1.Research on the Etiology of Pedophilia through Monozygotic Twins with Pedophilia : A Case Report.

Recent studies have suggested an association of genetic and neurodevelopmental factors with development of paraphilia. In this case report on monozygotic twins with pedophilia, we aim to offer additional evidence indicating that genetic vulnerability may be a significant risk factor along with environmental influences on development of pedophilia. These twin pairs, who have been arrested for sexual offenses against boys, had experienced emotional and physical abuse during their childhood, and the older twin brother is reported to have a history of sexual abuse. Both showed homosexual tendencies, and the younger twin brother was also diagnosed as exhibitionism. This case implies greater involvement of genetic factors and associated vulnerabilities in the etiology of paraphilia, including pedophilia, than environmental factors such as childhood abuse. In the future, conduct of research will be needed in order to determine the biological markers related to the etiology of pedophilia.

2.Evaluation of Pedophilia Patients of the National Forensic Hospital by Types of Offense according to the Federal Bureau of Investigation (FBI) Typologies of Child Molesters.

This study aimed to classify individuals who had perpetrated child sexual abuse according to the Federal Bureau of Investigation (FBI) typologies of child molesters and to assess the correlations between sociodemographic and criminal variables. We examined a total of 26 patients diagnosed with pedophilia (DSM-IV-TR criteria) who had been hospitalized in the National Forensic Hospital for Cure Custody. Data were obtained through self-administered questionnaires and reviews of patient records. Patients were classified into preferential offenders or situational offenders based on FBI typologies. Current and first offense ages of preferential offenders were younger than those of situational offenders. Preferential offenders were more likely to have been exposed to sexual abuse during childhood, and were more likely to watch child pornography than situational offenders (p < 0.05). Additionally, recidivists had lower IQs (intelligence quotient), lower levels of education, and longer illness durations than first-time offenders (p < 0.05). A multiple logistic regression analysis revealed that IQ and duration of illness might be risk factors of recidivism, although results were not statistically significant (p < 0.1). We found that classification of individuals who had sexually abused children into preferential or situational types, based on FBI typologies, provided on pedophile characteristics and probability of recidivism. We highly recommended that low IQ patients receive intensive treatment and education in order to reduce the possibility of future offenses.

3.A Clinical Review of Chemical Castration Against Sex Offenders for Children.

In the past, physical castration was adopted to punish sex offenders and prevent the recurrence of sexual crimes. However, it was abrogated because of human right issues and the irreversibility of fertility. Chemical castration of depot injection with hormones was introduced as an alternative method of physical castration. Antiandrogen is the most frequently used hormonal agents. Although there are several positive results such as changes of sexual behaviors and decrease of the recurrence rate of sexual crime after hormonal treatment, it also has serious limitations; difficulties in academic evaluation for control group and placebo effect; difficulties in the assessment of the therapeutic effect; and difficulties to decide the proper duration of treatment. Although the law for chemical castration to control sexual impulses in sex offenders was enforced since July, 2011 in Korea, there are not enough resources for treatment. We reviewed the appropriate references and suggested continuous long-term follow-up studies.

4.The Comparison of MMPI Profile between In-Family and Out-Family Child Sexual Offenders with Pedophilia.

Forty six patients (23 in-family and 23 out-family child sexual offenders) diagnosed with pedophilia participated in this study. For each patient, computerized objective data, obtained from the doctors, nurses, psychologists, and prosecutors involved, and the hospital information system, were collected. Immediately after the authors collected data that included any personal identifying information, it was replaced by random numbers to prevent bias and to protect privacy. Statistical analysis was performed using SPSS version 20.0 for MS Windows. Comparative items on demographic characteristics were evaluated by a paired t test and chi-square test. Out-family child sexual offenders were younger, assaulted younger victims, and possessed a higher sexual recidivism rate than in-family sexual offenders did (P<0.05). The four scales of Minnesota Multiphasic Personality Inventory showed a significant difference between in-family and out-family child sexual offenders. There was no statistically significant difference in the victim's gender and the incidence of comorbid psychiatric disease between in-family and out-family child sexual offenders.

5.Psychiatric and Psychological Characteristics of Sexual Offenders in Korea.

OBJECTIVES: In an effort to increase understanding of the relationship between sexual violence and mental illness, we conducted an investigation of the prevalence of psychiatric illness and associated psychological features of sexual offenders. METHODS: Twenty-two male sex offenders were referred for psychiatric diagnosis from the prosecutor's office and admitted to the hospital between August 2010 and May 2011. In order to identify Axis I and II disorders, including sexual disorders, we conducted the Korean version of the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorder, Fourth Edition. All participants completed psychological assessment batteries, including the Korean-Wechsler Adult Intelligence Scale, and self-report scales, including the Barratt Impulsiveness Scale, State-Trait Anger Expression Inventory, and the Rape Myth Acceptance Scale. Information on participants' legal, personal, and psychiatric family histories was also gathered. RESULTS: Many of the participants received diagnosis of Axis I disorders : Eight subjects (36.4%) had paraphilia, and five (22.7%) had pedophilia. Two subjects (9.1%) had schizophrenia, five (22.7%) had depressive disorder, and three (13.6%) had alcohol dependence. Participants also showed high rates of Axis II disorders : four of them (18.2%) had mental retardation or borderline intellectual functioning and seven (31.8%) had cluster B personality disorders. Participants' serum free testosterone level showed statistically significant correlation with motor impulsiveness (p<0.05). CONCLUSION: Sex offenders included in this study displayed high rates of mental illness, including paraphilia and intellectual disability. On the basis of our findings, we asserted that sex offenders should undergo careful evaluation for mental illness, and that management programs for sex offenders should include psychiatric evaluation and intervention.
