1.Efficacy analysis of different therapeutic methods in low-risk patients with clinical stage Ⅰ nonseminomatous germ cell testicular tumors

Objective To evaluate the effects of rigorous surveillance and retroperitoneal lymph node dissection (RPLND) in the treatment of low-risk patients with clinical stage Ⅰ nonseminomatous germ cell testicular tumors (NSGCT) after radical orchiectomy.Methods The data of 71 patients with clinical stage Ⅰ NSGCT were analyzed retrospectively in Hunan Provincial Tumor Hospital,Xiangya Third Hospital of Central South University and Hunan Provincial People's Hospital between Feb,2001 and Apr,2012.Excluding lymphatic and vascular invasion,percentage of embryonal carcinoma>50% and increasing tumour markers (AFP/β-HCG) following orchiectomy,46 low-risk patients out of 71 patients with clinical stage Ⅰ NSGCT were selected and divided into rigorous surveillance group (30 cases) and RPLND group (16 cases) according to different therapeutic methods after radical orchiectomy.Univariate analysis was used to confirm variables associated with disease progression,and the disease free survival rates (DFSR) were compared by using Kaplan-Meier analysis.Results Five cases were lost,and 41 cases were followed up for an average of 61 months (range,15-147 months),with 58 months in rigorous surveillance group (range,19-147months) and 65 months in RPLND group (range,15-144 months).The survival rate was 100% in 2 groups.The DFSR was 89% (24/27) and 86% (12/14),respectively,and there was no significant difference between the 2 groups (x2 =0.08,P=0.78).The DFSR was 83% in patients with small amout of embryonal (percentage of embryonal carcinoma < 50%),and 92% in patients without embryonal carcinoma,and there was no significant difference between the 2 groups (x2=1.07,P=0.30).Also there was no significant difference between the patients less than 15 years and patients more than 15 years (x2=1.59,P =0.21).Conclusions There is no significant difference in recurrence rate and DFSR between rigorous surveillance group and RPLND group.Low-risk patients with clinical stage Ⅰ NSGCT may achieve satisfactory prognosis with surveillance after radical orchiectomy.
2.Spatial epidemiological analysis of severe hand, foot and mouth disease in Guangxi, 2014-2018

Abstract: Objective To explore the spatial epidemiological characteristics of severe cases hand, foot and mouth disease (HFMD) in Guangxi, China, from 2014 to 2018, and to provide a basis for identifying the high-risk regions as well as the prevention and control of severe cases of HFMD in Guangxi. Methods Spatial-temporal scanning analysis, global and local spatial autocorrelation analysis were used to analyze the spatial clustering of HFMD. The trend surface analysis was used to evaluate the spatial distribution trend of HFMD. Results From 2014 to 2018, the incidence and severe case fatality rates of HFMD were 3.89/100 000 and 4.23%, respectively. Monte Carlo scanning analysis showed that the first cluster region was Cenxi City, the second cluster was mainly concentrated in northwest of Guangxi, and the aggregation time was mainly concentrated in April to May and August to October. The global spatial autocorrelation analysis showed that the severe HFMD was significant clustering distribution, and the Moran's I coefficients of the sever cases, severe morbidity and severe case fatality rate were 0.088, 0.118, 0.197, respectively (P<0.05). Local spatial autocorrelation analysis showed that hotspots of severe HFMD cases were concentrated in the southern Guangxi, mainly in Lingshan County. Anselin local Moran's I clustering and outlier analysis indicated that 5 high-high (H-H) clustering regions for fatality were Lingshan, Pubei, Zhongshan, Zhaoping and Pinggui County. There were 6 high-high (H-H) clustering regions for severe incidence rate, namely Lingshan, Qinnan, Lingyun, Youjiang, Bama Yao Autonomous and Pinggui County, and 1 high-low (H-L) clustering region, Cenxi County. The trend surface analysis showed that the overall number of severe cases of death decreased from east or west to the middle, and increased from north to middle, and then decreased to south. Conclusions Severe HFMD cases in Guangxi have obvious spatial-temporal clustering, and the hop spots are mainly concentrated in southern Guangxi. The prevention and control of HFMD in areas with high incidence of severe cases should be strengthened to reduce the burden of HFMD cases.
3.Imaging and histological changes of the healing process of experimental sheep femoral fracture using different fixation methods

BACKGROUND: Microwave treatment is a common physical therapy method that can increase the temperature and blood circulation of deep tissues, and is used for improving fracture repair. However, microwave treatment cannot be used if there is surgically implanted metal plate or screw. OBJECTIVE: To observe the dame of microwave treatment to the tissues surrounding the titanium alloy implants. METHODS: Forty-four New Zealand white rabbits were randomized into experimental and control groups. The model of the fracture at the middle of the femur was established in all rabbits, and the rabbits in the experimental group were implanted with titanium alloy internal fixation systems. A 30-day microwave treatment (2 450 MHz, 20 W or 40 W, 20 minutes daily) was applied to the fracture site in all rabbits at 3 days after operation. RESULTS AND CONCLUSION: After 20 W of wave microwave treatment, the temperature of tissues around the implants showed no significant increase or severe heat injury. While, 40 W of wave microwave treatment significantly increased the temperature of tissues around the implants and the tissue was damaged severely. Our results indicate that, the low-dosage microwave treatment may be a promising method in the rehabilitation therapy of fractures with titanium alloy internal fixation.
4.Evolution of lubricating adsorption films with protein aqueous solution under impact loads

Objective To study the dynamic growth of adsorption films on solid surfaces in point contacts with aqueous protein aqueous solutions and investigate the dominating factors.Methods The formation of the adsorption films within a ball-on-disc contact was measured using optical interference technique.The effects of impact loads,impact times and surface properties of the materials on the adsorption of BSA aqueous solution were also studied.Results Under dynamic conditions,the thickness of the adsorption film was much higher than that under static condition.The adsorption film was growing with increasing impact times and finally the stable thickness could be achieved.Small loads and hydrophobic materials were favorable for the growth of the adsorption film.The BSA adsorption film was easy to grow on the steel surface,but on the ceramic surface the adsorption film thickness was low.Conclusions The research about adsorbed lubricating films evolution of protein aqueous solution under impact loads is of clinical significance for treating and preventing of artificial prosthesis.
5.Metagenesis of intermediate host to Schistosoma japonicum Oncomelania snail in mountainous areas

Objective To anderstand the metagenesis of Oncomelania snails in the mountainous regions so as to control the spread of snails and the epidemics of schistosomiasis.Methods Observation spot was established at a typical snail habitat close to Puge county,Sichuan province from February 2008 to July 2009.Random sampling was applied to determine the place of each frame during the observation.All the snails in each frame were collected and numbers counted in the laboratory,with the number of mating pairs in each frame also observed.Snails being collected were measured for the body indices and the dissection was carried out to identify gender composition.survival status and the number of live snails in each frame counted.Line graphs of the body indices.mating pairs in each observed months,bar graphs of the snail density,proportions of gender together with the maturity of the snails in each month were drawn.Results The number of live snail existed the whole year and its density fluctuated.All the three kinds of body indices showed the same time trend and a dynamic circulation.The young snail existed all year around and arose constantly in proportion from May,becoming the dominant snailin October to replace the adult snails.The young and adult snails also showed a dynamic alternative.The gender composition showed no significant difference during each month.The number of the mating pairs was more on April.May and June annually,when were the snail's main multiplying stage.Conclusion In mountain area.the young snails existed through all the year while adult snails appeared to be dominantin each month except for October.Oncomelania snail showed a circular process of metagenesis which started in May and finished in October.The snail population presented a dynamic equilibrium.It was concluded that ecological studies on Oncomelania snail were extremely relevant,either to optimally apply the existing control measures or to develop alternative measures for snail control,ecologically or biologically.
6.Influence of the VEGF antibody targeted vascular therapy on the expression of collagen type I in hyperplastic.

OBJECTIVETo investigate the influence of the vascular endothelial growth factor( VEGF) antibody targeted vascular therapy on the expression of human collagen type I in hyperplastic scar of nude mice.
METHODSThe hyperplastic scar from one female burn patient with 1% TBSA deep-partial thickness burns were implanted into subcutaneous skin of scapular region of 48 nude mice. Three weeks later, the nude mice were divide into large dose (LA) , medium dose (MD) , small dose (SD) and control groups, with 12 mice in each group. The mice in LA,MD and SD groups were injected with 200 microl of 15,10, 5 microg/ml VEGF monoclonal antibody diluted in 0.01 mol/L PBS, respectively in the scar twice a week for 3 weeks, while those in C group were injected with equal amount of 0. 01 mol/L PBS. The area and volume of the scar in each group were calculated and histological changes were observed, and the expression of collagen type I mRNA and its protein in each group were determined 3 days after treatment.
RESULTSThe volume of scar in LA, MD, SD and C groups were (55.3 +/-4.1, 67.9 +/-5.7, 78.9 +/-5.5, 85.0 +7.3) mm(3), respectively. Compared with that in C group, the volume of the scar were significantly decreased in AD and MD groups ( P <0.05). A few number of vessels and fibroblasts were observed in LD, MD groups, with decreased number of collagen fibers arranged in order. Compared with that in C group ,The expression of procollagen type I mRNA and its protein in C group was obviously higher than those in LD and MD groups ( P < 0. 05) , but it was similar to those in SD group.
CONCLUSIONVEGF targeted vascular therapy is beneficial for the inhibition of the angiopoietins of hyperplastic scar, the expression of collagen , and the growth of scar.

7.Transcriptional activation of TMSG-1 by complex of KLF6 and Sp1.

OBJECTIVETo investigate the regulatory mechanism of the transcription of tumor metastasis suppressor gene TMSG-1.
METHODSLuciferase reporter assay and site-directed mutagenesis were used to analyze the regulatory region of TMSG-1. Electrophoretic mobility shift assay (EMSA) and chromatin immunoprecipitation (ChIP) were carried out to verify the interaction of KLF6 and Sp1 with the regulatory region of TMSG-1. Co-immunoprecipitation (CoIP) was performed to analyze the interaction between KLF6 and Sp1. TMSG-1 and wt-KLF6 mRNA expressions in cells with different metastatic capacities were quantitated by real-time PCR. Cell invasive capability was determined by Matrigel invasion assay.
RESULTSA 63 bp inducible regulatory region (+59 bp - +123 bp) in exon 1 was identified by luciferase assay using reporter plasmids with a series of TMSG-1 regulatory region deletions. Mutations in KLF6/Sp1 binding sites of this region resulted in a decrease of luciferase activity, while cotransfection with KLF6 or Sp1 expressing plasmids led to a remarkable increase of luciferase activity. EMSA and ChIP demonstrated that KLF6 as well as Sp1 interacted with this region. CoIP also indicated a possible interaction between KLF6 and Sp1 proteins. In the highly metastatic cell sublines, a low level of wild type KLF6 was associated synchronously with a low TMSG-1 level. Prostate carcinoma cells overexpressing KLF6 exhibited a higher TMSG-1 level and a lower invasive capability.
CONCLUSIONSTranscription factor complex of KLF6 and Sp1 may participate in the inducible transcriptional regulation of TMSG-1, and a decreased wild type KLF6 expression is likely associated with a low TMSG-1 level in the highly metastatic cell sublines.

8.Screening of phosphoprotein associated with glycosphingolipid microdomains 1 (PAG1) by cDNA microarray and influence of overexpression of PAG1 on biologic behavior of human metastatic prostatic cancer cell line in vitro.

OBJECTIVETo screen for novel gene(s) associated with tumor metastasis, and to investigate the effect of overexpression of phosphoprotein associated with glycosphingolipid microdomains 1 (PAG1) on the biological behaviors of human prostatic cancer cell line PC-3M-1E8 in vitro.
METHODSFour cDNA microarrays were constructed using cDNA library of prostatic cancer cells PC-3M-1E8 (high metastatic potential), PC-3M-2B4 (low metastatic potential), lung cancer cells PG-BE1 (high metastatic potential)and PG-LH7 (low metastatic potential)to screen genes which were differentially expressed according to their different metastatic properties. From a battery of differentially expressed genes, PAG1, which was markedly downregulated in both high metastatic sublines of PC-3M and PG was chosen for further investigation. Real-time PCR and Western blot were used to confirm the gene expression of PAG1 at mRNA and protein levels. Full-length coding sequence of human PAG1 was subcloned into plasmid pcDNA3.0 and the recombinant plasmids were stably transfected into PC-3M-1E8. The cell proliferation ability, anchorage-independent growth, cell cycle distribution, apoptosis rates and invasive ability were detected by MTT, and in addition, soft agar colony formation, flow cytometry analysis and matrigel invasion assay using Boyden chamber were also carried out respectively. All experiments contained pcDNA3.0-PAG1-transfected clones, vector transfected clones and non-transfected parental cells.
RESULTSA total of 327 differentially expressed genes were obtained between the high and low metastatic sublines of PC-3M cells, including 123 upregulated and 204 downregulated genes in PC-3M-1E8. A total of 281 genes, including 167 upregulated and 114 downregulated genes were obtained in PG-BE1 cells. Nine genes were simultaneously downregulated and 8 genes were upregulated in both high metastatic cell lines of PC-3M and PG. The expression of PAG1 at mRNA and protein level were decreased in the high metastatic subline PC-3M-1E8. Western blot revealed that PAG1 protein was downregulated in PC-3M-1E8 cell line which was in agreement with the gene expression at mRNA level. The proliferation ability and clonogenicity of PAG1 overexpression cells by stable transfection were markedly decreased in comparing with that of the control cells (P < 0.05). Colonies formed in stably PAG1-transfected cells, the vector-transfected clones and parental cells were 26.7 ± 5.2, 47.2 ± 3.2 and 52.3 ± 3.4 respectively (P < 0.05). Flow cytometry analysis showed that the stable PAG1-transfected cells at G₀-G₁ phase were significantly more than that of the control cells (P < 0.05). However, no difference of the apoptosis rate was found between PAG1-transfected cells and control cells (P > 0.05). The number of cells passing through the matrigel and multipore membrane was also decreased in the stable PAG1-transfected cells (35.1 ± 4.9) compared with those of the vector-transfected clones (127.6 ± 6.6) and parental cells (135.0 ± 5.0, P < 0.05).
CONCLUSIONSUsing cDNA microarray technique and differential gene expression analysis of sublines of the parental cancer cell lines enable of revealing the metastasis-related genes, among which PAG1 represents one of those under-expressed genes in the high metastatic subline PC-3M-1E8. Transfection expression of PAG1 suppresses cell proliferation, anchorage-independent growth and invasive ability of PC-3M-1E8 cells in vitro. Conclusively, PAG1 may play an important role in inhibiting the proliferation, invasion and metastasis of the cancer cells.

9.Establishment of a digital Evans-Jensen classification model of femoral intertrochanteric fracture.

OBJECTIVETo establish a digitized Evans-Jensen classification model of femoral intertrochanteric fracture.
METHODSThe hip of a healthy male volunteer was examined with 64-slice spiral CT, and the data were processed using Mimics 10.01 software to reconstruct the proximal femur model. According to Evans-Jensen classification criteria of femoral intertrochanteric fracture, each type of fracture model was simulated. Five orthopedic surgeons and 10 medical students undertook the preliminary evaluation of the digital fracture model.
RESULTSThe digital fracture model is intuitive, three-dimensional, and realistic with good visual effect. The fracture could be observed and charted from optional direction and angle. The corresponding animation allowed 360 degrees rotation. Evaluation by the 4 orthopedic surgeons and 10 medical students confirmed that the digital fracture model was intuitive and easy to understand.
CONCLUSIONThe digital model of femoral intertrochanteric fracture is intuitive, three-dimensional, realistic and dynamic, and may help in clinical practice and medical teaching.

10.Study on effectiveness of radiotherapy line image guidance for dose-error correction in patients with abdominal tumor

Objective To investigate the effect of the correction of the position error of the abdominal tumor by cone beam scanning. Methods Six patients with abdominal tumor were selected with double-blind method, and electronical portal imaing device was used to collect the information of the patient dosimetry during radiotherapy.EDOSE tool was applied to 3D dose reconstruction so as to execute real-time 3D dose verification of the treatment plan. Each patient had 5 sets of data acquired when image guidance existed and another 5 sets when image guidance was absent. The treatment effects with or without image guidance were compared.Results The overall γ passing rate with cone beam CT image guidance gained no advantages over that without image guidance, though the setup error was corrected to some extent. Image guidance contributed nothing to increasing target area coverage while decreasing exposure dose to normal tissue. Conclusion Setup error correction by the image guidance based on bone marker has no significant effect on improving 3D dose verification and dose accuracy during the radiotherapy of the abdominal tumor patient.
