OBJECTIVE: To evaluate the cost-effectiveness of Rabeprazole versus Lansoprazole for peptic ulcer. METHODS: A total of 101 patients with peptic ulcer were randomly assigned to receive either Rabeprazole+Clarithromycin sustained release capsules+tinidazole(treatment group) or Lansoprazole+Clarithromycin sustained release capsules+tinidazole(control group) for 14 days. RESULTS: In the treatment group vs. the control group, the effective rate was 96.0% vs. 84.3%(P0.05); the cost was 285.46 vs. 373.87 yuan; the cost-effectiveness ratio was 297.35 vs. 443.50. The incremental cost-effectiveness ratio for the control group was -755.64 as against the treatment group. CONCLUSION: Rabeprazole group is optimal for peptic ulcer in that it is more economical, more effective and safer than Lansoprazole group.

Background Choroidal neovascularization(CNV)has been descrihed as a main reason of visual loss in a lot of ocular diseases.Researches showed that local hypoxia and retinal pigment epithelial(RPE) cells play an important role in the formation of CNV.A closely relationship of angiopoietin-2 (Ang-2) and angiogenesis has been proved.However,whether the expression of Aog-2 in hypoxic cultured human RPE cells is associated with the pathogenesis of CNV is still below understood.Objective This study was to investigate the effects of hypoxia on expression of Ang-2 in cultured human RPE cells in vitro,and discuss the possible effects of Ang-2 in the formation of CNV.Methods Human RPE cells were cultured and passaged,and 4-7 generation of cells were used in the experiment.The cells were incubated in cultural plate at the density of 5×107 cells/L.The culture medium containing 200 μmol/L CoCl2 was used to establish the hypoxia model of human RPE cells culturecd in vitro for 0.5,1,2,4,6,12and 24 hours,and the RPE cells cultured under normoxia were as controls.Reverse transcription polymerase chain reaction(RT-PCR) was used to detect the expression of Ang-2 mRNA in cultured human RPE cells,and enzymelinked immunosorbnent assay(ELISA) was used to assay the content of Ang-2 protein in supernatant of cultured human RPE cells.Results The survival rate of human RPE cells was 90％ after resoscitation.The fourth generation of cells showed the fusiform with the less pigment in them.The Ang-2 mRNA/β-actin mRNA value in human RPE cells was significantly different among various groups(F=1086.30,P=0.00),The Ang-2 mRN A/β-actin mRNA value in hypoxia cultured for 0.5 hours group began to increase and peaked in hypoxia culture for 4-6 hours group,with the significant differences in comparison with normoxia control group(P＜0.05).The Ang-2 mRNA/β-actin mRNA value decreased to the baseline level at hypoxia for 24 hours.The ELISA analysis showed that the concentration of Ang-2 protein in supernatant of RPE cells showed significant difference among groups(F=1034.00,P=0.00).The concentration of Ang-2 protein increased at hypoxia culture for 0.5 hours and peaked at 6 hours,showing significant differences in comparison with the control group (P＜0.05).Conclusions Hypoxia could significantly up-regulate the expression of Ang-2 in human RPE cells cultured in vitro.Ang-2 expresses highly in the early stage of hypoxia,implying that Ang-2 participates in the formation of CNV.

The effects of sophoramine (SA) on immune system in normal mice were studied. SA 15. 20mg/kg reduced the weight of thymus,decreased the rate of E rosette,and also suppressed T lymphocyte transformation. The weight and the hemolytic plaque of spleen were not influenced by SA at 15. 20mg/kg. At the dose of 25mg/kg,SA reduced the weight of thymus and spleen,decreased the rate of E rosette and suppressed the T lymphocyte transformation. All of the doses of SA didn' t affect the phagocytotic function of peritoneal macrophage.

BACKGROUND: With the extensive development of reperfusion treatment on acute myocardial infarction, myocardial ischemia/reperfusion (I/R) injury cause clinical concern intensively. In recent years, nitric oxide (NO) and endothelin (ET)are hot point of researches. Myocardial protective role of Chinese traditional medicine is highly regarded.OBJECTIVE: To study the change of endothelial function during myocardial I/R injury of rabbits, the effect of shenmai injection (SMI) and mechanism.DESIGN: Randomized controlled animal study.SETTING: The First Hospital Affiliated to China Medical University.MATERIALS: The experiment was carried out in Laboratory Animal Center, Department of Biochemistry and the Second Electron Microscope Room of China Medical University from September 2002 to January 2003. Twenty-one healthy female Japanese white rabbits weighing 1.5-2.5 kg were in this study. SMI was produced by the Three Nine Medicine Industry Co., Ltd., Yaan (batch number: 010110).METHODS: ① Grouping: The animals were randomly divided into three groups, 7 cases of each group. Sham-operation group: Rabbits were gone through thread only and were not ligated coronary artery; I/R group and I/R + SMI group:Coronary artery occlusion was maintained 40 minutes at the ligature was released and reperfusion continued for 40 minutes. ② Administration: Rabbits in I/R group were injected 20 mL saline through ear margin vein (injected one third of saline at 10 minutes before ligature, two thirds of saline when unloosing the ligature). Rabbits in I/R + SMI group were injected with SMI which was diluted with saline to 20 mL (1.5 mL/kg). The method of injection was as same as I/R group.③ Index observation: 4 mL venous blood samples were obtained before ligature, at 40 minutes of ischemia and 40minutes of reperfusion. Half of the blood samples were separated serum to detect NO production (NOP) by nitric acid deacidizing enzyme process; another were separated plasma to detect ET by radiation immunity process. In the experiment ends, NOP and ET of myocardial tissue were detected; total of superoxide dismutase (T-SOD) of myocardial tissue were detected by sulphur purine oxidase process and malondialdehyde (MDA) were detected by thiobarbituric acid development process. Myocardial ultrastructure was observed with transmission electro microscope.MAIN OUTCOME MEASURES: ① Contents of serum NO and plasma ET in each.group before ligature, at 40 minutes of ischemia and 40 minutes of reperfusion; ② contents of NOP, ET, T-SOD and MDA of myocardial tissue; ③ Myocardial ultrastructure.RESULTS: All 21 rabbits were involved in the final analysis. ① Content of serum NOP: Content of serum NOP was lower in I/R group than that in sham-operation group at 40-minute ischemia and 40-minute perfusion (P ＜ 0.01), but was higher in I/R +SMI group than that in I/R group (P＜ 0.01). ② Content of plasma ET: Content of plasma ET was higher in I/R group than that in sham-operation group at 40-minute ischemia and 40-minute perfusion (P ＜ 0.01), but was lower in I/R +SMI group than that in I/R group (P ＜ 0.01). ③ Content of NOP was negative correlation with that of ET 40-minute ischemia and 40-minute perfusion (P ＜ 0.05, P ＜ 0.01). ④ Content of NOP and activity of T-SOD: Those were lower in I/R group than those in sham-operation group at 40-minute perfusion (P ＜ 0.01), but were higher in I/R +SMI group than those in I/R group (P＜ 0.01). ⑤ Contents of ET and MDA: Those were higher in I/R group than those in sham-operation group (P ＜ 0.01), but were lower in I/R +SMI group than those in I/R group (P ＜ 0.01). ⑥ Content of NOP was positive correlation with T-SOD activity (P ＜ 0.05), and negative correlation with content of MDA (P ＜ 0.05). In addition, content of ET was negative correlation with T-SOD activity (P ＜ 0.05), and positive correlation with content of MDA (P ＜ 0.05).CONCLUSION: Myocardial I/R leads to abnormality of vascular endothelial function, and SMI can improve endotheliual function and relieve myocardial I/R injury.

BACKGROUND:The effect of body mass index (BMI) on the outcomes ofin vitrofertilization (IVF) is controversial. Some scholars suggested that the higher BMI, the lower rate of pregnancy, and the higher rate of abortion. However, some others got the different results. To data, the effects of BMI on the pregnancy outcomes in females at different ages are rarely reported.
OBJECTIVE:To explore the effect of BMI on the pregnancy outcomes of IVF for infertile women atdifferent ages.
METHODS:A total of 902 cycles undergoing IVF were divided into two groups according to the age:≤ or> 35 years old groups. The patients in each group were then divided into four subgroups according to BMI: low weight, normal weight, excess weight, and obesity subgroups, respectively.
RESULTS AND CONCLUSION: With increasing BMI, the average dosage of gonadotropins was increased gradualy in two both of age groups (P < 0.05). In the≤ 35 years old, the levels of estrogens on the day of injecting human chorionic gonadotropin was significantly lower in obesity group than the other subgroups (P < 0.05); the rates of empty folicle in excess weight and obesity subgroups were significantly higher than that in normal weight subgroup (P < 0.008 23). The fertilization rate and clinical pregnancy rate of in obesity subgroup were significantly lower than those in the other subgroups (P < 0.05). In the > 35 years old group, the empty folicle rate in obesity subgroup was significantly higher than that in normal weight subgroup (P < 0.008 23). Fertilization rate in excess weight and obesity subgroups was significantly lower than that in the normal weight subgroup (P< 0.008 23). There were no significant differences in cleavage rate, cycle cancelation rate, implantation rate, biochemical pregnancy rate and abortion rate between four subgroups (P> 0.05). These findings suggest that in the females at the younger age, BMI not only affects the quality of folicles and fertilization rate; but with increasing BMI, clinical pregnancy rate is decreased obviously. However in the females at the older age, BMI only affects the folicular quality, embryo quality and fertilization rates; there are no significant differences in the outcomes of pregnancy in the four subgroups. BMI have effects on pregnancy outcomes of IVF, and the effects are much greater in the younger female group.

Objective To know the nursing countermeasures for the catheter associated complications during the course of hemofilitration by observation critical patients who had accepted hemofilitration.Methods Retrospective analyzed the nursing points and treatment effects among 68 critical patients with hemofilitration from June 2005 to October 2008.Results There were no venous catheters droped off,no blocked and obvious infection.6 cases with exudation,57 cases with vitro cogulation,35 cases with hypotension,21cases with hypothermia,2 cases with hypoglycemia.Conclusions Apropriate anticoagulant,observe the body temperature and keep the comfortable temperature can increase efficiency and reduce the venous catheter associated complications during the hemofilitration.
