This research is to investigate study the flavonoids from stems of Nelumbo nucifera and the cytotoxic activities of iso- lated compounds. The constituents were separated by column chromatography,and their structures were elucidated by spectroscopic data analyses. The isolated compounds were evaluated for cytoxic activities by MTT method. Twelve compounds were isolated and identified as rhamnazin-3-O-beta-D-glucopyranoside (1), luteolin-3', 4'-dimethylether-7-O-beta-D-glucoside (2), kaempferol-3-O-beta-D-xylopyranosyl-(1-->2)-O-beta-D-glucopyranoside (3), quercetin-3,3'-di-O-beta-D-glucopyranoside (4), 1, 8-dihydroxy-3,7-dimethoxyxanthone (5), isorhamnetin-3-O-beta-D-glucopyranoside(6) , kaempferol(7), isorhamnetin (8), quercetin(9), astragalin(10), hyperoside (11) and 1-hy- droxy-3,7,8-trimethoxyxanthone(12). All compounds were isolated from stems of this plant for the first time, and compounds 1-5 were firstly isolated from the family nelumbonaceae. Compounds 24 and 6 showed significant cytotoxic activities against BEL-7402 carcinoma cell lines at a concentration of 1 x 10(-5) mol x L(-1) with the inhibitory rate of 67.36%, 53.25%, 57.78%, 60.13% and 52.11%, respectively.

Objective To discuss the therapeutic method of the chronical septic hip replacements with discharging sinuses. Methods Ten cases of infected hip replacement with discharging sinuses, including 6 males and 4 females with a mean age of 59 years old (from 48 to 65 years old ) were reported. The preoper ative mean Harris score was 31 (range 26 to 38), mean erythrocyte sedimentation rate (ESR) was 56 mm/h (38 to 74 mm/h). Cultures and sensitive test before and d uring operation were both carried out. One stage revision was performed in eig ht cases, two stage revision was performed in two cases. Results Preoperation microorganism cultures of joint aspiration or pus in sinus were positive in eigh t cases and negative in two cases; six had staphylococcus and two Gram negative microorganism. Intraoperation microorganism cultures were positive in nine cases and negative in 1 case; seven had staphylococcus and two had Gram negative. Acc ording to the results of preoperation microorganism cultures, six cases of staph ylococcus infection two negative cases of microorganism culture were treated wit h one stage revision, the other two cases of infection by Gram negative microo rganism had undergone two stage revision. No recurrence was found after 19 mon ths (8 to 30 months) follow up. ESR after operation decreased to normal range. Chemical investigation was carried out to study the alkaloids from stems of Nelumbo nucifera and their cytotoxic activities. The constituents were separated by column chromatography, and their structures were elucidated by spectroscopic data analyses. The isolated compounds were evaluated for their cytotoxic activities by MTr method. Fifteen compounds were isolated from the total alkaloids extract and identified as asimilobine (1), isococlaurine (2), N-acetylnorarmepavine (3), crykonisine (4), velucryptine (5), pycnarrhine (6), liriodenine (7), nuciferine (8), nornuciferine (9), armepavine (10), N-methylasimilobine (11), coclaurine (12), N-norarmepavine (13), N-methylcoclaurine (14) and lysicamine (15). Compounds 1-7 and 12-15 were isolated from stems of this plant for the first time, and compounds 2-6 were firstly isolated from the family Nelumbonaceae. Compounds 7-10, 13 and 14 showed significant cytotoxic activities against HL-60 carcinoma cell line with inhibitory ratios of 51.36%, 59.09%, 52.51%, 53.93%, 51.43%, and 64.31% at concentration of 1 x 10(-5) mol x L(-1), respectively.

Objective To analyze the diagnostic value of serum procalcitonin ( PCT ) for early postoperative bacterial infection after pediatric living donor liver transplantation.Methods A retrospective study was conducted in pediatric patients after living donor liver transplantation recipients admitted to department of critical care medicine of Beijing Friendship Hospital affiliated to Capital Medical University during June 2013 to October 2015.According to the clinical data , all pediatric patients were divided into infection group(n=60) and non-infection group (n=100).Primary disease, PCT post operation day 1 to day 5 for non-infection group and day 1 to day 9 for infection group , temperature , white blood cell , cold ischemia time, warm ischemia time, operation time, volume of blood loss during operation were recorded.All parameters above were compared between groups.Receiver operating characteristic ( ROC) curve was plotted, and the diagnostic value of PCT was evaluated.Results PCT of both groups were elevated after liver transplantation , there was a markedly resolution in non-infection group within 48 to 72 hours.PCT of pediatric patients with bacterial infection was significantly higher than that of non-infected patients , and the difference was of greatly significant (4.62 ±1.39) ng/ml vs (0.85 ±0.19) ng/ml,t=26.56,P=0.00.ROC curve showed that the peak level of PCT might be valuable in the diagnosis of bacterial infection ( AUC=0.985).There was no significant difference of cold ischemia time [(109.92 ±19.22) min vs (108.04 ± 13.20) min, t=1.05, P=0.29], warm ischemia time[(1.49 ±0.17) min vs (1.52 ±0.12) min, t=1.08, P=0.28], operation time[(8.01 ±0.77)vs (8.00 ±1.05) h, t=0.06, P=0.94], WBC[(8.95 ±1.69) ×109/L vs (8.98 ±2.00) ×109/L,t=-0.08, P=0.93]and body temperature[(37.5 ±0.7) vs (37.5 ±0.8) ℃,t=-0.05, P=0.96] on the first day after surgery between infection and non-infection groups.Amount of bleeding in infection group was higher than that of non infection group [ ( 650.87 ± 90.36) ml vs (240.29 ±67.67) ml, t=32.33, P=0.00], there was longer length of ICU stay in the infection group[(11.01 ±1.81)d vs (6.03 ±1.65)d, t=17.78, P=0.00].Conclusion Peak PCT level was a valuable indicator for early postoperative bacterial infection after pediatric living donor liver transplantation.

Objective To investigate the epidemiological characteristics and mortality risk factors of severe sepsis patients admitted into intensive care unit (ICU).Methods The clinical data of 419 severe sepsis patients admitted to an adult ICU of Beijing Friendship Hospital from January 2009 to December 2012 were retrospectively analyzed and evaluated.Logistic regression analysis was employed to identify independent risk factors for the death of patients with severe sepsis during ICU stay.Results Overall ICU mortality was 43.9％ (184/419),and the respiratory tract was the most common site of infection (50.8％),followed by infection of the gastrointestinal tract and abdominal cavity (27.8％),and hematogenous infection (4.3％).Gram-negative bacteria were the most common pathogens [46.0％ (214/465)].Iatrogenic infections accounted for 53.7％ (225/419) of the enrolled patients.Logistic regression analysis showed that age [odds ratio (OR)=0.003,95％ confidence interval (95％CI) 1.015-1.056,P=0.O00],vasoactive drug application (OR=3.251,95％CI 1.562-6.768,P=0.002),failure of 3 or more organs (OR=2.452,95％ CI 1.015-5.924,P=0.046),and iatrogenic infection (OR =1.775,95％ CI 0.981-3.221,P=0.046) were independent risk factors for ICU mortality.Conclusions Severe sepsis is a common cause of ICU admission.Patients with risk factors for high mortality should be carefully monitored,and aggressive treatment should be administered.

OBJECTIVE:To study the ADR mechanism of conjunctival hyperemia in model rats with prostacyclin-induced high intraocular pressure. METHODS:50 rats were randomly divided into normal control group,model group,prostacyclin low-dose, medium-dose,high-dose groups(100,200,400 mg/kg),10 in each group. Except for normal control group,right eyes of rats in other groups were established high intraocular pressure model,dropping corresponding medicine once a day,for 1 week. After last administration,the right eyes cornel peripheral corneal endothelial cells of rats in each group were isolated in vitro and cultured. Vascular endothelial cell viability,cell apoptosis and proliferation-related factor(Ki-76),apoptosis-related factors(Bad,Bax),in-hibito of apoptosis-related factors (Bcl-2,Bcl-xl) protein expressions were detected. RESULTS:Compared with normal control group,vascular endothelial cell viability in model group were obviously decreased;apoptosis rate was obviously increased;Bad, Bax protein expressions were obviously enhanced;Bcl-2,Bcl-xl,Ki-76 protein expressions were obviously weakened,with statisti-cal significances (P<0.05 or P<0.01). Compared with model group,vascular endothelial cell viability in prostacyclin low-dose,medium-dose,high-dose groups were obviously decreased;apoptosis rate was obviously increased;Bcl-2,Bcl-xl protein expres-sions were obviously weakened,with statistical significances(P<0.05 or P<0.01);the Bad,Bax protein expressions in prostacy-clin medium-dose,high-dose groups were obviously enhanced(P<0.05 or P<0.01),the other indexes had no statistical differenc-es(P>0.05). CONCLUSIONS:Prostacyclin may cause conjunctival hyperemia through promoting the apoptosis of cornel peripher-al corneal endothelial cells of model rats with high intraocular pressure and decreasing the cell viability.

Glycosides are the active ingredients (AIs) of many Chinese herbs and have become hot spots along with the findings of their new functions,such as anti-inflammatory,antivirus,enhanced immunity and anti-cancer.It has been found that glycosides exert their effects by converting to aglycons or other AIs in vivo.Therefore,the transformation of glycosides to the corresponding AIs in vitro becomes very important to enhance their bioavailabilities.The microbial transformation has an unparalleled advantage in the transformation of Chinese herbs in vitro for its reaction specificity,less by-products,mild reaction conditions and environmental protection.This paper summarized and prospected researches of glycosides' microbial transformation.

Objective To investigate the bioactivities of controlled release bFGF microspheres (Ms) and their effects on the cultured osteoblasts. Methods The secondary cultured osteoblasts were divided into four groups according to the different ingredients being added to the DMEM culture medium, ie, control group,bFGF group, bFGF-PLGA-Ms group and bFGF-PELA-Ms group. The proliferation of the cultured osteoblasts was measured with cell counting method, MTT method and flow cytometry. The content of bone BGP secreted by osteoblast was also measured with RIA method. Results The in vitro cellular study showed no significant difference in the cell number and cell viability of four groups one day after plate culture.The cell number and cell viability in the bFGF-PLGA -Ms group were more than those in other three groups four and six days after plate culture. The cell number and cell viabilitythose in the bFGF group were more than those in the bFGF-PELA-Ms group six and eight days after plate culture with insignificant difference. The flow cytometrical examination showed that the G 2/M+S percentage in the bFGF group reached the highest two days after plate culture and the G 2/M+S percentage in the bFGF-PLGA-Ms group went the highest four and eight days after plate culture. Among four groups, the content of BGP in the bFGF-PLGA-Ms group was the highest and the bFGF-PELA-Ms group the next. Conclusions The effect of bFGF-PELA-Ms is not satisfactory,as indicates that the manufacturing method needs improving. However,the bFGF-PLGA-Ms can promote the proliferation and differentiation of the osteoblasts through a long period of controlled release of bFGF.
