BACKGROUND: Glass ionomer cement has been a commonly used filling matedal in the clinic. However, glass ionomer easily absorbs water and dissolves in early concretionary phase, which can reduce the leak tightness of the edge of obturator, resulting in slightly leakage. Therefore, protective agents are necessary following glass monomer filling. OBJECTIVE: To compare the effects of three protective agents (vaseline, cocoa butter, luminous paint) on edge closeness of glass ionomer cements. DESIGN, TIME AND SETTING: The controlled observation study was performed at the Central Laboratory, Hospital Affiliated to Medical College, Qingdao University from July to October 2007. MATERIALS: Forty freshly block teeth were used. After cleaning the facing, a 5 mm×3 mm×2 mm cavity was made at 1/3 part near to buccal surface using a high speed turbine engine under water cooling. Tooth facing was coated twice with colorless nail-polish at1 mm from the cavity. Apical foramen was covered with rod wax sheet. Teeth were equally and randomly assigned into control group, vaseline group, cocoa butter group and luminous paint group. METHODS: Glass ionomer cement was filled into the cavity. The filling surfaces were respectively coated with vaseline, cocoa utter and luminous paint in each group. The control group was left intact. Hot-cold cycle test and microleakage test were performed. MAIN OUTCOME MEASURES: The depth from dye penetration into the cavity wall to filling was recorded. RESULTS: Microleakage to different degrees was observed in each group under the microscope. However, the penetration depth was deeper in the control group than other three groups (P ＜ 0.05). No significant difference in penetration depth was detected among the vaseline, cocoa butter and luminous paint groups (P ＞ 0.05). CONCLUSION: Vaseline, cocoa butter and luminous paint have good effects on wet prevention.

OBJECTIVE To investigate and analyze the problems and challenges of HIV/AIDS epidemic on public health supervision in China.METHODS The main problems were found out by retrospective survey method,and could be solved through establishing a system of management.RESULTS The laws,regulations and specializations were strengthened and the full of public health supervision efforts were made.CONCLUSIONS It is essential to actively participate in widespread mass media education efforts to mainstream knowledge about HIV/AIDS prevention and care.But also to strengthen laws and specializations,to improve the quality of supervision,and to prevent and control of HIV/AIDS epidemic.

Objective To observe the hypoglycemic, hypolipidemic and antioxidant effects of Zhuye Shigao Decoction (ZSD) on experimental type 2 diabetes mellitus rats. Methods Type 2 diabetes mellitus rat model was induced by intragastric administration of high fat diet(10 mL/kg) and intraperitoneal administration of streptozotocin (30 mg·kg-1). Sixty male Sprague-Dawley rats were randomized into normal group, model group, low-, moderate-and high-dose ZSD groups (in the dosage of 3.6, 7.2, 14.4 g·kg-1·d-1 respectively), and Metformin group (0.15 g·kg-1·d-1) , 10 rats in each group. With fasting blood glucose, urine glucose, glycosylated hemoglobin, total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol(LDL-C), serum insulin, serum superoxide dismutase(SOD) and malondialdehyde (MDA) as the main indexes, we evaluated the influence of ZSD on the blood glucose and lipid levels as well as oxidative stress in type 2 diabetes mellitus rats. Results Compared with the model group, the levels of blood glucose, urine glucose and glycosylated hemoglobin, and the contents of TC, TG, MDA were decreased (P <0.05), whereas SOD activity and serum insulin level were obviously increased (P < 0.05) in ZSD groups. ZSD showed dose-related effectiveness in the treatment of diabetic rats. Conclusion ZSD has certain hypoglycemic, hypolipidemic and antioxidant effects in treating type 2 diabetes mellitus.

Hyperuricemia is closely associated with hypertension,diabetes,coronary heart disease.However,numerous studies have shown that as a natural antioxidant,uric acid has a strong antioxidant effect.It can enhance the body's overall antioxidant capacity,scavenge free radicals in tissue,decrease the level of oxidative stress,and improve the outcomes in patients with stroke.It may provide a new target for the treatment of stroke through investigating the neuroprotective effect of uric acid on ischemic stroke.

Objective To evaluate the safety of CT guided percutaneous lung biopsy in the elderly,and to analyze the risk factors for its complications.Methods Totally 204 elderly patients aged ≥65 years underwent transthoracic needle biopsy under CT guidance.Clinical data,complication and its risk factors were analyzed retrospectively.Results The complications included pneumothorax accounting for 15.2％ (31/204),hemoptysis 21.6％ (44/204),and no mortality was found.Logistic regression analysis showed that needle path ≥5.00 cm,lesion size ≤2.00 cm and emphysema around the lesion were the independent risk factors for postoperative pneumothorax (OR=4.05,2.54 and 3.97,all P＜0.05).Meanwhile,the enhancement degree by CT ＞40 HU,needle path ≥5.00 cm and lesion size ≤2.00 cm were the independent risk factors for postoperative bleeding (OR=2.27,2.82 and 2.33,all P＜0.05) in elderly patients undergoing lung biopsy.The sensitivity and specificity for postoperative pneumothorax and bleeding obtained by forecasting equation were 87.1％ and 49.1％,54.5％ and 80.8％ respectively.Conclusions The length of needle path,lesion size and emphysema around the lesion are correlated with pneumothroax,and length of needle path,lesion size and the CT enhancement degree are correlated with bleeding after percutaneous lung biopsy in the elderly.

Objective To evaluate contrast-enhanced MRI sequence for diagnosing cholangiocarcinoma.Methods Cholangiocarcinoma was confirmed by surgery and pathology in 17 cases,all underwent preoperative CT and MRI T1-VIBE scan with contrast-enhancement sequence.We retrospectively analyzed imaging signs in two scan methods,including lesion position,number,size,enhancement degree,expansion degree of bile duct,invasion of adjacent artery and portal vein,and portal vein tumor thrombosis.The differences on detecting lesions between two methods were compared.Results The contrast to noise ratio (CNR) between tumor and liver tissue in MRI T1-VIBE images was obviously superior to that in CT images.Peripheral lesion boundary in T1-VIBE enhanced images was clearer than CT.One hemorrhage lesion was shown in T1-VIBE images,and tumor thrombosis was visible in the left branch of portal vein in 1 case.Bile duct wall lesions in T1-VIBE enhanced images was more evident than CT in hilar cholangiocarcinoma and extrahepatic cholangiocarcinoma.The VIBE enhanced images find more lesions in 9 out of 11 multiple focus cases,compared with CT images.Conclusions Contrast-enhanced MRI T1-VIBE sequence can give more comprehensive and clear evaluation on cholangiocarcinoma,and has important clinical diagnostic values.

Objective To discuss the therapeutic method of the chronical septic hip replacements with discharging sinuses. Methods Ten cases of infected hip replacement with discharging sinuses, including 6 males and 4 females with a mean age of 59 years old (from 48 to 65 years old ) were reported. The preoper ative mean Harris score was 31 (range 26 to 38), mean erythrocyte sedimentation rate (ESR) was 56 mm/h (38 to 74 mm/h). Cultures and sensitive test before and d uring operation were both carried out. One stage revision was performed in eig ht cases, two stage revision was performed in two cases. Results Preoperation microorganism cultures of joint aspiration or pus in sinus were positive in eigh t cases and negative in two cases; six had staphylococcus and two Gram negative microorganism. Intraoperation microorganism cultures were positive in nine cases and negative in 1 case; seven had staphylococcus and two had Gram negative. Acc ording to the results of preoperation microorganism cultures, six cases of staph ylococcus infection two negative cases of microorganism culture were treated wit h one stage revision, the other two cases of infection by Gram negative microo rganism had undergone two stage revision. No recurrence was found after 19 mon ths (8 to 30 months) follow up. ESR after operation decreased to normal range. Objective To summarize the experiences of total hip replacement in the treatment of osteoarthritis secondary to congenital dislocation of hip in adults. Methods From April 1998 to April 2000, fourteen hips in nine adult patients with osteoarthritis secondary to congenital dislocation of hip undergone total hip replacement were included in this study. There were seven females and two males. The age of the patients at surgery ranged from 27-53 years (mean, 45.56 years). Five patients underwent bilateral total hip replacements and 4 had unilateral total hip replacement. Results The incision healed smoothly. The duration of follow up ranged from 6 months to 2 years. All the patients could walk independently after the surgery. They could take care of themselves and restored to previous job. The average Harris score was increased from 33.93 to 89.21 points after the surgery. Conclusion Total hip replacement is an effective method in the treatment of osteoarthritis secondary to congenital dislocation of hip in adults. The four special problems encountered in the procedure such as the position of the acetabular component, the implantation of the femoral stem, the reconstruction of the abductor power, and correction of the leg length discrepancies should be considered. It is a challenging work to the orthopaedic surgeon and the careful preoperative planning is recommended.

Objective To evaluated the diagnostic value of malignant biliary obstrutive disease magnetic resonance cholangiopancreatography(MRCP).Methods 21 cases with malignant biliary obstructive disease diagnosed by clinic and CT were examined by MRCP,the results of MRCP and operation-pathology were analysed.Results The accuracy of MRCP for evaluating the cause of obstructive were 90.5%(19/21),dilated biliary duct appeared "soft canes-like"and "sausage-like",obstructive ends of biliary duct exhibited "cut-off form","down cup mouth-form"and "mouse tail-form".Conclusion Malignant biliary obstruction can accurately diagnosed by MRCP,it may provide an efficient method for the patients who can not accept ERCP provide an efficient alternative to direct cholangiopancreatography.
