Objective To investigate the effect of surgical treatment combined with 131I in the treatment of thyroid papillary carcinoma.Methods45 cases of papillary thyroid cancer patients were selected from January 2008 to January 2012 in our hospital. All of the patients were treated with total thyroidectomy surgical, postoperative underwent thyroid hormone and 131I radionuclide therapy,The patients were followed up.ResultsAt 3 months or 6 months after operation, the levels of serum P53,Fas and TNF-a were significantly higher than immediate time after surgery (P<0.05) and cyclin E protein levels were significantly lower than immediate time after surgery (P<0.05).There were no difference of serum P53,Fas,TNF-and Cyclin E protein levels between at 3 months and 6 months after operation.During five years of follow-up, the 5-year recurrence rate of 4.4%,and the distant metastasis rate was 2.2%.ConclusionOperation combined with 131I radionuclide therapy has good clinical results, could be used as the first choice for clinical treatment.

Objective The present study was designed to investigate the efficiency of simvastatin therapy for experimental pul‐monary hypertension (PH) in rat ,and the effects on the number and function of circulating endothelial progenitor cells (EPC) . Methods Twenty four Sprague Dawley rats were divided into 3 groups randomly :model group ,treatment group and control group , 8 rats in each group .Rats were treated with a single subcutaneous injection of monocrotaline to induce PH (PBS used as control) . The rats in the experimental group were administrated with simvastatin 3 days following subcutaneous injection of monocrotaline .In the 21st day ,the number of circulating EPC ,the right ventricle systolic pressure of rat ,pulmonary vascular structural changes and the quantity of cultured EPC were measured .At the same time ,EPC in each group were cultured in vitro ,then the ability of prolif‐eration and function were analyzed and compared .Results The number of circulating EPC in model group was decreased signifi‐cantly compared to both control and model groups (P< 0 .01) .Compared with model group ,simvastatin treatment markedly de‐creased the RVSP and the ratio of media thickness to eternal diameter (P<0 .01) ,but the ratio of vessel area to total arterial area (VA/TAA) was definitively increased(P<0 .01) .After 7 days of culture in vitro ,both the output of EPC and the ability of prolif‐eration ,conglutination and migration of EPC in treatment group were up -regulated compared with those in model group (P<0 .01) .Conclusion This study confirmed that simvastatin effectively treat experimental PH through improving quantity and func‐tion of circulating EPC .

6.Sociocultural Factors Associated with Caregiver-Psychiatrist Relationship in Taiwan.
Psychiatry Investigation 2016;13(3):288-296
OBJECTIVE: Research on sociocultural factors associated caregiver-provider relationship is needed to enhance family involvement in psychiatric care. This study examines from the caregiver's perspective the associations of schizophrenia attributions, stigmatization, and caregiving experiences with caregiver-psychiatrist working relationship in Taiwan. METHODS: This cross-sectional study used a convenience sample of 152 Taiwanese family caregivers of persons diagnosed with schizophrenia, recruited from a grassroots organization, 4 community mental health rehabilitation centers and 2 psychiatric hospitals between July 2012 and March 2013. Multiple linear regression models were used for analysis. RESULTS: Biological attribution was positively associated with perceived family collaboration, and so was environmental attribution with perceived informational support. Internalized stigma was negatively associated with perceived family collaboration. Caregiving rewards were positively related to both perceived family collaboration and informational support, and so was experience of problems with services to perceived family collaboration. CONCLUSION: The examination of family perceptions informs Western psychiatric care providers of the importance of culturally sensitive practices in developing an effective working relationship with family caregivers, particularly in regards to caregivers' casual attributions, impact of stigma, and caregiving experiences.

Objective To identify gene expression profile in periphend blood mononuclear cell (PBMC)of patients with primary Sjogren's syndrome(pSS)complicated with hematology involvement and compare it to healthy volunteers. Methods Three Sjogren's syndrome(SS)patients with leucopenia and 3healthy volunteers were included. The cRNA probes prepared for total RNA were hybridized with three identical gene chips. The difference of gene expression of each patient and volunteers were compared. Results Significant difference in the expression of 82 genes could be detected between patients and volunteers.Among these,45 were upregulated, and 37 were downregulated.Statistical difierence was calculated between patients and volunteers especially in the following two pathways:the complement and coagulation pathways(including C2,PROS1,F2R and SEPPING1)and the cytokine-cytokine receptor interaction pathway(including FASLG,MPL,CCL20 and CXCL2)(P<0.01).Conclusion This study has identified distinct gene expression profiles in PBMC from patients with pSS patients with hematology involvement and healthy volunteers. This provides new knowledge about groups of genes that are upregulated during disease. It may form an excellent platform for future functional studies.

Objective To explore the changes of CD40 and CD40 ligand(CD40L) levels and investigate their significances in children with graft-versus-host disease(GVHD)after related-donor human leukocyte antigen(HLA) matching allogeneic hematopoietic stem cell transplantation(HSCT).Methods Nineteen patients with ?-thalassemia major and 1 patient with congenital inherent hemolytic anemia accepted umbilical cord blood transplantation(UCBT) and allogeneic peripheral blood stem cell transplantation(allo-PBSCT),respectively,and all cases were received successfully related-donor human leukocyte antigen matching allogeneic HSCT.Peripheral blood samples were obtained before and after transplantation, the time when GVHD happened, the expressions of CD40 and CD40L were measured by using immunofluresence asssy. Results Four UCBT children and 3 allo-PBSCT children had no acute GVHD.Thirteen children had acute GVHD(degreeⅠ-Ⅳ),the expressions of CD40L on CD4+ and CD8+ T cells in the patients with acute GVHD increased, especially in allo-PBSCT.Five cases of UCBT and 12 cases of allo-PBSCT patients had chronic GVHD,the expressions of CD40L+,CD25+ and CD69+ on CD4+ and CD8+ T cells in patients with chronic GVHD increased obviously. The expression of CD19+CD40+ was lower than normal within 3 months after transplantation. Conclusions The high expression of CD40L+T cells in periferal blood after HSCT was related to the activation and proliferation of T cells in the development of GVHD in HSCT.

Objective To discuss the value of moving darkroom and accompanying X-rays protective shield in endemic fluorosis areas. Methods Using moving and fixed darkrooms, the X-rays photos of the forearms and shanks of the 320 persons were developed in the endemic fluorosis areas, while the time for development and installing-uninstalling films was documented. The films develpoed in the darkroom were evaluated for the quality. Results Among the 320 films develpoed in the moving darkroom, the 268 had fingerprints,47 had scratches,298 were in good quality(93.13%),while in the fixed darkroom, the figure was correspondingly 735,384,227(70.93%);The moving darkroom increased excellent film rate significantly than fixed darkroom(χ2=53.43,P<0.01),The detectable rate of skelelal fluorosis and degree Ⅰ skelelal fluorosis were highter than that in the fixed darkroom did (χ2=10.34,χ2=9.56,P<0.01).Conclusions The films developed in moving darkroom have superior quality and higher diagnostic value,so it is important to use moving X-rays photographing in ecdemic fluorosis areas.
