST software.
RESULTSCompared with the healthy elderly subjects, the mild AD patients showed decreased ALFF in the right posterior cingulate cortex, right ventral medial prefrontal cortex, and in the bilateral dorsal medial prefrontal cortex. No brain region with increased ALFF was found in the AD group compared with the control group.
CONCLUSIONSThe reduced activity in the posterior cingulate cortex and medial prefrontal cortex observed in the present study suggest that the functional abnormalities of those areas are at an early stage of AD. The ALFF analysis may provide a useful tool in fMRI study of AD.

10.Research practices of conversion efficiency of resources utilization model of castoff from Chinese material medica industrialization.

The industrialization chains and their products, which were formed from the process of the production of medicinal materials-prepared drug in pieces and deep processed product of Chinese material medica (CMM) resources, have generated large benefits of social and economic. However, The large of herb-medicine castoff of "non-medicinal parts" and "rejected materials" produced inevitably during the process of Chinese medicinal resources produce and process, and the residues, waste water and waste gas were produced during the manufactured and deep processed product of CMM. These lead to the waste of resources and environmental pollution. Our previous researches had proposed the "three utilization strategies" and "three types of resources models" of herb-medicine castoff according to the different physicochemical property of resources constitutes, resources potential and utility value of herb-medicine castoff. This article focus on the conversion efficiency of resources model and analysis the ways, technologies, practices, and application in herb-medicine cast off of the conversion efficiency of resources model based on the recycling economy theory of resources and thoughts of resources chemistry of CMM. These data may be promote and resolve the key problems limited the industrialization of Chinese material medica for long time and promote the realization of herb-medicine castoff resources utilization.
