1.Proximal femoral nail antirotationversus artificial femoral head replacement for intertrochanteric fracture in the elderly:a meta-analysis

BACKGROUND:Proximal femoral nail antirotation and femoral head replacement could quickly recover hip function in intertrochanteric fractures in the elderly, but whose efficacy is better remains controversial. OBJECTIVE:To compare the differences in the effects of proximal femoral nail antirotation and femoral head replacement on intertrochanteric fractures in the elderly by using a meta-analysis. METHODS:The relevant literatures were searched in PubMed, Cochrane, CNKI, Wanfang database and VIP, and other relevant journal such asChinese Journal of Orthopaedicsand Orthopedic Journal of Chinafor articles published in recent five years. Randomized controled trials concerning proximal femoral nail antirotation and femoral head replacement for the treatment of intertrochanteric fractures in the Chinese elderly were colected. Baseline data, operation time, intraoperative blood loss, postoperative out-of-bed time, length of stay, Harris score, complication rate and number of death were colected and processed using RevMan 5.30 software for meta analysis. RESULTS AND CONCLUSION:Totaly 37 clinical controled trials with 3 216 patients were recruited. Meta-analysis results showed that compared with femoral head replacement, proximal femoral nail antirotation was at a disadvantage in postoperative out-of-bed time, length of stay and joint function in the early stage. No significant difference in complication and mortality was detected between proximal femoral nail antirotation and femoral head replacement. However, proximal femoral nail antirotation had some advantages such as short operation time, smal trauma, and less intraoperative blood loss, and showed good midterm and long-term outcomes of joint function.
2.Study of protective effect and mechanism for baicalin on bacillus pertussis infected brain tissue and its dose-effect relationship.

OBJECTIVETo study the possible mechanism of protective effect for Baicalin on Bacillus pertussis (BP) infected brain tissue and the dose-effect relationship.
METHODSBrain tissues slices were divided into 7 groups: (1) the normal group; (2) the model group: infected by 10% BP; (3) the baicalin group, which was pretreated with baicalin, infected by BP and subdivided into 5 sub-groups according to different doses of baicalin used; (4) the glutamic acid group: cultured with glutamic acid; (5) the baicalin plus glutamic acid group; (6) the peroxide group: cultured with hydrogen peroxide; and (7) the baicalin plus peroxide group. The lactate dehydrogenase (LDH) content in the supernatant of culture was determined and quantitative protein determination was conducted.
RESULTSThe LDH releasing was higher in the model group, glutamic acid group and peroxide group as compared with that in the normal group, 15.10 +/- 4.89 u/g. protein (the same unit below), 15.49 +/- 5.66 and 16.54 +/- 5.47 vs 6.10 +/- 2.87 respectively (P < 0.01). After being pretreated with 0.25 mmol/L baicalin, LDH level decreased significantly to 8.65 +/- 2.43, which was significantly different from that in the model group (P < 0.01), LDH was also decreased in the baicalin plus glutamic acid group (9.93 +/- 2.89) and baicalin plus peroxide group (9.54 +/- 2.82), which was significantly lower than that in the glutamic acid group and the peroxide group respectively (P < 0.01).
CONCLUSIONPretreatment of baicalin has protective effect on BP caused nerve cell injury in rat brain slices, the protection is possibly related with the reduction of glutamic acid and hydrogen peroxide induced damage on nerve cells in vitro.

3.Investigation about the postoperative rehabilitation and the health knowledge of patients with arteriosclerosis obliterans in lower extremity

Objective To investigate the condition of exercise, medicine compliance and the rehabilitation of patients with arteriosclerosis obliterans (ASO) in lower extremity after the surgery, and also to study the influence of self-efficiency and social support to their rehabilitation. Methods The questionnaire survey was performed in 102 patients who had surgical or interventional therapy of arteriosclerosis obliterans in the lower extremity in our hospital, investigated the general information of the patients, their knowledge of postoperative rehabilitation, medicine compliance, exercise condition, general self-efficiency and social support. Results The postoperative time for the patients with arteriosclerosis obliterans was 2 to 22months, the knowledge rate for postoperative rehabilitation was 7.8%-46.1%.The Morisky score for the patients of taking medicine for the treatment of vascular disease was 2.0,and there was only 6 patients(5.9%)with good medicine compliance. There were 22 patients(21.0%) had completed the ambulation training according to the guidelines, and there were 25 patients(24.5%) had completed the Buerger training. The self-efficiency score of the patients was 2.78 ± 0.76, and the gross score of social support was 32.38 ± 7.89. Spearman correlation analysis shows that the Morisky score for the patients of taking medicine for the treatment of vascular disease was in positive correlation with self-efficiency (r=0.289, P=0.003), but in negative correlation with social support (r=–0.286, P=0.006). Conclusions Most of the patients with arteriosclerosis obliterans have poor medicine compliance, and do not do exercise as required by the guidelines. Patients with high self-efficiency demonstrates medicine noncompliance, however the enhancement of social support will help patients improve their medicine compliance.
4.Study on risk factors of brain metastases of locally advanced non-small-cell lung cancer

Objective To evaluate the prognostic factors in locally advanced non-small-cell lung cancer (LA-NSCLC) for selectively carrying out prophylactic cranial irradiation (PCI).Methods 114 patients with LA-NSCLC between Jun 2006 and Oct 2010 were retrospectively analyzed. Related risk factors and features about brain metastases were analyzed.Results The 2-year incidence rate of brain metastases was 31.58 % (36/114),the first brain metastases was 20.18 % (23/114),and sole brain metastases was 9.65 %(11/114),respectively.Variables involved in the equation of binary logistic regression analysis were pathology (OR =5.892) and treatment mode(OR =2.888).The incidence rate of brain metastases in patients of non-squamous carcinoma and single treatment mode was higher than others (P < 0.01) Model fitting is better (P > 0.05).Overall accuracy rate of predicting brain metastases is 67.7 %.The increased rate of lactate dehydrogenase in the patients with brain metastases or death was 17.54 %, which was higher than that in the survival patients without brain metastases (P < 0.01).At the same time,the station number and the number of mediastinal lymph node metastases were positively correlated (r =0.716, P < 0.01).The incidence rate of brain metastases or mortality rate was higher in the adenocarcinoma cases than that in the squamous carcinoma cases (P < 0.01,P < 0.05),with more frequent occurrence of mediastinal metastases.The mean diameter of squamous carcinoma and adenocarcinoma were 5.8 cm and 3.9 cm, respectively (P < 0.01).Conclusions The incidence rate of brain metastases was higher in patients with single treatment.Large primary tumors, high lactate dehydrogenase, non-squamous carcinoma, multiple stations, and multiple mediastinal lymph nodes metastases can be regarded as risk factors of brain metastases to perform PCI.
5.Expression of intrahepatic CD4+ CD25+ regulatory T cells in patients with chronic hepatitis B.

OBJECTIVETo investigate the expression and distribution of intrahepatic CD4+ CD25+ regulatory T cells in immuno-tolerant and immuno-clearance phase of patients with chronic hepatitis B.
METHODSThe expression of FoxP3 was detected in 19 cases of immuno-tolerant phase and 12 cases of immuno-clearance phase by immunohistochemistry. The relation between the intrahepatic expression of FoxP3 and the clinicopathological features were analyzed.
RESULTSThe positive signal of FoxP3 is located in nuclear of lymphocyte and mainly aggregated in portal areas as well as occasionally scattered in hepatic sinusoids. The expression of intrahepatic FoxP3 in the group of immuno-tolerant phase was significantly increased than those in normal control (P < 0.01), and greatly decreased than those in immuno-clearance phase (P < 0.01). No correlation was observed among the expression of intrahepatic FoxP3, ALT, levels of HBV DNA, HBeAg positive, in patients of immuno-clearance phase, respectively. There were significant differences between immuno-tolerant phase and immuno-clearance phase age, ALT, TBIL, PTA, HBV-DNA and detection of HBeAg but not in sex and family history of HBV infection.
CONCLUSIONCD4+ CD25+ regulatory T cells may play important roles in the clearance of HBV as well as in liver inflammation and injury during chronic HBV infection.

6.SELENIUM STATUS AND DIETARY SELENIUM CONTENT OF POPULATIONS IN THE ENDEMIC AND NON-ENDEMIC AREAS OF KESHAN DISEASE

By using whole blood selenium, 24 hr urinary selenium and hair selenium contents as the indices of assessing human selenium status, it was found that the populations in the endemic areas of Keshan disease were practically in a selenium poor status. The selenium contents in locally grown staple grains and daily diets in the endemic areas were also lower than those in the non-endemic areas. In an area covering a cross section of Keshan disease geographic belt in our country, the hair selenium contents of agricultural populations were measured. The results indicated that all the hair selenium contents in the endemic sites were always at a lower level, whereas those in the non-endemic sites distant from the endemic areas were generally at a higher level; they decreased gradually until the endemic areas were reached; and finally, along the contiguous region of the endemic and non-endemic areas they were insignificantly different.The hair selenium contents among the agricultural populations were significantly lower than those among the non-agricultural ones in the same endemic areas. However, no regular correlation had been observed between the seasonal prevalence of Keshan disease and the variation of hair selenium contents in the same populations living in the same endemic sites.It is considered that the endemic areas of the disease seem to be a Se-deficiency belt, and Se-deficiency probably might be a pathogenic geo-gen in the prevalence of Keshan disease.
7.Analysis of a random telephone survey result of knowledge in prevention and treatment of iodine deficiency disorders in urban communities of Jiangsu province in 2010

Objective To provide updated recommendations for further adjustment of iodine deficiency disorders control strategy by evaluating awareness of the dangers of iodine deficiency disorders and their prevention and control measures among local residents and the willingness of choosing iodized or non-iodized salt in urban communities of Jiangsu province.Methods Trained health service workers did questionnaire investigation on iodine deficiency disorders prevention knowledge by calling computer-randomly selected family telephone numbers in every two blocks of provincial capital (Nanjing) and coastal city (Nantong) in Jiangsu province in 2010.The survey topics covered include:when supplied with both non-iodized and iodized salt,what kind of salt the local residents chose and the reasons; residents awareness of the dangers of iodine deficiency disorders and their prevention and control measures; resident's personal information(including only occupational category and age group).Key fields were rechecked from 10% randomly selected valid questionnaires.Results Four hundred and fifty-five valid questionnaires were obtained by calling 2021 randomized telephone numbers in Nanjing and Nantong cities.After 10.1%(46/455) of the valid questionnaires were rechecked,the total coincidence rate was 87.0%(40/46).In the two cities,there were 73.2% (333/455) of those 455 respondents saying they would choose iodized salt when iodized and non-iodized salt were supplied at the same time.There were 69.0%(314/455) of respondents had heard of iodine deficiency disorders,of which 79.6%(250/314) of them were aware of the dangers of iodine deficiency is endemic goiter,and 10.8% (34/314) of them knew that iodine deficiency can cause varying degrees of mental damage.With respect to useful preventive methods of iodine deficiency disorders,there were 69.4% (218/314) respondents know that eating iodized salt,and 41.1% (129/314) respondents know that taking laver or kelp would prevent iodine deficiency disorders.Conclusions Remarkable achievements on iodine deficiency disorders health promotion and health education have been gained in urban communities of both the provincial capital and the coastal city Jiangsu province.However,some of the people are still not very clear why we should eat iodized salt.Salt industry in the current management system,is still premature to provide iodized salt and non-iodized salt simultaneously now.
8.Effects of Exercise on Sleep Quality in Pregnant Women: A Systematic Review and Meta-analysis of Randomized Controlled Trials

Purpose:
Sleep quality was considered a priority concern facing pregnant women. Conventional wisdom argues that good sleep quality benefits pregnant women and their fetuses. The aim of this study is to assess the effects of a specific exercise program on the sleep quality in pregnant women.
Methods:
Searches were executed in seven databases since their inceptions until February 28, 2019, for randomized controlled trials evaluating the effects of an exercise program on the sleep quality and insomnia in pregnant women. A random-effects model was applied for meta-analysis, and odds ratio, mean differences (MDs), and 95% confidence intervals (CIs) are shown as parts of outcomes.
Results:
Seven studies were included for meta-analysis. Compared with their not-exercising counterparts, analyses showed that regularly exercising women had significantly enhanced sleep quality, with an odds ratio of 6.21 (95% CI, 2.02–19.11;p = .001; I2 = 80.2%), with a standardized MD of −0.93 (95% CI, −1.19 to −0.67; p < .001; I2 = 30.0%). However, exercising women showed no significant insomnia improvement, with an standardized MD of −2.85 (95% CI, −7.67 to 1.98; p = .250; I2 = 97.0%), relative to their not-exercising counterparts.
Conclusion
This research indicated that exercise has a positive impact on the sleep quality of pregnant women. Despite the aforementioned positive impact on sleep quality, the present study did not find evidence to support that exercise may also improve insomnia for pregnant women.

10.Effects of stereoscopic cultivation on soil microorganism, enzyme activity and the agronomic characters of Panax notoginseng.

Compartments of soil microorganism and enzymes between stereoscopic cultivation (three storeys) and field cultivation (CK) of Panax notoginseng were carried out, and the effects on P. notoginseng agronomic characters were also studied. Results show that concentration of soil microorganism of stereoscopic cultivation was lower than field cultivation; the activity of soil urea enzyme, saccharase and neutral phosphatase increased from lower storey to upper storey; the activity of soil urea enzyme and saccharase of lower and upper storeys were significantly lower than CK; agronomic characters of stereoscopic cultivated P. notoginsengin were inferior to field cultivation, the middle storey with the best agronomic characters among the three storeys. The correlation analysis showed that fungi, actinomycetes and neutral phosphatase were significantly correlated with P. notoginseng agronomic characters; concentration of soil fungi and bacteria were significantly correlated with the soil relative water content; actinomycete and neutral phosphatase were significantly correlated with soil pH and relative water content, respectively; the activities of soil urea enzyme and saccharase were significantly correlated with the soil daily maximum temperature difference. Inconclusion, The current research shows that the imbalance of soil microorganism and the acutely changing of soil enzyme activity were the main reasons that caused the agronomic characters of stereoscopic cultivated P. notoginseng were worse than field cultivation. Thus improves the concentration of soil microorganism and enzyme activity near to field soil by improving the structure of stereoscopic cultivation is very important. And it was the direction which we are endeavoring that built better soil ecological environment for P. notoginseng of stereoscopic cultivation.
