1.Investigation of survival situation of senile people' s home caregivers in Hangzhou city

Objective To study the survival situations,care skill,social support and demand condition of senile people with home care.Methods Many methods were used such as self-designed questionnaire,social support scale,interview and so on.Cluster sampling was used and totals of 202 hone caregivers of senile people from eight communities,old town of Hangzhou city were selected and were investigated with questionnaire in their homes and interviewed by face to face,then,the status of survival and care of home care-givers was analysed.Results Children(52.5% ) and spouses( 12.5% ) were main caregivers,65.8% caregivers provided 24 hours care,10.4% caregivers believed that the seriously effect of care work was sleeping,25.2% caregivers believed that they could not treated with senile person' s unusual mental well.The male score of social support was (29.02 ±5.51 ) and female was(27.50 ± 5.37 ),and the difference was statistically significant (t =1.906,P <0.05 ).Conclusions Community nurse and health manager should pay attention and assessment on the caregivers'needs and wishes,and provide feasible and appropriate social support service for caregivers.
2.Chemical constituents of Sapium sebiferum leaves.

Chemical constituents of ethyl acetate extract of Sapium sebiferum leaves were isolated and purified by various chromatographic methods, including column chromatographies over silica gel, macroporous adsorption resin, and Sephadex LH-20, as well as preparative TLC and semi preparative HPLC. As a results, 15 compounds were separated from Sapium sebiferum leaves and their structures were examined by spectral analysis including NMR and MS data and identified as( + )-(7R,7'R,7"S,7'"S,8S,8'S,8"S,8'"S)-4", 4"'-dihydroxy-3,3',3",3',5,5'-hexamethoxy-7,9';7',9-diepoxy-4,8";4',8'"-bisoxy-8,8'-dineo-lignan-7",7"',9",9"'-tetraol(1) ,1-(4'- hydroxy-3'-methoxyphenyl)-2-[4"-(3-hydroxypropyl) -2", 6"-dimethoxyphenoxy] propane-1, 3-diol (2), Thero-2, 3-bis-(4-hydroxy-3- methoxypheyl)-3-methoxy-propanol(3) , threo-5-hydroxy-3,7-dimethoxyphenyl propane-8,9-diol (4), boropinol B (5), threo-8S-7-methoxysyringylglycerol(6), 5-hydroxymethylfurfural(7), 5-( methoxy-methyl)-1H-pyrrole-2-carbaldehyde (8), quercetin (9) , kaempferol (10), ethyl gallate(11), coniferaldehyde(12), vanillin(13), 7-hydroxy-6-methoxy-2H-1-henzopyran-2-one(14),and 1-heptacosanol (15). All compounds except for compounds 9-11,14 were separated from this plant for the first time.

3.Antinociceptive effects of meptazinol and its isomers on carrageenan-induced thermal hyperalgesia in rats.

Using the latency of paw withdrawal (PWL) from a noxious thermal stimulus as a measure of hyperalgesia, the effects of i.p. injection of meptazinol and its isomers, 112824 and 112825, on carrageenan-induced thermal hyperalgesia were studied in awaked carrageenan-inflamed rats. Peripheral inflammation was induced by intraplantar (i.pl.) injection of carrageenan (2 mg/100 microl) into one hindpaw in rats. Carrageenan produced marked inflammation (edema and erythema) and thermal hyperalgesia in the injected paws, which peaked at 3 h after injection and showed little change in magnitude for another 3 h. Injection of 0.1 mg/kg meptazinol (i.p.) at 3 h after carrageenan had no effect on the PWLs of either inflamed or non-inflamed hindpaw during the next 100 min (P>0.05, n=8). At the dosage of 1 and 10 mg/kg, meptazinol produced marked anti-nociception and anti-hyperalgesia in non-inflamed and inflamed hindpaw, respectively (P<0.05, n=8-11). The prolonging effect of meptazinol on PWL in inflamed hindpaw was more potent than that in non-inflamed hindpaw. Pre-administration of 1.5 mg/kg naloxone significantly antagonized meptazinol-induced anti-nociception and anti-hyperalgesia. Intraperitoneal injection of an isomer of meptazinol, 112825 (1.5 mg/kg), but not 112824 (1 mg/kg), markedly increased the PWL of the non-inflamed hindpaw. Nevertheless, both the isomers produced similar anti-hyperalgesic effect to that of meptazinol (P<0.05, n=8), which was completely reversed by naloxone (1.5 mg/mg). The results suggest that meptazinol and its isomers have anti-nociceptive and anti-hyperalgesic properties with the former more potent. The effects are mainly mediated by mu opioid receptors. This study provides an important clue for extending clinical utilization of meptazinol and its isomers.

4.Use of bicyclol in treatment of chronic hepatitis B virus infection: a systematic review.

OBJECTIVETo evaluate the efficacy and safety of bicyclol for chronic hepatitis B.
METHODThe authors searched CBMDisk, CJFD (2000-2006), CJD-CD, PubMed (1966-2006) for randomized controlled trials (RCT) comparing bicyclol versus non-antiviral interventions, interferon alpha (IFN-a) and lamivudine for treatment of chronic hepatitis B. Two reviewers performed data extraction and quality assessment independently and discussed when there was different opinion. We analyzed the data with RevMan 4.2 software supplied by Cochrane Collabration.
RESULTSFourteen RCTs involving 1782 patients were included. ALT recovery rate of bicyclol group was 69.3 percent while that of the control group was 59.0 percent, the difference was statistical significant [RR 1.24, 95 percent CI (1.01, 1.52), P=0.04]. Loss of HBeAg in the bicyclol group (22.1 percent) was higher than that of the control group (13.5 percent) [RR 1.65, 95 percent CI (1.32, 2.06), P<0.00001]. No serious adverse events were reported.
CONCLUSIONBicyclol might be beneficial to recovery of liver function and loss of serum HBV marker. However, more high quality clinical trials are needed for confirmation.

7.Changes of renal cortex lipid peroxidation in renal injury induced by cisplatin in rats.

AIMTo study the relativity of the renocortical lipid peroxidation with renal tubules structure damage in renal injury induced by cisplatin in rats.
METHODSFemale Wistar rats were randomly divided into NS group, CDDP(I) group, CDDP(II) group and CDDP(III) group. All rats were injected via the tail vein with NS or cisplatin and NS qd in five days. The changes in content of Scr, BUN and MDA, the activity of SOD and GSH-Px of the renal cortex were measured. Alkaline phosphatase of renal tubular epithelia was stained by histochemistry and the slices of renal cortex were observed.
RESULTSThe contents of Scr and BUN of CDDP groups were significantly higher than those of NS group (P < 0.01). The content of renocortical MDA was significantly higher than that of NS group (P < 0.05). The activities of renoconical SOD and GSH-Px were lower than those of NS group (P < 0.05). The content of MDA, activities of renocortical SOD and GSH-Px with the content of Scr and BUN were significantly correlative. Alkaline phosphatase of renal tubular epithelia cells was losed largely and renal tubular epithelia cells were denaturative and necrotic partly in sections.
CONCLUSIONThe damage of renal cortex was correlative with its lipid peroxidation. The injury of renal cortex became heavier with cisplatin dose increased.

8.Correlation between fibrinogen polymorphisms and the type of cerebral infarction.

OBJECTIVETo study the association of beta-fibrinogen(Fg) gene -148 C/T and 448 G/A polymorphisms, plasma Fg concentration, molecular reactivity and the type of cerebral infarction.
METHODSGene polymorphisms were analyzed by polymerase chain reaction-restriction fragment length polymorphism. The plasma Fg concentration and the molecular reactivity were also determined.
RESULTSThe Fg concentration in MCI patients with T -148 allele was higher than that in PCI patients and controls. The MCI patients with A448 allele had higher Fg concentration, FMPV and FMPV/Amax when compared with controls, and had higher FMPV/Amax when compared with PCI patients.
CONCLUSIONFgB beta -148 and 448 mutational genotypes have impact on Fg concentrationì and therefore increase the risk of MCI.

9.The clinical and genetic studies in a family of cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy.

OBJECTIVETo investigate the clinical features and genetic diagnostic method of cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL).
METHODSA systematic study on the clinical manifestations, neuroimaging characteristics, therapeutic measures and molecular genetics was performed. An investigation on the onset and hereditary pattern of the family was also done.
RESULTSThe main clinical features including poor memory and history of stroke were found. And no risk factors of hypertension and arteriosclerosis were found. A positive family history was confirmed. Neuroimaging examination showed multiinfarct lesions and leukoencephalopathy. All these features are in conformity with those of CADASIL. A mutation in the third and fourth exon of the NOTCH3 gene was identified in the 10 cases of 4 generations. The clinical or subclinical onset in the 10 cases was consistent with classical autosomal dominant inheritance.
CONCLUSIONThe clinical and molecular genetic features of the family accord with CADASIL.

10.Study on the relationship between polymorphisms of XPA gene and susceptibility of esophageal cancer

Objective To explore the relationships between the polyrnorphisms of xeroderma pigmentosum A(XPA) and the susceptibility of esophageal cancer (EC),as well as its interaction with environmental factors-gene in Changzhi area,Shanxi province. Methods A case-control study was conducted,including 196 cases of EC and 201 controls.XPA 23G polymorphisms were determined with polymerase chain-restriction on fragment length polymorphism (PCR-RFLP).Results The risk of EC was significantly degraded in the individuals who had been carrying the XPA heterozygote (A/G) and mutation genotype (G/G),compared to those with wild genotype (X2=16.21,P<0.01) and the ORs were 0.58 (0.37-0.91) and 0.32 (0.18-0.56),respectively.There was negative interaction between XPA 23G mutation genotype and the consumption of pickled food (S=0.04,API=-0.77).Conclusion Genetic polymorphism in the XPA 23G might be associated with esophageal cancer in Changzhi area,and there was a negative action between XPA predisposing genotype and the consumption of pielded food.
