1.Localization and ultrastructure of corneal epithelial stem cells in human fetuses

Objective To evaluate the migrating regulation and ultrastructure of the corneal epithelial stem cells in human fetuses. Methods We examined the corneal cryosections of 14-38 weeks of gestation. Hematoxylin-eosin staining showed the stratified corneal epithelium and the corneal epithelial stem cells were localized by mouse monocolonal antibody against human 64-kilodalton keratin (mAE5), and the ultrastructure of the corneal epithelial stem cells was observed. Results At 14 weeks of gestation, the corneal epithelium was composed of a single basal cells layer and 1-2 superficial squamous cells layers. Some superficial squamous cells were mAE5 positive in the limbus as well as the central and peripheral cornea. At 17-29 weeks of gestation, the limbus epithelium developed from 3 to 5 cells layers and the central region from 2 to 3 cells layers. mAE5 positive cells were found in the suprabasal layers of all 3 regions examined but not in the basal layer. At 33-38 weeks of gestation, the corneal epithelium consisting of 4-6 cells layers was morphologically mature. mAE5 immunoreaction showed the negative cells were confined to limbus basal layer. The ultrastructure of basal layer cells showed they had more heterochromatin in the nucleus, less organells in the cytoplasm and less desmosomes among them. Conclusion The migration of corneal epithelial stem cells in the human fetuses was from the whole layers to basal layer and confined to limbus region finally, and their ultrastructure was immature.
2.Initial clinical result of the computer-assisted total knee replacement system

[Objective]To evaluate the initial effect of the computer-assisted total knee replacement system.[Method]The 24 computer-navigated knees in our department were involved,and were matched at ratio of 1∶3 by previous cases at sex,age and disease.All cases anteroposterior and lateral projection X-ray films were checked postoperatively.We compared ? and ? angles at anteroposterior films,? and ? angles ant lateral films of all involved cases[Result]Although there was no significant differences existing between two groups,we still observed the obvious reduce risk of malplacement both for femoral and tibial prosthesis,and improvement of mechanical alignment.[Conclusion]By using computer-navigated total knee replacement system during operating,the precise and repeatability could be ensured.
3.Bedside ultrasonographic diagnosis in children with superior vena cava syndrome after cardiac operations

Objective To explore the findings and clinical value of bedside ultrasonography in superior vena cava(SVC) syndrome in children after cardiac operations for congenital heart disease. Methods Bedside ultrasonography was performed on 6 children suspected of SVC syndrome after 5-38 hours[(22?11) hours] postoperatively,to view the structure and blood flow of SVC and its main branches. All the patients were reoperated as soon as diagnosed. Results Five children got severe SVC obstruction, marked with stenosis, turbulent, continuous and high velocity in SVC. Color Doppler flow imaging presented almost absence of flow due to nearly complete occlusion of the innominate vein in 1 child. Ultrasonographic diagnosis was confirmed by the reoperative findings. All the patients were cured by reoperation and discharged with normal ultrasonographic results. Conclusions Bedside ultrasonography can provide early and accurate diagnosis in children with SVC syndrome after cardiac operation.
4.Clinical correlation of post cardiac surgery hyperuricemia with cardiopulmonary bypass

Objective Determining the respective incidence, risk factors and prognosis of hyperuricemia post normothermic cardiac surgery with cardiopulmonary bypass. Methods Clinical data was collected from April 2002 to October 2004. 232 patients undergoing cardiac surgery with cardiopulmonary bypass (CPB). Patients were classified into three groups: nonhyperuricemia group, hyperuricemia group, and acute goutynephropathy group. Uricemia levels 24 hours post cardiac surgery, the respective incidence, risk factors, and prognosis among three groups were analyzed. Results Seventy patients (30.1%) had hyperuricemia, and twenty-two patients (9.5%) had acute goutynephropathy during their ICU stay. Objective:To establish the quality standard for Sonehus braehyotus DC. Methods:The pharmacognosy of Sonehus braehyotus DC. were studied,and the qualitative identification by TLC and the quantitative analysis by HPLC were established. Re-sults:The microscopic characteristics of Sonehus braehyotus DC. were obvious. The TLC spots were clear with high resolution,and the quantitative research of chlorogenic acid in 10 batches of Sonehus braehyotus DC. was performed by HPLC. Conclusion:The quality control standard for Sonehus braehyotus DC. is established through the experimental studies.
7.Effects of glipizide and metformin on serum insulin-like growth factor-1, 2 in patients with type Ⅱ diabetes mellitus

AimTo study the effects of glipizide and met formin on the serum IGF-1,IGF-2 in patients with type Ⅱ diabetes mellitus; Methods The effect of glipizide(n = 40) and metformin(n = 25) on serum IGF-1, IGF-2 in patients with type Ⅱ diabetes mellitus were compared with self- controlled study. Results In metformin-treated patients ,there were not significantly changes in fasting IGF-1 and IGF-2 concentrations, In glipizide-treated patients, there were markedly increased IGF-1 concentrations(181.8+ 104.5) vs (209.0+ 88.2) ng· ml-1(P<0.05) while serum IGF-2 was not change. There was a significant reduction of blood glucose in two groups at the end of treatment(both P<0.01), but C-peptide level was markedly increased(P<0.05) only in glipizide-treatedpatients.Conclusion The changes of IGF-1 is markedly different between metformin-treated and glipizide-treated patients with type Ⅱ diabetes mellitus.

9.Dynamic investigation of tibial biomechanical property endured persistent intensive stress.

For investigating the changes in continuous biomechanical nature of bone bearing intensive stress in vivo, we adopted the animal model of persistent intensive stress. Thirty-seven rabbits were involved in the experiment; they were randomly divided into control group (5 rabbits) and trained group (32 rabbits). All animals were forced to jump and run about 300 times everyday in the electric stimulation cage so as to simulate the persistent intensive stress on tibias for different periods. Subsequently, all animals were sacrificed at different times (1-11 weeks), and all tibias of them were collected for biochemical investigation. By torsion destroy test, the experiment showed that the changes of biomechanical nature appeared obviously in a period of 6 weeks or so in the experiment. Furthermore, during the 2nd-3rd week of each period, the biomechanical indexes decreased strikingly; the abilities of deformation resistant dropped, the twisting rigidity decreased (69.7% lower than control, P< 0.01), the flexible index inereased (203.2% higher than control, P<0.05), the angle of twisting destroy increased (102.9% greater than control, P< 0.05). And the tibial mechanical strength declined too; the destroy torsion, energy absorption and energy absorb density decreased (most of them being 50% lower than control, even accounting for 34.5% of control; P< 0.05 or P< 0.01). We noticed that the biomechanical properties of bone endured persistent intensive stress presented periodicity and the period of change in bone biomechanical nature covered about 6 weeks in the experiment. Therefore, we have ground to conclude that a weakness period did exist when rabbit tibias endured 2-3 weeks persistent intensive stress, and such weakness did exist at the 2nd-3rd week of each training period.

ObjectiveContrast research the operative effect of the pulmonary artery banding (PAB) in the congenital heart disease with pulmonary artery hypertension,underwent the single ventricle repair procedure in different age.Methods 49 cases,male 31 and female 18.Age (7.8 ± 3.8 ) years old,weight ( 8.82 ± 4.24) kg,percutaneous blood oxygen saturation ( SPO2 ) 0.90 ± 0.04,preoperative mean pulmonary artery pressure (mPAP) ( 54.6 ± 16.8 ) mm Hg.single ventricle 13cases,tricuspid atresia 12 cases,double outlet of right ventricle with left ventricular dysplasia 11 cases,tricuspid stenosis 5 cases,ventricular imbalance type complete atrioventricular canal defect 5 cases and cross heart cases 3 cases.divided into three groups:≤0.5 years old of 17 cases,0.5 to 2.0 years old of 17 cases,≥2.0 years old 15 cases.All cases taken PAB under anesthesia and cpb,inhaled oxygen concentration 40%,SPO2 0.85,MPAP 20 mmHg.comparatively analysis postoperative SPO2,MPAP,Ventilator using time,ICU stay time and operation mortality of the three groups.ResultsThe postoperative PAP obviously decreased.Three groups of postoperative SPO2,mPAP,ventilator using time,ICU stay time were not significantly different.More than were followed up 6-72 months,1 cases ( 1-2 months) dead in aspiration two months postoperation.the other three cases had been completed Glenn and/or Fontan.ConclusionPAB can effectively reduce the pulmonary artery pressure in the different age children with congenital heart disease and pulmonary artery hypertension,the effect is good.more than 2 years old children still PAB feasible operation,and likely to complete the Glenn and/or Fontan procedure.
