3.Otoplasty using expanded "bag-shape" opisthotic fascial flap

Objective To make an inquiry into how to reconstruct a lifelike ear, and which is one of the most thorny problem in plastic surgery. Methods The operation was divided into two stages: the first stage was that an expander was put into a patient's opisthotic subscutaneous region and lobe of the ear was transposed or laid aside; the second stage was that the expander was taken off by a 2.5~3.0 ㎝ incision at the top or bottom of the expanded flap. The reniform incision except the preauriclar pedical part which was 0.5~0.6 ㎝ away from the expander edge was slit until the deep fascia and dissected forword at the same layer in order to form a "bag-shape" flap. Then, an auricle model was put and fixed and aspirating tube was inserted in the "bag". The back of the reconstructive ear was repaired with the full thickness graft. Results It made operating procedure simple, negative pressure plasty looked good, the blood circuit of flap was increased and occurrence rate of complication reduced. The operation had been performed on 18 patients with a satisfactory results. Conclusion The success rate of the procedure is higher and the shape of the reconstructed ear is more lifelike than that of the traditional one.

Objective To monitor KBD prevalence rate in Changdu of Tibet.Methods The aged 7~12 year children are tested with X- ray and epidemiological investigation.Results Xizang are still high yet,the 3 of 4 porints X-rates are more than 20% and the highest one is 55.34%.Conclusions The Changdu is the severest point of KBD in our country.
5.Advance in mechanism research of the interaction between bone plant on osteoblast

The conformity between bone implant and bone tissue plays an important role in osseointegration.Protein molecules such as extracellular matrix proteins,cytoskeletal proteins,and attachment proteins,which produced by osteoblasts,promote the attachment of osteoblasts to bone implant and the surrounding ceils.In vitro studies show that nano-plated implant material bears excellent biocompatibility and superior physical and chemical features.Nano-scale material has the advantage of high surface roughness,wettability and electrochemistry features which erdaance the adherence and interaction of extracellular matrix proteins and promotes the attachmerit,proliferation and differentiation of osteoblasts.The expression of cytokines,such as prostaglandin E2(PGE2)and transforming growth factor-β(TGF-β)are also increased in nano-structured bone implants which in turn promote the new bone formation.Further studies need to be done to explain the mechanism of the effect of nano-tructured bone implant on osteoblasts.

7.Sunitinib induces autophagy via suppressing Akt/mTOR pathway in renal cell carci-noma

Objective:To determine the mechanism of sunitinib-induced autophagy in renal cell carci-noma cells.Methods:MTS assay was applied to detect the cell viability alteration under the treatment of sunitinib (2,8 μmol /L).The sunitinib-induced autophagy as well as cell apoptosis was measured and compared after knocking down autophagy-related protein Beclin1 and microtubule associated protein 1 light chain 3 fusion protein (LC3)by RNA interference.The transmission electron microscope was used to observe the formation of autophagosomes in ACHN cells.The fluorescence microscope was used to mo-nitor distribution and aggregation of endogenous LC3-Ⅱ.The expressions of protein such as LC3-Ⅱ,the autophagic regulation molecules protein kinase B /mammalian target of rapamycin (Akt/mTOR)and the symbol of apoptosis poly ADP-ribose polymerase (PARP)were capable to be detected by immunoblotting assay.Results:Sunitinib was able to significantly trigger cell viability loss in the renal carcinoma cell ACHN,which was both in a concentration-dependent and time-dependent manner (P <0.05 ).After reducing the autophagy by knocking down Beclin1 and LC3,the number of cleavage of PARP was in-creased remarkably,whereas there was nearly not any cleavage in the mock group.By the transmission electron microscope,there were more autophagic vacuoles in ACHN cells after being administrated with sunitininb compared with the control.And the nuclear-to-cytosol translocation as well as aggregation of LC3-Ⅱ was presented after sunitinib treatment by the fluorescence microscope,which was the proof of the enhanced autophagy.According to the immunoblotting,sunitinib was able to increase the accumula-tion of LC3-Ⅱ.At the same time,the result of sunitinib combined with chloroquine,a drug which blocked the fusion of autophagosomes and lysosomes,demonstrated that the increasing amount of LC3-Ⅱwas due to the enhanced autophagy flux by sunitinib treatment in ACHN cells.However,phosphorylation of Akt as well as mTOR was decreased at the same time.The rapamycin (mTOR inhibitor)or knocking down Akt subunits could change the sunitinib-induced LC3-Ⅱ accumulation,whereas overexpression of Akt subunits decreased the autophagic flux,indicating that Akt/mTOR was the target of sunitinib in auto-phagy.Conclusion:Sunitinib induced autophagy via suppressing Akt/mTOR pathway,and the auto-phagy was involved in apopotosis.
8.Effect of team hypnotic counseling for cancer patients on anxiety and depression

Objective To observe the effect of team hypnotic counseling for cancer patients on anxiety and depression,and to explore the effective measure to make cancer patients achieve long -term survival and quality of life.Methods The hypnotic treatment technology was carried in the application of team hypnotic counseling,and the emotional status was designed for cancer patients of team hypnotic counseling courses,and the self rating Anxiety Scale(ASA)and self rating Depression Scale(SDS)was contrasted before and after guidance.Results After 20times of team hypnotic counseling,55 cases of cancer patients were taken ASA,SDS measurement,before team hypnot-ic counseling,ASA was (45.5 ±6.2)points,SDS was (47.3 ±9.3)points,and after team hypnotic counseling,ASA was (37.1 ±3.3)points,SDS was (38.2 ±2.4)points,there was a significant difference before and after team hyp-notic counseling(ASA P <0.02,SDS P <0.05).Conclusion The team hypnotic counseling can significantly im-prove cancer patients with anxiety and depression,application of hypnotic technique in cancer team counseling is very practical,feasible and worthy of further exploration.
9.The regulation roles of protein kinase C alpha in enhanced expression of type Ⅰ inositol 1,4,5-trisphophate receptors induced by tumor necrosis factor alpha

Objective To investigate the signal mechanism of protein kinase C alpha(PKC-α)participated in enhanced expression of type Ⅰ inositol 1,4,5-trisphophate receptors (IP3 RI) induced by tumor necrosis factor alpha(TNFα),in order to delineate the mechanisms of decreased glomerular filtration rate (GFR) in hepatorenal syndrome caused by TNFα.Methods The glomerular mesangial cells (GMCs)line from rats was chosen as experimental material.GMCs were divided into control (D),TNFα-2 h,TNFα-4 h,TNFα-8 h,and TNFα-24 h groups.Moreover,another two groups were sanflngol-8h (S),TNFα + Sanfingol-8h(TS)groups.The effect of TNFα on the expression of IP3RI was detected by immunocytochemical staining,Western blotting,teal time-polymerase chain reaction (PCR) assays.Results Immunocytochemical staining demonstrated that IP3 RI was mainly distributed in cytoplasm of GMCs.Enhanced positive staining was determined in all TNFα-treated groups,especially in TNFα-8 h group.Western blotting demonstrated that the expression of IP3RI protein was significantly higher in TNFα-4 h,TNFα-8h and TNFα-24 h groups than control group(4 h:1.82 ± 0.63 ; 8 h:2.95 ± 0.66 ; 24 h:2.48 ± 0.72 ; D:1 ±0.02 ; F =9.24,P < 0.05).The expression of IP3 RI protein was the highest in TNFα-8 h and TNFα-24 h groups(P <0.05).No difference was found among S,TS,and control groups(S:1.39 ±0.65; TS:1.35± 0.37 ; P > 0.05).Real time-PCR found the expression of IP3 RImRNA was significantly higher in all TNFα-treated groups than control group(2 h:3.35 ± 1.97; 4 h:3.16 ± 1.35; 8 h:3.70 ± 1.76; 24 h:4.49±1.70; D:1 ±0.01; F =6.167,P <0.05).No difference was found among all TNFα-treated groups(P >0.05).No difference was found among S,TS,and control groups(S:1.53 ±0.79; TS:1.32 ± 0.38 ; P > 0.05).Conclusions IP3 RI was mainly distributed in cytoplasm of GMCs.TNFa could enhance the expression of IP3 RI protein and IP3 RI mRNA,which could be blocked by sanfingol,a PKCα inhibitor.It might be an important signal in the mechanisms of GFR decrease caused by TNFα in hepatorenal syndrome.
10.Impact of preoperative ureteral stenting on outcome of flexible ureteroscopic lithotripsy

Objective To assess the impact of preoperative ureteral stenting on outcome of flexible ureteroscopic lithotripsy . Methods The clinic data of flexible ureteroscope lithotripsy were analyzed retrospectively .All 52 eligible patients were divided into three group :goup A(no preoperative ureteral stenting );group B (preoperative ureteral stenting for 3-10 days);group C(preopera‐tive ureteral stenting for two weeks or more ) .The application of ureteral access sheath ,operation time ,stone free rates ,hospital stays ,complications were compared among the three groups .Results There were no significant differences in aging ,gender ,stone size , distribution ,average hospitalization days ,postoperative complications among the three groups (P>0 .05) .There were significantly differ‐ences between group A and group B ,group C(P<0 .05) ,and there were no difference between group B and group C (P>0 .05) on the suc‐cess rate of indwelling ureteral access sheath ,average operation time ,stone free rate .Conclusion Preoperative ureteral stenting could en‐hance the success rate of indwelling ureteral access sheath ,shorten the operation time ,improve the stone free rate .There was similar out‐come of flexible ureteroscopic lithotripsy between preoperative ureteral stenting for 3-10 days and two weeks or more .
